This study explores the implementation of computed tomography(CT)reconstruction and simulation techniques for patient-specific valves,aiming to dissect the mechanical attributes of calcified valves within transcatheter heart valve replacement(TAVR)procedures.In order to facilitate this exploration,it derives pertinent formulas for 3D multi-material isogeometric hyperelastic analysis based on Hounsfield unit(HU)values,thereby unlocking foundational capabilities for isogeometric analysis in calcified aortic valves.A series of uniaxial and biaxial tensile tests is executed to obtain an accurate constitutive model for calcified active valves.To mitigate discretization errors,methodologies for reconstructing volumetric parametric models,integrating both geometric and material attributes,are introduced.Applying these analytical formulas,constitutive models,and precise analytical models to isogeometric analyses of calcified valves,the research ascertains their close alignment with experimental results through the close fit in displacement-stress curves,compellingly validating the accuracy and reliability of the method.This study presents a step-by-step approach to analyzing themechanical characteristics of patient-specific valves obtained fromCT images,holding significant clinical implications and assisting in the selection of treatment strategies and surgical intervention approaches in TAVR procedures. 展开更多

Many people accept that valve replacement therapy should be performed if more than two valve leaflets are involved. The aim of the study to discuss if reconstructive surgery could be done to treat two valve leaflets involved in tricuspid valve endocarditis. Methods A 17-year-old boy with ventricular septal defect (VSD) and tricuspid valve subacute endocarditis was surgical treated through extra-corporeal circulation. two-thirds of the defective septal cusp, and half of the defective anterior cusp were ex-cised during operation. The tricuspid valve was reconstructed with autologous pericardial strip, cusp com-missuroplasty and 4-0 prolene sutures made as chordae tendineaes. VSD was repaired using a pericardial patch. Results The patient was discharged post-operation with excellent restoration of the tricuspid valve ac-tivity. Cardiac ultrasound revealed normal tricuspid valve activity and low degree of regurgitation two years follow-up. Conclusions It seems that tricuspid valve reconstruction could be performed for two defective leaflets or half of the tricuspid valve. 展开更多

We performed left ventricular reconstruction and mitral valve annuloplasty combined with papillary muscle relocation. Although left ventricular continued remodeling occurred during follow-up, left ventricular reconstruction and mitral valve annuloplasty combined with papillary muscle relocation provided durable mitral repair, and his functional status was good. Operating principle of water three-way valve with high flow for individual hydraulic prop in coal was presented in this paper, its strict and precise mathematical model was established, its flow field was simulated numerically by software Fluent, and its dynamic characteristics were analyzed during the work process such as raising leg, loading and overflow, the influence of the related parameters on high-flow water three-way valve was determined. The results as follows: during the raising leg stage and early raising leg stage, when the damping ratio increases, the overshoot of system decreases and the setting time reduces, and the dynamic response performance has a significant improvement. During the loading stage and the overflow stage, the pressure in plunger chamber of single hydraulic prop, the output flow and the displacement of the high-flow water three-way valve decrease with the decreasing of the external load. The spring stiffness of the safety valve directs the flow and the spool's displacement of the safety valve, and it can be used to control the high-flow three-way valve's sensitivity. 展开更多

In order to improve the dynamic response performance of the three-way solenoid. Methods Experimental studies have been conducted to investigate the effects of spool stroke, drive voltage, negative demagnetizing pulse and two drive schemes on the dynamic response performance of the three-way solenoid valve. Results The results show that the dynamic response performance of the three-way solenoid valve can be remarkably improved by shortening the spool stroke and increasing the drive voltage. Simultaneously, the difference between the response time of closing valve and that of opening valve decreases. At each different drive voltage, there exists an optimal negative demagnetizing pulse corresponding to the same positive exciting pulse. At this optimal pulse, the dynamic response performance of the three-way solenoid valve is the best. In addition, the high drive voltage can lead to the smaller optimal negative demagnetizing pulse. It is also indicated from the experiments that the dynamic response performance of the three-way solenoid valve is better when the NO.1 drive scheme is adopted. The lower drive voltage results in the larger difference between the dynamic response performances for the two drive schemes. Conclusion The dynamic response performance of a novel three-way solenoid valve is good. 展开更多

目的评估不同手术方法对Ebstein心脏畸形患者的治疗效果。方法回顾性分析2003年3月1日至2021年9月30日于河南省胸科医院心血管外科经手术治疗的74例Ebstein心脏畸形患者的临床资料。总结治疗方法,观察手术指标、手术效果和术后并发症。... 目的评估不同手术方法对Ebstein心脏畸形患者的治疗效果。方法回顾性分析2003年3月1日至2021年9月30日于河南省胸科医院心血管外科经手术治疗的74例Ebstein心脏畸形患者的临床资料。总结治疗方法,观察手术指标、手术效果和术后并发症。结果74例Ebstein心脏畸形患者中,男性34例、女性40例,年龄2~57岁,平均(17.4±7.9)岁,治愈出院71例(95.9%)。根据Carpentier分型采用不同手术方法:横向折叠房化心室+环缩瓣环术29例(39.2%),其中横向折叠房化心室后加双向格林术6例;纵向折叠房化心室+瓣环成形术18例(24.3%),其中加双向格林术4例;三尖瓣锥形重建术20例(27.0%);三尖瓣置换术6例(8.1%);二次手术三尖瓣前叶补片加宽修复术1例(1.4%)。术后住院时间7~15 d,平均(10.6±2.1)d;住ICU时间1.5~6 d,平均(2.5±1.3)d;呼吸机使用时间5~72 h,平均(26.5±4.4)h。不同手术方法术后3 d、术后1年的三尖瓣反流面积均小于术前(术后3 d与术前比较,t=6.766、6.113、9.144;术后1年与术前比较,t=6.803、6.243、9.129,均为P<0.01);手术前后纽约心脏病协会(NYHA)心功能分级分布比较差异有统计学意义(Z=17.915、32.083,均为P<0.01),且术后1个月、术后1年的NYHA心功能分级Ⅰ~Ⅱ级患者比例高于术前(χ2=45.106、45.106,均为P<0.01);71例治愈出院患者术后3 d、术后1年的三尖瓣反流面积、右心室功能指标均小于术前(术后3 d与术前比较,t=14.769、8.594、8.698、8.400、13.888;术后1年与术前比较,t=14.836、9.007、9.231、9.149、13.893,均为P<0.01)。术后并发症发生率为9.9%(7/71);术后死亡3例,死亡率为4.1%(3/74)。结论根据Carpentier分型采用不同手术方法治疗Ebstein心脏畸形均可得到较好治疗效果,建议D型病变采用纵向折叠房化心室+三尖瓣置换术,C型病变采用三尖瓣修复术+双向格林术,A、B型病变采用三尖瓣锥形重建术。 展开更多

为了实现损伤阀芯零件的高质量修复,提出了一种基于几何重构与路径位姿规划的阀芯再制造工艺方法。引入再制造边界预制原则对阀芯损伤部位进行预处理设计并制定其实施步骤,基于NURBS理论实现了再制造边界曲面及待修复阀芯模型的高精度重... 为了实现损伤阀芯零件的高质量修复,提出了一种基于几何重构与路径位姿规划的阀芯再制造工艺方法。引入再制造边界预制原则对阀芯损伤部位进行预处理设计并制定其实施步骤,基于NURBS理论实现了再制造边界曲面及待修复阀芯模型的高精度重构;采用等距截平面法生成再制造工艺路径,根据直线逼近和误差控制原理计算了各插补点坐标,并将光斑面积与插补点姿态相结合确定激光喷头动态位姿;运用ABB机器人系统进行路径与位姿校验并导出代码进行了阀芯再制造实践。结果表明:所提方法中规划路径及位姿能够有效跟随阀芯再制造边界曲面进行熔覆,形成了无气孔、无裂纹的致密涂层,涂层晶粒尺寸和平均硬度分别为14~54μm和573.6HV0.5±51.8HV0.5,耐冲蚀腐蚀性能提高了30.8%,满足阀芯再制造要求。 展开更多

基于2D-PIV的调节阀三维流场特性研究

调节阀流道内流动状态直接影响其服役寿命和系统工作稳定性,为了探究调节阀内部三维流场的分布特性,通过二维粒子图像测速(2D-PIV)装置获取三维坐标系下不同切面的流场特征,利用插值计算其余节点的流场信息,实现调节阀流场的三维重构。... 调节阀流道内流动状态直接影响其服役寿命和系统工作稳定性,为了探究调节阀内部三维流场的分布特性,通过二维粒子图像测速(2D-PIV)装置获取三维坐标系下不同切面的流场特征,利用插值计算其余节点的流场信息,实现调节阀流场的三维重构。实验结果表明:三维重构速度场与二维速度场整体呈一致趋势,油液在调节阀节流口处由于节流效应形成对冲射流,射流汇合后在阀芯头部下游形成高速的整体喷射;阀芯顶端区域由于两侧冲击射流的合并会产生一定的油液回流现象;整体流道速度随着拍摄平面外移均呈逐步下降的趋势,上游流道速度平稳下降到一定值后保持稳定,节流口附近速度呈先升高后下降的趋势且变化幅度较大,壁面阻力以及剪切力的存在使得近壁面速度保持平稳。下游区域速度呈同心圆分布,从湍流变为有序流动。仿真结果表明,三维重构流场与理论计算结果基本一致,最大重构误差为9.5%,出现在节流口处的高速流区域。实验重构结果与仿真重构结果具有高度相似性,流动平缓区域的重构精度较高。本研究可为优化调节阀结构和改善空蚀性能提供参考,也可为微小流道三维流场测量提供借鉴。 展开更多

Treatment of More Than Moderate Ischemic Mitral Regurgitation in Patients Undergoing Left Ventricular Reconstruction

Objective:While evidence-based clinical guidelines recommend chordal-sparing mitral valve replacement,rather than mitral valve repair,in patients with severe ischemic mitral regurgitation(IMR)undergoing coronary artery bypass grafting,there are no similar recommendations for patients undergoing left ventricular reconstruction(LVR).This study aimed to compare the clinical outcomes of mitral valve repair and replacement in patients undergoing LVR complicated by more than moderate IMR.Methods:In this single-center cohort study,a total of 74 consecutive patients who underwent LVR and mitral valve surgery(repair group:59;replacement group:15),during the period from March 2000 to March 2021 at Fuwai Hospital(Beijing,China)were retrospectively enrolled.Survival rates were calculated with the Kaplan-Meier method and compared using the log-rank test.Univariate Cox analysis was performed to evaluate possible confounders,followed by adjustment in multivariate analysis.The primary outcome was survival free of major adverse cardiovascular and cerebrovascular events(MACCE).Results:Median follow-up time was 59.4 months.Compared with mitral valve replacement,mitral valve repair was associated with increased risk of perioperative use of ventricular assist device(22.0%vs.0,P=0.045).There was no difference in overall survival(hazard ratio(HR),1.10;95%confidence interval(CI),0.31-3.93;Plogrank=0.888)and MACCE-free survival(HR,1.54;95%CI,0.65-3.65;Plogrank=0.319),even after multivariate Cox regression(HR,1.35;95%CI,0.37-4.88;PCox=0.646;and HR,1.41;95%CI,0.57-3.44;PCox=0.455,respectively).Furthermore,while no differences were observed in ejection fraction and left ventricular end-diastolic diameter on follow-up echocardiography,mitral valve repair was associated with an increased risk of recurrent mitral regurgitation(P=0.041).Conclusions:In patients undergoing LVR complicated by more than moderate IMR,both concomitant mitral valve repair and replacement can be successfully achieved with comparable overall and MACCE-free survival outcomes;however,mitral valve replacement may be superior to mitral valve repair for persistent correction of mitral dysfunction. 展开更多

目的:回顾性分析先天性二尖瓣关闭不全再次手术的问题。方法:自2002年至2009年间共438例先天性二尖瓣关闭不全(不包括完全性或部分性房室通道)患儿行二尖瓣成形手术,所有患儿中有15例(3.4%)行再次二尖瓣手术,其中再次行二尖瓣成形术12例... 目的:回顾性分析先天性二尖瓣关闭不全再次手术的问题。方法:自2002年至2009年间共438例先天性二尖瓣关闭不全(不包括完全性或部分性房室通道)患儿行二尖瓣成形手术,所有患儿中有15例(3.4%)行再次二尖瓣手术,其中再次行二尖瓣成形术12例(组1),二尖瓣置换术3例(组2)。再次手术的原因包括:严重溶血和残余或复发二尖瓣关闭不全。术中根据瓣膜病变情况选择合适的成形方法,包括二尖瓣瓣环环缩、腱索转移、双孔成形和瓣膜置换手术。对所有患者均行临床评估和超声心动图检查评估二尖瓣功能。结果:全组患儿无死亡病例,所有15例随访患儿亦无死亡。组2患儿年龄、体质量和心胸比均大于组1。组1患儿中术中见原缝线撕裂2例,瓣环扩大4例,瓣叶局部裂3例,组1患儿术后超声心动图见二尖瓣关闭不全较术前明显减轻(P<0.01)。组2中3例患儿均行二尖瓣双叶机械瓣置换。结论:二尖瓣成形术仍是先天性二尖瓣关闭不全再次手术的首选术式,对术前心功能差且瓣膜条件不理想的患儿行瓣膜置换手术是理智的选择。 展开更多

目的:评价二尖瓣成形术的早、中期效果。方法:回顾性分析1993年9月至1999年1月二尖瓣成形术121例,其中二尖瓣脱垂69例,风湿性心脏病35例,原发性感染性心内膜炎8例,先天性二尖瓣关闭不全7例(5.9%),其他2例。均在体外循环下行二尖瓣综合... 目的:评价二尖瓣成形术的早、中期效果。方法:回顾性分析1993年9月至1999年1月二尖瓣成形术121例,其中二尖瓣脱垂69例,风湿性心脏病35例,原发性感染性心内膜炎8例,先天性二尖瓣关闭不全7例(5.9%),其他2例。均在体外循环下行二尖瓣综合成形术。手术前、后应用彩色多普勒超声心动图观察二尖瓣返流的情况。结果:术中改行二尖瓣置换术8例;术后早期死亡3例(2.65%);随访4个月至4.7年,心功能(NYHA标准)恢复级61例,级19例,级6例,级2例;心尖部闻及收缩期杂音13例,再次行二尖瓣置换术3例;晚期死亡2例;术后3年超声心动图检查二尖瓣返流量少于1.0ml者占83.6%。结论:二尖瓣脱垂因腱索断裂或延长所致者,成形术的成功率较高;风心病患者年轻、瓣膜病变较轻者,成形效果较好;老年、病史较长以及瓣膜钙化严重者,成形术的远期效果较差。 展开更多
