Transient elastography(TE)is a new non invasive tool for measuring liver stiffness,which is correlated to the histologic stage of liver fibrosis.Several studies in chronic liver disease(CLD)have determined a good accu...Transient elastography(TE)is a new non invasive tool for measuring liver stiffness,which is correlated to the histologic stage of liver fibrosis.Several studies in chronic liver disease(CLD)have determined a good accuracy of TE in predicting significant fibrosis and an optimal accuracy in predicting cirrhosis.Normal liver stiffness ranges between 3.3-7.8 KPa and using a cut off of 7.1 KPa,significant fibrosis and cirrhosis can be excluded with a very high negative predictive value(NPV).Positive predictive value(PPV)for the diagnosis of cirrhosis is lower using just a single scan but increases to 90% if high stiffness values are confirmed by a second independent scan.However the presence of fatty liver and metabolic syndrome slightly increases the readings and may reduce the accuracy of the test.It is uncertain if this increase is related to the presence of steatofibrosis or ifit is caused by steatosis itself.TE can be used in screening patients attending the liver clinics to identify those with signifi cant fi brosis or cirrhosis and may be particularly useful in discriminating HBV inactive carriers from chronic hepatitis B patients.TE,however,is not reliable in predicting the presence of esophageal varices in cirrhotics.Another potential indication for TE is the systematic screening of populations at high risk for CLD,such as intravenous drug users and alcoholics,but further studies are needed to determine its diagnostic accuracy in these settings.展开更多

Spontaneous bacterial peritonitis (SBP) is defined as the infection of ascitic fluid (AF) in the absence of a contiguous source of infection and/or...Ascites remain the commonest complication of decompensated cirrhosis. Spontaneous bacterial peritonitis (SBP) is defined as the infection of ascitic fluid (AF) in the absence of a contiguous source of infection and/or an intraabdominal inflammatory focus. An AF polymorphonuclear (PMN) leucocyte count ≥ 250/mm 3 -irrespective of the AF culture resultis universally accepted nowadays as the best surrogate marker for diagnosing SBP. Frequently the results of the manual or automated PMN count do not reach the hands of the responsible medical personnel in a timely manner. However, this is a crucial step in SBP management. Since 2000, 26 studies (most of them published as full papers) have checked the validity of using leukocyte esterase reagent strips (LERS) in SBP diagnosis. LERS appear to have low sensitivity for SBP, some LERS types more than others. On the other hand, though, LERS have consistently given a high negative predictive value (> 95% in the majority of the studies) and this supports the use of LERS as a preliminary screening tool for SBP diagnosis. Finally, an AF-tailored dipstick has been developed. Within the proper setting, it is set to become the mainstream process for handling AF samples.展开更多

In this work,an elaborated 3D model fully compliant with the geometrical size of radial tire 185/60 R15 is worked up, for evaluating the effects of components properties and working conditions on deformation and stress/strain fields created inside the tire. For the simulation, the tire structure is assumed to be composed of tread, carcass ply, and bead. The mechanical behavior of rubber as main component of tire is described by Mooney-Rivlin material model. The comparison of the obtained results and laboratory tests demonstrates the validity and high accuracy of analysis.展开更多

Headspace solid phase microextraction chromatography (HS-SPME/GC) was evaluated as a tool in determining the rate of oxidation in oxidized soybean oil samples by measuring the production of hexanal as a secondary ma...Headspace solid phase microextraction chromatography (HS-SPME/GC) was evaluated as a tool in determining the rate of oxidation in oxidized soybean oil samples by measuring the production of hexanal as a secondary major volatile breakdown product of linoleic acid. Samples of the headspace taken from sealed 20 mL vials, incubated 30 min at 50 ~C followed by 5 min adsorption, were injected into a gas chromatograph with 2 min thermal desorption. In applying SPME, different analytical conditions were evaluated. The linearity of response of the volatiles for the HS-SPME/GC procedure using a carboxen-polydimethylsiloxane fiber was determined from 3 g of mineral oil spiked with a hexanal standard solution at different levels. Using the optimized extraction conditions, an R value close to unity (R = 0.999) was found, and the repeatability (n = 1 l) was 6.31%. The results indicated that hexanal is linearly related to peroxide value (PV) only intermediate PV ranges (10-18 meq/kg). The study also showed that HS-SPME/GC procedure was a simple and reproducible method for the analysis of hexanal in the HS of commercial soybean oil samples, and is useful as a quality control and research tool for the evaluation of flavor quality and shelf life of vegetable oils.展开更多

Polycrystalline diamond (PCD) tools with different grain s...The milling machinabilities of titanium matrix composites were comprehensively evaluated to provide a theoretical basis for cutting parameter determination. Polycrystalline diamond (PCD) tools with different grain sizes and geometries, and carbide tools with and without coatings were used in the experiments. Milling forces, milling temperatures, tool lifetimes, tool wear, and machined surface integrities were investigated. The PCD tool required a primary cutting force 15 % smaller than that of the carbide tool, while the uncoated carbide tool required a primary cutting force 10% higher than that of the TiA1N-eoated tool. A cutting force of 300 N per millimeter of the cutting edge (300 N/mm) was measured. This caused excessive tool chipping. The cutting temperature of the PCD tool was 20%-30% lower than that of the carbide tool, while that of the TiA1N-coated tool was 12% lower than that of the uncoated carbide tool. The cutting temperatures produced when using water-based cooling and minimal quantity lubrication (MQL) were reduced by 100 ~C and 200 ~C, compared with those recorded with dry cutting, respectively. In general, the PCD tool lifetimes were 2--3 times longer than the carbide tool lifetimes. The roughness Ra of the machined surface was less than 0.6μm, and the depth of the machined surface hardened layer was in the range of 0.15-0.25 mm for all of the PCD tools before a flank wear land of 0.2 mm was reached. The PCD tool with a 0.8 mm tool nose radius, 0% rake angle, 10% flank angle, and grain size of (30+2) μm exhibited the best cutting performance. For this specific tool, a lifetime of 16 rain can be expected.展开更多

Group I u...AIM: To define the evolution of ischemic lesions with 7T magnetic resonance imaging (7T-MRI) in an animal model of acute colonic ischemia. METHODS: Adult Sprague-Dawley rats were divided into two groups. Group I underwent inferior mesenteric artery (IMA) ligation followed by macroscopic observa- tions and histological analysis. In group H, 7T-MRI was performed before and after IMA ligation and followed by histological analysis. RESULTS: Morphological alterations started to develop 1 h after IMA ligation, when pale areas became evident in the splenic flexure mesentery and progressively wors-ened up to 8 h thereafter, when the mesentery was less pale, and the splenic flexure loop appeared very dark. The 7T-MRI results reflected these alterations, showing a hyperintense signal in both the intraperitoneal space and the colonic loop wall 1 h after IMA ligation; the lat- ter progressively increased to demonstrate a reduction in the colonic loop lumen at 6 h. Eight hours after IMA ligation, MRI showed a persistent colonic mural hyper- intensity associated with a reduction in peritoneal free fluid. The 7T-MRI findings were correlated with histolog- ical alterations, varying from an attenuated epithelium with glandular apex lesions at 1 h to coagulative necro- sis and loss of the surface epithelium detected 8 h after IMA ligation. CONCLUSION: MRI may be used as a substitute for invasive procedures in diagnosing and grading acute ischemic colitis, allowing for the early identification of pathological findings.展开更多

An abundance of scenarios literature exists in which numerous pictures ha...A description of the future as it might be without forecasts, predictions and trend analysis can be referred to as a 'future scenario'. An abundance of scenarios literature exists in which numerous pictures have been painted of changed future worlds. However, upon closer inspection it becomes apparent that virtually all of this literature refers to changes occurring above ground, ignoring the inevitable consequences (or opportunities) for sustainable use of underground space, not least in densely populated urban areas, some of which may be underlain by complex geological conditions. This paper reports on the development (to date) of an 'Urban Futures' (UF) toolkit for sub-surface environments (including infrastructure and utilities) which, through use of 'key' questions 'sustainable' indicators and a 'contextual' narrative, allows for better definition and measured performance of underground space in the UK, both today and in the future. The toolkit explores possible uses for underground space within 4 plausible and well-cited future scenarios (i.e. New Sustainability Paradigm, Fortress World, Market Forces and Policy Reform). This forms part of a much larger generic toolkit which the UF research team are developing for exploring possible future scenarios over a range of UK urban regeneration case study areas.展开更多

This study introduces ...The influence of an acoustic logging tool on borehole guided wave propagation should be considered in the processing and inversion of the guided waves for formation acoustic property estimation. This study introduces an equivalent-tool theory that models the tool response using an elastic rod with an effective modulus and applies the theory to multipole acoustic logging for both wireline and logging while drilling (LWD) conditions. The theory can be derived by matching the tool’s acoustic impedance/conductance to that of the multipole acoustic wavefield around the tool, assuming that tool radius is small compared to wavelength. We have validated the effectiveness and accuracy of the theory using numerical modeling and its practicality using field data. In field data applications, one can calibrate the tool parameters by fitting the theoretical dispersion curve to field data without having to consider the actual tool’s structure and composition. We use a dispersion correction example to demonstrate an application of the simple theory to field data processing and the validity of the processing result.展开更多
This study investigates how orthographic,semantic and contextual variables—including word length,concreteness,and contextual support—impact on the processing and learning of new words in a second language(L2)when fi...This study investigates how orthographic,semantic and contextual variables—including word length,concreteness,and contextual support—impact on the processing and learning of new words in a second language(L2)when first encountered during reading.Students learning English as a foreign language(EFL)were recruited to read sentences for comprehension,embedded with unfamiliar L2 words that occurred once.Immediately after this,they received a form recognition test,a meaning recall test,and a meaning recognition test.Eye-movement data showed significant effects of word length on both early and late processing of novel words,along with effects of concreteness only on late-processing eye-tracking measures.Informative contexts were read slower than neutral contexts,yet contextual support did not show any direct influence on the processing of novel words.Interestingly,initial learning of abstract words was better than concrete words in terms of form and meaning recognition.Attentional processing of novel L2 words,operationalized by total reading time,positively predicted L2 learners’recognition of new orthographic forms.Taken together,these results suggest:1)orthographic,semantic and contextual factors play distinct roles for initial processing and learning of novel words;2)online processing of novel words contributes to L2 learners’initial knowledge of unfamiliar lexical items acquired from reading.展开更多

The clinical presentation of adult cases of intussusception and enteric duplication is non-specific and thus poses a diagnostic challenge. A computed tomography (CT) scan is recommended in adult cases as the most sensitive diagnostic tool and the pathognomonic finding of outer intussuscepiens and central intussusceptum is diagnostic. A septum of a duplicated colon in a non-intussuscepted segment has been rarely reported in the literature. With advancements in radiological imaging technology and the increased availability of CT scanners, the capacity for a correct pre-operative diagnosis has been significantly enhanced. Our current case report illustrates the importance of considering an uncommon etiology for enteric intussusception and duplication as a differential diagnosis of acute abdomen in an adult patient. Our analyses of this patient also highlight the successful use of CT scanning to make this diagnosis.展开更多

文摘This study investigates how orthographic,semantic and contextual variables—including word length,concreteness,and contextual support—impact on the processing and learning of new words in a second language(L2)when first encountered during reading.Students learning English as a foreign language(EFL)were recruited to read sentences for comprehension,embedded with unfamiliar L2 words that occurred once.Immediately after this,they received a form recognition test,a meaning recall test,and a meaning recognition test.Eye-movement data showed significant effects of word length on both early and late processing of novel words,along with effects of concreteness only on late-processing eye-tracking measures.Informative contexts were read slower than neutral contexts,yet contextual support did not show any direct influence on the processing of novel words.Interestingly,initial learning of abstract words was better than concrete words in terms of form and meaning recognition.Attentional processing of novel L2 words,operationalized by total reading time,positively predicted L2 learners’recognition of new orthographic forms.Taken together,these results suggest:1)orthographic,semantic and contextual factors play distinct roles for initial processing and learning of novel words;2)online processing of novel words contributes to L2 learners’initial knowledge of unfamiliar lexical items acquired from reading.
文摘Colonic intussusception and gastrointestinal duplication are diseases that arise in young children. The clinical presentation of adult cases of intussusception and enteric duplication is non-specific and thus poses a diagnostic challenge. A computed tomography (CT) scan is recommended in adult cases as the most sensitive diagnostic tool and the pathognomonic finding of outer intussuscepiens and central intussusceptum is diagnostic. A septum of a duplicated colon in a non-intussuscepted segment has been rarely reported in the literature. With advancements in radiological imaging technology and the increased availability of CT scanners, the capacity for a correct pre-operative diagnosis has been significantly enhanced. Our current case report illustrates the importance of considering an uncommon etiology for enteric intussusception and duplication as a differential diagnosis of acute abdomen in an adult patient. Our analyses of this patient also highlight the successful use of CT scanning to make this diagnosis.
文摘The American writer Kate Chopin's masterpiece, The Awakening published in the late 19th century is one of the classics in female literature. The heroine Edna having the courage to pursue self-reliance and express her self-awareness, is regarded as a "new woman", while the comments on her husband, Mr. Pontellier are quite negative--He regards Edna as a private property and a sexual tool, leading to her awakening and suicide. In this paper, the author reinterprets Mr. Pontellier with the method of close reading in new criticism, deeming that it is the binary opposition in thinking that leads to the misreading. Mr. Pontellier was just a follower of the tradition and custom. The objective evaluation and reinterpretation to him can help students to develop the habit of critical thinking and the courage to pursue truth is case they form a simple thinking mode of the binary opposition.
