AIM:To evaluate the survival outcomes of patients with lacrimal gland adenoid cystic carcinoma who underwent eye-sparing surgery combined with ^(125I)seed implantation radiotherapy or local externalγ-ray radiotherapy.METHODS:In this retrospective comparative case series,the clinical records of 27 primary and 8 recurrent patients were reviewed.Univariate and multivariate analyses were used to identify risk factors associated with distant metastasis(DM),and the overall survival(OS)after the initial surgery was analyzed.RESULTS:The median follow-up after radiotherapy was 36 mo(range 6-120 mo).At the last follow-up after radiotherapy,26(74.3%)patients had no evidence of disease,7(20%)patients had DM,2(5.9%)patients died of DM,and 1 patient with DM was lost to follow-up.Univariate analyses showed that duration of symptoms,bone destruction,T stage classification,and wide excision surgery were risk factors influencing DM(P<0.05).The 5-year and 10-year OS rates after the initial surgery were 95.8%and 79.9%,respectively.The 5-year DM-free survival and disease-free survival rates after radiotherapy were 66.4%and 52.7%,respectively.CONCLUSION:^(125I)seed radiotherapy and local externalγ-ray radiotherapy may have similar therapeutic effects in preventing DM.Patients with T1/T2 stage disease have a better prognosis than those with T3/T4 stage disease. 展开更多

Early lesions respond well to hepatic resection or liver transplantation. However, only a few of HCC patients are suitable for surgical intervention. External beam radiation and chemotherapy is poorly efficacious. In the last 20 years, HCCs belonging to the radiosensitive tumor group has been confirmed. Along with the development of new radiotherapy technology and facilities, the research about brachytherapy(especially ^125I seed implantation therapy) has provoked more interests in the world. Radioactive seed implantation therapy is a form of interstitial brachytherapy, with the property of local "conformal radiotherapy" and the advantages of minimal invasion, convenience, high performance, and minimal adverse effects. It is a promising therapy for HCC, however the dosimetry hasn' t yet been identified and lacks verification in prospective research. AIM: To study the therapeutic value of combination o cryosurgery and 125iodine seed implantation for locally advanced pancreatic cancer. METHODS: Forty-nine patients with locally advanced pancreatic cancer (males 36, females 13), with a median age of 59 years, were enrolled in the study. Twelve patients had liver metastases. In all cases the tumors were considered unresectable after a comprehensive evaluation. Patients were treated with cryosurgery, which was performed intraoperatively or percutaneously unde guidance of ultrasound and/or computed tomography (CT), and 125iodine seed implantation, which was performed during cryosurgery or post-cryosurgery under guidance of ultrasound and/or CT. A few patients received regional celiac artery chemotherapy. RESULTS: Thirteen patients received intraoperative cryosurgery and 36 received percutaneous cryosurgery Some patients underwent repeat cryosurgery. 125Iodine seed implantation was performed during freezing procedure in 35 patients and 3-9 d after cryosurgery in 14 cases. Twenty patients, 10 of whom had hepaticmetastases received regional chemotherapy. At 3 mo after therapy, CT was repeated to estimate tumor response to therapy. Most patients showed varying degrees of tumor necrosis. Complete response (CR) of tumor was seen in 20.4% patients, partial response (PR), in 38.8%, stable disease (SD), in 30.6%, and progressive disease (PD), in 10.2%. Adverse effects associated with cryosurgery included upper abdomen pain and increased serum amylase. Acute pancreatitis was seen in 6 patients one of whom developed severe pancreatitis. All adverse effects were controlled by medical management with no poor outcome. There was no therapy-related mortality. During a median follow-up of 18 mo (range of 5-40), the median survival was 16.2 mo, with 26 patients (53.1%) surviving for 12 mo or more. Overall, the 6-, 12-, 24- and 36-mo survival rates were 94.9%, 63.1%, 22.8% and 9.5%, respectively. Eight patients had survival of 24 mo or more. The patient with the longest survival (40 mo) is still living without evidence of tumor recurrence. CONCLUSION: Cryosurgery, which is far less invasive than conventional pancreatic resection, and is associated with a low rate of adverse effects, should be the treatment of choice for patients with locally advanced pancreatic cancer. 125Iodine seed implantation can destroy the residual surviving cancer cells after cryosurgery. Hence, a combination of both modalities has a complementary effect. 展开更多

Excessive radiation exposure may lead to edema of the spinal cord and deterioration of the nervous system. Magnetic resonance imaging can be used to judge and assess the extent of edema and to evaluate pathological changes and thus may be used for the evaluation of spinal cord injuries caused by radiation therapy. Radioactive ^125I seeds to irradiate 90% of the spinal cord tissue at doses of 40–100 Gy (D90) were implanted in rabbits at T10 to induce radiation injury, and we evaluated their safety for use in the spinal cord. Diffusion tensor imaging showed that with increased D90, the apparent diffusion coefficient and fractional anisotropy values were increased. Moreover, pathological damage of neurons and microvessels in the gray matter and white matter was aggravated. At 2 months after implantation, obvious pathological injury was visible in the spinal cords of each group. Magnetic resonance diffusion tensor imaging revealed the radiation injury to the spinal cord, and we quantified the degree of spinal cord injury through apparent diffusion coefficient and fractional anisotropy. 展开更多

The present study investigated the nuclear transportation phenomenon of ^125I-nerve growth factor (NGF) and the DNA-damaging changes to U251 cells using microautoradiography and single cell electrophoresis. The results showed that ^125I-NGF inhibited the survival of p53 mutant U251 human glioma cell/tumor and enhanced the therapeutic effectiveness of vincristine in in vivo and in vitro models. In vitro experiments showed ^125I-NGF was transported into the nucleus and damaged the DNA in U251 cells. Moreover, ^125I-NGF locked the U251 cells in the G2 phase. Further investigation showed that ^125I-NGF decreased cyclin B1 protein levels in a dose dependent manner, but the level of cyclin B1 mRNA expression remained unchanged. ^125I-NGF increased phosphorylated Chkl, Chk2 and Cdc25c protein levels in U251 cells, but did not influence p53 and p21 protein expression. Moreover, ^125I-NGF and vincristine exhibited synergistic effects on reducing cyclin B1 protein levels. These results indicate that ^125I-NGF can provide anti-tumor effects by activating the ATM and ATR pathways through DNA damage. 展开更多

Objective The aim of this study was to investigate the relationship between miR-7-5p expression and intertissue-^(125)I irradiation sensitivity in pancreatic cancer tissues and to analyze the function of target genes.Methods Thirty-seven patients with unresectable pancreatic ductal adenocarcinoma(PDAC)treated with radioactive ^(125)I seed implantation were enrolled.RT-PCR was used to detect the expression level of miR-7-5p in cancer tissues and analyze the relationship between miR-7-5p expression and ^(125)I radiation sensitivity.Bioinformatic software and online tools were used to predict the miR-7-5p target genes and analyze their functional annotation and pathway enrichment.Results Radioactive ^(125)I seed implantation was followed up for 2 months.The objective response rate of the miR-7-5p high expression group was 65.0%(13/20),whereas the objective response rate of the miR-7-5p low expression group was 5.88%(1/17),and the difference between the two groups was statistically significant(χ^(2)=13.654,P<0.001).A total of 187 target genes were predicted using three databases.GO functional annotation showed that target genes were mainly involved in cellular response to insulin stimulus,regulation of gene expression by genetic imprinting,cytosol,peptidyl-serine phosphorylation,bHLH transcription factor binding,cargo loading into vesicles,cellular response to epinephrine stimulus,and nucleoplasm.KEGG pathway enrichment analysis showed that target genes were mainly involved in the ErbB signaling pathway,HIF-1 signaling pathway,axon guidance,longevity regulatory pathway,endocrine resistance,glioma,choline metabolism in cancer,and EGFR tyrosine kinase inhibitor drug resistance.Molecular complex detection analysis by Cytoscape revealed that PIGH,RAF1,EGFR,NXT2,PIK3CD,PIK3R3,ERBB4,TRMT13,and C5orf22 were the key modules of miR-7-5p target gene clustering.Conclusion The expression of miR-7-5p in pancreatic cancer tissues positively correlated with the radiosensitivity of ^(125)I seeds.Via targeted gene regulation,miR-7-5p acts on the network of multiple signaling pathways in PDAC and participates in its occurrence and development.Thus,miR-7-5p may become a predictive index of ^(125)I seed implantation therapy sensitivity in PDAC patients. 展开更多

目的 :探讨12 5I组织间植入疗法疗效及其毒副作用。方法 :对 39例各种恶性肿瘤病人实施手术根治术后 ,术中同时将一定数量的12 5I粒子按一定间距排布植入特定部位的组织内。术前、术中、术后分别在一定位置使用γ数字辐射仪对植入粒子... 目的 :探讨12 5I组织间植入疗法疗效及其毒副作用。方法 :对 39例各种恶性肿瘤病人实施手术根治术后 ,术中同时将一定数量的12 5I粒子按一定间距排布植入特定部位的组织内。术前、术中、术后分别在一定位置使用γ数字辐射仪对植入粒子工作人员和病人进行照射量率的监测。术后结合病人情况实施相应的化疗 ,术前、术后不同时间测定病人血清免疫学指标 (IgA、IgG、IgM、C3、C4、B、C -RP)和外周血白细胞。结果 :术后随访期 (1周~ 16个月 )内 ,患者无异常症候群发生 ,未见相关放射病及其并发症的发生也无12 5I粒子脱落或排出体外事件发生。经X线、ECT、CT、B超检查无一例发现复发灶或新的转移灶。ECT扫描结果提示 ,粒子植入部位放射性分布均匀。血清免疫学指标和外周血白细胞检查 ,术后第 3周各项指标 ,均有不同程度的降低 ,但经方差分析统计学处理 ,无显著性差异 (P >0 .0 5 )。结论 :肿瘤12 5I粒子术中组织间永久植入疗法简便、安全、疗效显著、毒副作用极小 ,可单独实施也可与其他疗法综合应用。病人易接受 ,值得推广应用。 展开更多

目的比较研究^(125)I放射性粒子植入联合多靶动脉减量灌注化疗治疗ⅢB期非小细胞肺癌(NSCLC)与联合静脉化疗临床疗效的差异。方法选取在2013年7月至2018年12月接受局部^(125)I放射性植入治疗联合化疗的ⅢB期NSCLC患者作为研究对象,收集... 目的比较研究^(125)I放射性粒子植入联合多靶动脉减量灌注化疗治疗ⅢB期非小细胞肺癌(NSCLC)与联合静脉化疗临床疗效的差异。方法选取在2013年7月至2018年12月接受局部^(125)I放射性植入治疗联合化疗的ⅢB期NSCLC患者作为研究对象,收集患者各项临床特征资料,包括性别、年龄、体力状况、吸烟指数、病理类型、化疗方式、肿瘤体积(gross tumor volume,GTV)和100%靶区所受剂量(D100)等,接受动脉灌注化疗的患者为MAC组,接受静脉化疗的患者为SVC组。随访并行短期疗效评价和记录生存时间。结果57例SVC组CR、PR、SD和PR患者分别为1.8%(1/57)、47.4%(27/57)、45.6%(26/57)和5.3%(3/57),OR为49.1%;19例MAC组患者CR、PR、SD和PR分别为15.8%(3/19)、52.6%(10/19)、21.1%(4/19)和10.5%(2/19),OR为68.4%;两组差异有统计学意义(χ^(2)=8.192,P=0.042)。SVC组患者1、2和3年生存率分别为73.7%、26.3%和8.8%,mOS为16.3个月;MAC组患者1、2和3年生存率为84.2%、42.1%和15.8%,mOS为20.0个月;两组差异有统计学意义(χ^(2)=3.916,P=0.048)。亚组分析显示单一支气管动脉供血的患者mOS为25.0个月,多靶动脉供血患者mOS为16.8个月,两组患者OS差异明显(χ^(2)=3.944,P=0.047)。应用Cox模型单因素分析显示:GTV(HR=3.090,95%CI:1.057-8.884,P=0.036)、D100(HR=4.463,95%CI:1.261-15.798,P=0.020)、供血动脉(HR=5.885,95%CI:1.877-18.449,P=0.002)与OS显著相关,多因素分析显示:多靶动脉供血是影响OS的独立危险因素(HR=4.776,95%CI:1.265-18.039,P=0.021)。结论^(125)I放射性粒子植入治疗联合多靶动脉减量灌注化疗治疗ⅢB期NSCLC在局部病灶控制、延长生存时间方面优于联合静脉化疗,可作为治疗手段之一进行临床推广。 展开更多

目的探讨125I放射粒子植入联合手术去势治疗局部晚期前列腺癌的临床效果。方法晚期前列腺癌患者82例,治疗组42例,采用经直肠B超引导下经直肠植入125I放射粒子并同时行双侧睾丸切除治疗。对照组40例施行双侧睾丸切除术。82例患者术后同... 目的探讨125I放射粒子植入联合手术去势治疗局部晚期前列腺癌的临床效果。方法晚期前列腺癌患者82例,治疗组42例,采用经直肠B超引导下经直肠植入125I放射粒子并同时行双侧睾丸切除治疗。对照组40例施行双侧睾丸切除术。82例患者术后同时服用比卡鲁胺内分泌化疗药物。结果所有患者手术过程顺利,术后随访24~48个月,中位随访时间36.5个月。治疗组平均植入粒子(38±15)粒。无因经直肠穿刺引起的严重并发症。1例术后16个月发生骨转移,有2例在术后平均18.6个月出现PSA生化复发,累计PSA无进展患者生存率为92.8%。对照组中1例于术后23个月死亡,3例在术后14个月发生骨转移,有5例在术后平均15.8个月出现PSA生化复发,累计PSA无进展患者生存率为77.5%。结论对于局部晚期前列腺癌。 展开更多

目的观察自制^(125)Ⅰ粒子敷贴式鼻饲营养管用于治疗食管癌恶性梗阻的可行性和安全性。方法对14例经病理证实的食管癌并伴Ⅲ~Ⅳ级吞咽困难高龄患者行DSA引导下^(125)Ⅰ粒子敷贴式鼻饲营养管置入术,使^(125)Ⅰ粒子完全覆盖肿瘤段上下各2... 目的观察自制^(125)Ⅰ粒子敷贴式鼻饲营养管用于治疗食管癌恶性梗阻的可行性和安全性。方法对14例经病理证实的食管癌并伴Ⅲ~Ⅳ级吞咽困难高龄患者行DSA引导下^(125)Ⅰ粒子敷贴式鼻饲营养管置入术,使^(125)Ⅰ粒子完全覆盖肿瘤段上下各2 cm行近距离放射治疗;观察技术成功率、临床治疗成功率、手术时间、肿瘤吸收剂量[D90(90%肿瘤体积所接受的剂量)]、有无核素脱落、术后并发症及肿瘤控制情况,并比较术前与术后6~8周Karnofsky评分及Neuhaus吞咽困难分级差异。结果对所有患者均成功置入^(125)Ⅰ粒子敷贴式鼻饲营养管,技术成功率100%(14/14);手术时间(27.37±4.82)min。临床治疗成功率85.71%(12/14),D90为(52.19±6.64)Gy。9例(9/14,64.29%)患者诉治疗期间胸骨后间断性疼痛,可耐受。未发生核素脱落及其他并发症,肿瘤局部控制率为92.86%(13/14)。术后6~8周Karnofsky评分(t=-2.75,P=0.01)及Neuhaus分级(Z=9.72,P<0.01)均较术前明显改善。结论^(125)Ⅰ粒子敷贴式鼻饲营养管制作及置入简单、便捷,可同时实现近距离放射治疗和胃肠营养。 展开更多

目的观察^(125)I粒子植入与间歇性内分泌联合治疗对中晚期前列腺癌临床疗效,以及对患者癌细胞PC-3、血管内皮生长因子(VEGF)的影响。方法选取2019年3月至2021年5月收治的54例晚期前列腺癌患者为研究对象,根据治疗方法的不同随机分为观... 目的观察^(125)I粒子植入与间歇性内分泌联合治疗对中晚期前列腺癌临床疗效,以及对患者癌细胞PC-3、血管内皮生长因子(VEGF)的影响。方法选取2019年3月至2021年5月收治的54例晚期前列腺癌患者为研究对象,根据治疗方法的不同随机分为观察组和对照组,每组27例。对照组患者采用间歇性内分泌治疗(IHT),观察组患者采用^(125)I粒子植入与间歇性内分泌联合治疗。比较2组治疗2个月后的临床疗效、国际前列腺症状评分(IPSS)、最大尿流率(Qmax)、前列腺体积(PV)、前列腺特异性抗原(PSA)、癌细胞PC-3凋亡率及VEGF水平的变化,观察2组治疗期间不良反应的发生率。结果2组患者治疗后,观察组患者治疗有效率(77.78%)明显高于对照组治疗有效率(44.44%),组间比较具有统计学意义(P<0.05)。2组患者治疗前IPSS、Qmax、PV、PSA水平,组间比较无统计学意义(P>0.05),治疗后观察组患者IPSS、PV、PSA水平低于对照组,Qmax水平高于对照组,组间比较具有统计学意义(P<0.05)。2组患者治疗后癌细胞PC-3细胞凋亡率较前明显升高、而VEGF水平较治疗前比较显著降低,且观察组较对照组上述指标改善更明显(P<0.05),组间比较具有统计学意义(P<0.05)。2组患者治疗期间不良反应发生率,观察组显著低于对照组,组间比较具有统计学意义(P<0.05)。结论^(125)I粒子植入与间歇性内分泌联合治疗晚期前列腺癌疗效显著,还能抑制癌细胞PC-3的增殖速度和病灶内血管的生长速度,且不良反应发生率低,具有一定的临床意义。 展开更多

Objectives: To explore the clinical efficacy and survival of CT-guided Iodine-125 radioactive seed implantation in the treatment of stage Ⅳ primary hepatocellular carcinoma.Methods: A retrospective study of 62 patients with primary hepatocellular carcinoma in our hospital from January2017 to December 2018 [60 males, 2 females, age(52.76 ± 10.82) years old], All patients were implanted with Iodine-125 radioactive seeds under CT guidance, followed up regularly after operation to observe the clinical efficacy, including comparison of changes in cancer size before and after treatment, tumor marker AFP, and improvement in complications such as abdominal pain and ascites. Follow-up 3–36 months to assess patient survival.Results: Among the 62 patients, 3 months after Iodine-125 radioactive seed implantation, 5 cases(8.1%) had complete remission of cancer, 33 cases(53.2%) had partial remission, 12 cases(19.4%) had stable lesions, and 12 cases(19.4%) had disease progression. The effective rate was 61.3%. The tumor volume(31.44 ± 14.51 cm3) was significantly smaller than before(50.96 ± 30.13 cm3)(t=5.303, p < 0.05). The tumor marker AFP(69.28 ±50.99) ug/L of 3 months after implantation was significantly lower than that before treatment(90.63 ± 68.58)ug/L(t=3.702, P < 0.05). The average survival time of Iodine-125 seed implantation for stageⅣhepatocellular carcinoma is 11.47 ± 0.85 months, and the median survival time is 9 months. The survival time of the group with better pathological differentiation(grade Ⅰ+ⅡⅠ) was significantly better than that of the group with poor differentiation(grade Ⅲ+Ⅳ)(x2=6.869 p < 0.05). Among the 38 patients with different degrees of abdominal pain,22 patients improved better than before;15 of 28 patients with different degrees of ascites were better than before. All patients had no serious complications related to treatment.Conclusions: Iodine-125 radioactive seed implantation therapy can safely and effectively treat hepatocellular carcinoma, and relieve the clinical symptoms of abdominal pain and ascites. 展开更多

目的:探讨^(125)I放射性粒子植入治疗肿瘤病人的心理弹性、家庭亲密度与适应性和孤独感的相关性。方法:采取便利抽样法,选取106例行^(125)I放射性粒子植入治疗的肿瘤病人作为研究对象,调查病人的一般资料,采用心理弹性量表中文版(CD-RI... 目的:探讨^(125)I放射性粒子植入治疗肿瘤病人的心理弹性、家庭亲密度与适应性和孤独感的相关性。方法:采取便利抽样法,选取106例行^(125)I放射性粒子植入治疗的肿瘤病人作为研究对象,调查病人的一般资料,采用心理弹性量表中文版(CD-RISC)评价病人的心理弹性水平,并使用家庭亲密度与适应性量表中文版(FACESIICV)及孤独感自评量表(UCLA)进行调查问卷。结果:^(125)I放射性粒子植入治疗肿瘤病人孤独感量表得分为(40.01±8.63)分,心理弹性量表得分为(61.79±14.54)分,家庭亲密度与适应性量表得分为(116.32±18.80)分;不同性别、职业、文化程度、家庭人均月收入的孤独感得分差异均有统计学意义(P<0.01),不同性别、家庭人均月收入心理弹性得分和家庭亲密度与适应性得分差异有统计学意义(P<0.01)。106例^(125)I放射性粒子植入治疗的肿瘤病人的孤独感量表得分和心理弹性量表得分呈显著负相关(r=-0.912,P<0.01),心理弹性量表得分和家庭亲密度与适应性量表得分呈显著正相关关系(r=0.884,P<0.01),孤独感量表得分和家庭亲密度与适应性量表得分呈显著负相关关系(r=-0.907,P<0.01);纳入性别、职业、文化程度、家庭人均月收入、心理弹性得分、家庭亲密度与适应性得分进行孤独感影响因素多元线性逐步回归分析,家庭亲密度与适应性得分和家庭人均月收入均有统计学意义(P<0.01)。结论:不同性别、职业、文化程度、家庭人均月收入是^(125)I放射性粒子植入治疗的肿瘤病人孤独感的影响因素,及早对孤独感程度进行评估并提前进行有效干预,改善病人家庭亲密度与适应性情况,对于降低其孤独感程度具有显著效果,对促进病人早日回归家庭与社会具有重大的意义。 展开更多

目的 评价^(125)I粒子三链鼻饲营养管近距离放射治疗晚期食管癌的近期疗效。方法 2020年1月至2021年5月采用经鼻置入^(125)I粒子三链鼻饲营养管近距离放射治疗18例晚期食管癌患者,6周后拔除鼻饲营养管。记录技术成功情况、90%体积肿瘤... 目的 评价^(125)I粒子三链鼻饲营养管近距离放射治疗晚期食管癌的近期疗效。方法 2020年1月至2021年5月采用经鼻置入^(125)I粒子三链鼻饲营养管近距离放射治疗18例晚期食管癌患者,6周后拔除鼻饲营养管。记录技术成功情况、90%体积肿瘤吸收剂量(D90)、术后并发症。评价术后6周食管通畅情况、肿瘤最大径、局部控制率、Stooler吞咽困难分级、VAS疼痛评分和Karnofsky功能状态评分。结果 18例患者均达到技术成功,平均D90为46.19 Gy,平均止血时间12.4 d,术后均达到局部控制。与术前相比,术后6周16例患者的食管通畅,肿瘤最大径平均缩小1.23 cm, Stooler吞咽困难评分平均下降了2.6分,VAS评分平均下降3.28分,Karnofsky评分平均升高了10分,差异均有统计学意义(均P<0.05)。无同位素脱落、大出血、食管穿孔等严重并发症。结论^(125)I粒子三链鼻饲营养管近距离放射治疗晚期食管癌安全可行,近期疗效显著。 展开更多
