TACE efficacy was evaluated according to m RECIST. The HCC location coefficient was calculated as the distance from the central portal portion to the HCC center(mm)/liver diameter(mm) on multiplanar reconstruction images rendered(MPR) to visualize bifurcation of the right and left branches of the portal vein and HCC center. The HCC location coefficient was compared between complete response(CR) and non-CR groups in Child-Pugh grade A and B patients.RESULTS The median location coefficient of HCC among all nodules, the right lobe, and the medial segment was significantly higher in the CR group than in the non-CR group in the Child-Pugh grade A patients(0.82 vs 0.62, P < 0.001; 0.71 vs 0.59, P < 0.01; 0.81 vs 0.49, P < 0.05, respectively). However, there was no significant difference in the median location coefficient of the HCC in the lateral segment between in the CR and in the non-CR groups(0.67 vs 0.65, P > 0.05). On the other hand, in the Child-Pugh grade B patients, the HCC median location coefficient in each lobe and segment was not significantly different between in the CR and in the non-CR groups.CONCLUSION Improved TACE efficacy may be obtained for HCC in the peripheral zone of the right lobe and the medial segment in Child-Pugh grade A patients.展开更多

The aim of this study was to evaluate the strategy for initiating treatment with Sofosbuvir/Daclatasvir, and also to assess its efficacy. Patients and Methods: Included were patients aged at least 15 years, with detectable hepatitis C viremia and treated with a pan-genotypic Sofosbuvir/Daclatasvir regimen at the Centre “Hospitalier Universitaire la Référence Nationale de N’Djamena” between October 2019 and October 2023. The APRI score was used to assess fibrosis. Cure was defined by an undetectable viral load 12 weeks after the end of treatment. Results: A total of 835 patients were included (sex ratio 1.55, mean age 50.5 ± 13.73, extremes 20 and 84 years). The 60 - 69 age group was the most represented. The mean APRI score was 0.42 ± 0.29, with extremes of 0.019 and 1.84. The mean viral load was 2,316,193.086 IU/mL. All subjects had good renal function. Cytolysis was absent in 70% of cases. Without genotyping, a pan-genotypic regimen was used in all patients: Sofosbuvir (400 mg)/Daclatasvir (60 mg). The cure rate was 99.4%, with good tolerability. However, four cases of failure were recorded out of the 668 patients who underwent follow-up HCV-PCR testing after 12 weeks of treatment. Conclusion: The Sofosbuvir/Daclatasvir regimen is highly effective, with an excellent safety profile. However, the still high cost of these molecules limits their accessibility to a large proportion of patients.展开更多

The existing evaluation methods rely too much on expertise and are difficult to carry out for the dynamic evaluation on time series. In order to solve these problems, a threat evaluation method based on the AR(p)(auto regressive(AR))-dynamic improved technique for order preference by similarity to ideal solution(DITOPSIS) method is proposed. The AR(p) model is adopted to predict the missing data on the time series. Then, the entropy weight method is applied to solve each index weight at the objective point. Kullback-Leibler divergence(KLD) is used to improve the traditional TOPSIS, and to carry out the target threat evaluation. The Poisson distribution is used to assign the weight value.Simulation results show that the improved AR(p)-DITOPSIS threat evaluation method can synthetically take into account the target threat degree in time series and is more suitable for the threat evaluation under the condition of missing the target data than the traditional TOPSIS method.展开更多

Arterial phase images were acquired with GSI for reconstruction of virtual nonenhanced images and color overlay images. Digital subtraction angiography (DSA) was performed in all these patients. Two blinded and independent readers evaluated the data in two reading sessions; standard nonenhanced, arterial, portalvenous, and delayed phase images were read in session A, and the optimal monochromatic images, iodine/water based images and spectrum features were read in session B. Sensitivity and specificity were calculated with the DSA data as the reference standard. The sensitivity and specificity were compared using the χ2 test.RESULTS: DSA revealed 154 lesions in 30 patients, and 100 of them had blood supply. Overall sensitivity and specificity were 72% (72/100) and 77.8% (42/54) for session A, and 97% (97/100) and 94.4% (51/54) for session B, respectively. The sensitivity and specificity of the two reading sessions were significantly different (χ2 = 23.04, χ2 = 7.11, P < 0.05).CONCLUSION: Compared with conventional CT, GSI could significantly improve the detection of small and multiple lesions without increasing the radiation dose. Based on spectrum features, GSI could assess tumor homogeneity and more accurately identify residual tumors and recurrent or metastatic lesions during efficacy evaluation and follow-up in HCC after TACE treatment.展开更多

The review led to a dissatisfactory finding of the low quality of many qualitative evaluation studies due to their insensitivity to the following issues: philosophical basis of the study, auditability (detailed documentation of the participants and data collecting procedure), biases (acknowledgement of biases and preoccupation, and steps to deal with them), credibility or trustworthiness (triangulation, peer checking and participant verification of the findings), consistency (reliability consciousness and audit trails), and critical interpretation of the data (alternative explanations, disconfirming evidence, and limitations of the study). It was recommended that researchers be cautious when utilizing findings from the published qualitative evaluation studies; that social workers be sensitive to the issue of quality when conducting qualitative evaluation studies; that researchers be critical when judging the qualitative evaluation studies in social work; that researchers develop a clear set of guidelines for qualitative studies; that social work training institutes design qualified qualitative research courses; that a database of social work in China be established; that researchers be engaged in more qualitative studies that demonstrate high quality; that myths in qualitative research be debunked; and that adequate training for social workers on qualitative evaluation studies be provided.展开更多

文摘Background:The efficacy of combining immune checkpoint inhibitors(ICIs)with chemotherapy in neoadjuvant therapy for locally advanced gastric cancer has been explored. However, limited research exists on its effectiveness in conversion therapy, and its superiority over standalone chemotherapy remains to be elucidated. This study aims to investigate the efficacy and survival outcomes of patients treated with ICIs in combination with conversion therapy for locally advanced gastric cancer. Methods:Retrospective data from patients with locally advanced gastric cancer treated with either oxaliplatin+S-1(SOX)alone or in combination with ICIs in conversion therapywere collected.Clinical andpathological characteristics,disease-free survival, andefficacy assessments in nonoperable patients were compared between the 2 treatment groups.Efficacy was further evaluated through dynamic changes in serum markers, and patients’quality of life was assessed using the QLQ-STO22(Gastric Cancer–Specific Quality of Life Questionnaire)quality-of-life measurement scale. Results:A total of 140 patients underwent conversion therapy:80 in the SOX alone group and 60 in the SOX combined with the ICIs group. There were no significant differences in baseline characteristics between the 2 groups.Compared with the SOX alone group, the SOX combined with ICIs group exhibited a higher conversion rate(83.3%vs 75%,P=0.23),R0 resection rate(90.0%vs 83.3%,P=0.31),pathological complete response(pCR)rate(18%vs 5%,P=0.02),median disease-free survival(21.4 vs 16.9 months,P=0.007),the objective response rate in nonoperable patients(60%vs 40%,P=0.301),and median progression-free survival time(7.9 vs 5.7 months,P=0.009).The QLQ-STO22 quality-of-life assessment revealed statistically significant improvements in pain, swallowing difficulties, and dietary restrictions in the combination therapy group compared with those in the monotherapy group. The enhanced efficacy of immune combination with SOX is evident,as demonstrated by the significantly prolonged surgical duration in operated patients(206.6±26.6 min vs 197.8±19.8 min,P=0.35)and intraoperative blood loss(158.9±21.2 mL vs 148.9±25.1 mL,P=0.59).No significant differences were observed in postoperative complications. Conclusions:Compared with the SOX conversion therapy regimen,SOX combined with ICIs demonstrated higher conversion rates,R0 resection rates, pathological response rates, and disease-free survival without increasing surgical difficulty or complications.Nonoperable patients also experienced longer progression-free survival and objective response rates.
文摘AIM To evaluate the relationship between the location of hepatocellular carcinoma(HCC) and the efficacy of transarterial chemoembolization(TACE).METHODS We evaluated 115 patients(127 nodules), excluding recurrent nodules, treated with TACE between January 2011 and June 2014. On the other hand, in the Child-Pugh grade B patients, the HCC median location coefficient in each lobe and segment was not significantly different between in the CR and in the non-CR groups.CONCLUSION Improved TACE efficacy may be obtained for HCC in the peripheral zone of the right lobe and the medial segment in Child-Pugh grade A patients.

文摘Ultraviolet(UV),ozone(O_(3)),and particulate matter(PM)in air pollutants in the external environment can induce skin aging through oxidative mechanisms such as free radicals, lipid peroxidation, and protein oxidation.Antioxidant cosmetics can be used to defend against oxidative damage caused by the external environment to the skin.In China, the claimed cosmetics need to be managed according to new efficacy cosmetics, but there are currently few reports on the evaluation methods for such new efficacy cosmetics. The efficacy evaluation methods of cosmetics with antioxidant mechanisms under UV,O_(3),and PM models were reviewed, and the skin damage caused by UV,O_(3),and PM were introduced.Suggestions for the application of each model in the efficacy evaluation of antioxidant cosmetics and raw materials were given to provide ideas for new efficacy claims and technical references for establishing a new efficacy evaluation system.
文摘Introduction: The treatment of viral hepatitis C (HCV), a major public health problem, has evolved considerably since the introduction of direct-acting anti-virals (DAAs). However, the still high cost of these molecules limits their accessibility to a large proportion of patients.

文摘Background: Goniosynechialysis is a surgical procedure that has been shown to slow the progression of glaucoma in oriental eyes with chronic angle closure glaucoma. This procedure was successful in 80% of cases, and the peripheral anterior syenchiae did not exist until one year later. Nonetheless, there is little evidence of its efficacy in our context. Our study aims to investigate the efficacy of goniosynechialysis during phacoemulsification in patients with primary angle closure disease and concomitant cataract. Methods: This was an observational, prospective study. The intra-ocular pressure, need for anti-glaucoma drugs, visual acuity, the extent of synechiae, anterior chamber depth, surgical success rate, and other indicators were monitored for at least three months following surgery. Results: This study included 114 patients (118 eyes), 61 with chronic angle closure glaucoma (51.69%), 33 with primary angle closure (27.97%), and 24 with acute attack angle closure (20.34%), who were surgically treated with phacoemulsification and goniosynechialysis (Phaco-GSL). The mean intra-ocular pressure had significantly decreased three months after surgery (pre- vs post-op: 22.04 ± 10.86 vs 15.41 ± 6.06 mmHg, p-value p-value p-value p-value p-value Conclusion: Regardless of the type of glaucoma, combined phacoemulsification-goniosynechialysis is effective in lowering pressure, restoring vision, reducing the need for anti-glaucoma drugs, and preventing the synechial recurrence. Success was higher in eyes with less extensive synechiae. Phaco-GSL is safe and effective in the treatment of primary angle closure diseases with co-existing cataract.
文摘Purpose: This is a retrospective, comparative, head-to-head, not commissioned study about the efficacy of bimato-prost 0.03%, brimonidine 0.2%, brinzolamide 1%, dorzolamide 2%, and travoprost 0.004%/timolol 0.5%-fixed combinations in patients affected by na?ve open-angle glaucoma and IOP > 25 mmHg. Patients and Methods: Files from 70 patients (35 M, 35 F, mean age 69.52 y, S.D. 11.56, range: 37-87y) in our Glaucoma Service were retrospectively analyzed as long as 12 months. Every subgroup, including 14 age- and sex-matched patients, was allocated to 1 of the 5 groups of the fixed combinations monotherapy. Data recorded after 3 months follow-up were statistically analyzed by descriptive and ANOVA statistics as percentage of IOP reduction from baseline. Results: All the fixed combinations were effective in lowering IOP. The mean percentage reduction was: brimonidine/timolol 43.57%, dorzolamide/timolol 37.67%, bimatoprost/timolol 35.60%, travoprost/timolol 33.25% and brinzolamide/timolol 23.0%. The brimonidine/timolol fixed combination showed to be statistically significant more effective only than brinzolamide/timolol fixed combination (p = 0.001). Setting the α error to 5%, the power of the study is 26%, phi: 0.842. Discussion: In all this cohort of patients the target IOP was successfully achieved. All the fixed combinations used in this study had a very good profile of efficacy. Brimonidine, dorzolamide, bimatoprost and travoprost/timolol fixed combinations statistically significantly reduced the percentage of IOP from baseline (p = 0.001) more than brinzolamide/timolol fixed combination.

文摘For the target threat evaluation of warships formation air defense, the sample data are frequently insufficient and even incomplete. The Poisson distribution is used to assign the weight value.Simulation results show that the improved AR(p)-DITOPSIS threat evaluation method can synthetically take into account the target threat degree in time series and is more suitable for the threat evaluation under the condition of missing the target data than the traditional TOPSIS method.
文摘AIM: To assess the value of gemstone spectral imaging (GSI) in efficacy evaluation in hepatocellular cancer (HCC) after transcatheter arterial chemoembolization (TACE) treatment.METHODS: Thirty patients with HCC underwent GSI, including nonenhanced, arterial, portalvenous and delayed phase scans, after TACE treatment. Based on spectrum features, GSI could assess tumor homogeneity and more accurately identify residual tumors and recurrent or metastatic lesions during efficacy evaluation and follow-up in HCC after TACE treatment.
文摘Objective: This paper reviewed and examined the quality of all the qualitative evaluation studies indexed by two key search terms of “qualitative” and “evaluation” in the Social Work Abstracts database from 1990 to 2003 against a number of criteria typically adopted in the field of qualitative research. It was recommended that researchers be cautious when utilizing findings from the published qualitative evaluation studies; that social workers be sensitive to the issue of quality when conducting qualitative evaluation studies; that researchers be critical when judging the qualitative evaluation studies in social work; that researchers develop a clear set of guidelines for qualitative studies; that social work training institutes design qualified qualitative research courses; that a database of social work in China be established; that researchers be engaged in more qualitative studies that demonstrate high quality; that myths in qualitative research be debunked; and that adequate training for social workers on qualitative evaluation studies be provided.

文摘The well-developed coal electricity generation and coal chemical industries have led to huge carbon dioxide(CO_(2))emissions in the northeastern Ordos Basin. The geological storage of CO_(2) in saline aquifers is an effective backup way to achieve carbon neutrality. In this case, the potential of saline aquifers for CO_(2) storage serves as a critical basis for subsequent geological storage project. This study calculated the technical control capacities of CO_(2) of the saline aquifers in the fifth member of the Shiqianfeng Formation(the Qian-5 member)based on the statistical analysis of the logging and the drilling and core data from more than 200 wells in the northeastern Ordos Basin,as well as the sedimentary facies, formation lithology, and saline aquifer development patterns of the Qian-5 member. The results show that(1)the reservoirs of saline aquifers in the Qian-5 member, which comprise distributary channel sand bodies of deltaic plains, feature low porosities and permeabilities;(2)The study area hosts three NNE-directed saline aquifer zones, where saline aquifers generally have a single-layer thickness of 3‒8 m and a cumulative thickness of 8‒24 m;(3)The saline aquifers of the Qian-5 member have a total technical control capacity of CO_(2) of 119.25×10^(6) t.With the largest scale and the highest technical control capacity(accounting for 61%of the total technical control capacity),the Jinjie-Yulin saline aquifer zone is an important prospect area for the geological storage of CO_(2) in the saline aquifers of the Qian-5 member in the study area.

文摘Taking the Lower Permian Fengcheng Formation shale in Mahu Sag of Junggar Basin,NW China,as an example, core observation, test analysis, geological analysis and numerical simulation were applied to identify the shale oil micro-migration phenomenon. The hydrocarbon micro-migration in shale oil was quantitatively evaluated and verified by a self-created hydrocarbon expulsion potential method, and the petroleum geological significance of shale oil micro-migration evaluation was determined. Results show that significant micro-migration can be recognized between the organic-rich lamina and organic-poor lamina. The organic-rich lamina has strong hydrocarbon generation ability. The heavy components of hydrocarbon preferentially retained by kerogen swelling or adsorption, while the light components of hydrocarbon were migrated and accumulated to the interbedded felsic or carbonate organic-poor laminae as free oil.About 69% of the Fengcheng Formation shale samples in Well MY1 exhibit hydrocarbon charging phenomenon, while 31% of those exhibit hydrocarbon expulsion phenomenon. The reliability of the micro-migration evaluation results was verified by combining the group components based on the geochromatography effect,two-dimension nuclear magnetic resonance analysis, and the geochemical behavior of inorganic manganese elements in the process of hydrocarbon migration.Micro-migration is a bridge connecting the hydrocarbon accumulation elements in shale formations, which reflects the whole process of shale oil generation, expulsion and accumulation, and controls the content and composition of shale oil.The identification and evaluation of shale oil micro-migration will provide new perspectives for dynamically differential enrichment mechanism of shale oil and establishing a“multi-peak model in oil generation”of shale.
