Modulation of gastric accommodation by duodenal nutrients

AIM: To determine the relative potency and contribution of intestinal nutrients to net gastric accommodative relaxation and conscious perception. METHODS: In 12 healthy subjects, we randomly tested duodenal loads of lipids and carbohydrates (12 mL administered in 4 rain) at various caloric concentrations (0.0125-0.8 kcal/mL) separated by 12-24 rain wash-out periods of saline infusion. Maximal gastric relaxation was induced at the end of each experiment by i.v glucagon (5 μg/kg), as reference. The reflex gastric response was measured by a barostat, and symptom perception by a 0-6 score questionnaire. RESULTS: Upids induced a dose-response gastric relaxation with a steep and early rise. Maximal effect (179±42 mL relaxation) reached at a relatively low concentration (0.2 kcal/mL), maximal lipid-induced relaxation was 61±6% of the glucagon effect. By contrast, duodenal infusion of carbohydrates induced weaker relaxation that became significant only at the high end of the physiological concentration range (65±14 ml with 0.8 kcal/mL). Intestinal nutrient loads, either of lipid or carbohydrates, did not induce significant changes in perception (0.6±0.4 and 0.1±0.4 score increase for the highest concentrations, respectively). CONCLUSION: Chyme entering the small bowel induces nutrient-spedfic gastric relaxatory reflexes by a physiologically saturable mechanism. Normally, neither the intestinal nutrient load nor the gastric accommodative response is perceived. 展开更多

Cystic dystrophy of the duodenal wall is a rare condition characterized by the development of cysts in heterotopic pancreatic tissue localized in the duodenal wall.A 38-year- old man was admitted to the hospital for abdominal pain and vomiting after food intake.The diagnosis of acute pancreatitis was initially suspected.Abdominal ultrasound examination revealed thickening of the second portion of duodenal wall within which,small cysts(diameter,less than 1 cm)were present in the vicinity of pancreatic head. The head of pancreas appeared enlarged(63 mm×42 mm) and hypoechoic.Upper endoscopy and barium X-ray series were performed revealing a severe circumferential deformation,as well as 4 cm long stenosis of the second portion of the duodenum.CT examination revealed multiple cysts located in an enlarged,thickened duodenal wall with moderate to strong post-contrast enhancement.We suspected that patient had cystic dystrophy of duodenal wall developed in the heterotopic pancreas and diagnosis was confirmed by endoscopic ultrasound(EUS).Endoscopic ultrasound(EUS)revealed circular stenosis from the duodenal bulb onwards.A twenty megahertz mini-prope examination further showed diffuse(intramural)infiltration of duodenal wall limited to the submucosa and muscularis propria of the second portion of duodenum with multiple microcysts within the thickened mucosa and submucosa.Patient was successfully surgically treated and pancreatoduodenectomy was performed.The pathological examination confirmed a diagnosis of cystic dystrophy of a heterotopic pancreas. Endoscopic ultrasonography features allow preoperative diagnosis of cystic dystrophy of a heterotopic pancreas in duodenal wall,with intralumina120 MHz mini probe sonography being more efficient in cases of luminal stenosis. 展开更多

Aim To analyze the secondary structure and neurotrophic effect of a specific protein in sensory neurons. Methods Comparison of the proteins expressed in the rat spinal sensory neurons and motor neurons was made by two dimensional electrophoresis. One specific protein in sensory neurons was isolated and purified by DEAE Sephacel ion exchange chromatography and high performance liquid chromatography. A primary analysis of its secondary structure by circular dichroism, and its neurotrophic effects were investigated using the model of dorsal root ganglia(DRG) cultured in vitro . Results The molecular weight and isoelectric point of the protein were 33 1 kDa and 5 52, respectively. Its circular dichroism showed that there were 20 8% α helix, 54 8% β sheet, 7 3% turn, and 17 1% random coil in its secondary structure. Biological experiments showed that the protein could promote the neurite outgrowth of DRG. Conclusion A specific protein in spinal sensory tissue with molecular weight of 33 1 kDa has been purified. There is mainly β sheet in the secondary structure of the protein. And the protein has neurotrophic effects in the model of DRG. 展开更多

AIM: To assess the clinical and economical validity of glutamine dipeptide supplemented to parenteral nutrition (PN) in patients undergoing abdominal surgery. METHODS: A meta-analysis of all the relevant randomized controlled trials (RCTs) was performed. The trials compared the standard PN and PN supplemented with glutamine dipeptide in abdominal surgery. RCTs were identified from the following electronic databases: the Cochrane Library, MEDLINE, EMBASE and ISI web of knowledge (SCI). The search was undertaken in April 2006. Literature references were checked by computer or hand at the same time. Clinical trials were extracted and evaluated by two reviewers independently. Statistical analysis was performed by RevMan4.2 software from Cochrane Collaboration. A P value of < 0.05 was considered statistically significant. RESULTS: Nine RCTs involving 373 patients were included. The combined results showed that glutamine dipeptide has a positive effect in improving postoperative cumulative nitrogen balance (weighted mean difference (WMD = 8.35, 95% CI [2.98, 13.71], P = 0.002), decreasing postoperative infectious morbidity (OR = 0.24, 95% CI [0.06, 0.93], P = 0.04), shortening the length of hospital stay (WMD= -3.55, 95% CI [-5.26, -1.84], P < 0.00001). No serious adverse effects were found. CONCLUSION: Postoperative PN supplemented with glutamine dipeptide is effective and safe to decrease the infectious rate, reduce the length of hospital stay and improve nitrogen balance in patients undergoing abdominal surgery. Further high quality trials in children and severe patients are required, and mortality and hospital cost should be considered in future RCTs with sufficient size and rigorous design. 展开更多

Characteristics of interaction between di-2-ethylhexyl phthalate(DEHP) and particulate in a eutrophic lake were studied in this paper. DEHP concentrations ranged from 89.9 to 247 μg/L with an average value of 146 μg/L in subsurface water (SSW) samples, and from 82.0 to 390 μg/L with an average value of 211 μg/L in water surface microlayer (SM) samples. The results indicate that there was only a weak correlation between the DEHP concentrations and suspended particulate material(SPM) concentrations in both SSW and SM, while the significant correlation between DEHP concentrations and chlorophyll a concentrations was found, which suggestes that DEHP was principally bound to phytoplankton in the eutrophic lake. Correlation between DEHP concentrations and total phosphor (TP) concentrations was also found in this investigation.Enrichment factors (EF) of DEHP in SM were in the range of 0.85 to 2.12 with an average value of 1.35. DEHP EFs were significantly related to the enrichment of chlorophyll a in SM. The results suggest that the enrichment of DEHP in SM of this eutrophic lake was mainly due to DEHP accumulation in phytoplankton and was controlled by distribution of phytoplankton between SM and SSW. 展开更多

In this paper, the analysis of the environmental impacts of a biopolymer based on starch is presented. These impacts were calculated from a cradle to gate LCA (Life Cycle Assessment), based on UNEEN ISO 14040:2006 and UNEEN ISO 14044, using as functional unit 1 kg of produced biomaterial. The impact categories analyzed were global wanning, ozone depletion, acidification and eutrophication. The results indicate that the electricity causes the highest impacts in the global warming and ozone depletion categories, but compared to synthetic polymers, the emissions are low. Finally, in the case of eutrophication, the impacts are similar to other biopolymers but higher in comparison with petroleum-based polymers like HDPE (High Density Polyethylene), LDPE (Low Density Polyethylene), PP (Polypropylene), PS (Polystyrene) and PET (Polyethylene Terephthalate). 展开更多

Fatty acids(FAs) provide energy and also can be used to trace trophic relationships among organisms.Sea cucumber Apostichopus japonicus goes into a state of aestivation during warm summer months.We examined fatty acid profiles in aestivated and non-aestivated A.japonicus using multivariate analyses(PERMANOVA,MDS,ANOSIM,and SIMPER).The results indicate that the fatty acid profiles of aestivated and non-aestivated sea cucumbers differed significantly.The FAs that were produced by bacteria and brown kelp contributed the most to the differences in the fatty acid composition of aestivated and nonaestivated sea cucumbers.Aestivated sea cucumbers may synthesize FAs from heterotrophic bacteria during early aestivation,and long chain FAs such as eicosapentaenoic(EPA) and docosahexaenoic acid(DHA) that produced from intestinal degradation,are digested during deep aestivation.Specific changes in the fatty acid composition of A.japonicus during aestivation needs more detailed study in the future. 展开更多

As part of the design process of the Sidab Coastguard harbour in Oman, 2-dimensional wave transformation modelling was carried out to generate inshore wave conditions (outside the harbour) using the MIKE21 Spectral Wave (SW) model of DHI (Danish Hydraulic Institute). The MIKE21 Boussinesq Wave (BW) model was then used to simulate the wave penetration so that acceptable wave conditions were achieved inside the harbour. Tide levels and currents (speeds and directions) inside and outside the harbour were simulated using the MIKE21 Hydrodynamic (HD) model. Finally, the MIKE21 model was used to simulate the flushing mechanism to ensure good water quality inside the harbour. Both the existing and proposed harbour layouts were modelled for operational and extreme conditions. The influence of cyclones on the design was also considered. An acceptable harbour layout was determined which provided good shelter from waves and with an exchange rate of water between the harbour and the sea which was high enough to prevent eutrophication and to allow removal of man-made pollutants such as accidental oil spills. The paper compares the findings of the present study with a range of internationally used guidelines on harbour flushing. Finally, the paper demonstrates the value of using numerical modelling technique in optimising the layout of a new harbour. 展开更多

Objective To investigate the roles of fructose-1,6-diphosphate(FDP)-added total parenteral nutrition (TPN)in septic animals and stressed patients.Methods Thirteen adult dogs were randomly assigned to one of two study groups 6 hours after the induction of severe intra-abdominal infection.Group TPN(n =6)received 70 kcal· kg-1· d-1 of nonprotein calorie(NPC)and 0.56 g· kg-1· d-1 of nitrogen.1 g/kg of FDP was also infused to the animals in group TPN + FDP(n = 7)everyday.In the clinical study,the control group received routine TPN,while the study group(n = 16)was treated with TPN plus FDP(5 g,two times a day)for 7 days.Results In dogs with TPN support,plasma ATP levels were not changed significantly,while the value in the TPN + FDP group increased significantly from 0.18 μmol/L to 0.46 μmol/L at 24 h and 0.51 μmol/L at 48 h(P < 0.01).Muscular ATP increased markedly in the TPN + FDP group.Muscular creatine phosphate alues were not significantly changed in the TPN group,but the values increased in the TPN + FDP group from 4.06 μmol/g·wt at the beginning to 4.93 μmol/g· wt at 24 h and 5.60 μmol/g·wt at 48 h(P < 0.05),with a cytochrome oxidase increase in immunohistochemistry stain.In the clinical study,plasma ATP levels increased and urinary 3-methylhistidine production significantly decreased with an improved value for positive accumulative nitrogen balance in the FDP-infused group.Conclusion Our results suggest that total parenteral nutrition support with the supplement of fructose-1,6-diphosphate has a positive role in body energy production and protein metabolism in septic animals and stressed patients. 展开更多

Anti-depression effects of electroacupuncture through up-regulating serum E_2 and BDNF and expression of BDNF in hippocampus in chronic depression rats 被引量:4

Objective:To investigate the effects of electroacupuncture (EA) at acupoints Baihui (GV 20), Neiguan (PC 6) and Sanyinjiao (SP 6) on the levels of estradiol (E2) and brain-derived neurotrophic factor (BDNF) in serum, as well as the expression of BDNF in hippocampus in chronic depression rat models. Methods:Thirty female Wistar rats were randomly divided into three groups, including a normal control (NC) group, a model group and an EA group, 10 rats in each group. The depression model was established by using chronic unpredicted mild stress (CUMS), such as cold-water swimming, tail clamping, electric shock to foot, etc., combined with individual caging for 21 d. Rats in the model group and EA group were randomly exposed to one of the 9 stressors each day and caged individually. After modeling, rats in the EA group were then treated with EA at Baihui (GV 20), Neiguan (PC 6) and Sanyinjiao (SP 6) once daily for 14 d, and the model group was not treated with EA but bounded in the same way as the EA group. Serum E2 was measured by radioimmunoassay and BDNF was assessed by an enzyme-linked immunosorbent assay (ELISA) and the expression of BDNF in hippocampus was detected by using immunohistochemistry. Results:After the stress stimulation, compared with the NC group, the model group and EA group showed a significant reduction of sucrose preference rate (P〈0.01) and remarkable increase of forced-swimming immobility time (P〈0.01). In addition, EA significantly reduced the depression-like behavior of rats in the EA group (P〈0.01). The expressions of E2 and BDNF in serum as well as the expression of BDNF in hippocampus were remarkably lower in the model rats than those in the NC group (P〈0.01,P〈0.01,P〈0.05). The expression of BDNF in rats’ serum and hippocampus in the EA group was significantly higher than that in the model group (P〈0.05), while serum E2 increased but insignificantly (P〉0.05). Conclusion:E2 and BDNF may contribute to the depression-like behaviors of the rats during CUMS period. EA may exert its anti-depression effects through promoting BDNF expression in serum and hippocampus. 展开更多

Moreover, when the dependence structure between different risk cells is modeled by the Frank copula, this approach is extended to the multi-dimensional setting. Annual wormwood(Artemisia annua L.) is the only viable source of artemisinin,an antimalarial drug.There is a pressing need to optimize production per cultivated area of this important medicinal plant;however,the effect of increasing atmospheric carbon dioxide(CO_2) concentration on its growth is still unclear.Therefore,a pot experiment was conducted in a free-air CO2 enrichment(FACE) facility in Yangzhou City,China.Two A.annua varieties,one wild and one cultivated,were grown under ambient(374μmol mol^(-1)) and elevated(577 μmol mol^(-1)) CO_2 levels to determine the dry matter accumulation and macronutrient uptake of aerial parts.The results showed that stem and leaf yields of both A.annua varieties increased significantly under elevated CO_2 due to the enhanced photosynthesis rate.Although nitrogen(N),phosphorus(P),and potassium(K) concentrations in leaves and stems of both varieties decreased under elevated CO_2,total shoot N,P,and K uptake of the two varieties were enhanced and the ratios among the concentrations of these nutrients(N:P,N:K,and P:K) were not affected by elevated CO_2.Overall,our results provided the evidence that elevated CO_2 increased biomass and shoot macronutrient uptake of two A.annua varieties. 展开更多

Methods: A total of 46 patients (31 males and 15 females, aged 7-68 years, ( 47 ± 9. 6 ) years on average ) with severe traumatic brain injury were randomized into two groups: Group G (n=23) and Group C (n =23). The patients in Group G received nutritional remedy with the dipeptide of glutamine and alanine, whereas the patients in Group C received routine nutritional therapy only. GCS changes, the length of stay in the neurosurgical intensive care unit (NICU), the mortality, the count of lymphocytes, related complications including lung infection and hemorrhage of alimentary tracts, etc, were examined and recorded. Results: The fatality rate and the length of stay in NICU in Group G was lower than these in Group C (P〈0.05), but no obvious difference was found in GCS changes of the patients between the two groups ( P 〉 0.05 ). The patients with lung infection and alimentary tract hemorrhage in Group G were less than those in Group C (P〈0.05). The count of lymphocytes in Group G was more than that in Group C (P 〈0.05), but no difference was found in other nutritional data. Conclusions: Dipeptide of glutamine and alanine can increase the resisting stress and anti-infection ability of patients with severe traumatic brain injury, which can also lower the mortality and shorten the NICU stay. 展开更多

We also find a binomial-like theorem between the single-variable Hermite polynomials and the two-variable Hermite polynomials. Application of these identities in deriving new integration formulas, but without really doing the integration in the usual sense, is demonstrated. 展开更多

Micronutrient status in soils can be affected by long-term fertilization and intensive cropping.A 19-year experiment (1990-2008) was carried out to investigate the influence of different fertilization regimes on micronutrients in an Aquic Inceptisol and maize and wheat grains in Zhengzhou,China.The results showed that soil total Cu and Zn markedly declined after 19 years with application of N fertilizer alone.Soil total Fe and Mn were significantly increased mainly due to atmospheric deposition.Applications of P and organic fertilizer with incorporation of straws resulted in dramatic increases in soil total Cu,Zn,Fe,and Mn.Soil diethylenetriamine pentaacetic acid (DTPA)-extractable Cu in all treatments sharply decreased from initially 1.12 to about 0.8 mg kg -1 .The treatments with organic fertilizer had the highest soil DTPA-extractable Cu,Zn,Fe,and Mn after 19 years of cropping and fertilization,thus demonstrating the important role of organic fertilizer application in improving available micronutrient status.Cu and Zn contents in wheat grains in the no-P treatments were significantly higher than those of the treatments with P application.In addition,Fe and Mn contents in wheat grains were positively correlated with their soil DTPA-extractable concentrations.These indicated that the long-term application of organic fertilizer resulted in significant increases in soil total and available micronutrient concentrations and remarkable reduction in wheat grain Cu and Zn contents,which was due to high soil available P. 展开更多

This paper studies the concepts of a totally compatible dialgebra and a totally compatible Lie dialgebra,defined to be a vector space with two binary operations that satisfy individual and mixed associativity conditions and Lie algebra conditions respectively.We show that totally compatible dialgebras are closely related to bimodule algebras and semi-homomorphisms.More significantly,Rota-Baxter operators on totally compatible dialgebras provide a uniform framework to generalize known results that Rota-Baxter related operators give tridendriform algebras.Free totally compatible dialgebras are constructed.We also show that a Rota-Baxter operator on a totally compatible Lie dialgebra gives rise to a PostLie algebra,generalizing the fact that a Rota-Baxter operator on a Lie algebra gives rise to a PostLie algebra. 展开更多
