Application of sensory and motor training in AIDET communication mode in patients after knee arthroplasty

BACKGROUND Patients with knee arthroplasty often have problems such as slow recovery of knee function,which may cause negative emotions and affect their postoperative rehabilitation.The application of sensory and motor training in the Acknowledge,Introduce,Duration,Evaluation,Thank You(AIDET)communication mode in patients after knee arthroplasty can provide reference for the selection of postoperative rehabilitation training measures.AIM To explore the sensory and motor training effects in AIDET communication mode on knee function recovery and resilience of patients after knee arthroplasty.METHODS One hundred patients who underwent knee arthroplasty at our hospital between January 2022 and January 2024 were randomly divided into two groups.The control group(n=50)received routine rehabilitation training.In the observation group(n=50),the AIDET communication mode was used to perform sensory and motor training,in addition to routine rehabilitation training.The rehabilitation training was administered for 8 weeks.After surgery,knee function,balance ability,walking ability,proprioception,and resilience were compared between the two groups.RESULTS The New York Hospital for Special Surgery knee scores of the observation group at the time of discharge and 8 weeks after intervention were(65.23±6.84,84.53±5.27),which was higher than those of the control group(61.03±7.15,74.92±6.52)(P<0.05).The balance ability of the observation group at the time of discharge and 8 weeks after the intervention was higher than that of the control group,the time of time up to go test was shorter than that of the control group,and proprioceptive function was higher than that of the control group(P<0.05).The resilience level in the observation group after the intervention was higher than that in the control group(P<0.05).CONCLUSION Sensory and motor training in AIDET communication mode promotes knee function recovery of patients after knee arthroplasty,improves their limb walking ability and balance function,and increases their resilience level. 展开更多

In order to study the on-board diagnosis network's security and reliability, a simulation model for the on-board diagnosis network of high-speed Maglev trains with the optimal network engineering tool (OPNET) was built to analyze the network's performance, such as response error and bit error rate on the network load, throughput, and node-state response. The simulation model was verified with an actual on-board diagnosis network structure. The results show that the model results obtained are in good agreement with actual system performance and can be used to achieve actual communication network optimization and control algorithms. 展开更多

The user signal quality as well as the performance of transmission link experiences severe loss due to wireless channel fading and propagation loss in high-speed railway scenario.To improve the quality at the receiving end,spatial diversity was realized by means of cooperative communication technology based on the uncorrelated characteristics of the channels.The model of mobile communication system in high-speed railway was set up,and a cooperative scheme based on statistics was proposed.Mathematical analysis and simulation results show that the quality of the received signal and the performance of the transmission link are significantly improved using cooperative communication technology compared to that in non-cooperative communication mode. 展开更多

Extremely large-scale multiple-input multiple-output(XL-MIMO)and terahertz(THz)communications are pivotal candidate technologies for supporting the development of 6G mobile networks.However,these techniques invalidate the common assumptions of far-field plane waves and introduce many new properties.To accurately understand the performance of these new techniques,spherical wave modeling of near-field communications needs to be applied for future research.Hence,the investigation of near-field communication holds significant importance for the advancement of 6G,which brings many new and open research challenges in contrast to conventional far-field communication.In this paper,we first formulate a general model of the near-field channel and discuss the influence of spatial nonstationary properties on the near-field channel modeling.Subsequently,we discuss the challenges encountered in the near field in terms of beam training,localization,and transmission scheme design,respectively.Finally,we point out some promising research directions for near-field communications. 展开更多

Objective: This study aimed to determine variables associated (predictors and correlates) with the learning of assessment and supportive skills in the context of a communication skills training for medical residents. Methods: Learning was measured by comparing residents’ communication skills in a simulated consultation before and after a communication skills training. Communication skills were transcribed and tagged with a computer-assisted program. Potential variables associated with learning (residents’ characteristics, contextual characteristics and pre-training communication skills) were measured before the training and entered in regression analysis. Results: Fifty-six residents followed the training between 2002 and 2006. Poor pre-training assessment and supportive skills predicted the respective learning of these skills. Better assessment skills’ learning was predicted by copings (i.e. lower level of emotional coping), lower levels of self-efficacy and depersonalization. Better supportive skills’ learning was predicted by a lower work experience and associated with a higher training attendance rate. Conclusions: Predictors and correlates of assessment and supportive skills learning were different. Trainers needed to detect certain residents’ characteristics (i.e. depersonalization) in order to optimize assessment skills learning. Trainers needed to be aware that supportive skills are difficult to learn and to teach and may need more training hours. 展开更多

Aims and objectives: To explore any changes in nurses’ skills at communicating with residents with dementia disease when using the validation method, as observed in one-to-one videotaped conversations. Background: Communication difficulties due to cognitive impairment among residents with dementia disease may complicate care situations. Training can improve nurses’ communication skills and increase care quality. The validation method aims to facilitate communication with residents with dementia disease through empathic and confirmatory approaches. Evaluations of the validation method have primarily focused on the residents’ perspective, and reports on nurses’ experiences are sparse. Improved communication and relationships with residents after validation method training have been described previously. Videotaped data could provide additional information about these earlier results. Design: A descriptive qualitative design. Methods: Eight nurses participated in a year of validation method training, including videotaped conversations with eleven residents. Videotapes with at least five months between the first and last recording were analysed and compared qualitatively. Results: The analysis revealed an overall pattern: nurses’ movements within and between various paths when improving their communication skills. This was based on three sub-patterns: from controlling communication towards developing attentiveness in communication, from ambiguous communication towards developing coherence in communication, and from being open and attentive towards having a refined attuned communication. Conclusions: All nurses developed their communication skills during the programme, albeit to different degrees. The findings are in congruence with the experiences described by nurses, and so it is reasonable to believe that the programme helped to improve the nurses’ skills in communicating with residents with dementia disease. Relevance to clinical practice: A validation method training programme could give nurses the possibility to develop their skills in communicating with residents with dementia disease. 展开更多

The zone control subsystem is a real-time control system,which requests the correctness of the control process.Train tracing scene is an important function of the zone controller(ZC)in the communication based train control(CBTC)system.In the process of deep development and design,to ensure the safety of the system,the system needs to be modeled,simulated and verified to discover the system design flaws.Unified modeling language(UML)is combined with timed automata,and timed automata network models of train-filter and train tracing demarcation-point are established.At the same time,the verification tool of UPPAAL is applied to simulate the system,and verify the requirements of performance and function of system.The results show that the function of train tracing demaraction-point meets the requirements of system safety and limited activity.Therefore,the method is feasible and can be applied to the modeling and verification of other scenes of train control system. 展开更多

Communication can promote coordination in cooperative Multi-Agent Reinforcement Learning(MARL).Nowadays,existing works mainly focus on improving the communication efficiency of agents,neglecting that real-world communication is much more challenging as there may exist noise or potential attackers.Thus the robustness of the communication-based policies becomes an emergent and severe issue that needs more exploration.In this paper,we posit that the ego system trained with auxiliary adversaries may handle this limitation and propose an adaptable method of Multi-Agent Auxiliary Adversaries Generation for robust Communication,dubbed MA3C,to obtain a robust communication-based policy.In specific,we introduce a novel message-attacking approach that models the learning of the auxiliary attacker as a cooperative problem under a shared goal to minimize the coordination ability of the ego system,with which every information channel may suffer from distinct message attacks.Furthermore,as naive adversarial training may impede the generalization ability of the ego system,we design an attacker population generation approach based on evolutionary learning.Finally,the ego system is paired with an attacker population and then alternatively trained against the continuously evolving attackers to improve its robustness,meaning that both the ego system and the attackers are adaptable.Extensive experiments on multiple benchmarks indicate that our proposed MA3C provides comparable or better robustness and generalization ability than other baselines. 展开更多

Objective To determine the impact of scenario-based lecture and personalized video feedback on anesthesia residents'communication skills during preoperative visits.Methods A total of 24 anesthesia residents were randomly divided into a video group and a control group.Residents in both groups took part in a simulated interview and received a scenario-based lecture on how to communicate with patients during preoperative visits.Afterwards,residents in the video group received personalized video feedback recorded during the simulated interview.One week later all the residents undertook another simulated interview.The communication skills of all the residents were assessed using the Consultation and Relational Empathy measure(CARE)scale by two examiners and one standardized patient(SP),both of whom were blinded to the group allocation.Results CARE scores were comparable between the two groups before training,and significantly improved after training in both groups(all P<0.05).The video group showed significantly greater increase in CARE score after the training than the control group,especially assessed by the SP(t=6.980,P<0.001).There were significant correlations between the examiner-assessed scores and SP-assessed scores(both P=0.001).Conclusion Scenario-based lectures with simulated interviews provide a good method for training communication skills of anesthesia residents,and personalized video feedback can enhance their performance on showing empathy during preoperative interview. 展开更多

The aim of this paper is to analyze the results from the implemented projects and trainings which were aimed to minimize violence in health care area in CR (the Czech Republic) in years 2010-2013. The paper describes the results and experience from the communication trainings which were gained within the projects: CR lacks the support of government agencies so as employers and research on the issue, medical staff are not professional enough in communication, there are more requirements for the quality of lecturers; there is no existing general system of monitoring of violent incidents; there is no relevant education at schools. The article contains also new general ways about how to resolve mentioned problems. These results include the special mini project where educated 550 staff members in the health care area in a short time period and taught them key skills for how to approach the aggressive patients-50.6% respondents used successfully the gained skills in their practice (negotiation with patients, aggressive related); 42.7% respondents also used successfully the gained skills in their private life. It was found out that with the used skills the courage by conflict resolution was created. 展开更多
