γ-谷氨酰转肽酶对非酒精性脂肪性肝病诊断价值

目的非酒精性脂肪性肝病(nonalcoholic fatty liver disease,NAFLD)是一种代谢应激性疾病,γ-谷氨酰转肽酶(gamma-glutamyl transpeptidase,GGT)与氧化应激和代谢综合征的关系得到了越来越多关注。本研究探讨GGT在NAFLD筛查中意义。方... 目的非酒精性脂肪性肝病(nonalcoholic fatty liver disease,NAFLD)是一种代谢应激性疾病,γ-谷氨酰转肽酶(gamma-glutamyl transpeptidase,GGT)与氧化应激和代谢综合征的关系得到了越来越多关注。本研究探讨GGT在NAFLD筛查中意义。方法收集2018-06-01-2019-04-30辽宁省人民医院诊治的114例NAFLD患者和同期96名健康体检人员临床资料,分析两组体质量指数(body mass index,BMI)、血压及生化指标差异;GGT与NAFLD关联性;评估GGT对NAFLD诊断效能。结果病例组BMI为(26.51±2.85)kg/m^2,高于对照组的(23.45±3.14)kg/m^2,t=7.270,P<0.001;病例组丙氨酸氨基转移酶(alanine aminotransferase,ALT)为(39.57±19.29)U/L,高于对照组的(26.14±7.57)U/L,t=6.480,P<0.001;病例组天门冬氨酸氨基转移酶(aspartate aminotransferase,AST)为(25.19±12.09)U/L,高于对照组的(19.26±5.73)U/L,t=4.431,P<0.001;病例组GGT为(41.35±29.45)U/L,高于对照组的(19.62±9.74)U/L,t=6.794,P<0.001;病例组胆碱酯酶为(10778.53±2454.08)U/L,高于对照组的(9155.04±1951.61)U/L,t=5.162,P<0.001;病例组总胆固醇(total cholesterol,TC)为(5.15±1.45)mmol/L,高于对照组的(4.77±0.89)mmol/L,t=2.186,P=0.030;病例组三酰甘油(triacylglycerol,TG)为(3.59±3.08)mmol/L,高于对照组的(1.46±1.42)mmol/L,t=4.255,P<0.001;病例组低密度脂蛋白胆固醇(low density lipoprotein cholesterol,LDL-C)为(3.52±0.82)mmol/L,高于对照组的(1.04±1.21)mmol/L,t=3.371,P=0.001;病例组血清尿酸(serum uric acid,SUA)为(346.54±92.42)μmol/L,高于对照组的(283.38±86.04)μmol/L,t=4.978,P<0.001;病例组高密度脂蛋白胆固醇(high density lipoprotein-cholesterol,HDL-C)为(1.14±0.37)mmol/L,低于对照组的(1.23±0.27)mmol/L,t=2.667,P=0.008。Logistic回归分析结果显示,在校正后,GGT与NAFLD仍具有关联性(OR=1.062,P<0.05)。GGT筛查NAFLD曲线下面积为0.807(0.747~0.866),当GGT值为诊断界值22.5U/L时,灵敏度为76.31%,特异度为75.00%,Kappa=0.512。结论利用GGT筛查NAFLD虽具有一定临床意义,但漏诊率和误诊率均较高,作为一个常用指标,需要结合病史等基础信息,辅助NAFLD临床筛查。 展开更多

将来源于Escherichia coli DH5α的γ-谷氨酰转肽酶基因克隆到高表达质粒pET-32α中,并转化E·coli BL21,构建茶氨酸生物合成的基因工程菌。工程菌在温度为20℃,OD600nm为1.5,IPTG浓度为0.1mmol/L,培养基初始pH为7的条件下,诱导培养... 将来源于Escherichia coli DH5α的γ-谷氨酰转肽酶基因克隆到高表达质粒pET-32α中,并转化E·coli BL21,构建茶氨酸生物合成的基因工程菌。工程菌在温度为20℃,OD600nm为1.5,IPTG浓度为0.1mmol/L,培养基初始pH为7的条件下,诱导培养6h,γ-谷氨酰转肽酶的酶活力达(4.41±0.17)U/mL,大约是出发菌株E·coli DH5α的18.4倍。直接以工程菌为催化剂,在200mmol/L谷氨酰胺、1.5mol/L乙胺盐酸盐、pH10、20℃的条件下,反应6h,茶氨酸合成量达12.6mg/mL,L-谷氨酰胺转化率为41.05%。 展开更多

AIM:This study aimed to determine whether metabolic syndrome is directly or indirectly,through fatty liver,associated with elevated gamma-glutamyl transpeptidase (GGT) levels in Japanese women. METHODS:From 4 366 women who received their annual health check-up,4 211 women were selected for analysis. All 4 211 women were negative for both hepatitis B surface antigen and antibody to hepatitis C virus.Clinical and biochemical variables were examined by using univariate and multivariate analysis. RESULTS:A raised GGT level (>68 IU/L) was seen in 258 (6.1%) of the 4 211 women.In univariate analysis,all variables examined (age,body mass index,blood pressure, hemoglobin concentration,fasting blood glucose, glycosylated hemoglobin Alc,cholesterol,triglyceride,and uric acid) were associated with the elevated GGT level, whereas in multivariate analysis,four variables (age≧50 yr, hemoglobin≧14 g/dL,triglyceride≧150 mg/dL,and presence of diabetes) were significantly and independently associated with raised GGT level.Clinical variables predicting the presence of ultrasonographic evidence of fatty liver were also examined by multivariate analysis;four variables were associated with the presence of fatty liver:BMI≧25 kg/m^2, hemoglobin≧14 g/dL,triglyceride≧150 mg/dL,and uric acid ≧ 7 mg/dL.There was no significant association between the raised GGT level and the presence of fatty liver. Hypertriglyceridemia was significantly and independently associated with both the raised GGT level and the presence of fatty liver. CONCLUSION:Metabolic syndrome seemed to be directly, not indirectly through fatty liver,associated with the raised GGT level in Japanese women. 展开更多

Correlation of Cerebrospinal Fluid Sialic Acid with Various Enzymes in Patients with Meningitis

Meningitis is often associated with cerebral compromise which may be responsible for neurological squeal in nearly half of the survivors. Little is known about the mechanisms of Central Nervous System (CNS) involvement in bacterial meningitis. The present study was undertaken to assess the changes in biochemical parameters including glucose, protein, Sialic Acid (SA) and various enzymes like Aspartate Aminotransferase (AST), Alanine Aminotransferase (ALT), Creatine Kinase (CK), Gamma Glutamyltranspeptidase 7 (GGTP) and Lactate Dehydrogenase (LDH) have been determined in the Cerebrospinal Fluid (CSF) of the meningitis patients (n = 26) and compared with control subjects (n = 26). Total Protein, CK, SA (P 〈 0.02), AST (P 〈 0.001) and ALT (P 〈 0.0001) of meningitis patients were significantly higher, whereas, glucose (P 〈 0.05) and Gamma Glutamyltranspeptidase (GGT) (P 〈 0.01 and P 〈 0.05, respectively) were significantly lower than those of the controls. This study suggests that loss of integrity of brain-CSF barriers, sialic acid and enzyme profiles may contribute to the severity and neurological complications of meningitis. 展开更多
