Planning a Patient-Centered Parkinson’s Disease Support Program: Insights from Narrative Medicine

Background: We desired relevant information from patients with Parkinson’s disease (PD) and their caregivers for help in planning a long-term support program. Methods: We compared 3 ap-proaches: 1) a standard needs assessment questionnaire, the Camberwell assessment of need in the elderly, 2) an open-ended listing of needs, and 3) participation in a narrative medicine program in which health care professionals were involved. Results: On the questionnaire, patients reported physical needs as the most important while their caregivers were more concerned about psychological needs. The open-ended listings revealed additional needs, particularly access to re-liable information about PD and to opportunities to participate in clinical research. The narrative medicine process yielded a number of new insights and proposed solutions, particularly related to the heterogeneity of needs across individuals, the importance of providing hope, and the fact that caregivers expressed that their needs had been largely neglected. Participation in the narrative medicine sessions led to an increase in measured empathy by health care professionals, an im-provement in measured depression among PD patients, but a worsening of measured depression among the caregivers. Conclusions: We have identified important needs and potential solutions to be considered in providing long-term support to patients with PD and their caregivers.

Traveling Wavefronts on Reaction Diffusion Systems with Spatio-Temporal Delays

In our results, we reduce the existence of traveling wave fronts to the existence of an admissible pair of upper solution and lower solution which are much easier to construct in practice.

Observations and modelling of the travel time delay and leading negative phase of the 16 September 2015 Illapel,Chile tsunami

The systematic discrepancies in both tsunami arrival time and leading negative phase(LNP)were identified for the recent transoceanic tsunami on 16 September 2015 in Illapel,Chile by examining the wave characteristics from the tsunami records at 21 Deep-ocean Assessment and Reporting of Tsunami(DART)sites and 29 coastal tide gauge stations.The results revealed systematic travel time delay of as much as 22 min(approximately 1.7%of the total travel time)relative to the simulated long waves from the 2015 Chilean tsunami.The delay discrepancy was found to increase with travel time.It was difficult to identify the LNP from the near-shore observation system due to the strong background noise,but the initial negative phase feature became more obvious as the tsunami propagated away from the source area in the deep ocean.We determined that the LNP for the Chilean tsunami had an average duration of 33 min,which was close to the dominant period of the tsunami source.Most of the amplitude ratios to the first elevation phase were approximately 40%,with the largest equivalent to the first positive phase amplitude.We performed numerical analyses by applying the corrected long wave model,which accounted for the effects of seawater density stratification due to compressibility,self-attraction and loading(SAL)of the earth,and wave dispersion compared with observed tsunami waveforms.We attempted to accurately calculate the arrival time and LNP,and to understand how much of a role the physical mechanism played in the discrepancies for the moderate transoceanic tsunami event.The mainly focus of the study is to quantitatively evaluate the contribution of each secondary physical effect to the systematic discrepancies using the corrected shallow water model.Taking all of these effects into consideration,our results demonstrated good agreement between the observed and simulated waveforms.We can conclude that the corrected shallow water model can reduce the tsunami propagation speed and reproduce the LNP,which is observed for tsunamis that have propagated over long distances frequently.The travel time delay between the observed and corrected simulated waveforms is reduced to<8 min and the amplitude discrepancy between them was also markedly diminished.The incorporated effects amounted to approximately 78%of the travel time delay correction,with seawater density stratification,SAL,and Boussinesq dispersion contributing approximately 39%,21%,and 18%,respectively.The simulated results showed that the elastic loading and Boussinesq dispersion not only affected travel time but also changed the simulated waveforms for this event.In contrast,the seawater stratification only reduced the tsunami speed,whereas the earth’s elasticity loading was responsible for LNP due to the depression of the seafloor surrounding additional tsunami loading at far-field stations.This study revealed that the traditional shallow water model has inherent defects in estimating tsunami arrival,and the leading negative phase of a tsunami is a typical recognizable feature of a moderately strong transoceanic tsunami.These results also support previous theory and can help to explain the observed discrepancies.

The Ethics of Touch in Women Travellers＇ First Encounters With Foreign Ethnicities

This paper compares 18th and 19th century travelogues written by women and men travelling the cultural contact zones of the empire, as well as fictional recreations of such first encounters. A juxtaposition of the writers＇ reaction to the dynamics of gazing and the ethics of touch yields surprising results. Many women travellers have no problem to acknowledge the reciprocity of the gaze, accepting, as a matter of course, that the objects of their ethnological interest will gaze at them in return. In comparison, male travellers often exhibit unease at becoming an object of appraisal and observation. Even more interestingly, male travellers often shy away from haptic contact with members of the indigenous population, whereas many （though not all） women are more tolerant of touch and proximity. Regarded as ＂unwomanly＂ by their contemporaries, they carved out for themselves roles which allowed for a more intimate interaction with foreign ethnicities; also, they wrote in different genres--private memoirs instead of official reports. But even in their （semi） fictional writings male authors seem to imagine inter-cultural encounters in different terms from women and tend not let their protagonists enter into close bodily contact with the indigenous population.

Travelling Waves: Interplay of Low to High Reynolds Number and Tan-Cot Function Method to Solve Burger’s Equations

We study the nonlinear parabolic equations for travelling wave solutions of Burger’s equations. The purpose of the present work is to study various types of Burger’s equations describing waves and those are based on nonlinear equations. We focus on to describe the analytic solution in the special pattern of travelling wave solutions using tan-cot function method. We discuss about inviscid and viscous version of Burger’s equation for fluid flow and investigate the effects of internal friction of a fluid via Reynolds number. By changing the velocity amplitude, the nature of flows with shock wave and disturbance are observed. For numerical solutions, the Crank-Nicolson scheme is introduced to establish the wave solutions.

Narration as Therapy in Natsume Sōseki's I Am a Cat

Natsume Sōseki(1867-1916) suffered from cyclic mental depressions through his life, and it was when he was in the state of the most terrible one that he started his career as a writer by writing I Am a Cat(1905-1906). There are some evidence by his wife and friends that Sōseki's condition of disease dramatically improved during its serialization on a magazine, and the writer himself admited humorously in the preface of other work that it was his depression and mania that enabled him to produce I Am a Cat and other early works. This paper aims at describing the process how his act of narrating had therapeutic effects on his state of mind focusing on the construction of complicitous relationship between an implied author and readers in the work by the unique functions of the first-‘person' narrator: A Cat. Among others, I will consider the influence of Laurence Sterne's Tristram Shandy(1759-1767), which Sōseki introduced to Japan for the first time, by discussing the frequent use of metalepsis that the two works have in common.

基于线性密集台阵的2023年甘肃积石山M_(S)6.2地震断层低速带研究

2023年12月18日,甘肃省积石山县发生M_(S)6.2逆冲型地震。该地震发生在祁连活动地块拉脊山北缘断裂带,在断裂两侧曾发生过20余次5级左右中强地震,地震活动频次高,因此研究拉脊山断裂带特征具有重要意义。根据拉脊山断裂走向和此次地震余震分布,在刘集乡布设一条线性密集台阵,利用不同方位的余震事件计算体波的走时延时和放大效应。结果表明,P波和S波分别存在6个采样点(约0.012 s)和15个采样点(约0.03 s)的走时延时,推断出台阵西侧存在一个约150 m宽近似垂直的断层低速带,且该结果与远震P波相似性系数矩阵结果一致。

Exact Traveling Wave Solutions of Nano-Ionic Solitons and Nano-Ionic Current of MTs Using the exp(-φ (ξ ))-Expansion Method

In this work, the exp(-φ (ξ )) -expansion method is used for the first time to investigate the exact traveling wave solutions involving parameters of nonlinear evolution equations. When these parameters are taken to be special values, the solitary wave solutions are derived from the exact traveling wave solutions. The validity and reliability of the method are tested by its applications to Nano-ionic solitons wave’s propagation along microtubules in living cells and Nano-ionic currents of MTs which play an important role in biology.

2-D crustal Poisson′s ratio from seismic travel time inversion in Changbaishan Tianchi volcanic region

Based on the inversion method of 2D velocity structure and interface, the crustal velocity structures of P-wave and S-wave along the profile L1 are determined simultaneously with deep seismic sounding data in Changbaishan Tianchi volcanic region, and then its Poisson's ratio is obtained. Calculated results show that this technique overcomes some defects of traditional forward calculation method, and it is also very effective to determine Poisson's ratio distribution of deep seismic sounding profile, especially useful for study on volcanic magma and crustal fault zone. Study result indicates that there is an abnormally high Poisson's ratio body that is about 30 km wide and 12 km high in the low velocity region under Tianchi crater. Its value of Poisson's ratio is 8% higher than that of surrounding medium and it should be the magma chamber formed from melted rock with high temperature. There is a high Poisson's ratio zone ranging from magma chamber to the top of crust, which may be the uprise passage of hot substance. The lower part with high Poisson's ratio, which stretches downward to Moho, is possibly the extrusion way of hot substance from the uppermost mantle. The conclusions above are consistent with the study results of both tomographic determination of 3D crustal structure and magnetotelluric survey in this region.
