BACKGROUND Surgery is an effective method for treating certain diseases.Factors such as disease,preoperative fear and tension,surgical stress,postoperative pain,and related complications directly affect the smooth pro...BACKGROUND Surgery is an effective method for treating certain diseases.Factors such as disease,preoperative fear and tension,surgical stress,postoperative pain,and related complications directly affect the smooth progression and outcome of surgery.Patients may experience a series of psychological and physiological changes during the perioperative period,resulting in anxiety and depression,which may reduce the pain threshold and worsen their prognosis.METHODS We enrolled 200 patients who underwent surgical care at The First People’s Hospital of Lin’an District,Hangzhou between January and December 2023.They were categorized into a routine intervention group(n=103)and a psychological intervention group(n=97),based on the intervention strategies used.Various assessment tools,including the self-rating anxiety scale(SAS),the self-rating depression scale(SDS),and the Connor–Davidson Resilience scale,were used to measure patients’negative states and emotions.The pre-and post-intervention scores for these metrics in the two groups were then analyzed.RESULTS In the psychological intervention group,the SAS and SDS scores(31.56±5.18 and 31.46±4.57,respectively)were significantly reduced compared to the routine intervention group(P<0.05).The visual analog scale pain scores at 12 and 24 hours after intervention(6.85±1.21,4.24±0.72)were notably higher than those in the routine intervention group(P<0.05).The psychological intervention group also demonstrated superior scores in perseverance(36.08±3.29),self-reliance(22.63±2.91),optimism(11.42±1.98),and resilience(70.13±5.37),compared to the routine intervention group(P<0.05).Additionally,the psychological intervention group’s confrontation score(23.16±4.29)was higher(P<0.05).This group also reported lower scores in avoidance(9.28±1.94)and yielding(6.19±1.92)(P<0.05).Lastly,the Short Form 36 Health Survey scores were significantly higher in the psychological intervention group,indicating a better quality of life(P<0.05).CONCLUSION Psychological intervention measures based on SCT can effectively alleviate pain,anxiety,and depression in periop-erative patients.展开更多
BACKGROUND The severe physical and psychological impact of pain on the physical and mental health of women during labor leads to increased risks and complications during childbirth,presenting a major public health con...BACKGROUND The severe physical and psychological impact of pain on the physical and mental health of women during labor leads to increased risks and complications during childbirth,presenting a major public health concern.Some studies have shown that cognitive behavioral therapy(CBT)has a positive effect on maternal psychology during delivery,reducing stress and shortening labor time.Thus,CBT training for mothers and delivery room staff may be beneficial in minimizing complications and adverse effects during natural birth.AIM To investigate the clinical effects of CBT training and psychological care during delivery,and their therapeutic effects on women in labor.METHODS This study used a retrospective analysis and included 140 mothers admitted to the maternity ward between January 2021 and January 2023.The study subjects were randomized into two groups:control(n=70)and observation(n=70).Routine care,CBT training,and psychological care were provided to mothers in both groups.Psychological status scores,delivery time,and satisfaction with care pre-and post-delivery were compared,and the incidence of complications after receiving care was analyzed between the two groups.RESULTS Although the psychological state of both groups improved significantly in the late stages of labor,the psychological state scores of the mothers in the observation group were significantly lower than those of the mothers in the control group(P<0.05).The duration of labor and incidence of complications in the observation group were significantly lower than those in the control group(P<0.05).The mothers in the observation group were significantly more satisfied with nursing care during the course of labor than those in the control group(P<0.05).CONCLUSION CBT training and psychological care for mothers in the midwifery process can effectively improve anxiety and depression,shorten labor duration,reduce postnatal complications,and improve nursing satisfaction and nurse-patient relationships.Its clinical application is effective and has popularization value,providing a new way to protect maternal mental health.展开更多

A total of 156 patients with OSCC (mean age: 62 years, M/F: 2.39 : 1) were recruited at the Universities of Palermo and Naples. Risk factors related to patient delay included: sociodemographic, health-related, cognitive and psychological variables. The analysis was conducted by considering two different delay ranges: dichotomous (≤1 month vs. 〉 1 month) and polytomous (〈1 month, 1-3 months, 〉3 months) delay. Data were investigated by univariate and multivariate analyses and a Pvalue≤0.05 was considered statistically significant. For both delay measurements, the most relevant variables were: 'Personal experience of cancer' (dichotomous delay: P=0.05, odds ratio (0R)=0.33, 95% confidence interval (CI)=0. 11-0.99; polytomous delay: P=0.006, Chi-square= 10.224) and 'Unawareness' (dichotomous delay: P〈0.01, 0R=4.96, 95% CI--2.16-11.37; polytomous delay: P=0.087, Chi-square=4.77). Also 'Denial' (P〈0.01, 0R=6.84, 95% CI=2.31-20.24) and 'Knowledge of cancer' (P=0.079, Chi-square=8.359) were found to be statistically significant both for dichotomous and for polytomous categorization of delay, respectively. The findings of this study indicated that, in the investigated cohorts, the knowledge about cancer issues is strongly linked to the patient delay. Educational interventions on the Mediterranean population are necessary in order to increase the patient awareness and to emphasize his/her key role in early diagnosis of OSCC.展开更多

In the secondary network, two secondary users (SUs) communicate with each other via an assist relay. The main point is to provide the best system performance to SUs while maintaining the interference power at primary user (PU) under a certain level. Using convex optimization, a closed-form solution is obtained when optimizing the power allocation among the two nodes and relay. Based on this result, a joint power control and relay selection scheme with fewer variable dimensions is proposed to maximize the achievable rate of the secondary system. Simulation results demonstrate that the sum rate of the cognitive two-way relay network increases compared with a random relay selection and fixed power allocation.展开更多
To promote reliable and secure communications in the cognitive radio network,the automatic modulation classification algorithms have been mainly proposed to estimate a single modulation.In this paper,we address the cl...To promote reliable and secure communications in the cognitive radio network,the automatic modulation classification algorithms have been mainly proposed to estimate a single modulation.In this paper,we address the classification of superimposed modulations dedicated to 5G multipleinput multiple-output(MIMO)two-way cognitive relay network in realistic channels modeled with Nakagami-m distribution.Our purpose consists of classifying pairs of users modulations from superimposed signals.To achieve this goal,we apply the higher-order statistics in conjunction with the Multi-BoostAB classifier.We use several efficiency metrics including the true positive(TP)rate,false positive(FP)rate,precision,recall,F-Measure and receiver operating characteristic(ROC)area in order to evaluate the performance of the proposed algorithm in terms of correct superimposed modulations classification.Computer simulations prove that our proposal allows obtaining a good probability of classification for ten superimposed modulations at a low signal-to-noise ratio,including the worst case(i.e.,m=0.5),where the fading distribution follows a one-sided Gaussian distribution.We also carry out a comparative study between our proposal usingMultiBoostAB classifier with the decision tree(J48)classifier.Simulation results show that the performance of MultiBoostAB on the superimposed modulations classifications outperforms the one of J48 classifier.In addition,we study the impact of the symbols number,path loss exponent and relay position on the performance of the proposed automatic classification superimposed modulations in terms of probability of correct classification.展开更多

Methods: From July 2020 to April 2021, 180 elderly patients who underwent radical esophageal cancer surgery in our hospital were randomly divided into 90 cases in the control group and 90 in the observation group. The control group used static aspiration compound general anesthesia, and the patients in the observation group used intravenous anesthesia to compare the cognitive function and psychological state of the two groups of patients. Results: There was no statistical difference in the cognitive function score of patients in the observation group 30 minutes before anesthesia, 1 h and 24 hours after anesthesia compared with that in the control group, P > 0.05;there was no statistical difference between the Hamilton Anxiety Scale (HAMA) scores 30 minutes before and 24 hours after anesthesia in the observation group compared with the control group, P > 0.05;the cognitive function score of patients in the observation group of 4 h after surgery and 12 h after operation was significantly higher than that of the control group;the HAMA scores of patients in the observation group of 1 h, 4 h and 12 h after surgery were significantly lower than that of the control group, P Conclusion: The application of total intravenous anesthesia in elderly patients with esophageal cancer surgery can reduce the impact of anesthesia on their cognitive function and psychological state, which is worth popularizing and applying in clinical practice.展开更多

In the network, the energy-constrained secondary user(SU) relay employs TS protocol to harvest energy from the signals sent by the circuit-powered primary user(PU) transmitter, and then exploits the harvested energy to perform information forwarding. Our aim is to maximize the sum rate of SUs under the constraints of the data rate of PU, the energy harvesting and the transmit power of the SU relay. To determine the beamforming matrix and TS ratio, we decouple the original non-convex problem into two subproblems which can be solved by semidefinite relaxation and successive convex optimization methods. Furthermore, we derive closed form expressions of the optimal solutions for each subproblem, which facilitates the design of a suboptimal iterative algorithm to handle the original sum rate maximization problem. Simulation results are presented to illustrate the effectiveness and superior performance of the proposed joint design against other conventional schemes in the literature.展开更多
Objective:To investigate ghrelin level change in combination with psychological stress in the hypertensive old people with cognitive impairment and to explore its effect as well as possible mechanism.Methods:The study...Objective:To investigate ghrelin level change in combination with psychological stress in the hypertensive old people with cognitive impairment and to explore its effect as well as possible mechanism.Methods:The study population of 300 elders was divided into 2 groups,148 with hypertension and 152 non-hypertension,who were screened for psychological distress and cognition function,and had blood drawn to measure plasma levels of ghrelin and total cortisol on the same day.Results:The rates of anxiety and cognitive impairment were higher in the hypertension elders,which were negatively correlated with plasma ghrelin level,resulted from chronic cortisol response to anxiety.Conclusion:Chronic plasma cortisol increase to long-term anxiety leads a reduce in ghrelin level which then adversely affects blood pressure and cognitive function in old people.So measuring ghrelin of elders may be a diagnostic tool to predict cognitive development and ghrelin may be a selective antihypertensive medicine for cognitive impairment elders with or without chronic psychological stress.展开更多

For effective use of spectrum, we propose a Multiple Access Broadcasting (MABC) two-way relaying scheme for cognitive networks. The two transceivers transmit their data towards the relays and then relays retransmit the processed form of signal towards the receiver. Our aim is to design the beamforming coefficients to maximize quality of service (QoS) for the secondary network while satisfying tolerable interference constraint for the primary network. We show that this approach yields a closed-form solution. Our simulation results show that the maximum achievable SINR improved while the tolerable interference temperature becomes not strict for primary receiver.展开更多

We know that biological ageing is an inescapable fac...This research aims to understand the psychological, the social and cognitive aspects of ageing and assess the relevance of driving a personal automobile to seniors. We know that biological ageing is an inescapable fact, but is there also a psychological and a cognitive ageing? If so, is it related to getting older or to other factors? How relevant is continuing to drive to seniors, and how can the activity influence psychological and cognitive types of their ageing and self well-being? To answer these questions, we study several authors and concepts about mental health, development of ageing and their relationship to driving. We also explore psychological and cognitive changes associated with ageing, the principal behavioural changes of healthy senior people (with normal cognitive function) and MCI (mild cognitive impairment), as well as how driving inhibition can induce and affect psychological and cognitive characteristics. We conclude that low mental functioning is caused by a set of biological, psychological and social factors, connected in turn to environmental factors. The slowdown of cognitive activities and performances in seniors is not inevitably accompanied by a decrease in intellectual capacity, low sensory acuity, or slowing psychomotor skills. Other factors should be considered--such as health status, limitations, anxiety, lack of interest, motivation and attitudes. The loss of cognitive abilities and the prevalence of mild cognitive impairment increase with age and are a growing concern, especially among seniors who drive cars. However, we still lack specific evidence based on tests and criteria--in particular, on psychological and cognitive domains--to assess senior drivers actually at risk. It is important to establish a base of evidence to allow objective risk determination. The safest senior drivers should continue driving for as long as possible, thereby contributing to their mental health, independence and quality of life.展开更多

Methods: One hundred fourteen SAD patients completed manualized group CBT. We examined associations between the personality dimensions of NEO Five Factor Index (NEO-FFI) and the subscales of Symptom Checklist-90 Revised (SCL-90-R) in SAD patients after CBT using multiple regression analysis. Results: High levels of conscientiousness at baseline predicted symptom reduction on 4 SCL-90-R scales, including somatization, obsessive-compulsive, anxiety and global severity index in patients with SAD after CBT. And high levels of agreeableness predicted symptom reduction on 2 SCL-90-R scales, including Hostility and Paranoid Ideation. High levels of openness predicted psychoticism. Conclusion: The present study suggested that high levels of three NEO-FFI dimensions (openness, agreeableness, conscientiousness) might predict comorbid psychological symptoms reduction in SAD patients after CBT. For the purpose of improving comorbid psychological symptoms with SAD patients, it might be useful to pay more attention to these dimensions of NEO-FFI at baseline.展开更多
BACKGROUND Recently,the efficacy of cognitive behavioral therapy(CBT)-based intervention on health outcomes in patients with coronary heart disease(CHD)has been recognized in randomized controlled trials(RCTs),but no ...BACKGROUND Recently,the efficacy of cognitive behavioral therapy(CBT)-based intervention on health outcomes in patients with coronary heart disease(CHD)has been recognized in randomized controlled trials(RCTs),but no comprehensive systematic review has been conducted.To address this research gap,our study aimed to evaluate whether comprehensive CBT-based interventions positively affect health outcomes in CHD patients.It was hypothesized that CBT-based interventions are effective in:(1)Reducing depression,anxiety,and stress symptoms;(2)Reducing body mass index,blood pressure,and lipid levels;and(3)Improving quality of life,and exercise endurance.AIM To verify the effectiveness of CBT-based interventions on CHD patients through a meta-analysis of previous publications.METHODS Relevant RCTs published in English were obtained by searching electronic databases,including PubMed,Embase,Cochrane Central Register of Controlled Trials,Scopus,and Proquest,with the retrieval time from inception to August 2020.The primary outcomes were psychological factors(depression,anxiety,and stress symptoms),physiological factors(body mass index,blood pressure,blood lipids).The secondary outcomes included quality of life and exercise endurance.We used Review Manager 5.3 to conduct the meta-analysis and used the Physiotherapy Evidence Database tool to evaluate the quality of studies.RESULTS A total of 22 RCTs comprising 4991 patients with CHD were included in the systematic review and meta-analysis.The main analysis revealed that CBT-based intervention can reduce depression symptoms:-2.00[95%confidence interval(CI):-2.83 to-1.16,P<0.001];anxiety symptoms:-2.07(95%CI:-3.39 to-0.75,P=0.002);stress symptoms:-3.33(95%CI:-4.23 to-2.44,P<0.001);body mass index:-0.47(95%CI:-0.81 to-0.13,P=0.006);and improve physical functioning:3.36(95%CI:1.63 to 5.10,P=0.000)and mental functioning:6.91(95%CI:4.10 to 9.73,P<0.001).Moreover,subgroup analysis results showed that CBT-based interventions were more effective for symptoms of depression and anxiety in CHD patients when individual,as opposed to group treatment,and psycho-education,behavioral and cognitive strategies were applied as the core treatment approaches.CONCLUSION CBT-based interventions are effective treatment strategies for CHD patients,significantly improving their symptoms of depression,anxiety and stress,body mass index,and health-related quality of life.展开更多
In this editorial we comment on the article by Cahn-Hidalgo D published in a recent issue of the World Journal of Psychiatry 2020;10(1);1-11.We focus on the importance of utilizing psychometrically valid cognitive scr...In this editorial we comment on the article by Cahn-Hidalgo D published in a recent issue of the World Journal of Psychiatry 2020;10(1);1-11.We focus on the importance of utilizing psychometrically valid cognitive screening tools when assessing for cognitive decline in older adults in a psychiatric outpatient setting.We compared the use of Cognivue®to use of the montreal cognitive assessment(MoCA)as a cognitive screening tool.A total of 58 patients aged 55 and over participated in this comparison study.Patients completed cognitive screening on Cognivue®,a new Food and Drug Administration-cleared computer screening device,and the MoCA.The results of patient performance using these two instruments were analyzed.Sixteen(28%)patients screened negative for cognitive impairment on both assessments.Forty-two(72%)patients screened positive on one or both of the assessments.There was 43%agreement between Cognivue®and the MoCA in identifying patients with cognitive impairment,and individual subtests were weakly correlated.The MoCA was determined to be the preferred instrument due to its high sensitivity and specificity(100%and 87%,respectively)when screening for cognitive impairment.We propose that the use of Cognivue®cognitive screening tool be closely reviewed until more research proves that the test meets the standards for reliability and validity.It is important for clinicians to remember that screeners should not be used to diagnosis patients with neurocognitive disorders;instead,they should be used to determine whether further evaluation is warranted.Additionally,misdiagnosing of neurocognitive disorders can pose unnecessary psychological and emotional harm to patients and their families and also lead to incorrect treatment and undue healthcare costs.展开更多

文摘BACKGROUND Surgery is an effective method for treating certain diseases.Factors such as disease,preoperative fear and tension,surgical stress,postoperative pain,and related complications directly affect the smooth progression and outcome of surgery.Patients may experience a series of psychological and physiological changes during the perioperative period,resulting in anxiety and depression,which may reduce the pain threshold and worsen their prognosis.METHODS We enrolled 200 patients who underwent surgical care at The First People’s Hospital of Lin’an District,Hangzhou between January and December 2023.They were categorized into a routine intervention group(n=103)and a psychological intervention group(n=97),based on the intervention strategies used.Various assessment tools,including the self-rating anxiety scale(SAS),the self-rating depression scale(SDS),and the Connor–Davidson Resilience scale,were used to measure patients’negative states and emotions.The pre-and post-intervention scores for these metrics in the two groups were then analyzed.RESULTS In the psychological intervention group,the SAS and SDS scores(31.56±5.18 and 31.46±4.57,respectively)were significantly reduced compared to the routine intervention group(P<0.05).The visual analog scale pain scores at 12 and 24 hours after intervention(6.85±1.21,4.24±0.72)were notably higher than those in the routine intervention group(P<0.05).The psychological intervention group also demonstrated superior scores in perseverance(36.08±3.29),self-reliance(22.63±2.91),optimism(11.42±1.98),and resilience(70.13±5.37),compared to the routine intervention group(P<0.05).Additionally,the psychological intervention group’s confrontation score(23.16±4.29)was higher(P<0.05).This group also reported lower scores in avoidance(9.28±1.94)and yielding(6.19±1.92)(P<0.05).Lastly,the Short Form 36 Health Survey scores were significantly higher in the psychological intervention group,indicating a better quality of life(P<0.05).CONCLUSION Psychological intervention measures based on SCT can effectively alleviate pain,anxiety,and depression in periop-erative patients.

文摘BACKGROUND The severe physical and psychological impact of pain on the physical and mental health of women during labor leads to increased risks and complications during childbirth,presenting a major public health concern.Some studies have shown that cognitive behavioral therapy(CBT)has a positive effect on maternal psychology during delivery,reducing stress and shortening labor time.Thus,CBT training for mothers and delivery room staff may be beneficial in minimizing complications and adverse effects during natural birth.AIM To investigate the clinical effects of CBT training and psychological care during delivery,and their therapeutic effects on women in labor.METHODS This study used a retrospective analysis and included 140 mothers admitted to the maternity ward between January 2021 and January 2023.The study subjects were randomized into two groups:control(n=70)and observation(n=70).Routine care,CBT training,and psychological care were provided to mothers in both groups.Psychological status scores,delivery time,and satisfaction with care pre-and post-delivery were compared,and the incidence of complications after receiving care was analyzed between the two groups.RESULTS Although the psychological state of both groups improved significantly in the late stages of labor,the psychological state scores of the mothers in the observation group were significantly lower than those of the mothers in the control group(P<0.05).The duration of labor and incidence of complications in the observation group were significantly lower than those in the control group(P<0.05).The mothers in the observation group were significantly more satisfied with nursing care during the course of labor than those in the control group(P<0.05).CONCLUSION CBT training and psychological care for mothers in the midwifery process can effectively improve anxiety and depression,shorten labor duration,reduce postnatal complications,and improve nursing satisfaction and nurse-patient relationships.Its clinical application is effective and has popularization value,providing a new way to protect maternal mental health.

Therefore, English teacher would change their teaching opinions, realizing teaching object, guiding student's minds and ideas to study by some measures, such as accordant teaching and study, making students as a base in study, and students' self-study. In other words, making student study actively rather than passively is much better for their English abilities.

文摘BACKGROUND The efficacy of cognitive behavioral group therapy(CBGT)for cognitive dysfunction and negative symptoms of schizophrenia is established,but more evidence is required.AIM To assess the effectiveness of CBGT combined with mental health education as a treatment for schizophrenia compared with mental health education alone.METHODS In all,120 schizophrenia out-patients were randomized into CBGT combined with mental health education or single mental health education.The primary outcomes were positive and negative symptoms,cognitive function,excitatory factor,anxiety and depression symptom improvements on the positive and negative syndrome scale score.Secondary outcome measures included social function and drug compliance.RESULTS There were significant differences between CBGT combined with mental health education and single mental health education on measures of positive and negative symptoms,cognitive functions,excitatory factor,anxiety and depression symptoms,and social functions.No other significant difference in outcomes was observed.CONCLUSION CBGT combined with mental health education may be relevant beneficial treatment method in reducing symptoms,cognitive and social functions of patients with schizophrenia.
文摘This retrospective study investigated, in two cohorts of subjects living in Southern Italy and awaiting treatment for oral squamous cell carcinoma (OSCC), the variables related to diagnostic delay ascribable to the patient, with particular reference to the cognitive and psychological ones. A total of 156 patients with OSCC (mean age: 62 years, M/F: 2.39 : 1) were recruited at the Universities of Palermo and Naples. Risk factors related to patient delay included: sociodemographic, health-related, cognitive and psychological variables. The analysis was conducted by considering two different delay ranges: dichotomous (≤1 month vs. 〉 1 month) and polytomous (〈1 month, 1-3 months, 〉3 months) delay. Data were investigated by univariate and multivariate analyses and a Pvalue≤0.05 was considered statistically significant. For both delay measurements, the most relevant variables were: 'Personal experience of cancer' (dichotomous delay: P=0.05, odds ratio (0R)=0.33, 95% confidence interval (CI)=0. 11-0.99; polytomous delay: P=0.006, Chi-square= 10.224) and 'Unawareness' (dichotomous delay: P〈0.01, 0R=4.96, 95% CI--2.16-11.37; polytomous delay: P=0.087, Chi-square=4.77). Also 'Denial' (P〈0.01, 0R=6.84, 95% CI=2.31-20.24) and 'Knowledge of cancer' (P=0.079, Chi-square=8.359) were found to be statistically significant both for dichotomous and for polytomous categorization of delay, respectively. The findings of this study indicated that, in the investigated cohorts, the knowledge about cancer issues is strongly linked to the patient delay. Educational interventions on the Mediterranean population are necessary in order to increase the patient awareness and to emphasize his/her key role in early diagnosis of OSCC.

文摘A joint power control and relay selection scheme is considered for a cooperative and cognitive radio system where a secondary network shares spectrum with the primary network. In the secondary network, two secondary users (SUs) communicate with each other via an assist relay. The main point is to provide the best system performance to SUs while maintaining the interference power at primary user (PU) under a certain level. Using convex optimization, a closed-form solution is obtained when optimizing the power allocation among the two nodes and relay. Based on this result, a joint power control and relay selection scheme with fewer variable dimensions is proposed to maximize the achievable rate of the secondary system. Simulation results demonstrate that the sum rate of the cognitive two-way relay network increases compared with a random relay selection and fixed power allocation.
文摘To promote reliable and secure communications in the cognitive radio network,the automatic modulation classification algorithms have been mainly proposed to estimate a single modulation.In this paper,we address the classification of superimposed modulations dedicated to 5G multipleinput multiple-output(MIMO)two-way cognitive relay network in realistic channels modeled with Nakagami-m distribution.Our purpose consists of classifying pairs of users modulations from superimposed signals.To achieve this goal,we apply the higher-order statistics in conjunction with the Multi-BoostAB classifier.We use several efficiency metrics including the true positive(TP)rate,false positive(FP)rate,precision,recall,F-Measure and receiver operating characteristic(ROC)area in order to evaluate the performance of the proposed algorithm in terms of correct superimposed modulations classification.Computer simulations prove that our proposal allows obtaining a good probability of classification for ten superimposed modulations at a low signal-to-noise ratio,including the worst case(i.e.,m=0.5),where the fading distribution follows a one-sided Gaussian distribution.We also carry out a comparative study between our proposal usingMultiBoostAB classifier with the decision tree(J48)classifier.Simulation results show that the performance of MultiBoostAB on the superimposed modulations classifications outperforms the one of J48 classifier.In addition,we study the impact of the symbols number,path loss exponent and relay position on the performance of the proposed automatic classification superimposed modulations in terms of probability of correct classification.

文摘At present,the academic researches on moral choices focus mainly on the individuals’external behaviors,without fully considering the influences of the individuals’internal psychological factors.Based on the model of complete information static games in the Non-cooperative Game theory,the present research has designed a“Red and Blue Experiment”to explore the psychological influencing factors of the new generation employees’moral choices.The research has conducted psychoanalysis on the new generation employees through experiments and interviews with 118 data sources,and concluded the characteristics in relation to the different stages of psychological processes and moral choices of the experimental population.Through comparative analysis,it is found that:the psychological factors,such as trust,doubt,and guilt will influence the new generation employees’moral choices,guiding them to make choices,such as risk aversion and mutual benefit and win-win,in the process.Additionally,the research has also provided effective scientific evidence for the new generation employees’mental health development and career planning.
文摘Objective: To compare the effects of total intravenous anesthesia and static aspiration combined general anesthesia on postoperative cognitive function and psychological state of elderly esophageal cancer patients. Methods: From July 2020 to April 2021, 180 elderly patients who underwent radical esophageal cancer surgery in our hospital were randomly divided into 90 cases in the control group and 90 in the observation group. The control group used static aspiration compound general anesthesia, and the patients in the observation group used intravenous anesthesia to compare the cognitive function and psychological state of the two groups of patients. Results: There was no statistical difference in the cognitive function score of patients in the observation group 30 minutes before anesthesia, 1 h and 24 hours after anesthesia compared with that in the control group, P > 0.05;there was no statistical difference between the Hamilton Anxiety Scale (HAMA) scores 30 minutes before and 24 hours after anesthesia in the observation group compared with the control group, P > 0.05;the cognitive function score of patients in the observation group of 4 h after surgery and 12 h after operation was significantly higher than that of the control group;the HAMA scores of patients in the observation group of 1 h, 4 h and 12 h after surgery were significantly lower than that of the control group, P Conclusion: The application of total intravenous anesthesia in elderly patients with esophageal cancer surgery can reduce the impact of anesthesia on their cognitive function and psychological state, which is worth popularizing and applying in clinical practice.

文摘In this paper, we investigate a joint beamforming and time switching(TS) design for an energy-constrained cognitive two-way relay(TWR) network. In the network, the energy-constrained secondary user(SU) relay employs TS protocol to harvest energy from the signals sent by the circuit-powered primary user(PU) transmitter, and then exploits the harvested energy to perform information forwarding. Our aim is to maximize the sum rate of SUs under the constraints of the data rate of PU, the energy harvesting and the transmit power of the SU relay. To determine the beamforming matrix and TS ratio, we decouple the original non-convex problem into two subproblems which can be solved by semidefinite relaxation and successive convex optimization methods. Furthermore, we derive closed form expressions of the optimal solutions for each subproblem, which facilitates the design of a suboptimal iterative algorithm to handle the original sum rate maximization problem. Simulation results are presented to illustrate the effectiveness and superior performance of the proposed joint design against other conventional schemes in the literature.
文摘Objective:To investigate ghrelin level change in combination with psychological stress in the hypertensive old people with cognitive impairment and to explore its effect as well as possible mechanism.Methods:The study population of 300 elders was divided into 2 groups,148 with hypertension and 152 non-hypertension,who were screened for psychological distress and cognition function,and had blood drawn to measure plasma levels of ghrelin and total cortisol on the same day.Results:The rates of anxiety and cognitive impairment were higher in the hypertension elders,which were negatively correlated with plasma ghrelin level,resulted from chronic cortisol response to anxiety.Conclusion:Chronic plasma cortisol increase to long-term anxiety leads a reduce in ghrelin level which then adversely affects blood pressure and cognitive function in old people.So measuring ghrelin of elders may be a diagnostic tool to predict cognitive development and ghrelin may be a selective antihypertensive medicine for cognitive impairment elders with or without chronic psychological stress.
文摘In this paper, we study the problem of distributed relay beamforming for a bidirectional cognitive relay network which consists of two secondary transceivers and K cognitive relay nodes and a primary network with a transmitter and receiver, using underlay model. For effective use of spectrum, we propose a Multiple Access Broadcasting (MABC) two-way relaying scheme for cognitive networks. The two transceivers transmit their data towards the relays and then relays retransmit the processed form of signal towards the receiver. Our aim is to design the beamforming coefficients to maximize quality of service (QoS) for the secondary network while satisfying tolerable interference constraint for the primary network. We show that this approach yields a closed-form solution. Our simulation results show that the maximum achievable SINR improved while the tolerable interference temperature becomes not strict for primary receiver.
文摘This research aims to understand the psychological, the social and cognitive aspects of ageing and assess the relevance of driving a personal automobile to seniors. We know that biological ageing is an inescapable fact, but is there also a psychological and a cognitive ageing? If so, is it related to getting older or to other factors? How relevant is continuing to drive to seniors, and how can the activity influence psychological and cognitive types of their ageing and self well-being? To answer these questions, we study several authors and concepts about mental health, development of ageing and their relationship to driving. We also explore psychological and cognitive changes associated with ageing, the principal behavioural changes of healthy senior people (with normal cognitive function) and MCI (mild cognitive impairment), as well as how driving inhibition can induce and affect psychological and cognitive characteristics. We conclude that low mental functioning is caused by a set of biological, psychological and social factors, connected in turn to environmental factors. The slowdown of cognitive activities and performances in seniors is not inevitably accompanied by a decrease in intellectual capacity, low sensory acuity, or slowing psychomotor skills. Other factors should be considered--such as health status, limitations, anxiety, lack of interest, motivation and attitudes. The loss of cognitive abilities and the prevalence of mild cognitive impairment increase with age and are a growing concern, especially among seniors who drive cars. However, we still lack specific evidence based on tests and criteria--in particular, on psychological and cognitive domains--to assess senior drivers actually at risk. It is important to establish a base of evidence to allow objective risk determination. The safest senior drivers should continue driving for as long as possible, thereby contributing to their mental health, independence and quality of life.
文摘Aim: The present study aimed to examine the predictors of comorbid psychological symptoms in social anxiety disorder (SAD) after cognitive-behavioral therapy (CBT). Methods: One hundred fourteen SAD patients completed manualized group CBT. We examined associations between the personality dimensions of NEO Five Factor Index (NEO-FFI) and the subscales of Symptom Checklist-90 Revised (SCL-90-R) in SAD patients after CBT using multiple regression analysis. Results: High levels of conscientiousness at baseline predicted symptom reduction on 4 SCL-90-R scales, including somatization, obsessive-compulsive, anxiety and global severity index in patients with SAD after CBT. And high levels of agreeableness predicted symptom reduction on 2 SCL-90-R scales, including Hostility and Paranoid Ideation. High levels of openness predicted psychoticism. Conclusion: The present study suggested that high levels of three NEO-FFI dimensions (openness, agreeableness, conscientiousness) might predict comorbid psychological symptoms reduction in SAD patients after CBT. For the purpose of improving comorbid psychological symptoms with SAD patients, it might be useful to pay more attention to these dimensions of NEO-FFI at baseline.
文摘BACKGROUND Recently,the efficacy of cognitive behavioral therapy(CBT)-based intervention on health outcomes in patients with coronary heart disease(CHD)has been recognized in randomized controlled trials(RCTs),but no comprehensive systematic review has been conducted.To address this research gap,our study aimed to evaluate whether comprehensive CBT-based interventions positively affect health outcomes in CHD patients.It was hypothesized that CBT-based interventions are effective in:(1)Reducing depression,anxiety,and stress symptoms;(2)Reducing body mass index,blood pressure,and lipid levels;and(3)Improving quality of life,and exercise endurance.AIM To verify the effectiveness of CBT-based interventions on CHD patients through a meta-analysis of previous publications.METHODS Relevant RCTs published in English were obtained by searching electronic databases,including PubMed,Embase,Cochrane Central Register of Controlled Trials,Scopus,and Proquest,with the retrieval time from inception to August 2020.The primary outcomes were psychological factors(depression,anxiety,and stress symptoms),physiological factors(body mass index,blood pressure,blood lipids).The secondary outcomes included quality of life and exercise endurance.We used Review Manager 5.3 to conduct the meta-analysis and used the Physiotherapy Evidence Database tool to evaluate the quality of studies.RESULTS A total of 22 RCTs comprising 4991 patients with CHD were included in the systematic review and meta-analysis.The main analysis revealed that CBT-based intervention can reduce depression symptoms:-2.00[95%confidence interval(CI):-2.83 to-1.16,P<0.001];anxiety symptoms:-2.07(95%CI:-3.39 to-0.75,P=0.002);stress symptoms:-3.33(95%CI:-4.23 to-2.44,P<0.001);body mass index:-0.47(95%CI:-0.81 to-0.13,P=0.006);and improve physical functioning:3.36(95%CI:1.63 to 5.10,P=0.000)and mental functioning:6.91(95%CI:4.10 to 9.73,P<0.001).Moreover,subgroup analysis results showed that CBT-based interventions were more effective for symptoms of depression and anxiety in CHD patients when individual,as opposed to group treatment,and psycho-education,behavioral and cognitive strategies were applied as the core treatment approaches.CONCLUSION CBT-based interventions are effective treatment strategies for CHD patients,significantly improving their symptoms of depression,anxiety and stress,body mass index,and health-related quality of life.
文摘In this editorial we comment on the article by Cahn-Hidalgo D published in a recent issue of the World Journal of Psychiatry 2020;10(1);1-11.We focus on the importance of utilizing psychometrically valid cognitive screening tools when assessing for cognitive decline in older adults in a psychiatric outpatient setting.We compared the use of Cognivue®to use of the montreal cognitive assessment(MoCA)as a cognitive screening tool.A total of 58 patients aged 55 and over participated in this comparison study.Patients completed cognitive screening on Cognivue®,a new Food and Drug Administration-cleared computer screening device,and the MoCA.The results of patient performance using these two instruments were analyzed.Sixteen(28%)patients screened negative for cognitive impairment on both assessments.Forty-two(72%)patients screened positive on one or both of the assessments.There was 43%agreement between Cognivue®and the MoCA in identifying patients with cognitive impairment,and individual subtests were weakly correlated.The MoCA was determined to be the preferred instrument due to its high sensitivity and specificity(100%and 87%,respectively)when screening for cognitive impairment.We propose that the use of Cognivue®cognitive screening tool be closely reviewed until more research proves that the test meets the standards for reliability and validity.It is important for clinicians to remember that screeners should not be used to diagnosis patients with neurocognitive disorders;instead,they should be used to determine whether further evaluation is warranted.Additionally,misdiagnosing of neurocognitive disorders can pose unnecessary psychological and emotional harm to patients and their families and also lead to incorrect treatment and undue healthcare costs.
文摘Irritable bowel syndrome(IBS)is a common disorder,reported to be found in 5%-20%of the general population.Its management accounts for up to 25%of a gastroenterologist’s workload in the outpatient department,and the main symptoms are abdominal pain,bloating,and altered bowel habits.Despite a great amount of available pharmacological treatments aimed at a wide variety of gastrointestinal and brain targets,many patients have not shown adequate symptom relief.In recent years,there has been increasing evidence to suggest that psychological treatments,in particular cognitive-behavioral therapy(CBT),are effective for the management of IBS.This review discusses CBT for the management of IBS.CBT has proved to be effective in alleviating the physical and psychological symptoms of IBS and has thus been recommended as a treatment option for the syndrome.
