Clinical Effect Analysis of Posterior Chamber Intraocular Lens Implantation for Correcting High Myopia and Astigmatism

Objective:To explore the corrective effect of posterior chamber intraocular lens implantation with phakic eyes in the treatment of high myopia and astigmatism.Methods:From May 2023,the hospital began to collect the case data of diagnosis and treatment of high myopia and astigmatism.By May 2024,310 cases were included,all of which were treated with posterior chamber intraocular lens implantation.The visual acuity,astigmatism and axial position of the intraocular lens were observed before and after treatment.Results:At different time points after the operation,the patient’s vision was significantly improved compared with that before the operation(P<0.05),and the vision level was equal to or greater than the best-corrected vision before the operation.At different time points after the operation,the average rotation of the intraocular lens was less than 5 degrees.Astigmatism was significantly lower than that before the operation(P<0.05).After the operation,the intraocular pressure increased in 11 cases,accounting for 3.55%,with no adverse complications such as lens turbidity,glare and obvious halo occurring.Conclusion:The posterior chamber intraocular lens implantation with phakic eyes has an ideal correction effect in the treatment of high myopia and astigmatism,which can effectively improve the vision level of patients and reduce the degree of astigmatism,and has high effectiveness and safety. 展开更多

AIM:To elucidate the safety and visual quality of implantable collamer lens with central hole(ICL V4c)implantation for correcting moderate and high myopia for at least 5y.METHODS:This retrospective study was conducted on 58 patients(114 eyes)who were followed up for at least 5y after ICL V4c implantation.The observation was done before and on 1d,1mo,1 and 5y or more after the surgical procedure.The visual acuity,subjective refraction,intraocular pressure,vault,axial length,central hole position,pupil diameter,visual quality,and adverse events were analyzed.The visual quality includes aberration,the modulation transfer function cutoff frequency(MTF cutoff),objective scattering index(OSI),Stroller's ratio(SR),and visual quality questionnaire.RESULTS:The average follow-up period was 69.25±3.80mo(range 60–82mo)and the preoperative spherical equivalent(SE)was-8.66±1.97 D.At 5y after operation,the safety index was 1.01±0.02 and the efficacy index was 0.99±0.42 and SE was-0.65±0.63 D.The 59.6%of the eyes achieved an uncorrected distance visual acuity of 20/20,76.3%of the eyes had SE within±1.0 D at the last visit.The axial length increased by 0.29±0.71 mm 5y after the surgery(t=-3.843,P<0.001).The mean vault at the last follow-up was 510.59±245.61μm.The central hole was on the temporal side in 80 eyes(84.2%).The visual quality questionnaire showed that 98.2%patients were satisfied with the surgical procedure.Adverse events occurred in 4 eyes(3.5%),including the posttraumatic toric ICL rotation(2 eyes),iris incarceration(1 eye),and posttraumatic ICL displacement(1 eye)at the last follow-up.CONCLUSION:Long-term ICL V4c implantation is safe,effective,and stable for correcting moderate and to high myopia,and the visual quality with patients is excellent and satisfactory,but the progression of axial length still needs attention after surgery. 展开更多

AIM:To evaluate the refractive correction for standard automated perimetry(SAP)in eyes with refractive multifocal contact lenses(CL)in healthy young participants.METHODS:Twenty-nine eyes of 29 participants were included.Accommodation was paralyzed in all participants with 1%cyclopentolate hydrochloride.SAP was performed using the Humphrey SITA-standard 24-2 and 10-2 protocol under three refractive conditions:monofocal CL corrected for near distance(baseline);multifocal CL corrected for distance(m CL-D);and m CL-D corrected for near vision using a spectacle lens(m CL-N).Primary outcome measures were the foveal threshold,mean deviation(MD),and pattern standard deviation(PSD).RESULTS:The foveal threshold of m CL-N with both the24-2 and 10-2 protocols significantly decreased by 2.2-2.5 d B(P〈0.001),while that of m CL-D with the 24-2 protocol significantly decreased by 1.5 d B(P=0.0427),as compared with that of baseline.Although there was no significant difference between the MD of baseline and m CL-D with the24-2 and 10-2 protocols,the MD of m CL-N was significantly decreased by 1.0-1.3 d B(P〈0.001)as compared with that of both baseline and m CL-D,with both 24-2 and 10-2 protocols.There was no significant difference in the PSD among the three refractive conditions with both the 24-2 and 10-2 protocols.CONCLUSION:Despite the induced mydriasis and the optical design of the multifocal lens used in this study,our results indicated that,when the dome-shaped visual field test is performed with eyes with large pupils and wearing refractive multifocal CLs,distance correction without additional near correction is to be recommended. 展开更多

目的:探讨白内障手术中植入LENTIS Comfort Toric人工晶状体后的视觉质量。方法:回顾性对照临床研究,收集数字化导航辅助下白内障超声乳化摘除联合人工晶状体植入术患者40例40眼,植入LENTIS Comfort Toric(MF15T)人工晶状体为A组(24眼)... 目的:探讨白内障手术中植入LENTIS Comfort Toric人工晶状体后的视觉质量。方法:回顾性对照临床研究,收集数字化导航辅助下白内障超声乳化摘除联合人工晶状体植入术患者40例40眼,植入LENTIS Comfort Toric(MF15T)人工晶状体为A组(24眼),植入单焦点Toric人工晶状体为B组(16眼);观察术后3mo不同距离视力、术后散光质心值、脱镜率、离焦曲线变化以及人工晶状体稳定性等。结果:两组患者术后均无严重并发症出现,两组术后3mo远视力无差异(P>0.05);A组中视力、近视力明显优于B组(P<0.01);A组术后脱镜率为99%,B组为45%(P<0.05);两组术后3mo散光质心值均较术前改善,但组间无差异(P>0.05);术后3mo A组离焦曲线于+0.50、-1.75D分别出现峰值,形成平台并平缓下降,B组于+0.25、-0.25D出现峰值后呈陡峭趋势下降。术后1d A组出现1眼(4%)人工晶状体轴位旋转大于10°;B组出现6眼(38%)人工晶状体轴位旋转大于10°,2眼(33%)顺时针旋转,4眼(67%)逆时针旋转;两组均无人工晶状体移位发生。结论:两种人工晶状体均可矫正散光,其中LENTIS Comfort Toric人工晶状体能为白内障合并散光人群提供个性化的选择,可同时解决散光和获得全程视力,术后光学干扰发生率低,稳定性更好,视觉质量及满意度高。 展开更多

AIM: To assess the sutureless scleral fixation technique for posterior chamber foldable intraocular lens(PCIOL) implantation in aphakic eyes with insufficient or no capsular support. METHODS: A technique for sutureless intrascleral fixation of the haptics of a standard 3-piece PCIOL was used which ensures sutureless fixation by permanent incarceration of the haptics in a scleral tunnel parallel to the limbus. All patients were evaluated for preoperative status [visual acuity, refractive error, K readings, intraocular pressure(IOP) measurement, slit lamp examination, fundus examination and optical biometry], postoperative status and complications. Ultrasound biomicroscopy(UBM) was done for 10 cases to evaluate optic tilt. RESULTS: The study evaluated 42 eyes of 42 patients. The follow-up period was 6 mo. Improvement of best corrected visual acuity(BCVA) one line occurred in 10 cases(23.8%) and loss of one line in 3 cases(7.1%). Intraoperative complications included: haptic kink in 4 cases(9.5%), haptic breakage in 1 case(2.4%), haptic dislocation in 1 case(2.4%), haptic slippage in 3 cases(7.1%), IOL dislocation in 1 case(2.4%) and sclerotomy related bleeding in 1 case(2.4%). Postoperative complications included: transient mild vitreous hemorrhage in 3 cases(7.1%), choroidal detachment in 1 case(2.4%), cystoid macular edema(CME) in 1 case(2.4%), optic capture in 1 case(2.4%), subconjunctival haptic in 2 cases(4.8%), ocular hypotony in 4 cases(9.5%) and ocular hypertension in 1 case(2.4%). There were no cases of retinal detachment or endophthalmitis. UBM showed optic tilt in 3 cases(30%). CONCLUSION: Fixation of three-piece foldable IOL haptics in scleral tunnel parallel to the limbus-providedaxial stability and proper centration of the IOL with minimal or no tilt in most cases and a low complication rate during the follow up period which lasted 6 mo. 展开更多

随着我国油气勘探程度的不断加深,陆相页岩油气等非常规油气逐渐成为我国勘探开发的重点,陆相页岩非均质性强、储层薄且纵横向变化剧烈等典型特征导致含油气性预测精度差,影响后续井位部署和生产等.时频变换方法是储层含油气性预测最有... 随着我国油气勘探程度的不断加深,陆相页岩油气等非常规油气逐渐成为我国勘探开发的重点,陆相页岩非均质性强、储层薄且纵横向变化剧烈等典型特征导致含油气性预测精度差,影响后续井位部署和生产等.时频变换方法是储层含油气性预测最有效的工具.为了提高储层含油气性预测的精度,本文提出了一种频率矫正广义S变换(Frequency-Corrected Generalized S-Transform, FCGST),该变换既具有S变换多尺度多分辨率的特性,又可以通过自适应选取窗函数参数以控制窗函数的带宽和主频等,这一特性可以适用于不同工区地震数据的处理与解释.与传统的S变换和广义S变换不同,该变换修正了传统S变换和广义S变换中的尺度因子,改善了S变换和广义S变换等传统方法中时频谱主频向高频方向偏移的问题,有利于精确描述复杂储层含油气性.合成数据算例表明,频率矫正广义S变换提升了地震信号局部时频特征的表征能力;将频率矫正广义S变换应用于鄂尔多斯盆地某工区储层含油气性预测中,实钻结果数据进一步验证了该时频分析方法在储层含油气性解释中的有效性和潜力. 展开更多

We propose a method for the compensation and phase correction of the amplitude spectrum based on the generalized S transform. The compensation of the amplitude spectrum within a reliable frequency range of the seismic record is performed in the S domain to restore the amplitude spectrum of reflection. We use spectral simulation methods to fit the time-dependent amplitude spectrum and compensate for the amplitude attenuation owing to absorption. We use phase scanning to select the time-, space-, and frequencydependent phases correction based on the parsimony criterion and eliminate the residual phase effect of the wavelet in the S domain. The method does not directly calculate the Q value; thus, it can be applied to the case of variable Q. The comparison of the theory model and field data verify that the proposed method can recover the amplitude spectrum of the strata reflectivity, while eliminating the effect of the residual phase of the wavelet. Thus, the wavelet approaches the zero-phase wavelet and, the seismic resolution is improved. 展开更多

In this paper, the S p q current systems are analyzed in the corrected geomagnetic coordinates (CGM) instead of the conventional geomagnetic coordinates (GM), and the symmetries of the S p q current in different systems are compared. An Improved 6S Code for Atmospheric Correction Based on Water Vapor Content

Water vapor content in the atmosphere is very significant for atmospheric correction of optical remote sensing data. Nowadays, the common atmospheric correction models use a single value of the average water vapor content of the study area to perform atmospheric correction. As the distribution of water vapor content varies greatly with time and space, it is obviously inaccurate to represent the total water vapor conditions of the whole area by just reading the average water vapor content. In this study, we altered the 6S sources so that it could read the water vapor content image which was retrieved from MODIS 1 km data. Atmospheric correction was implemented for the band 1 of MODIS 500 m data pixel-by-pixel using the improved 6S model. In comparison with the traditional 6S model, this improved 6S model is more reasonable in atmospheric correction, for it considers the spatial distribution of the water vapor content retrieved from MODIS data in the near infrared to define the atmospheric conditions for simulating the atmospheric radiative transfer. The results corrected by the improved 6S model showed more reasonable in pixel spatial distribution and closer histogram with the original image than those by traditional 6S model. 展开更多

Introduction: Congenitally corrected transposition of the great arteries (cc-TGA) is a congenital heart disease rarely described among adult population, especially if it is associated with other abnormalities such as Ebstein’s anomaly and abnormal origin of coronary arteries. Case presentation: Twenty-two-year-old woman admitted to intensive care unit with acute decompensated heart failure. The transthoracic echocardiography demonstrated atrioventricular and ventriculoarterial discordance. The left-sided atrioventricular valve, in this case, the tricuspid valve, showed apical displacement of the septal valve suggesting Ebstein’s anomaly. The computed tomography corroborated these findings and additionally showed a left superior vena cava and an abnormal origin of the coronary arteries. The patient is in NYHA functional class II, receiving treatment with diuretic, digitalis, beta-blockers and angiotensin-converting enzyme (ACE) inhibitor. Conclusion: This is a very interesting case of an adult in the third decade of life with cc-TGA associated with abnormalities not described in the literature that had reached adulthood. 目的分析白内障超声乳化联合人工晶体(IOL)植入术后并发囊袋收缩综合征(CCS)的危险因素。方法选择2020年1月至2020年12月在宝鸡市人民医院眼科接受白内障超声乳化联合IOL植入术的100例患者展开研究,根据患者术后3个月CCS的发生情况分为... 目的分析白内障超声乳化联合人工晶体(IOL)植入术后并发囊袋收缩综合征(CCS)的危险因素。方法选择2020年1月至2020年12月在宝鸡市人民医院眼科接受白内障超声乳化联合IOL植入术的100例患者展开研究,根据患者术后3个月CCS的发生情况分为CCS组7例,无CCS组93例。比较两组患者术前及术后3个月的最佳矫正视力(BCVA)、对比敏感度(CSF),并对影响白内障超声乳化联合IOL植入术后并发CCS的原因进行单因素分析和多因素Logistic回归分析。结果CCS组患者术前BCVA以及CSF 1.5c/d、6c/d、18c/d分别为(0.48±0.07)LogMAR、1.45±0.13、1.06±0.14、0.05±0.01,明显低于无CCS组的(0.57±0.11)LogMAR、1.59±0.16、1.22±0.16、0.07±0.02,CCS组患者术后3个月时,BCVA以及CSF 1.5c/d、6c/d、18c/d分别为(0.69±0.08)LogMAR、1.68±0.17、1.48±0.16、0.21±0.03,明显低于无CCS组的(0.83±0.07)LogMAR、1.87±0.15、1.76±0.18、0.35±0.05,差异均有统计学意义(P<0.05);经单因素分析结果显示,两组患者的年龄以及有无糖尿病、青光眼、葡萄膜炎、高度近视、视网膜色素变性、IOL植入材料比较差异均有统计学意义(P<0.05);经多因素Logistic回归分析结果显示,年龄、糖尿病、青光眼、葡萄膜炎、高度近视、视网膜色素变性、IOL植入材料均是导致白内障超声乳化联合IOL植入术后并发CCS的危险因素(P<0.05)。结论白内障超声乳化联合IOL植入术后并发CCS的危险因素包括年龄、糖尿病、青光眼、葡萄膜炎、高度近视、视网膜色素变性、IOL植入材料,临床上可根据上述危险因素早期干预,以期减少CCS的发生。 展开更多

From the perspective of individual learner differences,this paper aims to show the readers an outline of the influence of individual learner differences on written corrective feedback.Based on 200 eighth and ninth grade junior high school students’thinking patterns and tests,60 students were selected as subjects in this study.Based on the comparative analysis and error analysis in second language acquisition as the main theoretical basis,this study analyzed the common types of errors in the use of nonpredicate verbs in junior high school students and their causes.There are three main questions in this study:how about the error frequency of the five forms(present participle,past participle,infinitive,gerund and independent nominative structure)in the nonpredicate verb learning of junior high school students?What kind of mistakes do junior high school students make in the process of learning non-predicate verbs?According to the conclusion of the investigation and research,the author puts forward some suggestions from the aspects of teachers and students in the last part of the article. 展开更多

鱼眼镜头非线性光学畸变导致鱼眼图像行人检测算法精度低,且校正算法也无法完全克服鱼眼图像的边缘严重变形。针对上述问题,文中以Faster R-CNN架构为基础,建立了鱼眼图像校正光路模型。针对鱼眼图像畸变,提出一种基于微分方程的鱼眼图... 鱼眼镜头非线性光学畸变导致鱼眼图像行人检测算法精度低,且校正算法也无法完全克服鱼眼图像的边缘严重变形。针对上述问题,文中以Faster R-CNN架构为基础,建立了鱼眼图像校正光路模型。针对鱼眼图像畸变,提出一种基于微分方程的鱼眼图像校正模型,并提出一种改进算法用于鱼眼图像的行人检测。构建了ResNet 50融合特征金字塔网络结构,以增强网络的多尺度特征提取能力,提高网络对行人小目标的定位和识别能力;优化平滑L1损失函数解决大梯度难学样本与小梯度易学样本间的不平衡问题,提高训练效果。实验结果表明,文中算法与现有鱼眼图像行人检测算法相比,检测精度提高了39.68%。在边缘轻微畸变及小尺度行人的检测精度可以达到90%以上,有助于提高极端条件下鱼眼图像的行人检测性能。 展开更多

为了提升光子多普勒测速仪(Photon Doppler Velocimetry, PDV)对目标进行多点数的区域覆盖式或阵列式速度测量时的测量点数量,使用Zemax软件设计了像面直径10 mm的双远心光学系统,实现同时对平面靶5.6 mm范围内进行多点测速。该镜头由4... 为了提升光子多普勒测速仪(Photon Doppler Velocimetry, PDV)对目标进行多点数的区域覆盖式或阵列式速度测量时的测量点数量,使用Zemax软件设计了像面直径10 mm的双远心光学系统,实现同时对平面靶5.6 mm范围内进行多点测速。该镜头由4片单透镜和4片双胶合透镜组成,工作波长为1 550 nm,工作物距为300 mm,系统总长250 mm,有效焦距500 mm,远心度小于0.01°,均方根半径在聚焦位置处最小为0.055μm,最大为0.920μm。均方根半径及全部弥散斑半径均在艾里斑范围内,调制传递函数曲线与衍射极限重合,线条平整,系统达到衍射极限。同时畸变小于0.025%,像差校正满足设计要求。 展开更多
