Purpose: To report a novel technique of laparoscopic 10 mm Single Port Access IntraPeritoneal Onlay Mesh (SPA-IPOM) using bioresorbable composite mesh fixed with Endoclose suture (percutaneous subcutaneous suture) in ...Purpose: To report a novel technique of laparoscopic 10 mm Single Port Access IntraPeritoneal Onlay Mesh (SPA-IPOM) using bioresorbable composite mesh fixed with Endoclose suture (percutaneous subcutaneous suture) in 3 cases of bilateral inguinal hernia. Methods: Laparoscopic SPA-IPOM is done through a 10 mm trocar with one 10 mm instrument that has 5 mm optical lens and 5 mm channel for grasper. After inserting 10 mm trocar at umbilicus using semi-open technique, intraperitoneal anatomical landmark of inguinal her-nia is identified. A 10 × 15 cm pre-tied bioresorbable composite mesh is then placed to cover hernia defect and all three potential area of indirect, direct and femoral hernia. Using Endoclose? needle, each pair of pre-tied sutures is retrieved percutaneously through a needle wound and extracorporeal tied with knot in subcutaneous space. After the upper half of mesh is sutured to the posterior surface of abdominal wall, the lower half of mesh is fixed by hernia tacker to Symphysis Pubis, Cooper Ligament and Iliopubic tract. Re-sults: Three men, average 48 year olds were operated by laparoscopic 10 mm SPA-IPOM for bilateral in-guinal hernia repair using bioresorbable composite mesh. Average operative time was 36 minutes. No imme-diate complication. All patients were discharge on the 2nd post-operative day and average 6 months follow up has no recurrence. Conclusions: Laparoscopic SPA-IPOM is an optional operation and is much easier to be performed. Benefits include operative time saving, cosmesis, early discharge and early return to work. Bio-resorbable composite mesh prevents bowel adhesion, however, is much more expensive. Long term follow up study for complications and recurrence is needed.展开更多

First, through inductive and deductive reasoning, we propose the following hypothesis: the regional distribution of different types of PSSE would show different spatial agglomeration-decentralization tendency; and there would be distinct regional differentiation in the industrial structure of the enterprises. Second, based on data obtained from enterprises, empirical research is conducted using Gini coefficient and spatial interpolation simulation methods. Results show that: 1) The overall enterprise distribution is decentralized within a city. 2) Different types of enterprises show different spatial agglomeration-decentralization tendencies. At 3000 m×3000 m grid scale, there is an agglomeration tendency along seas and rivers in the spatial distribution of enterprises. Shanghai has been identified consistently as a hot spot. 3) There is significant regional differentiation in 12 port cities with respect to the industrial structures of enterprises. Finally, the transportization and the increase of shipping service demand, the globalization and the expansion of multinational corporate activities, the hierarchization and the cooperation among port cities as well as the decentralization and the behavioral difference between the central and local states can be seen as main driving mechanism of the spatial phenomenon.展开更多

Objective:To elucidate the awareness and approach to pre-operative human immunodeficiency virus(HIV) testing and emphasis on the attendant components of HIV testing.Methods: The study was conducted at the theatres of ...Objective:To elucidate the awareness and approach to pre-operative human immunodeficiency virus(HIV) testing and emphasis on the attendant components of HIV testing.Methods: The study was conducted at the theatres of the University of Port Harcourt Teaching Hospital among 123 nursing,anaesthetic,and operating staff.A descriptive cross sectional design was adopted with stratified sampling.The study instrument was a structured,self administered pro forma.Results:All respondents were all aware of HIV infection and all had transmission through infected blood and tissues(100.0%).88.6%had infection through needle stick injuries, 62.6%through vertical transmission,and 98.4%through blood transfusion.Sixty three percent of respondents correctly knew what preoperative testing was,while 58.5%were aware of the preoperative testing policy of the hospital.All respondents favoured the policy of preoperative testing.Attitudinal values to seropositive patients were not very different as 72.4%of respondents claimed they treated every patient as high risk,and all respondents used personal protective equipment.Conclusions:Routine HIV testing now represents a conventional means providing patients with knowledge of their HIV status.Such testing should be accompanied by informed consent,counselling,confidentiality,protection,and access to treatment.展开更多

Colonoscopy and a barium enema study revealed a submucosal tumor over the cecum, but computed tomography showed an ileal lipoma. There was no definitive diagnosis preoperatively, but ileocolic intussusception was noted during surgery. Single port laparoscopic radical right hemicolectomy was performed because intra-operative reduction failed. The histological diagnosis of the resected tumor was lipoma. Single port laparoscopic surgery has recently been proven to be safe and feasible. There are advantages compared with conventional laparoscopic surgery, such as smaller incision wounds, fewer port site complications, and easier conversion. However, there are some drawbacks which need to be overcome, such as difficulties in triangulation and instrument clashing. If there are no contraindications to laparoscopy, single port laparoscopic surgery can be performed safely and should be considered for diagnosis and treatment of intussusception in adults. Here, we report the first case of ileocolic intussusception successfully treated by single port laparoscopic surgery.展开更多

Long period (from 24 hours to a few days) and wide amplitude are their main features. Experience showed that this phenomenon is a risk for harbor navigation and mooring. It may be a serious obstacle to waterside port activities. The first observation of the data does not link the super-highs and the super-dry tides to the meteorological dynamic of sea-atmosphere interaction (i.e. Storm Effect). More, the attempt to compensate for these sea depth fluctuations by means of the well-known offshore hydrobarometric inverse parameter (1 [cm] sea level variation for ǃ [hPa] atmospheric pressure variation) underestimates the amplitude of the phenomenon. In the first years of 2000, the Italian Ministry of Transport launched a national project for the definition of thee-amplitude and time of these tidal anomalous waves. The measure of harbors hydrobarometric inverse factors showed a much more intense value than the theoretical one (computed for the offshore). These amplification factors are characteristic of each harbor and can be double (in exceptional cases even more) than offshore. The delay between the arrival times of these tidal waves (induced phenomenon) respect to the time of arrival of the atmospheric pressure variation (inducing phenomenon) depends on 1) morphology of the port basin sea floor and 2) harbor and off-shre water dynamics relationship (first approximation). For these reasons the hydro barometric inverse parameter is not effectiveness computable but it can be determined statistically.展开更多

Consequently, an examination is made as far as port sector is concerned, with an ever more demanding enhancement in operational efficiency. Moreover, a past research on West and Central Africa Ports, as well as an overview on port logistics was made, making it possible to determine what has been done (prospects) and what is still to be done (ineffectiveness), in prospective of a more reformed and efficient port. One of the principles of this paper is also to confer all the aspects that operate as obstacles to the WCA port’s improvement and development;using thus the evaluation model called EWCA (Evaluation of West and Central Africa Ports), which gives a descriptive analysis of the present situation of the latter’s port logistics, including the aid of multiple regression model to determine an impact of explanatory variables on the study probability. Applying the method of strategic development, this paper thus deals with exploring typical problems and quandaries of West and Central Africa ports. The findings suggested how to fight back or overcome the constrictions and strangulations;enabling West and Central Africa port logistics attain their desired strategic development aims, despite the ineffectiveness. Urban centers are consistently exhibiting higher temperature than its surrounding suburban rural areas. The large amount of heat generated from urban structures such as road materials and pavement materials and other ...Urban centers are consistently exhibiting higher temperature than its surrounding suburban rural areas. The large amount of heat generated from urban structures such as road materials and pavement materials and other anthropogenic heat sources are the main causes of Urban Heat Island (UHI). The sources of data for this research included primary and secondary sources. Other techniques employed for data collection were direct measurement and readings on the road and pavement materials. The research found out that, there was consistency in rising temperature at different time of the day by the different road and pavement materials. Asphalt has the greatest effect of increasing the urban temperature four degrees higher, followed by concrete, three degree rise in temperature, and earth (ordinary ground) by two degree rise and vegetation (grass) by one degree rise in temperature. The overall effect on the residents of the study area ranges from increase in hotness of the day;44.6% respondents agrees;while 34.3% says it affects the ambient air quality of the area, and other effects such as increased ground level ozone, suffocation, sleeplessness and restlessness as a result of excessive high temperature especially at night are identified in this paper. The research recommends the review or redesigning of the entire Port Harcourt city Master Plan to make provision for creation of more green areas rather than pavements and concreted areas to reduce the effects of (UHI) and ultimately improve the comfort and living conditions of the people in a the garden city Port Harcourt.展开更多

METHODS: Literature review and patient s...OBJECTIVE: To calculate and compare the cost of Port and PICC's application in long-term intravenous administration, and to support the decision making of hospital manager. METHODS: Literature review and patient survey in 2 oncology centers in China were carried out to investigate the cost and impact of Port and PICC for patients. The cost at different time of intravenous administration was calculated and compared. One-way sensitivity analysis was performed and tornado graph was drawn. RESULTS: Direct cost of Port at 0.5, 1, 1.5, and 2 years were7442, 8005, 8553, and 9131 CNY, and 4700, 9399, 14032, 18799 CNY for PICC respectively. Direct & indirect cost at 0.5, 1, 1.5, and 2 years were 9291, 11704, 14101, 16529 CNY for Port and 9697, 19393, 29023, 38787 CNY for PICC. Sensitivity analysis showed that productivity loss and device maintenance cost were the most in?uential factors to the result. CONCLUSION: Port had higher cost in short term and less in long term compared with PICC. Patients expected to get intravenous administration more than 0.5 year should use Port if both direct and indirect costs were included.展开更多

Laparoscopic surgery(LS), also termed minimal access surgery, has brought a paradigm shift in the approach to modern surgical care. Early postoperative recovery, less pain, improved aesthesis and early return to work ...Laparoscopic surgery(LS), also termed minimal access surgery, has brought a paradigm shift in the approach to modern surgical care. Early postoperative recovery, less pain, improved aesthesis and early return to work have led to its popularity both amongst surgeons and patients. Its application has progressed from cholecystectomies and appendectomies to various other fields including gastrointestinal surgery, urology, gynecology and oncosurgery. However, LS has its own package of complications. Port site infection(PSI), although infrequent, is one of the bothersome complications which undermine the benefits of minimal invasive surgery. Not only does it add to the morbidity of the patient but also spoils the reputation of the surgeon. Despite the advances in the field of antimicrobial agents, sterilization techniques, surgical techniques, operating room ventilation, PSIs still prevail. The emergence of rapid growing atypical mycobacteria with multidrug resistance, which are the causative organism in most of the cases, has further compounded the problem. PSIs are preventable if appropriate measures are taken preoperatively, intraoperatively and postoperatively. PSIs can often be treated non-surgically, with early identification and appropriate management. Macrolides, quinolones and aminoglycosides antibiotics do show promising activity against the atypical mycobacteria. This review article highlights the clinical burden, presentations and management of PSIs in LS as shared by various authors in the literature. We have given emphasis to atypical mycobacteria, which are emerging as a common etiological agent for PSIs in LS. Although the existing literature lacks consensus regarding PSI management, the complication can be best avoided by strictly abiding by the commandments of sterilization techniques of the laparoscopic instruments with appropriate sterilizing agent.展开更多

The sources of data for this research included primary and secondary sources. Other techniques employed for data collection were direct measurement and readings on the road and pavement materials. The research found out that, there was consistency in rising temperature at different time of the day by the different road and pavement materials. Asphalt has the greatest effect of increasing the urban temperature four degrees higher, followed by concrete, three degree rise in temperature, and earth (ordinary ground) by two degree rise and vegetation (grass) by one degree rise in temperature. The overall effect on the residents of the study area ranges from increase in hotness of the day;44.6% respondents agrees;while 34.3% says it affects the ambient air quality of the area, and other effects such as increased ground level ozone, suffocation, sleeplessness and restlessness as a result of excessive high temperature especially at night are identified in this paper. The research recommends the review or redesigning of the entire Port Harcourt city Master Plan to make provision for creation of more green areas rather than pavements and concreted areas to reduce the effects of (UHI) and ultimately improve the comfort and living conditions of the people in a the garden city Port Harcourt.
文摘OBJECTIVE: To calculate and compare the cost of Port and PICC's application in long-term intravenous administration, and to support the decision making of hospital manager. METHODS: Literature review and patient survey in 2 oncology centers in China were carried out to investigate the cost and impact of Port and PICC for patients. The cost at different time of intravenous administration was calculated and compared. One-way sensitivity analysis was performed and tornado graph was drawn. RESULTS: Direct cost of Port at 0.5, 1, 1.5, and 2 years were7442, 8005, 8553, and 9131 CNY, and 4700, 9399, 14032, 18799 CNY for PICC respectively. Direct & indirect cost at 0.5, 1, 1.5, and 2 years were 9291, 11704, 14101, 16529 CNY for Port and 9697, 19393, 29023, 38787 CNY for PICC. Sensitivity analysis showed that productivity loss and device maintenance cost were the most in?uential factors to the result. CONCLUSION: Port had higher cost in short term and less in long term compared with PICC. Patients expected to get intravenous administration more than 0.5 year should use Port if both direct and indirect costs were included.

文摘Laparoscopic surgery(LS), also termed minimal access surgery, has brought a paradigm shift in the approach to modern surgical care. Early postoperative recovery, less pain, improved aesthesis and early return to work have led to its popularity both amongst surgeons and patients. Its application has progressed from cholecystectomies and appendectomies to various other fields including gastrointestinal surgery, urology, gynecology and oncosurgery. However, LS has its own package of complications. Port site infection(PSI), although infrequent, is one of the bothersome complications which undermine the benefits of minimal invasive surgery. Not only does it add to the morbidity of the patient but also spoils the reputation of the surgeon. Despite the advances in the field of antimicrobial agents, sterilization techniques, surgical techniques, operating room ventilation, PSIs still prevail. The emergence of rapid growing atypical mycobacteria with multidrug resistance, which are the causative organism in most of the cases, has further compounded the problem. PSIs are preventable if appropriate measures are taken preoperatively, intraoperatively and postoperatively. PSIs can often be treated non-surgically, with early identification and appropriate management. Macrolides, quinolones and aminoglycosides antibiotics do show promising activity against the atypical mycobacteria. This review article highlights the clinical burden, presentations and management of PSIs in LS as shared by various authors in the literature. We have given emphasis to atypical mycobacteria, which are emerging as a common etiological agent for PSIs in LS. Although the existing literature lacks consensus regarding PSI management, the complication can be best avoided by strictly abiding by the commandments of sterilization techniques of the laparoscopic instruments with appropriate sterilizing agent.
