Middle lobe torsion occurred after right upper and lower lobectomy in a 74-year-old man. Immediate re-exploratory thoracotomy using the 3-cm uniportal VATS approach was performed. The torsion was corrected, and the lobe was anchored to the anterior chest wall with Prolene stitches. The patient recovered well postoperatively with daily improvements in chest radiographic findings. Follow-up examination was performed using fiberbronchoscopy, which revealed an unobstructed right middle lobe bronchus and sticky yellow sputum. Follow-up chest computed tomography was performed 3 months after the primary surgery and revealed increased expansion of the right middle lobe. We repositioned the right middle lobe successfully by using the 3-cm uniportal VATS approach, but more cases are needed to confirm the feasibility of the approach. Lobectomy remains the primary treatment option for such cases.展开更多

With regard to degeneration, intervertebral discs become less flexible and stiffer. In this study, a testing protocol using bending and torsion loading was developed to gain the flexibility curves and stiffness often cadaveric lumbar discs. Measurements of rotation in the sagittal plane (flexion-extension), coronal plane (right-lefl lateral bending) and transverse plane (torsion) due to a 5 N-m load are reported. This paper presents the construction of a highly compact HSLDS torsional vibration isolator by connecting positive and negative stiffness components in paral lel. Based on mechanical model analysis, the restoring torque of negative stiffness components is de rived from their springs and connecting rods, while that of positive stiffness components is obtained through their circular section flexible rods. The quasizero stiffness characteristics of the HSLDS iso lator are achieved through a combination of static structural simulation and experimental test. The tor sional vibration isolation performance is assessed by means of numerical simulation and theory analy sis. Finally, the frequency-sweep vibration test is conducted. The test results indicate that the HSLDS torsional vibration isolator exhibits superior low-frequency isolation performance compared to its linear counterpart, rendering it a promising solution for mitigating low-frequency torsional vi bration in ship shafting.展开更多

文摘Bone is a multi-phase,non-homogeneous material that exhibits strain rate sensitivity, and it may fail under compression, tension, torsion,or a combination of these loading. The mechanical properties of cortical bone with strain rate effect under compression and tension have been obtained through the application of the split Hopkinson pressure/tension bar technique, but no such studies have been reported for determining the strain rate behavior properties of bony materials under torsion. In this study, the shear stress-strain curves with the rate-dependent cortical bone subjected to dynamic torsional loading were first obtained using a torsional split Hopkinson bar system.Based on the experiments,an improved mathematical model consisting of elastic, viscoelastic, and viscoplastic components was used to identify the material parameters of the cortical bone.Detailed material properties are derived through constitutive relations. The results may assist researchers in developing more accurate models of cortical bone behavior under different load conditions.
文摘High-static-low-dynamic stiffness (HSLDS) vibration isolators have been demonstrated to be an effective means of attenuating low-frequency vibrations, and may be utilized for ship shafting applications to mitigate torsional vibration. The test results indicate that the HSLDS torsional vibration isolator exhibits superior low-frequency isolation performance compared to its linear counterpart, rendering it a promising solution for mitigating low-frequency torsional vi bration in ship shafting.

文摘Previous studies on the hollow cylinder torsional shear test(HCTST)have mainly focused on the macroscopic behavior, while the micromechanical responses in soil specimens with shaped particles have rarely been investigated. This paper develops a numerical model of the HCTST using the discrete element method(DEM).The method of bonded spheres in a hexagonal arrangement is proposed to generate flexible boundaries that can achieve real-time adjustment of the internal and external cell pressures and capture the inhomogeneous deformation in the radial direction during shearing.Representative angular particles are selected from Toyoura sand and reproduced in this model to approximate real sand particles. The model is then validated by comparing numerical and experimental results of HCTSTs on Toyoura sand with different major principal stress directions.Next,a series of HCTSTs with different combinations of major principal stress direction(a)and intermediate principal stress ratio(b)is simulated to quantitatively characterize the sand behavior under different shear conditions. The results show that the shaped particles are horizontally distributed before shearing, and the initial anisotropic packing structure further results in different stressestrain curves in cases with different a and b values. The distribution of force chains is affected by both a and b during the shear process, together with the formation of the shear bands in different patterns. The contact normal anisotropy and contact force anisotropy show different evolution patterns when either a or b varies, resulting in the differences in the non-coaxiality and other macroscopic responses. This study improves the understanding of the macroscopic response of sand from a microscopic perspective and provides valuable insights for the constitutive modeling of sand.
文摘BACKGROUND Cystic lymphangioma is a rare benign tumor that affects the lymphatic system.Mesenteric lymphangiomas in the small bowel are extremely uncommon.CASE SUMMARY We present a 21-year-old female patient who complained of abdominal pain. The diagnosis of ovarian torsion was suspected after abdominopelvic unenhanced computed tomography and ultrasound revealed a large cyst in contact with the bladder, ovary, and uterus. The patient underwent emergency laparotomy per-formed by gynecologists, but it was discovered that the cystic tumor originated from the jejunum.Gastrointestinal surgeons were then called in to perform a cystectomy.Pathological examination confirmed the diagnosis of cystic lymphangioma of the mesentery. The patient had an uneventful postoperative recovery.CONCLUSION Mesenteric lymphangiomas can cause abdominal pain, and imaging techniques can help determine their characteristics, location, and size.Complete surgical excision and pathological examination are considered the standard treatment and diagnostic method.

文摘In order to achieve combined mechanical and electrical properties,multi-walled carbon nanotubes(MWCNTs)reinforced Cu/Ti_(3)SiC_(2)/C nanocomposites were further processed by high-pressure torsion(HPT).The maximum microhardness values of central and edge from the composites with 1 wt.%MWCNTs reached HV 130.0 and HV 363.5,which were 43.9%and 39.5%higher than those of the original samples, respectively. With the same content of MWCNTs,its electrical conductivity achieved 3.42×10^(7) S/m,which was increased by 78.1%compared with that of original samples. The synergistic improvement of mechanical and electrical properties is attributed to the obtained microstructure with increased homogenization and refinement,as well as improved interfacial bonding and reduced porosity. The strengthening mechanisms include dispersion and refinement strengthening for mechanical properties,as well as reduced electron scattering for electrical properties.

文摘BACKGROUND Testicular torsion is the most common acute scrotum worldwide and mainly occurs in children and adolescents.Studies have demonstrated that the duration of symptoms and torsion grade lead to different outcomes in children diagnosed with testicular torsion.AIM To predict the possibility of testicular salvage(TS)in patients with testicular torsion in a tertiary center.METHODS We reviewed the charts of 75 pediatric patients with acute testicular torsion during a 12-year period from November 2011 to July 2023 at the Suzhou Hospital of Anhui Medical University.Univariate and multivariate logistic regression analyses were used to determine independent predictors of testicular torsion. The data included clinical findings, physical examinations, laboratory data, color Doppler ultrasound findings, operating results, age, presenting institution status, and follow-up results.RESULTS Our study included 75 patients.TS was possible in 57.3%of all patients;testicular torsion occurred mostly in winter, and teenagers aged 11-15 years old accounted for 60%.Univariate logistic regression analyses revealed that younger age(P=0.09),body mass index(P=0.004),torsion angle(P=0.013),red blood cell count(P=0.03),neutrophil-to-lymphocyte ratio(P=0.009),and initial presenting institution(P<0.001)were associated with orchiectomy. In multivariate analysis, only the initial presenting institution predicted TS(P<0.05).CONCLUSION The initial presenting institution has a predictive value for predicting TS in patients with testicular torsion.Children with scrotal pain should be admitted to a tertiary hospital as soon as possible.
