Aiming at the basic and key technical problems in prevention and control of sugarcane white leaf disease(SCWL),this study systematically overcame key technical bottleneck of prevention and control of new SCWL after 10...Aiming at the basic and key technical problems in prevention and control of sugarcane white leaf disease(SCWL),this study systematically overcame key technical bottleneck of prevention and control of new SCWL after 10 years of collaborative research,and achieved several innovative achievements.SCWL phytoplasmas newly recorded in China and the new subgroup of SCWL phytoplasmas(16SrXI-D)were discovered for the first time in Yunnan,and the whole genome analysis of the epidemic subgroup was completed.The main transmission source of SCWL pathogens has been identified as infected seed canes,and Tettigoniella viridis and Clovia conifer were newly discovered as vectors for virus transmission.The disease resistance of 25 main varieties was identified,and 10 control varieties were selected.The prevention and control strategy of"emphasizing early warning,strictly carrying out quarantine,blocking the vectors and controlling residual plants"was put forward,and a comprehensive prevention technique was established through integration of various techniques,and standardized technical regulations were formulated for demonstration application.The promotion and application of these achievements have realized scientific prevention and control of SCWL,effectively curbed the spread of SCWL,and ensured the safety of sugarcane producing areas in China,achieving great economic,social and ecological benefits and providing technical support for high-quality development,loss reduction and efficiency improvement of China's sugar industry.展开更多

[Methods]The identification technology of sugarcane resistance to SCWL was systematically studied and explored from the aspects of sugarcane material treatment and planting, inoculation liquid preparation, inoculation method, disease investigation, grading standard formulation, etc., and two sets of simple, efficient, stable, standardized and practical accurate identification methods for sugarcane resistance to SCWL were created for the first time, namely, the seed cane coating inoculation method and the stem-cutting inoculation method at the growth stage. The seed cane coating inoculation method includes the steps of directly screening SCWL phytoplasma, extracting juice from cane and adding 10 times of sterile water to prepare an inoculation liquid, spraying seed cane on plastic film to keep moisture, planting the inoculated materials in barrels in an insect-proof greenhouse for cultivation, investigating the incidence rate 30 d after inoculation, and evaluating the disease resistance according to the 1-5 level standard. The method of stem-cutting inoculation includes the steps of directly screening sugarcane stems carrying SCWL phytoplasma and adding 10 times of sterile water to prepare an inoculation liquid, cultivating the identification materials in an insect-proof greenhouse, dropping 100 μl of the inoculation liquid into each root incision with a pipette gun at the age of 6 months, investigating the incidence rate 20 d after planting, and evaluating the disease resistance according to the 1-5 level standard. [Results] The two methods are similar to the natural transmission method. After inoculation, SCML occurred significantly, with high sensitivity and good reproducibility. The results of resistance identification were consistent with those of natural disease in the field. Through the two inoculation methods and field natural disease investigation, the resistance of 10 main cultivars to SCML was identified, which was true and reliable. [Conclusions] This study can provide standard varieties for identification of SCML resistance in the future.展开更多

Then, Koch's Postulate was adopted to inoculate the pathogen. Finally, the identification was carried out according to its biological characteristics. [ Result] The pathogen of white silk disease belongs to weak hyperparasite. Because of its poor direct invasion ability, the pathogen usually invades into host through wound infection. According to its hyphae morphology and pathogenicity, the pathogen of G. elata White Silk Disease has been identified as Sc/erotium ro/fsii Sacc., which belongs to deuteromycotina, hyphomycetes, agonomycetales, fungi of sclerotium genus. Determination of the corres...A simple and highly sensitive analysis by electrochemical voltammetry has been developed for diagnosis of the most destructive crop disease in Thailand known as sugarcane white leaf (SCWL). Determination of the corresponding DNA interaction has been obtained from the voltammetric signals of electroactive redox methylene blue (MB) by means of cyclic and differential pulse voltammetry. In this study, a chitosan-modified glassy carbon electrode (GCE) was created by self-assembly to produce electrostatic platform for effective immobilization of the DNA. Fabrication of SCWL-DNA hybridization detection system was performed by immobilizing the ssDNA probe as a specific sensor onto chitosan-modified GCE. Hybridization of complementary DNA from the real samples could then be detected by its respective MB signal. This fabricated DNA probe sensor was shown to be capable for discriminative identification among the DNAs from SCWL plants, mosaic virus infected sugarcane and healthy sugarcane plants. Relationship between the specific hybridization signal and DNA target concentration was also observed under optimal condition. The detection limit of 4.709 ng/μl with the regression coefficient (R2) of 0.998 and overall RSD of 2.44% were obtained by response curve fit analysis. The actual SCWL-ssDNA immobilization and hybridizing event were subsequently confirmed by an observation under atomic force microscope. Thus these experiments demonstrate the first successful and effective DNA based voltammetric electrochemical determination for a verification of the specific pathogenic infection within plants from the real epidemic field.展开更多

Although some progresses have been made, there is no evident study on applying multifractal analysis to evaluate the white matter structural changes on MRI for Alzheimer's disease(AD) research. In this paper, to explore multifractal analysis of white matter structural changes on 3D MRI volumes between normal aging and early AD, we not only extend the traditional box-counting multifractal analysis(BCMA) into the 3D case, but also propose a modified integer ratio based BCMA(IRBCMA) algorithm to compensate for the rigid division rule in BCMA. We verify multifractal characteristics in 3D white matter MRI volumes. In addition to the previously well studied multifractal feature,△α, we also demonstrated △ f as an alternative and effective multifractal feature to distinguish NC from AD subjects.Both △α and △ f are found to have strong positive correlation with the clinical MMSE scores with statistical significance.Moreover, the proposed IRBCMA can be an alternative and more accurate algorithm for 3D volume analysis. Our findings highlight the potential usefulness of multifractal analysis, which may contribute to clarify some aspects of the etiology of AD through detection of structural changes in white matter.展开更多

BACKGROUND Several studies have reported that the walking trail making test(WTMT)completion time is significantly higher in patients with developmental coordination disorders and mild cognitive impairments.We hypothes...BACKGROUND Several studies have reported that the walking trail making test(WTMT)completion time is significantly higher in patients with developmental coordination disorders and mild cognitive impairments.We hypothesized that WTMT performance would be altered in older adults with white matter hyperintensities(WMH).AIM To explore the performance in the WTMT in older people with WMH.METHODS In this single-center,observational study,25 elderly WMH patients admitted to our hospital from June 2019 to June 2020 served as the WMH group and 20 participants matched for age,gender,and educational level who were undergoing physical examination in our hospital during the same period served as the control group.The participants completed the WTMT-A and WTMT-B to obtain their gait parameters,including WTMT-A completion time,WTMT-B completion time,speed,step length,cadence,and stance phase percent.White matter lesions were scored according to the Fazekas scale.Multiple neuropsychological assessments were carried out to assess cognitive function.The relationships between WTMT performance and cognition and motion in elderly patients with WMH were analyzed by partial Pearson correlation analysis.RESULTS Patients with WMH performed significantly worse on the choice reaction test(CRT)(0.51±0.09 s vs 0.44±0.06 s,P=0.007),verbal fluency test(VFT,14.2±2.75 vs 16.65±3.54,P=0.012),and digit symbol substitution test(16.00±2.75 vs 18.40±3.27,P=0.010)than participants in the control group.The WMH group also required significantly more time to complete the WTMT-A(93.00±10.76 s vs 70.55±11.28 s,P<0.001)and WTMT-B(109.72±12.26 s vs 82.85±7.90 s,P<0.001).WTMT-A completion time was positively correlated with CRT time(r=0.460,P=0.001),while WTMT-B completion time was negatively correlated with VFT(r=-0.391,P=0.008).On the WTMT-A,only speed was found to statistically differ between the WMH and control groups(0.803±0.096 vs 0.975±0.050 m/s,P<0.001),whereas on the WTMT-B,the WMH group exhibited a significantly lower speed(0.778±0.111 vs 0.970±0.053 m/s,P<0.001)and cadence(82.600±4.140 vs 85.500±5.020 steps/m,P=0.039),as well as a higher stance phase percentage(65.061±1.813%vs 63.513±2.465%,P=0.019)relative to controls.CONCLUSION Older adults with WMH showed obviously poorer WTMT performance.WTMT could be a potential indicator for cognitive and motor deficits in patients with WMH.展开更多

Functional connectivity reflects the synchrony of function...The main symptom of patients with Alzheimer’s disease is cognitive dysfunction. Alzheimer’s disease is mainly diagnosed based on changes in brain structure. Functional connectivity reflects the synchrony of functional activities between non-adjacent brain regions, and changes in functional connectivity appear earlier than those in brain structure. In this study, we detected resting-state functional connectivity changes in patients with Alzheimer’s disease to provide reference evidence for disease prediction. Functional magnetic resonance imaging data from patients with Alzheimer’s disease were used to show whether particular white and gray matter areas had certain functional connectivity patterns and if these patterns changed with disease severity. In nine white and corresponding gray matter regions, correlations of normal cognition, early mild cognitive impairment, and late mild cognitive impairment with blood oxygen level-dependent signal time series were detected. Average correlation coefficient analysis indicated functional connectivity patterns between white and gray matter in the resting state of patients with Alzheimer’s disease. Functional connectivity pattern variation correlated with disease severity, with some regions having relatively strong or weak correlations. We found that the correlation coefficients of five regions were 0.3–0.5 in patients with normal cognition and 0–0.2 in those developing Alzheimer’s disease. Moreover, in the other four regions, the range increased to 0.45–0.7 with increasing cognitive impairment. In some white and gray matter areas, there were specific connectivity patterns. Changes in regional white and gray matter connectivity patterns may be used to predict Alzheimer’s disease;however, detailed information on specific connectivity patterns is needed. All study data were obtained from the Alzheimer’s Disease Neuroimaging Initiative Library of the Image and Data Archive Database.展开更多
BACKGROUND Studies have demonstrated a potential role for fecal biomarkers such as fecal calprotectin(FC)and fecal lactoferrin(FL)in monitoring inflammatory bowel diseases(IBD)-Crohn's disease(CD)and ulcerative co...BACKGROUND Studies have demonstrated a potential role for fecal biomarkers such as fecal calprotectin(FC)and fecal lactoferrin(FL)in monitoring inflammatory bowel diseases(IBD)-Crohn's disease(CD)and ulcerative colitis(UC).However,their correlation to endoscopic scores,disease severity and affected intestinal surface has not been extensively investigated.AIM To correlate FL,and for comparison white blood cell(WBC)and C-reactive protein(CRP),with endoscopic scores,disease extent and location in CD and UC.METHODS Retrospective analysis in 188 patients who had FL,CRP and WBC determined within 30 d of endoscopy.Disease location,disease extent(number of intestinal segments involved),disease severity(determined by endoscopic scores),timing of FL testing in relation to colonoscopy,as well as the use of effective fast acting medications(steroids and biologics)between colonoscopy and FL measurement,were recorded.RESULTS In 131 CD and 57 UC patients,both CRP and FL-but not WBC-distinguished disease severity(inactive,mild,moderate,severe).In patients receiving fastacting(steroids or biologics)treatment in between FL and colonoscopy,FL showed a higher correlation to endoscopic scores when tested before vs after the procedure(r=0.596,P<0.001,vs r=0.285,P=0.15 for the Simple Endoscopic Score for CD;and r=0.402,P=0.01 vs r=0.054 P=0.84 for Disease Activity Index).Finally,FL was significantly correlated with the diseased mucosal surface(colon-ileocolon>small bowel)and the number of inflamed colon segments.CONCLUSION FL and CRP separated disease severity categories with FL showing lower discriminating P-values.FL showed a close correlation with the involved mucosal surface and with disease extent and was more closely correlated to endoscopy when determined before the procedure–this indicating that inflammatory activity changes associated with therapy might be rapidly reflected by FL levels.FL can accurately and timely characterize intestinal inflammation in IBD.展开更多
AIM: To investigate whether autofluorescence imaging (AFI) endoscopy can distinguish non-erosive reflux disease (NERD) from functional heartburn (FH).METHODS: In this prospective observational trial, 127 patients pres...AIM: To investigate whether autofluorescence imaging (AFI) endoscopy can distinguish non-erosive reflux disease (NERD) from functional heartburn (FH).METHODS: In this prospective observational trial, 127 patients presenting with typical reflux symptoms for > 6 mo were screened. All the participants underwent endoscopy, during which white light imaging (WLI) was followed by AFI. Finally 84 patients with normal esophageal appearance on WLI were enrolled. It was defined as being suggestive of NERD if one or more longitudinal purple lines longer than one centimeter were visualized in the distal part of the esophagus during AFI endoscopy. Ambulatory 24-h multichannel intraluminal impedance and pH monitoring was also performed. After standard proton-pump inhibitor (PPI) tests, subjects were divided into an NERD group and an FH group and the diagnostic performance of AFI endoscopy to differentiate NERD from FH was evaluated.RESULTS: Of 84 endoscopy-negative patients, 36 (42.9%) had a normal pH/impedance test. Of these, 26 patients with favorable responses to PPI tests were classified as having NERD. Finally 10 patients were diagnosed with FH and the others with NERD. Altogether, 68 (81.0%) of the 84 patients were positive on AFI endoscopy. In the NERD group, there were 67 (90.5%) patients with abnormal esophageal findings on AFI endoscopy while only 1 (10%) patient was positive on AFI endoscopy in the FH group. The sensitivity and specificity of AFI in differentiating NERD from FH were 90.5% (95%CI: 81.5%-96.1%) and 90.0% (95%CI: 55.5%-99.7%), respectively. Meanwhile, the accuracy, positive predictive value and negative predictive value of AFI in differentiating between NERD and FH were 90.5% (95%CI: 84.2%-96.8%), 98.5% (95%CI: 92.1%-99.9%) and 56.3% (95%CI: 30.0%-80.2%), respectively.CONCLUSION: Autofluorescence imaging may serve as a complementary method in evaluating patients with NERD and FH.展开更多

In this study, we sought to explore clinical characteristics of its substypes and biomarkers for prognosis of BAD. A total of 176 consecutive patients with BAD were classified into two groups: paramedianpontine artery group(PPA group, n=70) and lenticulostriate artery group(LSA group, n=106). Bivariate analyses were used to explore the relationship between white matter hyperintensities(WMHs), National Institutes of Health Stroke Scale(NIHSS) scores and prognosis evaluated by the modified Rank Scale(m RS) at 6th month after stroke. The differences in prevalence of diabetes mellitus and a history of ischemic heart disease were statistically significant between PPA group and LSA group(χ~2=8.255, P=0.004; χ~2=13.402, P〈0.001). The bivariate analyses demonstrated a positive correlation between NIHSS and poor prognosis in patients with BAD and in the two subtype groups, and a positive correlation between WMHs and poor prognosis in the PPA group. It is concluded that a significantly higher prevalence of diabetes mellitus and a history of ischemic heart disease exist in the PPA group than in the LSA group. In addition, high grades of NIHSS scores imply poor prognosis in patients with BAD and in the two subtype groups. Moreover, WMHs are a positive predictor for poor prognosis in patients in the PPA group.展开更多

Efforts to modify the disease process after the onset of cognitive symptoms have been unsuccessful in recent years. Thus, future trials must begin during the preclinical phases of the disease before symptom onset. Age related cognitive decline is often the result of two coexisting brain pathologies: Alzheimer's disease(amyloid, tau, and neurodegeneration) and vascular disease. This review article highlights some of the common neuroimaging techniques used to visualize the accumulation of neurodegenerative and vascular pathologies during the preclinical stages of dementia such as structural magnetic resonance imaging, positron emission tomography, and white matter hyperintensities. We also describe some emerging neuroimaging techniques such as arterial spin labeling, diffusion tensor imaging, and quantitative susceptibility mapping. Recent literature suggests that structural imaging may be the most sensitive and cost-effective marker to detect cognitive decline, while molecular positron emission tomography is primarily useful for detecting disease specific pathology later in the disease process. Currently, the presence of vascular disease on magnetic resonance imaging provides a potential target for optimizing vascular risk reduction strategies, and the presence of vascular disease may be useful when combined with molecular and metabolic markers of neurodegeneration for identifying the risk of cognitive impairment.展开更多

The search was updated until February 2016 and limited to articles in English.RESULTS Fifty-five original articles were included in this review, highlighting the role of single photon emission tomography and positron emission tomography. CONCLUSION To date, molecular imaging represents a useful tool to detect active disease in IBD. However, the available data need to be validated in prospective multicenter studies on larger patient samples.展开更多

文摘[Objectives]This study was conducted to establish simple, efficient, stable, standardized and practical identification methods for sugarcane resistance to white leaf disease(SCWL), and promote the breeding for sugarcane resistance to SCWL. [Methods]The identification technology of sugarcane resistance to SCWL was systematically studied and explored from the aspects of sugarcane material treatment and planting, inoculation liquid preparation, inoculation method, disease investigation, grading standard formulation, etc., and two sets of simple, efficient, stable, standardized and practical accurate identification methods for sugarcane resistance to SCWL were created for the first time, namely, the seed cane coating inoculation method and the stem-cutting inoculation method at the growth stage. The seed cane coating inoculation method includes the steps of directly screening SCWL phytoplasma, extracting juice from cane and adding 10 times of sterile water to prepare an inoculation liquid, spraying seed cane on plastic film to keep moisture, planting the inoculated materials in barrels in an insect-proof greenhouse for cultivation, investigating the incidence rate 30 d after inoculation, and evaluating the disease resistance according to the 1-5 level standard. The method of stem-cutting inoculation includes the steps of directly screening sugarcane stems carrying SCWL phytoplasma and adding 10 times of sterile water to prepare an inoculation liquid, cultivating the identification materials in an insect-proof greenhouse, dropping 100 μl of the inoculation liquid into each root incision with a pipette gun at the age of 6 months, investigating the incidence rate 20 d after planting, and evaluating the disease resistance according to the 1-5 level standard. [Results] The two methods are similar to the natural transmission method. After inoculation, SCML occurred significantly, with high sensitivity and good reproducibility. The results of resistance identification were consistent with those of natural disease in the field. Through the two inoculation methods and field natural disease investigation, the resistance of 10 main cultivars to SCML was identified, which was true and reliable. [Conclusions] This study can provide standard varieties for identification of SCML resistance in the future.

文摘[ Objective] The aim was to identify the pathogen of Gastrodia elata white silk disease and explore prevention and treatment of the disease.[ Method] Infected G. elata, collected from low-mountain regions in Yichang city of Hubei Province, were isolated and purified. Then, Koch's Postulate was adopted to inoculate the pathogen. Finally, the identification was carried out according to its biological characteristics. [ Result] The pathogen of white silk disease belongs to weak hyperparasite. Because of its poor direct invasion ability, the pathogen usually invades into host through wound infection. According to its hyphae morphology and pathogenicity, the pathogen of G. elata White Silk Disease has been identified as Sc/erotium ro/fsii Sacc., which belongs to deuteromycotina, hyphomycetes, agonomycetales, fungi of sclerotium genus. [ Conclusion ] The study will provide references for the control of G. elata White Silk Disease caused by S. rolfsii.
文摘A simple and highly sensitive analysis by electrochemical voltammetry has been developed for diagnosis of the most destructive crop disease in Thailand known as sugarcane white leaf (SCWL). Determination of the corresponding DNA interaction has been obtained from the voltammetric signals of electroactive redox methylene blue (MB) by means of cyclic and differential pulse voltammetry. In this study, a chitosan-modified glassy carbon electrode (GCE) was created by self-assembly to produce electrostatic platform for effective immobilization of the DNA. Fabrication of SCWL-DNA hybridization detection system was performed by immobilizing the ssDNA probe as a specific sensor onto chitosan-modified GCE. Hybridization of complementary DNA from the real samples could then be detected by its respective MB signal. This fabricated DNA probe sensor was shown to be capable for discriminative identification among the DNAs from SCWL plants, mosaic virus infected sugarcane and healthy sugarcane plants. Relationship between the specific hybridization signal and DNA target concentration was also observed under optimal condition. The detection limit of 4.709 ng/μl with the regression coefficient (R2) of 0.998 and overall RSD of 2.44% were obtained by response curve fit analysis. The actual SCWL-ssDNA immobilization and hybridizing event were subsequently confirmed by an observation under atomic force microscope. Thus these experiments demonstrate the first successful and effective DNA based voltammetric electrochemical determination for a verification of the specific pathogenic infection within plants from the real epidemic field.

文摘Applications of multifractal analysis to white matter structure changes on magnetic resonance imaging(MRI) have recently received increasing attentions. Although some progresses have been made, there is no evident study on applying multifractal analysis to evaluate the white matter structural changes on MRI for Alzheimer's disease(AD) research. In this paper, to explore multifractal analysis of white matter structural changes on 3D MRI volumes between normal aging and early AD, we not only extend the traditional box-counting multifractal analysis(BCMA) into the 3D case, but also propose a modified integer ratio based BCMA(IRBCMA) algorithm to compensate for the rigid division rule in BCMA. We verify multifractal characteristics in 3D white matter MRI volumes. In addition to the previously well studied multifractal feature,△α, we also demonstrated △ f as an alternative and effective multifractal feature to distinguish NC from AD subjects.Both △α and △ f are found to have strong positive correlation with the clinical MMSE scores with statistical significance.Moreover, the proposed IRBCMA can be an alternative and more accurate algorithm for 3D volume analysis. Our findings highlight the potential usefulness of multifractal analysis, which may contribute to clarify some aspects of the etiology of AD through detection of structural changes in white matter.

文摘BACKGROUND Several studies have reported that the walking trail making test(WTMT)completion time is significantly higher in patients with developmental coordination disorders and mild cognitive impairments.We hypothesized that WTMT performance would be altered in older adults with white matter hyperintensities(WMH).AIM To explore the performance in the WTMT in older people with WMH.METHODS In this single-center,observational study,25 elderly WMH patients admitted to our hospital from June 2019 to June 2020 served as the WMH group and 20 participants matched for age,gender,and educational level who were undergoing physical examination in our hospital during the same period served as the control group.The participants completed the WTMT-A and WTMT-B to obtain their gait parameters,including WTMT-A completion time,WTMT-B completion time,speed,step length,cadence,and stance phase percent.White matter lesions were scored according to the Fazekas scale.Multiple neuropsychological assessments were carried out to assess cognitive function.The relationships between WTMT performance and cognition and motion in elderly patients with WMH were analyzed by partial Pearson correlation analysis.RESULTS Patients with WMH performed significantly worse on the choice reaction test(CRT)(0.51±0.09 s vs 0.44±0.06 s,P=0.007),verbal fluency test(VFT,14.2±2.75 vs 16.65±3.54,P=0.012),and digit symbol substitution test(16.00±2.75 vs 18.40±3.27,P=0.010)than participants in the control group.The WMH group also required significantly more time to complete the WTMT-A(93.00±10.76 s vs 70.55±11.28 s,P<0.001)and WTMT-B(109.72±12.26 s vs 82.85±7.90 s,P<0.001).WTMT-A completion time was positively correlated with CRT time(r=0.460,P=0.001),while WTMT-B completion time was negatively correlated with VFT(r=-0.391,P=0.008).On the WTMT-A,only speed was found to statistically differ between the WMH and control groups(0.803±0.096 vs 0.975±0.050 m/s,P<0.001),whereas on the WTMT-B,the WMH group exhibited a significantly lower speed(0.778±0.111 vs 0.970±0.053 m/s,P<0.001)and cadence(82.600±4.140 vs 85.500±5.020 steps/m,P=0.039),as well as a higher stance phase percentage(65.061±1.813%vs 63.513±2.465%,P=0.019)relative to controls.CONCLUSION Older adults with WMH showed obviously poorer WTMT performance.WTMT could be a potential indicator for cognitive and motor deficits in patients with WMH.

文摘Diffuse changes in white matter resulting from cerebral microvascular disease contribute to cognitive impairment (Jokinen et al., 2011), declines in global functionality (Inzitari et al., 2009), and even death (Debette and Markus, 2010). Twenty years ago, estimations of the clinical incidence of ce- rebral microvascular disease approached 11 million per year in the US alone (Leary and Saver, 2003). More recent estima- tions suggest the prevalence of diffuse white matter disease and silent brain infarction approaches 20% and increases dramatically in the presence of cardiovascular risk factors (Fanning et al., 2014).

文摘The main symptom of patients with Alzheimer’s disease is cognitive dysfunction. Alzheimer’s disease is mainly diagnosed based on changes in brain structure. Functional connectivity reflects the synchrony of functional activities between non-adjacent brain regions, and changes in functional connectivity appear earlier than those in brain structure. In this study, we detected resting-state functional connectivity changes in patients with Alzheimer’s disease to provide reference evidence for disease prediction. Functional magnetic resonance imaging data from patients with Alzheimer’s disease were used to show whether particular white and gray matter areas had certain functional connectivity patterns and if these patterns changed with disease severity. In nine white and corresponding gray matter regions, correlations of normal cognition, early mild cognitive impairment, and late mild cognitive impairment with blood oxygen level-dependent signal time series were detected. Average correlation coefficient analysis indicated functional connectivity patterns between white and gray matter in the resting state of patients with Alzheimer’s disease. Functional connectivity pattern variation correlated with disease severity, with some regions having relatively strong or weak correlations. We found that the correlation coefficients of five regions were 0.3–0.5 in patients with normal cognition and 0–0.2 in those developing Alzheimer’s disease. Moreover, in the other four regions, the range increased to 0.45–0.7 with increasing cognitive impairment. In some white and gray matter areas, there were specific connectivity patterns. Changes in regional white and gray matter connectivity patterns may be used to predict Alzheimer’s disease;however, detailed information on specific connectivity patterns is needed. All study data were obtained from the Alzheimer’s Disease Neuroimaging Initiative Library of the Image and Data Archive Database.

文摘BACKGROUND Studies have demonstrated a potential role for fecal biomarkers such as fecal calprotectin(FC)and fecal lactoferrin(FL)in monitoring inflammatory bowel diseases(IBD)-Crohn's disease(CD)and ulcerative colitis(UC).However,their correlation to endoscopic scores,disease severity and affected intestinal surface has not been extensively investigated.AIM To correlate FL,and for comparison white blood cell(WBC)and C-reactive protein(CRP),with endoscopic scores,disease extent and location in CD and UC.METHODS Retrospective analysis in 188 patients who had FL,CRP and WBC determined within 30 d of endoscopy.Disease location,disease extent(number of intestinal segments involved),disease severity(determined by endoscopic scores),timing of FL testing in relation to colonoscopy,as well as the use of effective fast acting medications(steroids and biologics)between colonoscopy and FL measurement,were recorded.RESULTS In 131 CD and 57 UC patients,both CRP and FL-but not WBC-distinguished disease severity(inactive,mild,moderate,severe).In patients receiving fastacting(steroids or biologics)treatment in between FL and colonoscopy,FL showed a higher correlation to endoscopic scores when tested before vs after the procedure(r=0.596,P<0.001,vs r=0.285,P=0.15 for the Simple Endoscopic Score for CD;and r=0.402,P=0.01 vs r=0.054 P=0.84 for Disease Activity Index).Finally,FL was significantly correlated with the diseased mucosal surface(colon-ileocolon>small bowel)and the number of inflamed colon segments.CONCLUSION FL and CRP separated disease severity categories with FL showing lower discriminating P-values.FL showed a close correlation with the involved mucosal surface and with disease extent and was more closely correlated to endoscopy when determined before the procedure–this indicating that inflammatory activity changes associated with therapy might be rapidly reflected by FL levels.FL can accurately and timely characterize intestinal inflammation in IBD.
文摘AIM: To investigate whether autofluorescence imaging (AFI) endoscopy can distinguish non-erosive reflux disease (NERD) from functional heartburn (FH).METHODS: In this prospective observational trial, 127 patients presenting with typical reflux symptoms for > 6 mo were screened. All the participants underwent endoscopy, during which white light imaging (WLI) was followed by AFI. Finally 84 patients with normal esophageal appearance on WLI were enrolled. It was defined as being suggestive of NERD if one or more longitudinal purple lines longer than one centimeter were visualized in the distal part of the esophagus during AFI endoscopy. Ambulatory 24-h multichannel intraluminal impedance and pH monitoring was also performed. After standard proton-pump inhibitor (PPI) tests, subjects were divided into an NERD group and an FH group and the diagnostic performance of AFI endoscopy to differentiate NERD from FH was evaluated.RESULTS: Of 84 endoscopy-negative patients, 36 (42.9%) had a normal pH/impedance test. Of these, 26 patients with favorable responses to PPI tests were classified as having NERD. Finally 10 patients were diagnosed with FH and the others with NERD. Altogether, 68 (81.0%) of the 84 patients were positive on AFI endoscopy. In the NERD group, there were 67 (90.5%) patients with abnormal esophageal findings on AFI endoscopy while only 1 (10%) patient was positive on AFI endoscopy in the FH group. The sensitivity and specificity of AFI in differentiating NERD from FH were 90.5% (95%CI: 81.5%-96.1%) and 90.0% (95%CI: 55.5%-99.7%), respectively. Meanwhile, the accuracy, positive predictive value and negative predictive value of AFI in differentiating between NERD and FH were 90.5% (95%CI: 84.2%-96.8%), 98.5% (95%CI: 92.1%-99.9%) and 56.3% (95%CI: 30.0%-80.2%), respectively.CONCLUSION: Autofluorescence imaging may serve as a complementary method in evaluating patients with NERD and FH.
文摘The theory of branch atheromatous disease(BAD) has been commonly underused in clinical practice and research since it was proposed in 1989. In this study, we sought to explore clinical characteristics of its substypes and biomarkers for prognosis of BAD. A total of 176 consecutive patients with BAD were classified into two groups: paramedianpontine artery group(PPA group, n=70) and lenticulostriate artery group(LSA group, n=106). Bivariate analyses were used to explore the relationship between white matter hyperintensities(WMHs), National Institutes of Health Stroke Scale(NIHSS) scores and prognosis evaluated by the modified Rank Scale(m RS) at 6th month after stroke. The differences in prevalence of diabetes mellitus and a history of ischemic heart disease were statistically significant between PPA group and LSA group(χ~2=8.255, P=0.004; χ~2=13.402, P〈0.001). The bivariate analyses demonstrated a positive correlation between NIHSS and poor prognosis in patients with BAD and in the two subtype groups, and a positive correlation between WMHs and poor prognosis in the PPA group. It is concluded that a significantly higher prevalence of diabetes mellitus and a history of ischemic heart disease exist in the PPA group than in the LSA group. In addition, high grades of NIHSS scores imply poor prognosis in patients with BAD and in the two subtype groups. Moreover, WMHs are a positive predictor for poor prognosis in patients in the PPA group.
文摘Structural brain changes indicative of dementia occur up to 20 years before the onset of clinical symptoms. Efforts to modify the disease process after the onset of cognitive symptoms have been unsuccessful in recent years. Thus, future trials must begin during the preclinical phases of the disease before symptom onset. Age related cognitive decline is often the result of two coexisting brain pathologies: Alzheimer's disease(amyloid, tau, and neurodegeneration) and vascular disease. This review article highlights some of the common neuroimaging techniques used to visualize the accumulation of neurodegenerative and vascular pathologies during the preclinical stages of dementia such as structural magnetic resonance imaging, positron emission tomography, and white matter hyperintensities. We also describe some emerging neuroimaging techniques such as arterial spin labeling, diffusion tensor imaging, and quantitative susceptibility mapping. Recent literature suggests that structural imaging may be the most sensitive and cost-effective marker to detect cognitive decline, while molecular positron emission tomography is primarily useful for detecting disease specific pathology later in the disease process. Currently, the presence of vascular disease on magnetic resonance imaging provides a potential target for optimizing vascular risk reduction strategies, and the presence of vascular disease may be useful when combined with molecular and metabolic markers of neurodegeneration for identifying the risk of cognitive impairment.
文摘AIM To present the current state-of-the art of molecular imaging in the management of patients affected by inflammatory bowel disease(IBD).METHODS A systematic review of the literature was performed in order to find important original articles on the role of molecular imaging in the management of patients affected by IBD. The search was updated until February 2016 and limited to articles in English.RESULTS Fifty-five original articles were included in this review, highlighting the role of single photon emission tomography and positron emission tomography. CONCLUSION To date, molecular imaging represents a useful tool to detect active disease in IBD. However, the available data need to be validated in prospective multicenter studies on larger patient samples.

文摘目的:探讨脑白质病变(white matter lesions,WML)对帕金森病(Parkinson’s disease,PD)患者运动症状、认知功能和情绪的影响。方法:纳入2018年1月至2023年12月就诊于安徽医科大学第二附属医院神经内科PD患者123例,应用年龄相关的白质改变(age-related white matter changes,ARWMC)量表评估患者头颅MRI影像学的脑白质病变程度,根据评分结果将患者分为轻度WML组(46例),中重度WML组(77例);使用帕金森病统一评定量表第三部分、Hoehn&Yahr(H-Y)分级量表评定运动症状及疾病严重程度,简易精神状态检查(mini-mental state examination,MMSE)评估PD的认知功能,汉密尔顿抑郁量表,汉密尔顿焦虑量表评定情绪状态,Barthel指数评定患者日常生活功能状态。比较两组患者的运动症状、认知及情绪评分等差异,应用Logistic回归分析PD患者的WML与临床症状的关系。结果:中重度WML组的高血压发生率(41.6%vs.23.9%,P<0.05)高于轻度WML组。中重度WML组的H-Y分级高于轻度WML组[2.5(1.5,3.0)vs.2.0(1.5,2.5),P<0.05],UPDRS-Ⅲ评分高于轻度WML组[(34.0±16.5)vs.(24.09±11.04),P<0.01]。此外,中重度WML组的MMSE评分低于轻度WML组[24.0(18.5,27.0)vs.26.0(23.8,27.0),P<0.01],Barthel得分亦低于轻度WML组[(77.4±17.4)vs.(83.5±11.7),P<0.05],差异有统计学意义。相关分析显示,ARWMC总分与HY及UPDRS-Ⅲ得分呈正相关,与年龄亦呈正相关。ARWMC总分与MMSE得分及Barthel评分呈负相关。Logistic回归分析显示,重度WML损害与认知障碍有相关性(回归系数β=1.072,95%CI=1.078~7.918,P=0.035)。结论:WML与PD运动障碍、认知功能损害及生活质量下降相关。
