AIM:To adapt the low vision-related quality of life(LVQOL) instrument into Turkish language and to assess its validity and reliability.METHODS:The study was conducted in 387 patients attending the Centre of Low Vision Rehabilitation,Faculty of Medicine,Ankara University.For statistical analyses,the Spearman's correlation coefficient,Cronbach's alpha coefficient and Confirmatory Factor Analysis(CFA) were used.RESULTS:According to results of CFA,the item in the 'Adjustment' subscale because of having the factor loading below 0.40,was excluded from the questionnaire.The reliability of the questionnaire was assessed according to Cronbach's alpha coefficients.The reliability of the 'Distance Vision,Mobility,and Lighting' subscale was ?琢=0.863;of the 'Adjustment' subscale was ?琢=0.694;'Reading and Fine Work' was ?琢=0.791,and 'Activities of Daily Living' was ?琢= 0.770.So these results indicate that the questionnaire is reliable to measure the vision related quality of life of low-vision patients.The correlations between the subscales were also analyzed,and the correlation between 'Adjustment' and 'Reading and Fine Work' was found to be the lowest(rs=0.336,P <0.001),whereas the strongest correlation was found between the 'Reading and Fine Work',and 'Activities of Daily Living'.Additionally,the 'Adjustment' dimension showed the strongest correlation with only 'Distance Vision,Mobility,and Lighting' dimension.CONCLUSION:After removing the last item in the second dimension,the Turkish adaptation of all dimensions of the LVQOL has been shown to be reliable,valid and suitable for use in patients with low vision in Turkey. 展开更多

Background:To determine patient-reported vision-related quality of life(VR-QoL)following Boston type I keratoprosthesis(BI-KPro)surgery and its association with postoperative best-corrected visual acuity(BCVA).Methods:Descriptive cross-sectional study.Consecutive consenting patients with BI-KPro were included.The French National Eye Institute Visual Function Questionnaire-25 administered at 51±18 months postoperatively measured VR-QoL.Clinical charts were reviewed for demographics,indications for BI-KPro,baseline and postoperative(at time of interview)BCVA.For patients operated unilaterally,stratification of VR-QoL scores based on BCVA in the non-operated eye was performed.Multivariate linear regression was carried out,using VR-QoL scores as dependent variables,and demographics and postoperative BCVA as covariates.For patients operated bilaterally,Spearman correlation between VR-QoL scores and BCVA was performed.P<0.05 indicated statistical significance.Results:Sixty-four patients,aged 59±14 years,52%male,with a follow-up of 54±19 months,were included.Postoperative BCVA increased from baseline in all operated eyes(P=0.000).In patients with unilateral BI-KPro(n=52),the VR-QoL overall score was 70.7±25.1.Scores on all questionnaire subscales were greater when BCVA in the non-operated eye was>20/200 compared to 20/200(P=0.000).BCVA in the non-operated eye was positively associated with all subscales(P<0.01)independently of age,sex,follow-up duration and postoperative BCVA in the operated eye.In patients with bilateral BI-KPro(n=12)the VR-QoL overall score was 63.0±18.7.BCVA in the best eye positively correlated with Near/Distance activities,and social functioning subscales(P<0.05).There was no significant difference between VR-QoL scores of patients operated unilaterally vs.bilaterally.Conclusions:We describe VR-QoL more than 4 years after BI-KPro surgery.Compared to data at 1 year previously reported,our results suggest that,as vision progressively deteriorates in the operated eye,patients increasingly rely on their non-operated eye.VR-QoL after bilateral BI-KPro is assessed for the first time,and appears comparable to that after unilateral surgery.Larger,prospective,long-term studies,with assessment at baseline,are warranted. 展开更多

We analyzed a large POAG patien... Background Primary open angle glaucoma (POAG) is a common cause of irreversible blindness. The variable etiology of POAG poses significant challenges for treatment and rehabilitation. We analyzed a large POAG patient cohort during treatment to reveal possible causes of vision disorder, assess vision-related quality of life (VRQL), and to evaluate the efficacy of rehabilitative treatments. Methods We analyzed the visional disturbances in 500 POAG patients (890 eyes) by regular ophthalmic examination and visual field examination using Humphrey 30-o perimetry. Appropriate rehabilitative treatments for POAG were prescribed based on results of clinical examination and included correction of ametropia, health education, counseling, and the fitting of typoscopes. VRQL was assessed before and after treatment by a VRQL self-assessment questionnaire. Results Scores on the VRQL self-assessment were significantly lower compared to healthy controls. The primary cause of the vision disturbances was ametropia (97.99%), and 51.61% of the ametropia eyes had not received appropriate correction. The secondary causes of visual impairment were glaucomatous neurodegeneration (26.29%), complicated cataract, or other accompanying eye diseases. The causes of the clinical low vision (44 patients) were glaucomatous neurodegeneration (32 eyes), fundus diseases (23 eyes), keratopathy (11 eyes), and other eye diseases (10 eyes). The VRQL scores of patients improved significantly after rehabilitation and the correction of ametropia (P 〈0.01). Twenty-five patients with low vision were provided with typoscopes, and 21 (84%) experienced significant functional recovery, while the remaining low vision patients could see letter lines two or more levels lower (smaller) on visual charts in a near vision test. Conclusions Vision disorders in POAG patients are common and severe. Appropriate rehabilitation, especially the correction of ametropia, can significantly improve VRQL as revealed by the self-assessment of POAG patients. 展开更多

Background:Age-related macular degeneration (AMD) is the leading cause of irreversible vision loss among the older population.In China,treatment of age-related ocular diseases is becoming a priority in eye care services.This study was to investigate the clinical characteristics and quality of life of Chinese patients with wet AMD and current treatment types,to evaluate short-term gains in different treatments,and to investigate associations between visual function and vision-related quality of life (VRQoL).Methods:A prospective,observational,noninterventional study was conducted.Basic data were collected from patients with clinical diagnoses of wet AMD before clinical assessments at baseline.VRQoL was measured with the Chinese version of the National Eye Institute Visual Function Questionnaire-25 (NEI VFQ-25).Correlations of the NEI VFQ-25 subscale scores with best-corrected visual acuity (BCVA) and between-group differences were analyzed.Results:A total of 80 wet AMD patients were enrolled,with the mean age of 68.40 years.About one-quarter ofwetAMD patients received intravitreal (IVT) ranibizumab treatment,and 67% of them were treated on apro re nata basis.The visual acuity of patients treated with IVT ranibizumab at month 3 after treatment was significantly increased,whereas patients treated with traditional Chinese medicine achieved no significant improvement.Cronbach's α for the NEI VFQ-25 subscales ranged from 0.697 to 0.843.Eight subscale and overall composite scores were moderately correlated with the BCVA of the better-seeing eye.Significant differences in the overall NEI VFQ-25 scores and other subscales were observed between patients with BCVA in the better-seeing eye of less than 50 letters and the others.Conclusions:Patients treated with IVT ranibizumab experienced better vision improvement at short-term follow-up.The Chinese version of the NEI VFQ-25 is a valid and reliable tool for assessing the VRQoL of Chinese wet AMD patients. 展开更多

Pediatric uveitis is an inflammatory ocular disease that can lead to sight-threatening complications.Pediatric patients have distinct challenges in the diagnosis and management of uveitis,secondary to difficulties in performing ophthalmic examinations in young children,delayed diagnosis due to lack of adherence with recommended screening schedules,medication side-effects,and increased burden of disease into adulthood.Measurement of outcomes in pediatric uveitis has traditionally relied upon the ophthalmic examination and general quality of life(QOL)measures.However,the ophthalmic examination does not take into account the impact of uveitis on a child’s QOL and general QOL measures do not adequately assess the specific effects of vision.Several vision-related quality of life(VR-QOL)instruments have been used to measure outcomes in both adults and children including:the National Eye Institute Visual Function Questionnaire(NEI VFQ-25),Vision-related Quality of Life of Children and Young People(VQoL_CYP),the Children’s Visual Function Questionnaire(CVFQ),and the Effect of Youngsters’Eyesight on Quality of Life(EYE-Q).However,the NEI VFQ-25 is not a valid or applicable measure in children,and the VQoL_CYP and CVFQ are not uveitis specific and may not characterize disease specific burdens.The EYE-Q is the only uveitis-specific pediatric questionnaire that measures visual functioning and VR-QOL in 5-18 years old children and adolescents with uveitis.It has been shown to be a valid and reliable assessment tool in several cohorts of children with uveitis.A comprehensive assessment of the impact of uveitis on a child that includes a vision-specific measure,such as the EYE-Q,allows for better understanding of the true burden of uveitis in children.For this review,we describe traditional outcome measures in uveitis studies,general QOL measures and vision-specific measures in adults and in children. 展开更多
