目的:探究真武汤加味治疗急性心力衰竭(简称急性心衰)的临床疗效,为疾病的诊治提供临床依据。方法:收集2017年4月-2018年10月我院心脏内科治疗的急性左心衰竭患者共100例,采用随机数表法将其随机分成对照组和治疗组,各50例患者。对照组... 目的:探究真武汤加味治疗急性心力衰竭(简称急性心衰)的临床疗效,为疾病的诊治提供临床依据。方法:收集2017年4月-2018年10月我院心脏内科治疗的急性左心衰竭患者共100例,采用随机数表法将其随机分成对照组和治疗组,各50例患者。对照组患者进行常规治疗包括吸氧、利尿、扩冠、强心、镇静等方面。治疗组患者在上述对照组常规治疗之上,予口服真武汤加减治疗,嘱患者于每日早、晚饭后半小时温服水煎服,每次服用100 mL。分别对两组患者进行为期14 d的持续性治疗。经过14 d治疗疗程后进行疗效评估,观察治疗后两组患者的中医证候评分、急性生理慢性健康状况评分(APACHE Ⅱ评分)、氨基末端脑钠肽前体(NT-proBNP)、血乳酸含量、左室射血分数(Left Ventricular Ejection Fraction,LVEF)水平变化情况。结果:治疗后两组患者中医证候评分、APACHE Ⅱ评分、NT-proBNP、血乳酸含量、LVEF较治疗前均有所改善,且治疗后治疗组明显优于对照组,差异具有统计学意义(P <0.05)。结论:采用真武汤加减治疗急性心衰可以提高心肌收缩能力,改善心脏缺血组织结构,降低心肌耗氧量,提高心脏功能,大大降低急性心衰患者痛苦,提高其生活质量而重返社会。 展开更多

To solve the contradiction between supply and demand of land, the con- struction of land resource security guarantee system is an important part to coordi- nate the economic development and cultivated land protection in the core grain pro- duction region. Based on the deep analysis on the land resources characteristics and problems using the combined method of literature review method with qualita- tive and quantitative analysis, we constructed the land resource security system for different categories according to the land use principles of optimization, intensivism, improvement, and overall planning, and proposed the corresponding supporting mea- sures. The results showed that controlling the new increase, promoting the reclama- tion and improvement of land are the basis for land resource security, clearing the idle, improving the utilization rate of stock are the core of the efficient use of land resources, and establishing a unified urban and rural construction land market is the key to promoting the economical and intensive utilization of land resources, provid- ing scientific basis for the sustainable utilization of land resources in the core grain production regions. 展开更多

Objective To investigate the performance of dual-source computed tomography (DSCT) using high-pitch spiral fliPS) mode for coronary stents patency. Methods We conducted a prospective study on 120 patients with 260 previous stents implanted due to recurred suspicious symptoms of angina scheduled for invasive coronary angiography (ICA), while DSCT were conducted using HPS mode. Results There was no significant impact of age, body mass index or heat rate (HR) on image quality (P 〉 0.05), while HR variability had a slight impact on that (P 〈 0.05). Sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) of DSCT in detection of in-stent restenosis (ISR) based per-patient were 92.3%, 96.7%, 88.9%, and 97.8%, respectively. And those based per-stent were 87%, 96.8%, 83.3%, and 97.7% with un-assessment stents, 97.4%, 99.5%, 97.4%, and 99.5% without un-assessment stents. There was significant differ- ence on sensitivity, specificity, PPV and NPV between diameter 〉 3.0 mm group (93.3%, 97.9%, 87.5%, and 98.9%) and diameter 〈 3.0 mm group (80%, 93.3%, 80.0%, and 93.3%) (P 〈 0.05), and that between stent number 〉 3 group (82.3%, 77.8%, 66.7%, and 60%) with 〈 3 group (97.3%, 80%, 96.5%, and 75%). The effective dose of DSCT (1.4 ± 0.5 mSv) is significantly less than that by invasive coronary angiography [4.0 ± 0.8 mSv (P 〈 0.01)]. Conclusion DSCT using HPS mode provides good diagnostic performance on stent patency with lower effective dose in patients with HR 〈 65 beats/rain. 展开更多

Decreasing the flow completion time(FCT) and increasing the throughput are two fundamental targets in datacenter networks(DCNs), but current mechanisms mostly focus on one of the problems. In this paper, we propose OFMPC, an Open Flow based Multi Path Cooperation framework, to decrease FCT and increase the network throughput. OFMPC partitions the end-to-end transmission paths into two classes, which are low delay paths(LDPs) and high throughput paths(HTPs), respectively. Short flows are assigned to LDPs to avoid long queueing delay, while long flows are assigned to HTPs to guarantee their throughput. Meanwhile, a dynamic scheduling mechanism is presented to improve network efficiency. We evaluate OFMPC in Mininet emulator and a testbed, and the experimental results show that OFMPC can effectively decrease FCT. Besides, OFMPC also increases the throughput up to more than 84% of bisection bandwidth. 展开更多

It is conducted by having questionnaires and interviews among 220 students and by analyzing the teaching materials. The findings to the study indicate that the main obstacles are caused from the following aspects: neglecting the importance of developing vocabulary in the advanced study; neglecting activating the students' directing sense and detecting sense to difficult words; low rate of word recurrence in the teaching materials; requiring distinguishing the shade of the difference between synonyms excessively and too meticulously; overemphasizing the "negative transfer" of mother tongue; and some other obstacles to the development of lexical competence. 展开更多

Calculations show that the thermostat p... We have studied three plans for re-use of the abandoned mine roadway tunnels as an energy center. These are the thermostat plan, the thermal accumulator plan, and the CAES plan. Calculations show that the thermostat plan can provide over 15,000 m2 of building air-conditioning/heating load for each kilo- meter of roadway, but electric power is needed to run the system. Numerical research proved that the accumulation of hot water in the roadway for seasonal heating purposes (a temperature swing from 90 to 54 ℃) is a viable possibility. The CAES plan proposes using the discarded coal mine tunnel as a pea ing power station with an energy storage density over 7000 kj/m3. It can be concluded that presently abandoned coal mines could be reformed into future energy centers for a city. 展开更多

Objective To prospectively investigate the diagnostic accuracy for coronary artery stenosis of prospectively electrocardiogram-triggered spiral acquisition mode (high pitch mode) dual-source computed tomography coronary angiography (CTCA) in patients with relatively higher heart rates (HR) compared with catheter coronary angiography (CCA). Methods Forty-seven consecutive patients with relatively higher HR (>65 and <100 bpm) (20 male, 27 female; age 55±10 years) who both underwent dual-source CTCA and CCA were prospectively included in this study. All patients were performed CTCA using high pitch mode setting at 20%-30% of the R-R interval for the image acquisition. All coronary segments were evaluated by two blinded and independent observers with regard to image quality on a three-point scale (1: excellent to 3: non-diagnostic) and for the presence of significant coronary stenoses (defined as diameter narrowing exceeding 50%). Considered CCA as the standard of reference, the sensitivity, specificity, positive predictive value and negative predictive value were calculated. Radiation dose values were calculated using the dose-length product. Results Image quality was rated as being score 1 in 92.4% of segments, score 2 in 6.1% of segmentsand score 3 in 1.5% of segments. The average image quality score per segment was 1.064±0.306. The HR variability of patients with image score 1, 2 and 3 were 2.29±1.06 bpm, 5.17±1.37 bpm, 8.88±1.53 bpm, respectively. The average HR variability of patients with different image scores were significantly different (F=170.402, P=0.001). The sensitivity, specificity, positive and negative predictive values were 92.6%, 97.0%, 87.6%, 98.3%, respectively, per segment and 90.0%, 95.2%, 85.3%, 96.9%, respectively, per vessel and 100%, 63.6%, 90.0%, 100%, respectively, per patient. The effective radiation dose was on average 0.86±0.16 mSv. Conclusion In patients with HR more than 65 bpm and below 100 bpm without cardiac arrhythmia, the prospectively electrocardiogram-gated high-pitch spiral acquisition mode with image acquired timing set at 20%-30% of the R-R interval provides a high diagnostic accuracy for the assessment of coronary stenoses combined with a 1.5% of non-diagnostic coronary segments and a radiation dose below 1 mSv. 展开更多

This study was to understand the physiological changes of four kinds of crop seeds including baby corn, cowpea, radish and rape- seed, under the treatment of ultra-drying. Large amounts of seeds as experimental materials ( 10 kg for each material) were respectively dried to the suitable water content consistent with the ultra-dried seed criterion via heating at 50 ~C; then the physiological indices of these ultra-dried seeds such as water content, seed germination, dehydrogenase activity and SOD activity were measured. The results showed that except cowpea seeds, whose ultra-drying treatment should be controlled between two and four days, other three kinds of seeds were tolerant to high tem- perature and low water content. The ultra-dried seeds assumed similar or even higher activities than CKs, but showed no damage symptom. Our result proves that ultra-drying treatment via heating at 50 ℃ is safe and efficient to experimental seeds. 展开更多

Borges's poetics of reading and dreaming require another poetics of the work as a text that calls attention to its own artifice. In reading Borges' fiction, the separate roles and identities of reader, writer, and work of fiction merge and exchange roles, powers, and identities and are transformed into a single act of dreaming, which assumes cosmogonic and apocalyptic risks. In the dominant role given to the reader, the work of fiction as an object or work of art does not exist unless it is read. There is no determinate text, only a version of our own we re-write and invent every time we read the text. The author's reading is not a spontaneous evocation of vision, but an artifice, as artificial as the writing of the fiction. As writers and readers we are composed of texts and schemata, alphabets and artifacts, not merely mental perceptions and ideas. The reader requires a prior text to copy, translate, and recreate, and that text only exists as a fictional microcosm in so far as it is being read by a Reader who is able to actualize the revelation only imminent within it. 展开更多

The relationship between cross-regional cultural landscapes does not currently receive enough attention in cultural landscape conservation. Cultural landscapes in China are faced with the crisis of fragmentation and islanding, which makes it necessary to strengthen the idea of integrated conservation. The Great Wall cultural belt in Beijing is a cross-regional linear cultural landscape in the northwest mountainous area of Beijing. With the Great Wall as a link connecting the surrounding natural, social, and cultural resources to form a banded structure, it has the typical and practical significance of integrated conservation. Based on the theory of the cultural landscape security pattern and its ‘node – setting – connection' model, this research studies the spatial structure and shaping mechanism of the Great Wall cultural belt in Beijing from the perspective of tourist flow, using Flickr geo-tagged photos and Wikiloc tracks. The results identify the core nodes and intermediate nodes of the Great Wall cultural belt in Beijing and show that it has a multicore, multi-group hierarchical nesting structure differing from the geographic space, which has groups following the evolution orbit of ‘linear string – circumnuclear star – complete network.' To explain the differences between the cultural association network(CAN) and the geospatial network(GSN), we find that the cultural associations between nodes rely on geophysical channels, including heritage corridors and traffic passages, as physical carriers, but the nodes' attraction and their setting elements, containing natural geography environments, settlements, administrative divisions, etc., change the channel resistance, so CAN and GSN present different characteristics. From the perspective of systematic and spatial network thoughts, this study has made an attempt to adopt new analysis and research methods to achieve the integrated conservation and inheritance of linear cultural landscapes in mountainous areas. 展开更多

The sources and enrichment of organic matter in a sediment core in the first member of the Xiagou Formation(K_1g^1) from the Chang 2-2 borehole of the Jiuquan Basin,NW China,have been examined using Rock-Eval,maceral,carbon isotopes and biomarker data.This data indicates that highly variable organic matter sources and preservation conditions in response to climate change.TOC content,HI,and δ^(13)C value were strongly correlated with the abundance of gammacerane,woody organic matter content,steranes/hopanes ratio,and C_(29) sterane content.This correlation demonstrates the importance that the control of the salinity of the depositional environment and organic matter sources can have upon the enrichment,type,and carbon isotopic composition of organic matter.In the Jiuquan Basin's relatively high temperature and arid climate,high salinity lakes with high primary productivity of algae,planktons,and bacteria,and good organic matter preservation conditions(anoxic bottom water) resulted in the enrichment of isotopically-light algae-bacterial organic matter.In the Jiuquan Basin's regions with a relatively low temperature and wet climate,fresh lakes with low primary productivity of algae,planktons,and bacteria received significant terrigenous high plants input,resulting in the deposition of a low abundance of isotopically heavier terrestrial organic matter. 展开更多

The Business Waste Reduction Assistance Program: A Collaborative Economic Stimulus and Energy Reduction Initiative in Northwest Ohio, USA

In 2005, an innovative program was formed between the local government and The University of Toledo to help improve environmental and economic conditions in Lucas County, Ohio, USA by providing no cost energy assessments to various types of organizations. Unlike the Industrial Assessment Centers, which focus on manufacturing and are funded by the Federal Government, this program demonstrates that successful partnerships can be established at the local government level to aid various types of organizations in energy conservation and cost reduction. Since 2005, the program completed ten energy assessments and identified over 143,000 kwh and 103,000 kg of CO2 for reduction. Additionally, over $12,000 has been identified as annual cost savings for Lucas County businesses. The purpose of this paper is to provide a complete overview and framework of this program so that other institutions may learn from it and adopt similar programs at the local level. A focus of this paper is a discussion of a case study that details the process and results of a typical energy assessment conducted through the project and comparison to similar programs in the US. 展开更多
