AIM To examine the changes of the ghrelin/ghrelin O-acyltransferase(GOAT) axis and the mammalian target of rapamycin(m TOR) pathway in the hypothalamus after sleeve gastrectomy.METHODS A total of 30 obese type-2 diabe...AIM To examine the changes of the ghrelin/ghrelin O-acyltransferase(GOAT) axis and the mammalian target of rapamycin(m TOR) pathway in the hypothalamus after sleeve gastrectomy.METHODS A total of 30 obese type-2 diabetes Sprague-Dawley(SD) rats, 6 wk of age, fed with high-sugar and highfat fodder for 2 mo plus intraperitoneal injection of streptozotocin were randomly divided into three groups: non-operation group(S0 group, n = 10), sham operation group(Sh group, n = 10) and sleeve gastrectomy group(SG group, n = 10). Data of body mass, food intake, oral glucose tolerance test(OGTT), acylated ghrelin(AG) and total ghrelin(TG) were collected and measured at the first day(when the rats were 6 wk old), preoperative day 3 and postoperative week 8. The m RNA expression of preproghrelin, GOAT and neuropeptide Y(NPY), and protein expression of ghrelin, GOAT, GHSR and the m TOR pathway(p-Akt, p-m TOR and p-S6) were measured in the hypothalamus.RESULTS SG can significantly improve metabolic symptoms by reducing body mass and food intake. The obese rats showed lower serum TG levels and no change in AG, but the ratio of AG/TG was increased. When compared with the S0 and Sh groups, the SG group showed decreased TG(1482.03 ± 26.55, 1481.49 ± 23.30 and 1206.63 ± 52.02 ng/L, respectively, P < 0.05), but unchanged AG(153.06 ± 13.74, 155.37 ± 19.30 and 144.44 ± 16.689 ng/L, respectively, P > 0.05). As a result, the ratio of AG/TG further increased in the SG group(0.103 ± 0.009, 0.105 ± 0.013 and 0.12 ± 0.016, respectively, P < 0.05). When compared with the S0 group, SG suppressed m RNA and protein levels of preproghrelin(0.63 ± 0.12 vs 0.5 ± 0.11, P < 0.05) and GOAT(0.96 ± 0.09 vs 0.87 ± 0.08, P < 0.05), but did not change NPY m RNA expression(0.61 ± 0.04 vs 0.65 ± 0.07, P > 0.05) in the hypothalamus. The protein levels of p-Akt, p-m TOR and p-S6 were higher in the SG group, which indicated that the hypothalamic m TOR pathway was activated after SG at the postoperative week 8. CONCLUSION The reduction of ghrelin expression and activation of the m TOR pathway might have opposite effects on food intake, as SG improves obesity and T2 DM.展开更多

Greater reduction of TC frequency over WNP happened during the decaying phase of eastern Pacific El Nio(EPEN) than CPEN, due to the difference of the anomalous Philippine Sea anticyclone strength. After compositing three representative ENSO indices,El Nio events have been divided into an eastern pattern(EP) and a central pattern(CP).By using EOF,correlation and composite analysis,the relationship and possible...After compositing three representative ENSO indices,El Nio events have been divided into an eastern pattern(EP) and a central pattern(CP).By using EOF,correlation and composite analysis,the relationship and possible mechanisms between Indian Ocean Dipole(IOD) and two types of El Nio were investigated.IOD events,originating from Indo-Pacific scale air-sea interaction,are composed of two modes,which are associated with EP and CP El Ni o respectively.The IOD mode related to EP El Nio events(named as IOD1) is strongest at the depth of 50 to 150 m along the equatorial Indian Ocean.Besides,it shows a quasi-symmetric distribution,stronger in the south of the Equator.The IOD mode associated with CP El Nio(named as IOD2) has strongest signal in tropical southern Indian Ocean surface.In terms of mechanisms,before EP El Nio peaks,anomalous Walker circulation produces strong anomalous easterlies in equatorial Indian Ocean,resulting in upwelling in the east,decreasing sea temperature there;a couple of anomalous anticyclones(stronger in the south) form off the Equator where warm water accumulates,and thus the IOD1 occurs.When CP El Nio develops,anomalous Walker circulation is weaker and shifts its center to the west,therefore anomalous easterlies in equatorial Indian Ocean is less strong.Besides,the anticyclone south of Sumatra strengthens,and the southerlies east of it bring cold water from higher latitudes and northerlies west of it bring warm water from lower latitudes to the 15° to 25°S zone.Meanwhile,there exists strong divergence in the east and convergence in the west part of tropical southern Indian Ocean,making sea temperature fall and rise separately.Therefore,IOD2 lies farther south.展开更多

In the classic Lee-Carter model of mortality, the time trend and the age-specific pattern of mortality over age group are linear, this is not the feature of mortality model. To avoid this disadvantage, O-U type processes will be used to model the log-mortality in this paper. In fact, this model is an AR(1) process, but with a nonlinear time drift term.Based on the mortality data of America from Human Mortality database(HMD), mortality projection consistently indicates a preference for mortality with O-U type processes over those with the classical Lee-Carter model. By means of this model, the low bounds of mortality rates at every age are given. Therefore, lengthening of maximum life expectancies span is estimated in this paper.展开更多

METHODS A thorough review of the scientific literature on diabetes care a...AIM To review impacts of interventions involving self-management education, health coaching, and motivational interviewing for type 2 diabetes. METHODS A thorough review of the scientific literature on diabetes care and management was executed by a research team. RESULTS This article summarizes important findings in regard to the validity of developing a comprehensive behavioral system as a framework for empirical investigation. The behavioral system framework consists of patients' knowledge(K), motivation(M), attitude(A), and practice(P) as predictor variables for diabetes care outcomes(O). Care management strategies or health education programs serve as the intervention variable that directly influences K, M, A, and P and then indirectly affects the variability in patient care outcomes of patients with type 2 diabetes. CONCLUSION This review contributes to the understanding of the KMAP-O framework and how it can guide the care management of patients with type 2 diabetes. It will allow the tailoring of interventions to be more effective through knowledge enhancement, increased motivation, attitudinal changes, and improved preventive practice to reduce the progression of type 2 diabetes and comorbidities. Furthermore, the use of health information technology for enhancing changes in KMAP and communications is advocated in health promotion and development.展开更多
