Gangrene of the External Genitalia at Chu-Abeche

Introduction: Gangrene of the external genitalia organs (EGO) is a medico-surgical emergency that is still very common in Third World countries. The main etiologies are urogenital, dermocutaneous and proctological. Mortality remains very high despite therapeutic advances. Our aim was to report on the epidemiological, diagnostic and therapeutic aspects of gangrene of the external genitalia, and to identify prognostic factors. Patients and Method: This was a 5-year retrospective descriptive study, from February 2016 to February 2021, of cases of gangrene of the external genitalia admitted to and treated in the Urology Department of Abeche University Hospital. Results: We collected 49 cases of gangrene of the external genitalia. The mean age of patients was 42 ± 16.81, with extremes of 20 and 81 years. The age group most concerned was between 20 and 29. The most frequent reasons for consultation were suppuration of the external genitalia and scrotal swelling. The average consultation time was 19.05 ± 16.02 days. The most common comorbidity was diabetes (35.9%). The most frequent pathological antecedents were urinary tract infections and endourethral maneuvers, reported in 40.5% and 38.7% respectively. Urogenital aetiology was predominant in 54.7%, and idiopathic in 35.9%. Lesions involved the scrotum in 60.6%, the scrotum and penis in 16.5%, and the penis alone in 5.7% of cases. Lesions extended to the perineum in 13.6% of cases, and to the abdomen in 3.6%. Vascular filling via the central venous line was performed in 58.6% of cases, and via the peripheral venous line in 41.4%. 3rd-generation cephalosporins and associated imidazoles were the most commonly used antibiotics. Necrosectomy was performed in 37 patients (73.4%), debridement combined with bypass cystostomy in 26.6% of cases. Colostomy was performed in 4% of cases. 90.8% of patients were cured and 9.2% died. The average hospital stay was 30 ± 75 days. Conclusion: The gangrene of the external genitalia is a medical and surgical emergency which has become rare in Europe, but which is still very common in the context of our practice in Chad. The severity of the disease is linked to delayed consultation and co-morbidity factors. Mortality remains very high despite therapeutic advances.

Clinical study on the treatment of acute pancreatitis with external application of Yi-Dan Therapy based on the method of“stagnation requiring dispersion”:study protocol for a randomized controlled trial

Background:Acute pancreatitis is an unpredictable and potentially lethal disease,causing tremendous pain in patients.The initial treatment of acute pancreatitis in modern medicine is supportive,but it is generally ineffective in relieving abdominal pain and distension.Traditional Chinese medicine has been shown to be more effective in regulating the body’s homeostasis and reducing the clinical symptoms of pancreatitis.Yi-Dan ointment,derived from Dahuang-Mudan Decoction,is an effective external ointment for treating acute pancreatitis.The aim of this trial is to investigate the clinical efficacy of Yi-Dan ointment,providing a valuable complement to existing treatment options for pancreatitis.Methods:This is a randomized controlled clinical trial with two parallel groups.Patients in the control group were given basic treatment and nursing for 7 days;in the treatment group,Yi-Dan ointment was applied externally in addition to basic treatment and nursing.The main indicator is the overall efficacy,serum amylase,acute physiology and chronic health evaluationⅡscore,symptom score,inflammatory markers,and classification of computed tomography.Conclusion:The trial results will provide high-quality evidence for Yi-Dan ointment,and provide a complement to existing treatment options for pancreatitis.

Current Research Status of Traditional Chinese Medicine External Treatment for Diarrhea Type Irritable Bowel Syndrome

Irritable bowel syndrome(IBS-D)with diarrhea is a common gastrointestinal functional disease in clinical practice,which seriously affects the quality of life of patients.Cur‐rently,Western medicine has poor therapeutic effects,while traditional Chinese medi‐cine has unique advantages in relieving IBS-D symptoms and preventing recurrence.In recent years,especially with external treatment of traditional Chinese medicine,it has become a new treatment direction in clinical practice and has achieved good therapeutic effects.This article will provide a review of recent research on the treatment of IBS-D using traditional Chinese medicine external treatment methods.

Practical Application and Development Trend of External Treatment of Zhuang Medicine Based on CiteSpace

The external treatment of Zhuang medicine is a treatment method commonly used from outside the human body.The advantages of external treatment methods are easy operation and small side effects.People are more familiar with medicated thread moxibustion,meridian tendon massage,and acupuncture therapy of Zhuang medicine.Although the curative effect of external treatment is significant,it is little known due to insufficient publicity and other factors.The purpose of this project was to comprehensively analyze and organize the latest relevant scientific and technological literature graphs in the study on clinical external treatment of Zhuang medicine using CiteSpace software and adopting the research method of visual literature graph search.Current research status and research development process of the external treatment of Zhuang medicine in Chinese literature were summarized,which has been able to present the current trends and future research prospects of the development and research of external treatment of Zhuang medicine in China in detail and comprehensively.Finally,307 papers were selected and included.Wei Yingcai and Pang Yuzhou were the authors with the largest number of documents and had formed a stable core research team.The analysis showed that Guangxi University of Chinese Medicine published the largest number of research documents,and there was less cooperation between institutions.At present,the research on the external treatment methods of Zhuang medicine is still in the development stage,which provides ideas and reference for quickly mastering the research hotspot of the external treatment method.

Clinical efficacy of external treatment with Zi Cao Yang He Tang combined with hormone therapy for granulomatous mastitis and its effect on cellular pyroptosis proteins

Objective:To observe the clinical effect of external application of Zi Cao Yang He Tang combined with dexamethasone ointment in the treatment of granulomatous lobular mastitis in the mass stage and the effect on cellular pyroptosis-related proteins.Methods:Forty patients with granulomatous lobular mastitis were randomly divided into 20 cases in the observation group and 20 cases in the control group.Both groups were given dexamethasone ointment for treatment,and the observation group was treated with external application of Zi Cao Yang He Tang on the basis of the control group.Breast lump vol-ume changes,breast pain and pus symptoms,pathology and histology and immunohistochemistry,cellular death-related indicators expression were compared between the two groups.Results:After treatment,the difference in mass volume of the observation group was significantly greater than that of the control group(16.7±21.5 in the control group and 32.9±27.4 in the experimental group)and the difference between the two groups was statistically significant(P<0.05);the objective remission rate of the patients in the observation group(ORR=85%)was significantly higher than that of the treatment group(ORR=50%,P<0.05).In terms of pus formation rate and pain level,the breast pain score of the observation group was significantly lower than that of the control group after two weeks or three weeks of treatment(P<0.05);the pus formation rate of the observation group was 30%,and the pus formation rate of the control group was 20%,and the difference was not statistically significant(P>0.05).The immunohistochemical expression levels of caspase-1 and GSDMD in the observation group were significantly lower than those in the control group(P<0.05);and in terms of the expression of cellular pyroptosis-related indexes,the expression of GSDMD,cas-pase-1,and NLRP3 in the observation group was significantly lower compared with that in the control group(P<0.05).Conclusion:The efficacy of external application of Zi Cao Yang He Tang combined with dexamethasone ointment in treating granulomatous lobular mastitis in the mass stage is remarkable,which can effectively reduce breast pain,reduce the size of the mass,de-crease the expression of cellular pyroptosisrelated indexes,and have a good anti-inflammatory effect.

Traditional Chinese medicine(TCM)believes that the main pathogenesis of ulcerative colitis is spleen deficiency;damp heat in the large intestine as the symptom,the treatment must be warming,clearing,eliminating,tonifying,astringing concurrently.From the concept of combining holism and the local treatment in TCM,idea of combining internal and external treatment is adopted.That is,the external treatment follows the therapeutic principle of using herbs with a bitter and cold nature to eliminate stagnation,and the seven carbon decoction,which can clear heat and dampness,cool the blood,and stop dysentery,is used as a rectal drip to retain the enema.Internal treatment follows the therapeutic principle of warmly tonifying and cultivating vital qi,with oral Chinese herbal medicine named Maodie Tang to strengthen the spleen and tonify the kidney,clearing heat and inducing dampness.

A Study of the Treatment of Disfiguring Diseases in the “Qian Jin Fang”

Traditional Chinese medicine(TCM)has been passed down for more than 2,000 years.TCM beauty products driven by the principles of identification and treatment with various dosage forms and exhibit mild and safe efficacy are bound to become a main component of the beauty industry in the future.Xu once commented that“Qian Jin Fang”is different from the traditional prescription system,which emphasizes the use of medicines.In this paper,we mainly selected the prescriptions for the treatment of disfigurement diseases such as acne,scarring,black dryness,face hyperpigmentation,black mole,and rosacea from the article titled,“Upper Seven Orifices Disease-Facial Medicine IX”in the Bei Ji Qian Jin Yao Fang[1].By studying and analyzing the original composition of the formula,understanding the etiology and pathogenesis of the disease,and exploring the characteristics of the formula concerning the efficacy of TCM,their properties,flavors,and meridians,we summarized the characteristics of the Qian Jin Fang.It is based on the external treatment of disfigurement diseases,with a wide variety of drugs,flexibility,and accuracy.This method draws upon analogies while utilizing excipients and harmonizers to focus on the combined healing approach of treatment and nourishment.

Preventive and therapeutic effect of long snake moxibustion on psoriasis with yang deficiency and external cold syndrome based on the theory of "treatment of winter disease in summer"

Psoriasis is a chronic inflammatory skin disease with multiple layers of silvery white scales on the surface of erythema papule as the main clinical characteristics,of which yang-deficiency external cold psoriasis is refractory,recurrent and severe in winter and mild in summer,which has a great impact on patients'physical and mental health and quality of life.Traditional Chinese medicine has unique insights in the treatment of psoriasis,both curative effect and safety,have obvious advantages.Based on the concept of"winter disease and summer treatment",long-snake moxibustion gives full play to its functions of warming the kidney,invigorating the marrow,warming the meridians,activating blood circulation and removing blood stasis,so as to restore the deficiency of Yang in patients with psoriasis with external cold syndrome,enhance disease resistance,and prevent psoriasis recurrence or aggravation in winter.Therefore,based on the theory of"winter disease and summer treatment",this paper preliminarily discusses the mechanism of long-snake moxibustion in the prevention and treatment of psoriasis with external cold syndrome of Yang deficiency,and provides new ideas and directions for the prevention and treatment of psoriasis with traditional Chinese medicine.

The Characteristics of the Drug Composition in the Treatment of Insomnia by External Bath Prescription of Traditional Chinese Medicine

[Objectives]The research aimed to carry out a retrospective analysis on the characteristics of the drug composition in the treatment of insomnia by external bath prescription of traditional Chinese medicine,to provide ideas for the research and development of new Chinese medicine for insomnia.[Methods]The external bath prescriptions of traditional Chinese medicine related to insomnia were collected,and the appearance frequency of drug was analyzed,and statistical analysis of drug with high appearance frequency was conducted.[Results]In the external bath prescriptions of traditional Chinese medicine related to insomnia,the top ten herbs with high appearance frequency were as follows:Chaozaoren,Drgonsbones,Margaritifera,Yejiaoteng,Schisandrae Chinensis Fructus,Lilii Bulbus,Platycladi Semen,Poria,Coptidis Rhizoma,and Scutellariae Radix.According to the classification by efficacy,these herbs mostly have the effects of calming,nourishing and heat clearing.[Conclusions]Traditional Chinese medicine is most closely related to heart,kidney,liver and other viscera.Tranquilizing and sedative drugs should be chosen,which corresponds with the understanding on pathogenesis of insomnia in traditional Chinese medical science,and embodies the characteristics of external bath prescription of traditional Chinese medicine in the treatment of insomnia.

Spontaneous External Digestive Fistula of Tuberculosis Origin, Medical Treatment: About a Case at the Timbuktu Hospital

In the literature, the management of enterocutaneous fistula of tuberculosis origin is rare. The aim of this work was to report a case of external digestive fistula of tuberculosis origin in a 10-year-old girl in northern Mali in a situation of armed conflict. She was a 10-year-old patient who was admitted to our general surgery ward for external digestive fistula evolving for more than (4) four years, at admission the general. The karnofsky index was at 40%, she could not sit or hold a cup to drink water. A 3 cm enterocutaneous fistula deafened foul-smelling liquid stools, on clinical examination, the conjunctivas were pale, the lips were dry, the eyes were sunk in the eye sockets, the ribs visible from a distance. The paraclinical aspects (Itra-Dermo-Reaction (IDR) to tubercria, anatomopathology) were positive, specifying the place of medical treatment and evolution under medical treatment. Digestive fistula of tuberculosis origin is a rare condition in surgical settings.

Network meta-analysis of the effectiveness of TCM external treatment in the treatment of mycoplasmal pneumonia

Objective: To compare the efficacy and safety of different TCM external treatment combined with azithromycin in the treatment of Mycoplasma pneumoniae pneumonia. Methods: The keywords and free words were combined to search the literatures of clinical randomized or quasi-randomized controlled trials on the effects of TCM external treatment combined with azithromycin in the treatment of children with MPP in CNKI, VIP, CBM, Wan Fang Date, PubMed, Sciencedirect and Google Academic Database. The search time limit is from August 2019. After two independent reviewers selected the literature, extracted data and literature quality evaluation according to the inclusion and exclusion criteria, the ADDI software, RevMan5.3 and stata14.0 software were used for mesh meta-analysis. Results: A total of 18 randomized controlled literatures were included, involving 1536 children with MPP, 6 Chinese medicine external treatment methods (ultra-short wave, acupoint application, cupping, enema, Chinese medicine patch, massage). The results of mesh meta-analysis showed that treatment In terms of MPP efficiency, ultrashort wave, acupoint application, cupping, enema, Chinese medicine patch, massage combined with azithromycin treatment is more effective than azithromycin alone, and the probability distribution shows that the Chinese medicine patch combined with azithromycin treatment is the best solution (P= 0.34). In the reduction of adverse reactions, because the ultrashort wave and traditional Chinese medicine patch literature did not mention the non-performing rate, this study only analyzed the other four external treatment methods, acupoint application, cupping, enema, massage combined with azithromycin is better than single With azithromycin, the probability distribution showed that the probability of massage combined with azithromycin was the best (P=0.67). Conclusion: In terms of efficiency and reduction of adverse reactions, each Chinese medicine external treatment combined with azithromycin has an advantage over single azithromycin. Probability distribution shows that in the treatment of MPP efficiency, Chinese medicine patch + Achi > ultrashort wave + AZM > enema + AZM > cupping + AZM > massage + Aqi, Chinese medicine patch + azithromycin program is the optimal program;In terms of reducing adverse reactions, massage + AZM > enema + AZM > acupressure application + AZM > cupping +AZM.

Exploration on medication patterns of “External Treatment” in the ancient Chinese medical book Liu Juanzi Guiyi Fang

Background:To obtain the medication patterns of“External Treatment”by analyzing the prescriptions for the“External Treatment”in a Chinese ancient book called Liu Juanzi Guiyi Fang(Liu Juanzi’Ghost-Bequeathed Prescriptions,written by Liu Juanzi in 499 C.E.).Methods:The prescriptions for“External Treatment”and primary traditional Chinese medicines in Liu Juanzi’Ghost-Bequeathed Prescriptions were analyzed using data mining methods,such as frequency analysis and cluster analysis,after screening for prescriptions related to“External Treatment”in Liu Juanzi’Ghost-Bequeathed Prescriptions.The Database of Prescriptions for External Treatment in Liu Juanzi Guiyi Fang was then established.Results:The earliest Chinese characters for“Yin”and“Yang”were found on“oracle bones”.These bones were the skeletal remains of different animals,and were used in ancient Chinese divination practices going all the way back to the 14th century B.C.Simply put,“Yin”is the cold and“Yang”is the warm.In traditional Chinese medicine,this concept of dualism and its fundamental principle underlie physiology,pathology,and the treatment of illnesses.The“External Treatment”discussed in Liu Juanzi’Ghost-Bequeathed Prescriptions primarily adopts the methods of“Warming Yang”,“Cooling Heat(Yang)”,and“Qu Xie(dispelling pathogenic factors of epidemic diseases)”,as well as“Huo Xue(promoting blood circulation)and promoting tissue regeneration.In Chinese medicine,“Si Qi(cold as winter,cool as autumn,warm as spring,hot as summer)”,“Wu Wei(five flavors:sweet,pungent,salty,sour,and bitter)”used to describe the properties of herbs.In Liu Juanzi’Ghost-Bequeathed Prescriptions,most of the medicines used were more“Sweet”,“Pungent”,“Warm”,and the“Channel Tropism”were primarily distribute to the“Liver”,“Heart”,“Stomach”,and“Large intestine meridian”.Conclusion:The data mining method systematically summarized the medication characteristics of the prescriptions for“External Treatment”in Liu Juanzi’Ghost-Bequeathed Prescriptions.The characteristics of the“External Treatment”in this book include“supporting healthy Qi”and“promoting Qi and Blood circulation”which as the top priority,applying both“Attacking and Supplementing methods”as the focus,and combining“Supplementing methods”with“Elimination methods”.It has provided theoretical guidance in“External Treatment”for surgery of traditional Chinese medicine.

Application of external treatment of traditional Chinese medicine in the treatment of cancer-related fatigue

External treatment of traditional Chinese medicine(TCM),also known as external treatment of internal diseases,has many advantages,such as simple,easy to operate,cost-effective and so on.Therefore,external treatment of TCM is favored by the majority of medical workers.By consulting the domestic literature reports on the treatment of cancer-related fatigue(CRF),it is found that the external treatment of TCM has a significant effect on the intervention of CRF,and has a good effect on reducing fatigue,improving the quality of life and prolonging the survival time.The purpose of this paper is to summarize the application of several external treatments in this disease,and to provide a variety of references for clinical treatment of CRF.

Study on the Correlation between Syndrome Differentiation of Malignant Pleural Effusion Treated by External Treatment of Traditional Chinese Medicine and Immunohistochemistry of Biopsy Tissue Based on Medical Video-assisted Thoracoscope

Objective:Guided by the theory of syndrome differentiation of yin and yang in traditional Chinese medicine surgery,through visual observation of internal medicine thoracoscope,comprehensive observation of pleural cavity and immunohistochemistry of biopsy tissue,to classify malignant pleural effusion according to syndrome differentiation,and to explore the scientific nature of its theory.Methods:From March 1,2014 to February 28,2015,40 cases of malignant pleural effusion were treated in Beijing Chaoyang Hospital affiliated to Capital Medical University.According to the proposed TCM diagnostic criteria for yin and yang syndrome differentiation,and collect age,gender,course of disease,clinical symptoms,tumor primary focus,histomorphological manifestations and immunohistochemical results and other related information,and carry out statistical data processing.Results:The positive syndrome was mainly metastatic lung adenocarcinoma,which accounted for the majority of all MPE cases,up to 75%.The immunohistochemical results of biopsy tissues were mainly CEA and TTF-1 positive;While pleural effusion caused by pleural mesothelioma was the main type of yin syndrome,and the results of immunohistochemistry combined with biopsy were mainly positive for D2-40,Calretinin,WT-1 and CK5/6.Conclusion:TCM syndrome differentiation of MPE based on internal thoracoscopy combined with biopsy immunohistochemical results has sufficient theoretical basis and certain scientific nature,and further clinical research is needed to verify its effectiveness and practicability in the future.

Professor Renting Li's Combination of Internal and External Treatment of Traditional Chinese Medicine in Treating Radiation Enteritis

Radiotherapy is a common treatment for abdominal and pelvic malignancies with bone metastases.However,some patients develop radiation enteritis(RE)during the treatment or 2 months or more after the treatment,which seriously affects their quality of life.At present,the curative effect of western medicine is poor.Professor Renting Li believes that radiation kills tumor cells but at the same time,it damages the healthy Qi of the human body.Radiation causes heat and poison to accumulate in the intestinal tract,consumes and hurts Yin fluid,bums the vein,leads to deficiency of healthy Qi and poor detoxification,as well as cause blood stasis after a period of time.All these result in the combination of heat,blood stasis,and poison,manifesting as abdominal pain,diarrhea,mucus in stools,purulent and bloody stools,etc.Therefore,Professor Renting Li proposed the use of traditional Chinese medicine retention enema combined with oral traditional Chinese medicine to reduce symptoms,remove toxins,and improve the quality of life of patients.

"External main and internal auxiliary" treatment on refractory radiation proctitis

Radiotherapy is an essential method in the comprehensive treatment of malignant tumors.However,radiation proctitis(RP)is a common complication of pelvic tumors after radiotherapy.Due to RP's various etiology and complex pathogenesis,it is currently no standard for traditional Chinese medicine(TCM)treatment.Professor Yuan-hong Zhao believes that refractory RP,which is named chronic hemorrhagic radiation proctitis(CHRP),should be classified as"Intestinal Wind Bleeding"(Bleeding like a note before defecation,bright red blood,no swelling and pain in the anus)and"Intestinal Afflux"(Refers to the symptoms of pus and blood in the lower mucus,similar to ulcerative colitis,peptic ulcer and other diseases in modern medicine.)in TCM.The critical pathogenesis of CHRP lies in local stasis and collaterals injury of intestinal collaterals.In general,it is a syndrome of excess and scarcity.External treatment with TCM is the preferred treatment strategy for CHRP,and the primary way is to disperse blood stasis and detoxify and nourish the intestinal collaterals.
