Objective Contact precautions,especially the initiation of isolation,are important measures to prevent and control multidrug-resistant organisms(MDROs).However,the implementation in clinical practice remains weak.This...Objective Contact precautions,especially the initiation of isolation,are important measures to prevent and control multidrug-resistant organisms(MDROs).However,the implementation in clinical practice remains weak.This study aimed to analyze the impact of multidisciplinary collaborative intervention on isolation implementation in multidrug-resistant infection,and determine the factors that affect the implementation of isolation measures.Methods A multidisciplinary collaborative intervention related to isolation was conducted at a teaching tertiary hospital in central China on November 1,2018.The information of 1338 patients with MDRO infection and colonization at 10 months before and after the intervention was collected.Then,the issuance of isolation orders was retrospectively analyzed.Univariate analysis and multivariate logistic regression analysis were performed to analyze the factors that affected the isolation implementation.Results The overall issuance rate of isolation orders was 61.21%,which increased from 33.12%to 75.88%(P<0.001)after the implementation of the multidisciplinary collaborative intervention.The intervention(P<0.001,OR=0.166)was a promoting factor for the issuance of isolation orders,in addition to the length of stay(P=0.004,OR=0.991),department(P=0.004),and microorganism(P=0.038).Conclusion The isolation implementation remains far lower than policy standards.Multidisciplinary collaborative interventions can effectively improve the compliance to isolation measures implemented by doctors,thereby promoting the standardized management of MDROs,and providing reference for further improving the quality of hospital infection management.展开更多
AIM:To evaluate impact of radiation therapy dose escalation through intensity modulated radiation therapy with simultaneous integrated boost(IMRT-SIB).METHODS:We retrospectively reviewed the patients who underwent fou...AIM:To evaluate impact of radiation therapy dose escalation through intensity modulated radiation therapy with simultaneous integrated boost(IMRT-SIB).METHODS:We retrospectively reviewed the patients who underwent four-dimensional-based IMRT-SIBbased neoadjuvant chemoradiation protocol.During the concurrent chemoradiation therapy,radiation therapy was through IMRT-SIB delivered in 28 consecutive daily fractions with total radiation doses of 56 Gy to tumor and 5040 Gy dose-painted to clinical tumor volume,with a regimen at the discretion of the treating medical oncologist.This was followed by surgical tumor resection.We analyzed pathological completion response(p CR) rates its relationship with overall survival and event-freesurvival.RESULTS:Seventeen patients underwent dose escalation with the IMRT-SIB protocol between 2007 and 2014 and their records were available for analysis.Among the IMRT-SIB-treated patients,the toxicity appeared mild,the most common side effects were grade 1-3 esophagitis(46%) and pneumonitis(11.7%).There were no cardiac events.The Ro resection rate was 94%(n = 16),the p CR rate was 47%(n = 8),and the postoperative morbidity was zero.There was one mediastinal failure found,one patient had local failure at the anastomosis site,and the majority of failures were distant in the lung or bone.The 3-year diseasefree survival and overall survival rates were 41%(n = 7) and 53%(n = 9),respectively.CONCLUSION:The dose escalation through IMRT-SIB in the chemoradiation regimen seems responsible for down-staging the distal esophageal with well-tolerated complications.展开更多
This study develops crash rate prediction models based on the premise that crash frequencies observed from adjacent paired non-weaving and weaving freeway segments are spatially correlated and therefore requires a sim...This study develops crash rate prediction models based on the premise that crash frequencies observed from adjacent paired non-weaving and weaving freeway segments are spatially correlated and therefore requires a simultaneous equation modeling approach. Simultaneous equation models for paired freeway non-weaving segments and weaving segments along with combined three freeway segments upstream and downstream were developed to investigate the relationship of crash rate with freeway characteristics. The endogenous variables have significant coefficients which indicate that unobserved variables exist on these contiguous segments, resulting in different crash rates. AADT is a variable that can show the interaction between the traffic and crashes on these contiguous segments. The results corroborate such an interaction. By comparing the simultaneous equation model and the multiple linear regression model, it is shown that more model parameters in the simultaneous models are significant than those from linear regression model. This demonstrates the existence of the correlation between the interchange and between-interchange segments. It is crucial that some variables like segment length can be identified significant in the simultaneous model, which provides a way to quantify the safety impact of freeway development.展开更多

This system gained pop...The Orthogonal Frequency Division Multiplexing (OFDM) system is already used in commercial applications and is capable to deal with Intersymbolic Interference (ISI) caused by multipath channels. This system gained popularity after the application of the Fast Fourier Transform (FFT) and its inverse (IFFT) to modulate the signal in many subcarriers. This paper discusses implementation aspects of an OFDM system;such system is characterized by considering real constraints, including the memory consumption and the processing time. The OFDM modulator, channel samples and OFDM demodulator were implemented entirely in the DSP TMS320C6678 platform. As a proof-of-concept, a 256-QAM OFDM BER performance is compared with theoretical values. Moreover, the memory size is not demanding, consuming very few resources. It was observed a very high number of DSP clock cycles needed for the OFDM signal modulation, corresponding to more than 4 times the number used in demodulating the signal.展开更多
