It was the subject of a priority axis of the national plan for monitoring and evaluating the fight against HIV (2006-2010) that was aimed at strengthening the capacities of actors in this area. To increase the critical mass of competent human resources in the short term, the National Institute of Public Health (NIPH) of Côte d’Ivoire organized monitoring and evaluation training sessions for healthcare professionals from 2011 to 2016. Methods: A single case study with multiple levels of analysis was carried out, combining a qualitative survey and a literature review. An evaluation was carried out six months after each training session. In addition, the results of the pre- and post-tests and of the daily and final evaluations that accompanied the various training sessions were used to provide further information. The qualitative data collected were analyzed using INVIVO 15 software. Results: Some 89 health professionals (69% men and 31% women) working at the national level (51% at the central level, including 58% in health programs) and in development partner agencies (37%) participated in this capacity building program. Most participants were senior health managers (56%), data managers (23%), and statisticians and computer scientists (10%). Almost all the trainings were financed by 16 technical and financial partners (85%), mainly the MEASURE Evaluation project (27%). Conclusion: M&E training, despite all its imperfections, has made it possible to identify M&E training needs at the national level and to increase the critical mass of national skills and to have some culture in M&E.展开更多

These disturbances increase human pressure on biodiversity and species habitats, highlighting the need for conservation. This study aimed to assess the abundance and distribution of large mammals in different habitat types within Nimule National Park (NNP) and understand the impacts of human activities on them. Data on the abundance and distribution of large mammals and their respective habitat types were collected through line transect surveys. Human activity signs were observed and recorded along the transect lines. To estimate the impacts of human activities on the diversity, abundance, and distribution of large mammal species, as well as to identify any significant differences between them and their habitat types, the study utilized the Kruskal Wallis test, Polynomial multiple regressions, and diversity indices. The findings from the Shannon-Weiner and Simpson indices indicated that large mammal species were more diverse inside the park (H’ = 1.136;D = 0.570) compared to the buffer zone (H’ = 0.413;D = 0.171), with 85% (443 out of 510 samples) recorded within Nimule National Park. The species abundance showed a semi-balanced status (0.58). The diversity results among different habitat types revealed that large mammals were more diverse and highly distributed in both open woodlands (244) and dense woodlands (192), while riverine vegetation had the lowest diversity (8). Statistical tests demonstrated a highly significant difference at a 99% confidence interval (p-value = 0.01) between habitat types and identified species of large mammals. Additionally, the results highlighted the high abundance of Uganda kob (274), baboons (141), and warthog (57) across most habitat types, accounting for at least 75% of their distribution. The most prevalent human activities observed were cattle footprints (27%) and cattle dung (14%). Human footprints and tree cutting combined accounted for 9%, indicating the practice of livestock grazing, poaching, encroachment, and fuelwood collection by local communities. However, these activities did not appear to significantly impact the diversity, abundance, and distribution of large mammals in Nimule National Park. Therefore, it is crucial to foster shared responsibilities and engage relevant stakeholders in the management and conservation of large wildlife species. Regular community awareness programs should be implemented to cultivate a sense of ownership. Moreover, it is recommended that a comprehensive survey be conducted on the population status of all mammal species in Nimule National Park, including its surrounding Buffer Zone. Monitoring the impact of human activities on their behavior and habitats using satellite images should also be carried out at least every five to ten years.展开更多

As a result, the decline in motor and cognitive functions became more pronounced. Purpose: The study examined how activities combining passive horticultural therapy, which is always introduced, with active functional recovery training, which is used in the functional recovery training of patients with hemiplegia, influence the behavior of elder people using the facility. Methods: 36 elder people with care need 1 or 2 who regularly used the day-care facilities after the pandemic ended and who agreed to participate in the study was included. The assessment data were every three months from December 2021 to December 2023 after the COVID-19 pandemic, the MMSE and Moca-J, TMIG-Index, and GDS15. AI-based text mining was used to compare changes in visit reports before and after the introduction of the Posture Chart and Red Code, and the relationship between the language used. Results: The results of MMSE and Moca-J were both significantly higher in Group B and C than in Group A. TIMG assessment, was significantly higher in Group B and C than in Group A, and Group C was significantly higher than Group B. The GDS, an assessment of depression in old age, tended to be significantly higher in Group A than in Groups B and C. In the “Co-Occurrence Network Analysis” shown after the introduction of the latest functional recovery training equipment such as the Red Cord, its results showed a high association between the words Bathing and CARE and words such as POSTURE, Red-Code, and AI, App, which are used in functional training and rehabilitation, and their analysis. Conclusion: At day-care facilities specializing in functional recovery training and rehabilitation, a combination of horticultural therapy and the latest functional training equipment found to help maintain the physical and mental health of elderly people certified as requiring nursing care. Since this is a short-term study following behavioral restrictions, long-term research should continue to study the effects of adding functional training, handled by rehabilitation professionals, to horticultural therapy.展开更多

文摘Purpose:The aim of this umbrella review was to determine the impact of resistance training(RT)and individual RT prescription variables on muscle mass, strength, and physical function in healthy adults. Methods:Following Preferred Reporting Items for Systematic Reviews and Meta-Analyses(PRISMA)guidelines,we systematically searched and screened eligible systematic reviews reporting the effects of differing RT prescription variables on muscle mass(or its proxies),strength,and/or physical function in healthy adults aged>18 years. Results:We identified 44 systematic reviews that met our inclusion criteria. The methodological quality of these reviews was assessed using A Measurement Tool to Assess Systematic Reviews;standardized effectiveness statements were generated. We found that RT was consistently a potent stimulus for increasing skeletal muscle mass(4/4 reviews provide some or sufficient evidence),strength(4/6 reviews provided some or sufficient evidence),and physical function(1/1 review provided some evidence).RT load(6/8 reviews provided some or sufficient evidence),weekly frequency(2/4 reviews provided some or sufficient evidence),volume(3/7 reviews provided some or sufficient evidence),and exercise order(1/1 review provided some evidence)impacted RT-induced increases in muscular strength. We discovered that 2/3 reviews provided some or sufficient evidence that RT volume and contraction velocity influenced skeletal muscle mass, while 4/7 reviews provided insufficient evidence in favor of RT load impacting skeletal muscle mass. There was insufficient evidence to conclude that time of day,periodization,inter-set rest, set configuration, set end point, contraction velocity/time under tension,or exercise order(only pertaining to hypertrophy)influenced skeletal muscle adaptations.A paucity of data limited insights into the impact of RT prescription variables on physical function. Conclusion:Overall,RT increased muscle mass, strength, and physical function compared to no exercise.RT intensity(load)and weekly frequency impacted RT-induced increases in muscular strength but not muscle hypertrophy.RT volume(number of sets)influenced muscular strength and hypertrophy.
文摘Objectives:This study aimed to assess the feasibility of an online compassion training program for nursing students and preliminarily investigate its effects on mindfulness,self-compassion,and stress reduction. Methods:This study employed a randomized controlled trial design.Second-year students from a nursing college in Guangzhou,China, were recruited as research participants in August 2023.The intervention group participated in an 8-week online compassion training program via the WeChat platform, comprising three stages:mindfulness(weeks 1e2),self-compassion(weeks 3e5),and compassion for others(weeks 6 e8).Each stage included four activities:psychoeducation, mindfulness practice, weekly diary, and emotional support.Program feasibility was assessed through recruitment and retention rates, program engagement, and participant acceptability.Program effectiveness was measured with the Mindful Attention Awareness Scale,Self-Compassion Scale-Short Form, and Perceived Stress Scale. Results:A total of 28 students completed the study(13 in the intervention group,15 in the control group). The recruitment rate was 36.46%,with a high retention rate of 93.3%.Participants demonstrated high engagement:69.2%accessed learning materials every 1e2 days,93.3%practiced mindfulness at least weekly, with an average of 4.69 diary entries submitted per person and 23.30 WeChat interactions with instructors.Regarding acceptability, all participants expressed satisfaction with the program, with 92.4%finding it“very helpful”or“extremely helpful.”In terms of intervention effects, the intervention group showed a significant increase in mindfulness levels from pre-intervention(51.54±10.93)to postintervention(62.46±13.58)(P<0.05),while no significant change was observed in the control group.Although there were no statistically significant differences between the two groups in post-intervention self-compassion and perceived stress levels, the intervention group showed positive trends:selfcompassion levels increased(35.85±8.60 vs.40.85±5.54),and perceived stress levels slightly decreased(44.77±8.65 vs.42.00±5.77).Conclusions:This pilot study demonstrated the feasibility of an online compassion training program for nursing students and suggested its potential effectiveness in enhancing mindfulness,self-compassion,and stress reduction.Despite limitations such as small sample size and lack of long-term follow-up,preliminary evidence indicates promising prospects for integrating such training into nursing education.Further research is warranted to confirm thesefindings and assess the sustained impact of this approach on nursing education and practice.

文摘The reduction of energy consumption is an increasingly important topic of the railway system.Energy-efficient train control(EETC)is one solution, which refers to mathematically computing when to accelerate, which cruising speed to hold, how long one should coast over a suitable space, and when to brake. Most approaches in literature and industry greatly simplify a lot of nonlinear effects, such that they ignore mostly the losses due to energy conversion in traction components and auxiliaries. To fill this research gap,a series of increasingly detailed nonlinear losses is described and modelled. We categorize an increasing detail in this representation as four levels. We study the impact of those levels of detail on the energy optimal speed trajectory. To do this,a standard approach based on dynamic programming is used, given constraints on total travel time. This evaluation of multiple test cases highlights the influence of the dynamic losses and the power consumption of auxiliary components on railway trajectories, also compared to multiple benchmarks. The results show how the losses can make up 50%of the total energy consumption for an exemplary trip.Ignoring them would though result in consistent but limited errors in the optimal trajectory.Overall, more complex trajectories can result in less energy consumption when including the complexity of nonlinear losses than when a simpler model is considered. Those effects are stronger when the trajectory includes many acceleration and braking phases.
文摘Background: In the context of the fight against HIV, a lack of skills in monitoring and evaluating the personnel in charge of activities has been identified at the national level. Conclusion: M&E training, despite all its imperfections, has made it possible to identify M&E training needs at the national level and to increase the critical mass of national skills and to have some culture in M&E.

文摘Automation advancements prompts the extensive integration of collaborative robot(cobot)across a range of industries.Compared to the commonly used design approach of increasing the payload-to-weight ratio of cobot to enhance load capacity, equal attention should be paid to the dynamic response characteristics of cobot during the design process to make the cobot more flexible. In this paper,a new method for designing the drive train parameters of cobot is proposed.Firstly, based on the analysis of factors influencing the load capacity and dynamic response characteristics, design criteria for both aspects are established for cobot with all optimization design criteria normalized within the design domain.Secondly, with the cobot in the horizontal pose, the motor design scheme is discretized and it takes the joint motor diameter and gearbox speed ratio as optimization design variables. Finally, all the discrete values of the optimization objectives are obtained through the enumeration method and the Pareto front is used to select the optimal solution through multi-objective optimization.Base on the cobot design method proposed in this paper,a six-axis cobot is designed and compared with the commercial cobot. The result shows that the load capacity of the designed cobot in this paper reaches 8.4 kg,surpassing the 5 kg load capacity commercial cobot which is used as a benchmark. The minimum resonance frequency of the joints is 42.70 Hz.

文摘As the velocity of a train increases, the corresponding air pumping power consumption of the brake discs increases proportionally. In the present experimental study,a standard axle-mounted brake disc with circumferential pillars was analyzed using a 1:1 scale model and a test rig in a wind tunnel. In particular, three upstream velocities were selected on the basis of earlier investigations of trains operating at 160,250,and 400 km/h,respectively.Moreover,3D steady computational fluid dynamics(CFD)simulations of the flow field were conducted to compare with the wind tunnel test outcomes. The results for a 3-car train at 180 km/h demonstrated:(1)good agreement between the air resistance torques obtained from the wind tunnel tests and the related numerical results, with differences ranging from 0.95%to 5.88%;(2)discrepancies ranging from 3.2 to 3.8 N·m;(3)cooling ribs contributing more than 60%of the air resistance torque;(4)the fast rotation of brake discs causing a significantly different flow field near the bogie area, resulting in 25 times more air pumping power loss than that obtained in the stationary brake-disc case.

文摘Globally, human activities have a significant impact on the diversity, abundance, and distribution of large mammals in Protected Areas (PAs). Monitoring the impact of human activities on their behavior and habitats using satellite images should also be carried out at least every five to ten years.

文摘Purpose–The safety and reliability of high-speed trains rely on the structural integrity of their components and the dynamic performance of the entire vehicle system. This paper aims to define and substantiate the assessment of the structural integrity and dynamical integrity of high-speed trains in both theory and practice. The key principles and approacheswill be proposed, and their applications to high-speed trains in Chinawill be presented.Design/methodology/approach–First,the structural integrity and dynamical integrity of high-speed trains are defined, and their relationship is introduced. Then, the principles for assessing the structural integrity of structural and dynamical components are presented and practical examples of gearboxes and dampers are provided. Finally, the principles and approaches for assessing the dynamical integrity of highspeed trains are presented and a novel operational assessment method is further presented.Findings–Vehicle system dynamics is the core of the proposed framework that provides the loads and vibrations on train components and the dynamic performance of the entire vehicle system. For assessing the structural integrity of structural components,an open-loop analysis considering both normal and abnormal vehicle conditions is needed. For assessing the structural integrity of dynamical components,a closed-loop analysis involving the influence of wear and degradation on vehicle system dynamics is needed. The analysis of vehicle system dynamics should follow the principles of complete objects, conditions and indices.Numerical, experimental and operational approaches should be combined to achieve effective assessments.Originality/value–The practical applications demonstrate that assessing the structural integrity and dynamical integrity of high-speed trains can support better control of critical defects, better lifespan management of train components and better maintenance decision-making for high-speed trains.
文摘BACKGROUND Gallbladder cancer is the most common malignancy of the biliary tract.Neo-adjuvant chemotherapy(NACT)has improved overall survival by enabling R0 resection.Currently, there is no consensus of guidelines for neoadjuvant therapy in gallbladder cancer. As investigations continue to analyze the regimen and benefit of NACT for ongoing care of gallbladder cancer patients,we examined American College of Surgeons National Surgical Quality Improvement Program(NSQIP)database to determine if there was higher morbidity among the neo-adjuvant group within the 30-day post-operative period. We hypothesized patients who underwent NACT were more likely to have higher post-operative morbidity.AIM To investigate the 30-day post-operative morbidity outcomes between patients who received NACT and underwent surgery and patients who only had surgery.METHODS A retrospective analysis of the targeted hepatectomy NSQIP data between 2015 and 2019 was performed to determine if NACT in gallbladder cancer increased the risk for post-operative morbidity(bile leak, infection rate, rate of converting to open surgery, etc.)compared to the group who only had surgery. To calculate the odds ratio for the primary and secondary outcomes,a crude logistic regression was performed.RESULTS Of the 452 patients,52 patients received NACT prior to surgery. There were no statistically significant differences in the odds of morbidity between the two groups, including bile leak[odds ratio(OR),0.69;95%confidence interval(95%CI):0.16-2.10;P=0.55],superficial wound infection(OR,0.58;95%CI:0.03-3.02;P=0.61),and organ space wound infection(OR,0.63;95%CI:0.18-1.63;P=0.61).CONCLUSION There was no significant difference in the risk of 30-day post-operative morbidity between the NACT and surgery group and the surgery only group.

文摘Since the Beijing 2022 Winter Olympics was the first Winter Olympics in history held in continental winter monsoon climate conditions across complex terrain areas, there is a deficiency of relevant research, operational techniques, and experience. This made providing meteorological services for this event particularly challenging. The China Meteorological Administration(CMA)Earth System Modeling and Prediction Centre, achieved breakthroughs in research on short-and medium-term deterministic and ensemble numerical predictions.Several key technologies crucial for precise winter weather services during the Winter Olympics were developed.A comprehensive framework, known as the Operational System for High-Precision Weather Forecasting for the Winter Olympics, was established. Some of these advancements represent the highest level of capabilities currently available in China. The meteorological service provided to the Beijing 2022 Games also exceeded previous Winter Olympic Games in both variety and quality. This included achievements such as the“100-meter level, minute level”downscaled spatiotemporal resolution and forecasts spanning 1 to 15 days.Around 30 new technologies and over 60 kinds of products that align with the requirements of the Winter Olympics Organizing Committee were developed, and many of these techniques have since been integrated into the CMA’s operational national forecasting systems. These accomplishments were facilitated by a dedicated weather forecasting and research initiative,in conjunction with the preexisting real-time operational forecasting systems of the CMA.This program represents one of the five subprograms of the WMO’s high-impact weather forecasting demonstration project(SMART2022),and continues to play an important role in their Regional Association(RA)II Research Development Project(Hangzhou RDP).Therefore, the research accomplishments and meteorological service experiences from this program will be carried forward into forthcoming highimpact weather forecasting activities. This article provides an overview and assessment of this program and the operational national forecasting systems.
文摘Language is an important carrier of human culture and a specific method for communication between humans. It is one of the most important symbols of human identity and flags of dividing group identity. At present,in the modern national state, the government is paying more attention to language policy and being more and more prudent to it.In the process of making and implementing language policy, the government must take into account a range of factors such as the country’s national history, politics, culture, economy, and relations between different social groups. From the mid-nineteenth century, when New Zealand became a British colony, until the mid-twentieth century, the Maori people were inspired to defend their rights and respect for themselves by the rise of Maori nationalist ideology and Maori political radicalism in a struggle that lasted more than 100 years.Until 1994,there were more than 800 kohanga reo, early childhood institutions that established Maori language instruction independent of the school system, and the nationalism drives to revitalize the Maori national language was at its peak. The process of revolution on Maori language policies in New Zealand reflects a trend of nationalist thoughts from Maori becoming more deep.Exploring the process of change in New Zealand’s Maori language policy will help to develop the study of the social history of indigenous peoples during the colonial period and will contribute to the study of the development of colonized countries and regions in the post-colonial period. The change in New Zealand’s Maori language policy has provided an important example of the preservation and revitalization of the indigenous languages of the world’s former colonies, which is of great significance for the preservation of linguistic and cultural diversity.

文摘Background: In Japan, due to the COVID-19 pandemic, various behavioral restrictions were implemented from March 2020, resulting in an increased number of elderly people with reduced participation in care and rehabilitation. Since this is a short-term study following behavioral restrictions, long-term research should continue to study the effects of adding functional training, handled by rehabilitation professionals, to horticultural therapy.
