Introduction: Gangrene of the external genitalia organs (EGO) is a medico-surgical emergency that is still very common in Third World countries. The main etiologies are urogenital, dermocutaneous and proctological. Mortality remains very high despite therapeutic advances. Our aim was to report on the epidemiological, diagnostic and therapeutic aspects of gangrene of the external genitalia, and to identify prognostic factors. Patients and Method: This was a 5-year retrospective descriptive study, from February 2016 to February 2021, of cases of gangrene of the external genitalia admitted to and treated in the Urology Department of Abeche University Hospital. Results: We collected 49 cases of gangrene of the external genitalia. The mean age of patients was 42 ± 16.81, with extremes of 20 and 81 years. The age group most concerned was between 20 and 29. The most frequent reasons for consultation were suppuration of the external genitalia and scrotal swelling. The average consultation time was 19.05 ± 16.02 days. The most common comorbidity was diabetes (35.9%). The most frequent pathological antecedents were urinary tract infections and endourethral maneuvers, reported in 40.5% and 38.7% respectively. Urogenital aetiology was predominant in 54.7%, and idiopathic in 35.9%. Lesions involved the scrotum in 60.6%, the scrotum and penis in 16.5%, and the penis alone in 5.7% of cases. Lesions extended to the perineum in 13.6% of cases, and to the abdomen in 3.6%. Vascular filling via the central venous line was performed in 58.6% of cases, and via the peripheral venous line in 41.4%. 3rd-generation cephalosporins and associated imidazoles were the most commonly used antibiotics. Necrosectomy was performed in 37 patients (73.4%), debridement combined with bypass cystostomy in 26.6% of cases. Colostomy was performed in 4% of cases. 90.8% of patients were cured and 9.2% died. The average hospital stay was 30 ± 75 days. Conclusion: The gangrene of the external genitalia is a medical and surgical emergency which has become rare in Europe, but which is still very common in the context of our practice in Chad. The severity of the disease is linked to delayed consultation and co-morbidity factors. Mortality remains very high despite therapeutic advances. 展开更多

Background:Acute pancreatitis is an unpredictable and potentially lethal disease,causing tremendous pain in patients.The initial treatment of acute pancreatitis in modern medicine is supportive,but it is generally ineffective in relieving abdominal pain and distension.Traditional Chinese medicine has been shown to be more effective in regulating the body’s homeostasis and reducing the clinical symptoms of pancreatitis.Yi-Dan ointment,derived from Dahuang-Mudan Decoction,is an effective external ointment for treating acute pancreatitis.The aim of this trial is to investigate the clinical efficacy of Yi-Dan ointment,providing a valuable complement to existing treatment options for pancreatitis.Methods:This is a randomized controlled clinical trial with two parallel groups.Patients in the control group were given basic treatment and nursing for 7 days;in the treatment group,Yi-Dan ointment was applied externally in addition to basic treatment and nursing.The main indicator is the overall efficacy,serum amylase,acute physiology and chronic health evaluationⅡscore,symptom score,inflammatory markers,and classification of computed tomography.Conclusion:The trial results will provide high-quality evidence for Yi-Dan ointment,and provide a complement to existing treatment options for pancreatitis. 展开更多

The external treatment of Zhuang medicine is a treatment method commonly used from outside the human body.The advantages of external treatment methods are easy operation and small side effects.People are more familiar with medicated thread moxibustion,meridian tendon massage,and acupuncture therapy of Zhuang medicine.Although the curative effect of external treatment is significant,it is little known due to insufficient publicity and other factors.The purpose of this project was to comprehensively analyze and organize the latest relevant scientific and technological literature graphs in the study on clinical external treatment of Zhuang medicine using CiteSpace software and adopting the research method of visual literature graph search.Current research status and research development process of the external treatment of Zhuang medicine in Chinese literature were summarized,which has been able to present the current trends and future research prospects of the development and research of external treatment of Zhuang medicine in China in detail and comprehensively.Finally,307 papers were selected and included.Wei Yingcai and Pang Yuzhou were the authors with the largest number of documents and had formed a stable core research team.The analysis showed that Guangxi University of Chinese Medicine published the largest number of research documents,and there was less cooperation between institutions.At present,the research on the external treatment methods of Zhuang medicine is still in the development stage,which provides ideas and reference for quickly mastering the research hotspot of the external treatment method. 展开更多

Objective:To observe the clinical effect of external application of Zi Cao Yang He Tang combined with dexamethasone ointment in the treatment of granulomatous lobular mastitis in the mass stage and the effect on cellular pyroptosis-related proteins.Methods:Forty patients with granulomatous lobular mastitis were randomly divided into 20 cases in the observation group and 20 cases in the control group.Both groups were given dexamethasone ointment for treatment,and the observation group was treated with external application of Zi Cao Yang He Tang on the basis of the control group.Breast lump vol-ume changes,breast pain and pus symptoms,pathology and histology and immunohistochemistry,cellular death-related indicators expression were compared between the two groups.Results:After treatment,the difference in mass volume of the observation group was significantly greater than that of the control group(16.7±21.5 in the control group and 32.9±27.4 in the experimental group)and the difference between the two groups was statistically significant(P<0.05);the objective remission rate of the patients in the observation group(ORR=85%)was significantly higher than that of the treatment group(ORR=50%,P<0.05).In terms of pus formation rate and pain level,the breast pain score of the observation group was significantly lower than that of the control group after two weeks or three weeks of treatment(P<0.05);the pus formation rate of the observation group was 30%,and the pus formation rate of the control group was 20%,and the difference was not statistically significant(P>0.05).The immunohistochemical expression levels of caspase-1 and GSDMD in the observation group were significantly lower than those in the control group(P<0.05);and in terms of the expression of cellular pyroptosis-related indexes,the expression of GSDMD,cas-pase-1,and NLRP3 in the observation group was significantly lower compared with that in the control group(P<0.05).Conclusion:The efficacy of external application of Zi Cao Yang He Tang combined with dexamethasone ointment in treating granulomatous lobular mastitis in the mass stage is remarkable,which can effectively reduce breast pain,reduce the size of the mass,de-crease the expression of cellular pyroptosisrelated indexes,and have a good anti-inflammatory effect. 展开更多

In the literature, the management of enterocutaneous fistula of tuberculosis origin is rare. The aim of this work was to report a case of external digestive fistula of tuberculosis origin in a 10-year-old girl in northern Mali in a situation of armed conflict. She was a 10-year-old patient who was admitted to our general surgery ward for external digestive fistula evolving for more than (4) four years, at admission the general. The karnofsky index was at 40%, she could not sit or hold a cup to drink water. A 3 cm enterocutaneous fistula deafened foul-smelling liquid stools, on clinical examination, the conjunctivas were pale, the lips were dry, the eyes were sunk in the eye sockets, the ribs visible from a distance. The paraclinical aspects (<em>Itra-Dermo-Reaction</em> (IDR) to <em>tubercria</em>, anatomopathology) were positive, specifying the place of medical treatment and evolution under medical treatment. Digestive fistula of tuberculosis origin is a rare condition in surgical settings. 展开更多

Network meta-analysis of the effectiveness of TCM external treatment in the treatment of mycoplasmal pneumonia

Objective: To compare the efficacy and safety of different TCM external treatment combined with azithromycin in the treatment of Mycoplasma pneumoniae pneumonia. Methods: The keywords and free words were combined to search the literatures of clinical randomized or quasi-randomized controlled trials on the effects of TCM external treatment combined with azithromycin in the treatment of children with MPP in CNKI, VIP, CBM, Wan Fang Date, PubMed, Sciencedirect and Google Academic Database. The search time limit is from August 2019. After two independent reviewers selected the literature, extracted data and literature quality evaluation according to the inclusion and exclusion criteria, the ADDI software, RevMan5.3 and stata14.0 software were used for mesh meta-analysis. Results: A total of 18 randomized controlled literatures were included, involving 1536 children with MPP, 6 Chinese medicine external treatment methods (ultra-short wave, acupoint application, cupping, enema, Chinese medicine patch, massage). The results of mesh meta-analysis showed that treatment In terms of MPP efficiency, ultrashort wave, acupoint application, cupping, enema, Chinese medicine patch, massage combined with azithromycin treatment is more effective than azithromycin alone, and the probability distribution shows that the Chinese medicine patch combined with azithromycin treatment is the best solution (P= 0.34). In the reduction of adverse reactions, because the ultrashort wave and traditional Chinese medicine patch literature did not mention the non-performing rate, this study only analyzed the other four external treatment methods, acupoint application, cupping, enema, massage combined with azithromycin is better than single With azithromycin, the probability distribution showed that the probability of massage combined with azithromycin was the best (P=0.67). Conclusion: In terms of efficiency and reduction of adverse reactions, each Chinese medicine external treatment combined with azithromycin has an advantage over single azithromycin. Probability distribution shows that in the treatment of MPP efficiency, Chinese medicine patch + Achi > ultrashort wave + AZM > enema + AZM > cupping + AZM > massage + Aqi, Chinese medicine patch + azithromycin program is the optimal program;In terms of reducing adverse reactions, massage + AZM > enema + AZM > acupressure application + AZM > cupping +AZM. 展开更多

Background:To obtain the medication patterns of“External Treatment”by analyzing the prescriptions for the“External Treatment”in a Chinese ancient book called Liu Juanzi Guiyi Fang(Liu Juanzi’Ghost-Bequeathed Prescriptions,written by Liu Juanzi in 499 C.E.).Methods:The prescriptions for“External Treatment”and primary traditional Chinese medicines in Liu Juanzi’Ghost-Bequeathed Prescriptions were analyzed using data mining methods,such as frequency analysis and cluster analysis,after screening for prescriptions related to“External Treatment”in Liu Juanzi’Ghost-Bequeathed Prescriptions.The Database of Prescriptions for External Treatment in Liu Juanzi Guiyi Fang was then established.Results:The earliest Chinese characters for“Yin”and“Yang”were found on“oracle bones”.These bones were the skeletal remains of different animals,and were used in ancient Chinese divination practices going all the way back to the 14th century B.C.Simply put,“Yin”is the cold and“Yang”is the warm.In traditional Chinese medicine,this concept of dualism and its fundamental principle underlie physiology,pathology,and the treatment of illnesses.The“External Treatment”discussed in Liu Juanzi’Ghost-Bequeathed Prescriptions primarily adopts the methods of“Warming Yang”,“Cooling Heat(Yang)”,and“Qu Xie(dispelling pathogenic factors of epidemic diseases)”,as well as“Huo Xue(promoting blood circulation)and promoting tissue regeneration.In Chinese medicine,“Si Qi(cold as winter,cool as autumn,warm as spring,hot as summer)”,“Wu Wei(five flavors:sweet,pungent,salty,sour,and bitter)”used to describe the properties of herbs.In Liu Juanzi’Ghost-Bequeathed Prescriptions,most of the medicines used were more“Sweet”,“Pungent”,“Warm”,and the“Channel Tropism”were primarily distribute to the“Liver”,“Heart”,“Stomach”,and“Large intestine meridian”.Conclusion:The data mining method systematically summarized the medication characteristics of the prescriptions for“External Treatment”in Liu Juanzi’Ghost-Bequeathed Prescriptions.The characteristics of the“External Treatment”in this book include“supporting healthy Qi”and“promoting Qi and Blood circulation”which as the top priority,applying both“Attacking and Supplementing methods”as the focus,and combining“Supplementing methods”with“Elimination methods”.It has provided theoretical guidance in“External Treatment”for surgery of traditional Chinese medicine. 展开更多

Objective:Guided by the theory of syndrome differentiation of yin and yang in traditional Chinese medicine surgery,through visual observation of internal medicine thoracoscope,comprehensive observation of pleural cavity and immunohistochemistry of biopsy tissue,to classify malignant pleural effusion according to syndrome differentiation,and to explore the scientific nature of its theory.Methods:From March 1,2014 to February 28,2015,40 cases of malignant pleural effusion were treated in Beijing Chaoyang Hospital affiliated to Capital Medical University.According to the proposed TCM diagnostic criteria for yin and yang syndrome differentiation,and collect age,gender,course of disease,clinical symptoms,tumor primary focus,histomorphological manifestations and immunohistochemical results and other related information,and carry out statistical data processing.Results:The positive syndrome was mainly metastatic lung adenocarcinoma,which accounted for the majority of all MPE cases,up to 75%.The immunohistochemical results of biopsy tissues were mainly CEA and TTF-1 positive;While pleural effusion caused by pleural mesothelioma was the main type of yin syndrome,and the results of immunohistochemistry combined with biopsy were mainly positive for D2-40,Calretinin,WT-1 and CK5/6.Conclusion:TCM syndrome differentiation of MPE based on internal thoracoscopy combined with biopsy immunohistochemical results has sufficient theoretical basis and certain scientific nature,and further clinical research is needed to verify its effectiveness and practicability in the future. 展开更多
