Objective:Compared with long-term renal replacement therapy,kidney transplantation is the ideal treatment for end-stage renal disease(ESRD),significantly extending patient life and improving quality of life.Kidney tra...Objective:Compared with long-term renal replacement therapy,kidney transplantation is the ideal treatment for end-stage renal disease(ESRD),significantly extending patient life and improving quality of life.Kidney transplant patients need to adhere to lifelong immunosuppressive medication regimens,but their medication adherence is generally poor compared with other organ transplant recipients.Medication adherence is closely related to medication literacy and psychological status,yet related studies are limited.This study aims to investigate the current status of medication adherence,inner strength,and medication literacy in kidney transplant patients,analyze the relationships among these 3 factors,and explore the mediating role of inner strength in the relationship between medication literacy and medication adherence.Methods:A cross-sectional survey was conducted from March to October 2023 involving 421 patients aged≥18 years who visited kidney transplantation outpatient clinics at 4 tertiary hospitals in Hunan Province.The inner strength,medication literacy,and medication adherence of kidney transplant patients were investigated using the Inner Strength Scale(ISS),the Chinese version of the Medication Literacy Assessment in Spanish and English(MedLitRxSE),and the Chinese version of the Morisky Medication Adherence Scale-8(C-MMAS-8),respectively.Univariate analysis was performed to examine the effects of demographic and clinical data on medication adherence.Correlation analysis was conducted to explore the relationships among medication literacy,medication adherence,and inner strength.Significant variables from univariate and correlation analyses were further analyzed using multiple linear regression,and the mediating effect of inner strength was explored.Results:Among the 421 questionnaires collected,408 were valid,with an effective rate of 96.91%.The scores of C-MMAS-8,MedLitRxSE,and ISS were 6.64±1.16,100.63±14.67,and 8.47±4.03,respectively.Among the 408 patients,only 86(21.08%)patients had a high level of medication adherence,whereas 230(56.37%)patients had a medium level of medication adherence,and 92(22.55%)patients had poor medication adherence.Univariate analysis indicated that the kidney transplant patients’age,marital status,education levels,years since their kidney transplant operation,number of hospitalizations after the kidney transplant,and adverse drug reactions showed significant differences in medication adherence(all P<0.05).Correlation analysis showed that inner strength positively correlated with both medication literacy(r=0.183,P<0.001)and medication adherence(r=0.201,P<0.001).Additionally,there was a positive correlation between medication adherence and medication literacy(r=0.236,P<0.001).Inner strength accounted for 13.22%of the total effect in the mediating role between medication literacy and medication adherence.Conclusion:The level of medication adherence among kidney transplant patients needs improvement,and targeted intervention measures are essential.Inner strength mediates the relationship between medication literacy and medication adherence in these patients.Healthcare professionals should focus on enhancing medication literacy and supporting patients’inner strength to improve medication adherence.展开更多

It is pointed out that mastic Epure content, temperature and loading pressure are the main factors influencing permanent deformation of guss-asphalt; fine to coarse aggregate proportion influences its maximum bending strain and crack energy at low temperature. Many patients with AF are elderly and may suf...Background Medication adherence is an integral part of the comprehensive care of patients with atrial fibrillation (AF) receiving oral anticoagulations (OACs) therapy. Many patients with AF are elderly and may suffer from some form of cognitive impairment. This study was conducted to investigate whether cognitive impairment affects the level of adherence to anticoagulation treatment in AF patients. Me- thods The study involved 111 AF patients (mean age, 73.5±8.3 years) treated with OACs. Cognitive function was assessed using the Mini Mental State Examination (MMSE). The level of adherence was assessed by the 8-item Mot^sky Medication Adherence Scale (MMAS-8). Scores on the MMAS-8 range from 0 to 8, with scores 〈 6 reflecting low adherence, 6 to 〈 8 medium adherence, and 8 high adherence. Re- sults 46.9% of AF patients had low adherence, 18.8% had moderate adherence, and 33.3% had high adherence to OACs. Patients with lower adherence were older than those with moderate or high adherence (76.6 ±8.7 vs. 71.3 ~ 6.4 vs. 71.1 ± 6.7 years) and obtained low MMSE scores, indicating cognitive disorders or dementia (MMSE = 22.3 ± 4.2). Patients with moderate or high adherence obtained high MMSE test results (27.5 ±1.7 and 27,5 ± 3.6). According to Spearman's rank correlation, worse adherence to treatment with OACs was determined by older age (rs = -0.372) and lower MMSE scores (rs = 0.717). According to multivariate regression analysis, the level of cognitive function was a significant independent predictor of adherence (b = 1.139). Conclusions Cognitive impairment is an independent determinant of compliance with pharmacological therapy in elderly patients with AF. Lower adherence, beyond the assessment of cognitive function, is related to the age of patients.展开更多

Better compliance can be obtained by reducing the number of tablets and times of administration.Given that the 5-ASA formulations have different delivery systems that split the active moiety in various regions of the intestine,it is particularly important that an adequate dose of the drug arrives at the inflamed part of the colon.5-ASA Multi matrix(MMx)is a novel,high strength(1.2 g),oral formulation designed for oncedaily dosing.It releases the active moiety throughout the colon.Different studies with this compound have shown that it is as effective as 5-ASA enema in the treatment of mild-to-moderate,left-sided UC,and is comparable to a pH-dependent,delayed release 5-ASA (Asacol ),even if given once daily.Recently,the effectiveness in the acute phase of UC has been confirmed also in maintenance.In conclusion,at present,5-ASA MMx seems theoretically the best agent for maintaining patient compliance,and consequently,treatment effectiveness.展开更多
Objective:This study aims to evaluate the effects of a symptom management intervention(SMI)based on symptom management group sessions combined with a mobile health(mHealth)application(app)on the knowledge of symptom m...Objective:This study aims to evaluate the effects of a symptom management intervention(SMI)based on symptom management group sessions combined with a mobile health(mHealth)application(app)on the knowledge of symptom management,the certainty of symptom self-management,symptom severity,symptom distress,medication adherence,social support,and quality of life among persons living with HIV(PLWH)in China.Methods:A parallel randomized controlled trial with 61 PLWH was conducted in Shanghai,China.The participants in the control group(n¼30)downloaded the Symptom Management(SM)app according to their needs and preferences,and received routine follow-ups.The participants in the intervention group(n¼31)were guided to download and use the SM app,and received four tailored weekly group sessions at routine follow-ups.Each group session lasted for approximately 2 h and targeted one of the major modules of the SM app.All the outcomes were assessed at baseline and post-intervention.The study was registered with the Chinese Clinical Trial Registry(ChiCTR1900024821).Results:The symptom management knowledge and certainty of symptom self-management were significantly improved after the intervention(all P<0.01).Compared with the control group,the scores of symptoms reasons knowledge score improved 11.47 points(95%CI:3.41,19.53)and scores of symptoms self-management knowledge score improved 12.80 points(95%CI:4.55,21.05)in the intervention group after controlling for covariates.However,other outcomes did not show statistically significant differences between the intervention group and the control group(P>0.05).Conclusion:The SMI could improve PLWH’s symptom management knowledge and certainty of symptom self-management.Multi-center studies with larger sample sizes and longer follow-ups are needed to further understand the effects of SM app on ameliorating symptom severity and symptom distress.More innovative strategies are also needed to promote and maintain the sustainability of the SM app.展开更多

This retrieval model suggests an extension to the existing language modeling (LM) approach to IR by introducing dependency models for both query and document. Relevance between document and query is then evaluated by reference to the Kullback-Leibler divergence between their dependency models. This paper introduces a novel hybrid dependency structure, which allows integration of various forms of dependency within a single framework. A pseudo relevance feedback based method is also introduced for constructing query dependency model. The basic idea is to use query-relevant top-ranking sentences extracted from the top documents at retrieval time as the augmented representation of query, from which the relationships between query terms are identified. A Markov Random Field (MRF) based approach is presented to ensure the relevance of the extracted sentences, which utilizes the association features between query terms within a sentence to evaluate the relevance of each sentence. This dependency retrieval model was compared with other traditional retrieval models. Experiments indicated that it produces significant improvements in retrieval effectiveness.展开更多
Objective:To standardize the distress management of gastric cancer patients receiving chemotherapy,the adapted Cancer-related Distress Management Guidelines were implemented in nursing practice among gastric cancer pa...Objective:To standardize the distress management of gastric cancer patients receiving chemotherapy,the adapted Cancer-related Distress Management Guidelines were implemented in nursing practice among gastric cancer patients receiving chemotherapy based on A Guideline Adaptation and Implementation Planning Resource(CAN-IMPLEMENT).Methods:Based on the theoretical framework of CAN-IMPLEMENT,A multidisciplinary team was established,barriers and facilitators obstacles of guidelines implementation in medical oncology units were assessed,corresponding solutions were formulated,the guidelines implementation process was monitored,and implementation results were evaluated.Results:The multidisciplinary team developed review criteria,standardized work paths,assessment tools,training manuals for healthcare professionals,education manuals for patients and their caregivers.After guidelines implementation,the completion rate of the distress management record came up to 97.9%(189/193).From September 2017 to December 2018,the compliance of medical staff on most items in the audit checklist was improved,ranging from 57.1%(100/175)to 100.0%(193/193).The positive distress rate of gastric cancer patients receiving chemotherapy was decreased from 22.7%(32/141)to 9.3%(18/193)(P<0.05),and the Median(range)of the distress score declined from 2(0e9)to 0(0e7)(P<0.001).Conclusions:The implementation of guidelines based on CAN-IMPLEMENT promotes the establishment of a distress management system in the medical oncology units.The review standards,standardized work paths,and evaluation tools for distress in cancer patients formulated by the multidisciplinary team had clinical applicability and effectiveness.Quality control in the practice of distress management was effective.The compliance of healthcare professionals with distress management was improved.The distress of gastric cancer patients receiving chemotherapy was alleviated effectively.展开更多

Methods: A descriptive-c...Objectives: The study assessed if the level of knowledge of HIV-infected about HAART and waiting-times in the PHC (primary healthcare) clinic have an influence on antiretroviral adherence. Methods: A descriptive-cross-sectional study was conducted in South Africa. Data collected uses a standardized-questionnaire and face-to-face-exit interviews. Pill-count technique was performed and a value of≥ 95% acceptable. Data were analysed using SPSS. Univariate-factors associated with poor-adherence to knowledge about HAART and waiting times were assessed using ANOVA and p ≤ 0.05 considered statistically significant. Key findings: Of 86 enrolled, 63(73.3%) were females and 23(26.7%) males, with mean-age (± SD) of 35.6(±9.6) years and on HAART for 35.5(± 31.8) months ranging from 1-137. Of these, 27(31.40%) and 25(29.07%) were on WHO stages 2 and 3 respectively. Adherence-rates computed from 32 patients, 23(71.9%) revealed poor adherence-rates. The level of knowledge about HAART in terms of names of tablets, correct-dose, frequency, adverse-effects had no influence on ARV-adherence (p _〉 0.05). Of 23 non-compliant, 10 (40%) gave the reason of drugs-unavailability, 7(30%) adverse-effects, 5(20%) drugs' complexity, and 1(10%) too busy to take them. Waiting areas associated with poor ARV-adherence were reception (p = 0.028), doctors (p = 0.027), while nurse's station (p = 0.29) and pharmacy (p = 0.43) revealed acceptable ARV-adherence.展开更多
Objectives:Compliance to treatment is important for the management of type 2 diabetes mellitus(DM)and the prevention of complications.The purpose of the study was to develop a scale and test its psychometric propertie...Objectives:Compliance to treatment is important for the management of type 2 diabetes mellitus(DM)and the prevention of complications.The purpose of the study was to develop a scale and test its psychometric properties for the treatment compliance of the diabetic Turkish population.Methods:This multicenter study was conducted in four training and research hospitals.The convenience sample consisted of 350 patients with type 2 DM.The items of the scale were determined after a literature review and qualitative interviews with the patients.Items were psychometrically analyzed.Content validity of the scale was evaluated using opinions from experts and a pilot study.Principal component analysis and the varimax rotation technique were used to evaluate construct validity in exploratory factor analysis.Criterion validity was evaluated with the Attitudes Towards Diabetes Scale,a subgroup of the Diabetes Care Profile scale.Reliability was evaluated with Cronbach'sαcoefficient and test-retest analysis of internal consistency.Results:The scale consisted of 7 factors that explained 47.36%of the total variance.The KMO test was conducted to determine whether the sample size was sufficient before the factor analysis.The KMO test result of the data was 0.75.The Cronbach'sαvalue of the sample was 0.77.The test-retest reliability analysis result was r=0.991.We found a positive correlation between total scores of the developed scale and the Attitudes Towards Diabetes Scale(r=0.31).Conclusion:The results of the study demonstrated that the scale with 30 items is a valid and reliable scale for the evaluation of patient compliance with type 2 DM treatment.Thus,by using this scale,nurses and healthcare providers can evaluate the treatment compliance of patients with type 2 DM.展开更多
To evaluate the effectiveness and tolerability of a split-dose 2 L polyethylene glycol (PEG)/ascorbic acid (AA) regimen for healthy examinees who visited for comprehensive medical check-up in the early morning.METHODS...To evaluate the effectiveness and tolerability of a split-dose 2 L polyethylene glycol (PEG)/ascorbic acid (AA) regimen for healthy examinees who visited for comprehensive medical check-up in the early morning.METHODSFrom February 2015 to March 2015, examinees of average risk who were scheduled for a colonoscopy in the morning were retrospectively enrolled.RESULTSThe 189 examinees were divided into split-dose and non-split-dose groups. The adequacy of bowel preparation for the split-dose group vs the non-split-dose group was 96.8% vs 85.2%, respectively, P < 0.001, and the compliance of the last meal restriction was 74.6% vs 58.2%, respectively, P < 0.001. The sleep disturbance (P < 0.001) was more prevalent in the split-dose group, however the willingness to repeat the same preparation method (P = 0.243) was not different in both groups. The split-dose regimen was the most important factor influencing adequate bowel preparation in multivariate analysis (HR = 10.89, 95%CI: 6.53-18.17, P < 0.001).CONCLUSIONA split-dose regimen of 2 L PEG/AA for an early morning colonoscopy was more effective and showed better compliance for diet restriction without any difference in satisfaction and discomfort. Introducing a split-dose regimen of 2 L PEG/AA to morning colonoscopy examinees is effective and tolerable in a comprehensive medical check-up setting.展开更多

Samples from consolidated alloys were analyzed by XRD for the determination of lattice parameters. Using these lattice parameters, the mechanical properties of the alloys were calculated theoretically with WIEN2k and EMTO programs, respectively. The elasticity moduli of the experimentally produced alloys were compared with the theoretical calculation results. The calculated mechanical properties of A13Sc phase and A1-Sc alloys were discussed to determine the optimum Sc content of Al-Sc alloys. It may be concluded that the Sc content should be the maximum about 1.0%, much more Sc addition could not improve the mechanical properties of the alloys.展开更多

The present calculations show that gallium can form planar chains in linear-, zigzag- and ladder-form one-dimensional structures. The most stable one among the studied structures is the zigzag chain with a unit cell rather close to equilateral triangles with four nearest neighbors, and all the other structures are metastable. The relative structural stability, the energy bands and the charge densities are discussed based on the ab initio calculations and the Jahn-Teller effect.展开更多

Results: Adherence is defined as patients collaboration with health care provider, active involvement in thetreatment regimen, and persistence in practice characterized by self-efficacy and relapse-prevention. Defining Attributesof adherence include collaboration relationship; self-efficacy promotion and relapse-prevention. Antecedents ofadherence include health care provider’s prescription, illness perceptions, social-economic and environmental factors.Patients adherence to cardiac rehabilitation programme can improve clinical outcomes. However, there is no standardinstrument to measure adherence. Conclusion: Based on the concept analysis, in order to improve patients’ adherence tocardiac rehabilitation, health care provider should invite patients active involvement in making rehabilitation plan andpromote patients’ self-efficacy and prevention relapse.展开更多

NF (non-specific factors) involved in treatment are all those effects that do not depe...Introduction: The effectiveness of treatment depends on the efficacy of the therapy and the level of compliance of the patient. NF (non-specific factors) involved in treatment are all those effects that do not depend on the pharmacological properties of the drug. Materials and Methods: The job lasted a year. The sample consisted of patients with mental health disorders and was divided into two groups. Treatment compliance was measured with the Morisky-Green Test. Results were compared using the chi square test and relative risk. Results and Discussion: Group A had 23 patients ending 91.3% of them and group B of 22 patients, ending the 77.27%. At the beginning of the study, we found in the group A 0.86 NF/patients while in the group B 0.82 NF/patient. At the end of the study there was a 54.65% decrease in group A while in group B the proportion remained. At the beginning of the study both groups had approximately 40% of compliant patients. Data that remained in the control group rose to 80.95% in group A at the end of the study. Conclusions: The work demonstrates the negative influence of these factors on adherence to treatment.展开更多

The results indicate that there exists an optimum mastic Epure content, and a critical operation temperature as well as a critical loading pressure for guss-asphalt in operation.
文摘Traditional syntactic or semantic theories failed to provide a satistactory explanation for the unique features of non-restrictive relational clauses in people's daily use of English language. This paper would adopt the theoretical framework of pragmatics and use relevance theory as well as face theory to discuss the unique pragmatic functions of non-restrictive relational clauses in English news report discourse. The three major pragmatic functions are: (1) optimizing the relevance of information; (2) seeking for the consistence with readers; and (3) avoiding imposing the author's opinion on readers.
文摘Background Medication adherence is an integral part of the comprehensive care of patients with atrial fibrillation (AF) receiving oral anticoagulations (OACs) therapy. Many patients with AF are elderly and may suffer from some form of cognitive impairment. This study was conducted to investigate whether cognitive impairment affects the level of adherence to anticoagulation treatment in AF patients. Me- thods The study involved 111 AF patients (mean age, 73.5±8.3 years) treated with OACs. Cognitive function was assessed using the Mini Mental State Examination (MMSE). The level of adherence was assessed by the 8-item Mot^sky Medication Adherence Scale (MMAS-8). Scores on the MMAS-8 range from 0 to 8, with scores 〈 6 reflecting low adherence, 6 to 〈 8 medium adherence, and 8 high adherence. Re- sults 46.9% of AF patients had low adherence, 18.8% had moderate adherence, and 33.3% had high adherence to OACs. Patients with lower adherence were older than those with moderate or high adherence (76.6 ±8.7 vs. 71.3 ~ 6.4 vs. 71.1 ± 6.7 years) and obtained low MMSE scores, indicating cognitive disorders or dementia (MMSE = 22.3 ± 4.2). Patients with moderate or high adherence obtained high MMSE test results (27.5 ±1.7 and 27,5 ± 3.6). According to Spearman's rank correlation, worse adherence to treatment with OACs was determined by older age (rs = -0.372) and lower MMSE scores (rs = 0.717). According to multivariate regression analysis, the level of cognitive function was a significant independent predictor of adherence (b = 1.139). Conclusions Cognitive impairment is an independent determinant of compliance with pharmacological therapy in elderly patients with AF. Lower adherence, beyond the assessment of cognitive function, is related to the age of patients.
文摘5-aminosalicylic acid(5-ASA)compounds are a highly effective treatment for ulcerative colitis(UC).While UC patient compliance in clinical studies is over 90%, only 40%of patients in every day life take their prescribed therapy.Adherence to medication has been emphasized recently by a Cochrane meta-analysis that has suggested that future trials of 5-ASA in UC should look at patient compliance rather than drug efficacy. Better compliance can be obtained by reducing the number of tablets and times of administration.Given that the 5-ASA formulations have different delivery systems that split the active moiety in various regions of the intestine,it is particularly important that an adequate dose of the drug arrives at the inflamed part of the colon.5-ASA Multi matrix(MMx)is a novel,high strength(1.2 g),oral formulation designed for oncedaily dosing.It releases the active moiety throughout the colon.Different studies with this compound have shown that it is as effective as 5-ASA enema in the treatment of mild-to-moderate,left-sided UC,and is comparable to a pH-dependent,delayed release 5-ASA (Asacol ),even if given once daily.Recently,the effectiveness in the acute phase of UC has been confirmed also in maintenance.In conclusion,at present,5-ASA MMx seems theoretically the best agent for maintaining patient compliance,and consequently,treatment effectiveness.

文摘Objective:This study aims to evaluate the effects of a symptom management intervention(SMI)based on symptom management group sessions combined with a mobile health(mHealth)application(app)on the knowledge of symptom management,the certainty of symptom self-management,symptom severity,symptom distress,medication adherence,social support,and quality of life among persons living with HIV(PLWH)in China.Methods:A parallel randomized controlled trial with 61 PLWH was conducted in Shanghai,China.The participants in the control group(n¼30)downloaded the Symptom Management(SM)app according to their needs and preferences,and received routine follow-ups.The participants in the intervention group(n¼31)were guided to download and use the SM app,and received four tailored weekly group sessions at routine follow-ups.Each group session lasted for approximately 2 h and targeted one of the major modules of the SM app.All the outcomes were assessed at baseline and post-intervention.The study was registered with the Chinese Clinical Trial Registry(ChiCTR1900024821).Results:The symptom management knowledge and certainty of symptom self-management were significantly improved after the intervention(all P<0.01).Compared with the control group,the scores of symptoms reasons knowledge score improved 11.47 points(95%CI:3.41,19.53)and scores of symptoms self-management knowledge score improved 12.80 points(95%CI:4.55,21.05)in the intervention group after controlling for covariates.However,other outcomes did not show statistically significant differences between the intervention group and the control group(P>0.05).Conclusion:The SMI could improve PLWH’s symptom management knowledge and certainty of symptom self-management.Multi-center studies with larger sample sizes and longer follow-ups are needed to further understand the effects of SM app on ameliorating symptom severity and symptom distress.More innovative strategies are also needed to promote and maintain the sustainability of the SM app.

文摘This paper explores the application of term dependency in information retrieval (IR) and proposes a novel dependency retrieval model. This retrieval model suggests an extension to the existing language modeling (LM) approach to IR by introducing dependency models for both query and document. Relevance between document and query is then evaluated by reference to the Kullback-Leibler divergence between their dependency models. This paper introduces a novel hybrid dependency structure, which allows integration of various forms of dependency within a single framework. A pseudo relevance feedback based method is also introduced for constructing query dependency model. The basic idea is to use query-relevant top-ranking sentences extracted from the top documents at retrieval time as the augmented representation of query, from which the relationships between query terms are identified. A Markov Random Field (MRF) based approach is presented to ensure the relevance of the extracted sentences, which utilizes the association features between query terms within a sentence to evaluate the relevance of each sentence. This dependency retrieval model was compared with other traditional retrieval models. Experiments indicated that it produces significant improvements in retrieval effectiveness.

文摘Objective:To standardize the distress management of gastric cancer patients receiving chemotherapy,the adapted Cancer-related Distress Management Guidelines were implemented in nursing practice among gastric cancer patients receiving chemotherapy based on A Guideline Adaptation and Implementation Planning Resource(CAN-IMPLEMENT).Methods:Based on the theoretical framework of CAN-IMPLEMENT,A multidisciplinary team was established,barriers and facilitators obstacles of guidelines implementation in medical oncology units were assessed,corresponding solutions were formulated,the guidelines implementation process was monitored,and implementation results were evaluated.Results:The multidisciplinary team developed review criteria,standardized work paths,assessment tools,training manuals for healthcare professionals,education manuals for patients and their caregivers.After guidelines implementation,the completion rate of the distress management record came up to 97.9%(189/193).From September 2017 to December 2018,the compliance of medical staff on most items in the audit checklist was improved,ranging from 57.1%(100/175)to 100.0%(193/193).The positive distress rate of gastric cancer patients receiving chemotherapy was decreased from 22.7%(32/141)to 9.3%(18/193)(P<0.05),and the Median(range)of the distress score declined from 2(0e9)to 0(0e7)(P<0.001).Conclusions:The implementation of guidelines based on CAN-IMPLEMENT promotes the establishment of a distress management system in the medical oncology units.The review standards,standardized work paths,and evaluation tools for distress in cancer patients formulated by the multidisciplinary team had clinical applicability and effectiveness.Quality control in the practice of distress management was effective.The compliance of healthcare professionals with distress management was improved.The distress of gastric cancer patients receiving chemotherapy was alleviated effectively.
文摘Objectives: The study assessed if the level of knowledge of HIV-infected about HAART and waiting-times in the PHC (primary healthcare) clinic have an influence on antiretroviral adherence. Methods: A descriptive-cross-sectional study was conducted in South Africa. Data collected uses a standardized-questionnaire and face-to-face-exit interviews. Pill-count technique was performed and a value of≥ 95% acceptable. Data were analysed using SPSS. Univariate-factors associated with poor-adherence to knowledge about HAART and waiting times were assessed using ANOVA and p ≤ 0.05 considered statistically significant. Key findings: Of 86 enrolled, 63(73.3%) were females and 23(26.7%) males, with mean-age (± SD) of 35.6(±9.6) years and on HAART for 35.5(± 31.8) months ranging from 1-137. Of these, 27(31.40%) and 25(29.07%) were on WHO stages 2 and 3 respectively. Adherence-rates computed from 32 patients, 23(71.9%) revealed poor adherence-rates. The level of knowledge about HAART in terms of names of tablets, correct-dose, frequency, adverse-effects had no influence on ARV-adherence (p _〉 0.05). Of 23 non-compliant, 10 (40%) gave the reason of drugs-unavailability, 7(30%) adverse-effects, 5(20%) drugs' complexity, and 1(10%) too busy to take them. Waiting areas associated with poor ARV-adherence were reception (p = 0.028), doctors (p = 0.027), while nurse's station (p = 0.29) and pharmacy (p = 0.43) revealed acceptable ARV-adherence.

文摘Objectives:Compliance to treatment is important for the management of type 2 diabetes mellitus(DM)and the prevention of complications.The purpose of the study was to develop a scale and test its psychometric properties for the treatment compliance of the diabetic Turkish population.Methods:This multicenter study was conducted in four training and research hospitals.The convenience sample consisted of 350 patients with type 2 DM.The items of the scale were determined after a literature review and qualitative interviews with the patients.Items were psychometrically analyzed.Content validity of the scale was evaluated using opinions from experts and a pilot study.Principal component analysis and the varimax rotation technique were used to evaluate construct validity in exploratory factor analysis.Criterion validity was evaluated with the Attitudes Towards Diabetes Scale,a subgroup of the Diabetes Care Profile scale.Reliability was evaluated with Cronbach'sαcoefficient and test-retest analysis of internal consistency.Results:The scale consisted of 7 factors that explained 47.36%of the total variance.The KMO test was conducted to determine whether the sample size was sufficient before the factor analysis.The KMO test result of the data was 0.75.The Cronbach'sαvalue of the sample was 0.77.The test-retest reliability analysis result was r=0.991.We found a positive correlation between total scores of the developed scale and the Attitudes Towards Diabetes Scale(r=0.31).Conclusion:The results of the study demonstrated that the scale with 30 items is a valid and reliable scale for the evaluation of patient compliance with type 2 DM treatment.Thus,by using this scale,nurses and healthcare providers can evaluate the treatment compliance of patients with type 2 DM.
文摘To evaluate the effectiveness and tolerability of a split-dose 2 L polyethylene glycol (PEG)/ascorbic acid (AA) regimen for healthy examinees who visited for comprehensive medical check-up in the early morning.METHODSFrom February 2015 to March 2015, examinees of average risk who were scheduled for a colonoscopy in the morning were retrospectively enrolled.RESULTSThe 189 examinees were divided into split-dose and non-split-dose groups. The adequacy of bowel preparation for the split-dose group vs the non-split-dose group was 96.8% vs 85.2%, respectively, P < 0.001, and the compliance of the last meal restriction was 74.6% vs 58.2%, respectively, P < 0.001. The sleep disturbance (P < 0.001) was more prevalent in the split-dose group, however the willingness to repeat the same preparation method (P = 0.243) was not different in both groups. The split-dose regimen was the most important factor influencing adequate bowel preparation in multivariate analysis (HR = 10.89, 95%CI: 6.53-18.17, P < 0.001).CONCLUSIONA split-dose regimen of 2 L PEG/AA for an early morning colonoscopy was more effective and showed better compliance for diet restriction without any difference in satisfaction and discomfort. Introducing a split-dose regimen of 2 L PEG/AA to morning colonoscopy examinees is effective and tolerable in a comprehensive medical check-up setting.

文摘Al-1.1%Sc and A1-2%Sc (mass fraction) alloys were prepared using gas atomized alloy particles. Samples from consolidated alloys were analyzed by XRD for the determination of lattice parameters. Using these lattice parameters, the mechanical properties of the alloys were calculated theoretically with WIEN2k and EMTO programs, respectively. The elasticity moduli of the experimentally produced alloys were compared with the theoretical calculation results. The calculated mechanical properties of A13Sc phase and A1-Sc alloys were discussed to determine the optimum Sc content of Al-Sc alloys. It may be concluded that the Sc content should be the maximum about 1.0%, much more Sc addition could not improve the mechanical properties of the alloys.

文摘The structural stabilities and electronic structures of Ga atomic chains are studied by the first-principles plane wave pseudopotential method based on the density functional theory. The present calculations show that gallium can form planar chains in linear-, zigzag- and ladder-form one-dimensional structures. The most stable one among the studied structures is the zigzag chain with a unit cell rather close to equilateral triangles with four nearest neighbors, and all the other structures are metastable. The relative structural stability, the energy bands and the charge densities are discussed based on the ab initio calculations and the Jahn-Teller effect.
文摘Objective: To explore and identify the concept of adherence in the context of cardiac rehabilitation. This conceptanalysis will provide a framework for clinical decision-making and intervention to improve patients’ adherence tocardiac rehabilitation programme. Method: Walker and Avant’s framework was used to analysis the concept ofadherence. Results: Adherence is defined as patients collaboration with health care provider, active involvement in thetreatment regimen, and persistence in practice characterized by self-efficacy and relapse-prevention. Defining Attributesof adherence include collaboration relationship; self-efficacy promotion and relapse-prevention. Antecedents ofadherence include health care provider’s prescription, illness perceptions, social-economic and environmental factors.Patients adherence to cardiac rehabilitation programme can improve clinical outcomes. However, there is no standardinstrument to measure adherence. Conclusion: Based on the concept analysis, in order to improve patients’ adherence tocardiac rehabilitation, health care provider should invite patients active involvement in making rehabilitation plan andpromote patients’ self-efficacy and prevention relapse.

The prevalence of AF burden and associated arterial and venous thrombosis progressively increases with age. At the same time, representative national data regarding stroke incidence in AF patients aged 80 and older are limited.
文摘Introduction: The effectiveness of treatment depends on the efficacy of the therapy and the level of compliance of the patient. NF (non-specific factors) involved in treatment are all those effects that do not depend on the pharmacological properties of the drug. Materials and Methods: The job lasted a year. The sample consisted of patients with mental health disorders and was divided into two groups. Treatment compliance was measured with the Morisky-Green Test. Results were compared using the chi square test and relative risk. Results and Discussion: Group A had 23 patients ending 91.3% of them and group B of 22 patients, ending the 77.27%. At the beginning of the study, we found in the group A 0.86 NF/patients while in the group B 0.82 NF/patient. At the end of the study there was a 54.65% decrease in group A while in group B the proportion remained. At the beginning of the study both groups had approximately 40% of compliant patients. Data that remained in the control group rose to 80.95% in group A at the end of the study. Conclusions: The work demonstrates the negative influence of these factors on adherence to treatment.
