This study assessed the potent... Hepatitis B virus (HBV)-induced liver failure is an emergent liver disease leading to high mortality. The severity of liver failure may be reflected by the profile of some metabolites. This study assessed the potential of using metabolites as biomarkers for liver failure by identifying metabolites with good discriminative performance for its phenotype. The serum samples from 24 HBV-indueed liver failure patients and 23 healthy volunteers were collected and analyzed by gas chromatography-mass spectrometry (GC-MS) to generate metabolite profiles. The 24 patients were further grouped into two classes according to the severity of liver failure. Twenty-five eommensal peaks in all metabolite profiles were extracted, and the relative area values of these peaks were used as features for each sample. Three algorithms, F-test, k-nearest neighbor (KNN) and fuzzy support vector machine (FSVM) combined with exhaustive search (ES), were employed to identify a subset of metabolites (biomarkers) that best predict liver failure. Based on the achieved experimental dataset, 93.62% predictive accuracy by 6 features was selected with FSVM-ES and three key metabolites, glyeerie acid, cis-aeonitie acid and citric acid, are identified as potential diagnostic biomarkers. 展开更多

AIM: To describe the diagnostic criteria for acute liver failure due to Wilson disease (WD), which is an uncommon cause of acute liver failure (ALF). METHODS: We compared findings of patients presenting with ALF due to WD to those with ALF of other etiologies. RESULTS: Previously described criteria, such as low alkaline phosphatase activity, ratio of low alkaline phosphatase to total bilirubin or ratio of high aspartate arninotransferase (AST) to alanine arninotransferase (ALT), failed to identify patients with ALF due to WD. There were significant differences in low ALT and AST activities (53 ± 43 vs 1982 ± 938, P 〈 0.0001 and 87 ± 44 vs 2756 ± 2941, P = 0.037, respectively), low choline esterase activity (1.79 ± 1.2 vs 4.30 ± 1.2, P = 0.009), high urine copper concentrations (93.4 ± 144.0 vs 3.5 ± 1.8, P = 0.001) and low hemoglobin (7.0 ± 2.2 vs 12.6 ± 1.8, P 〈 0.0001) in patients with ALF caused by WD as compared with other etiologies. Interestingly, 4 of 7 patients with ALF due to WD survived without liver transplantation. CONCLUSION: In ALF, these criteria can help establish a diagnosis of WD. Where applicable, slitlamp examination for presence of Kayser-Fleischer rings and liver biopsy for determination of hepatic copper concentration still remain important for the diagnosis of ALF due to WD. The need for liver transplantation should be evaluated carefully as the prognosis is not necessarily fatal. 展开更多

The role of whole-body FDG [(~(18)F)2-fluoro-2-deoxy-glucose] positron emission tomography(PET)scan-ning as an imaging modality in the management of patients with malignancy has evolved enormously over the past two decades.FDG-PET has demonstrated signifi cant effi cacy in the staging,prognostication and detection of occult metastatic disease in malignancies of the gastrointestinal tract,in addition to assessment of the response to cytotoxic chemotherapy in a more timely manner than has traditionally been possible by more conventional imaging tools.The sensitivity and specif icity of FDG-PET for the detection and staging of malignancy depend not only on the site and size of the primary tumor and metastases,but also on histologi-cal cell type,reflecting underlying disparities in glucose metabolism.The metabolic response to neo-adjuvant chemotherapy or to chemo-radiotherapy in cancers of the gastro-esophageal junction or stomach has been demonstrated in several prospective studies to correlate signifi cantly with both the histological tumor response to treatment and with consequent improvements in overall survival.This may offer a future paradigm ofpersonalized treatment based on the PET response to chemotherapy.FDG-PET has been less successful in efforts to screen for and detect recurrent upper gastro-intestinal malignancies,and in the detection of low vol-ume metastatic peritoneal disease.Efforts to improve the accuracy of PET include the use of novel radiotrac-ers such as(~(18)F)FLT(3-deoxy-3-fluorothymidine)or 11C-choline,or fusion PET-CT with concurrent high-res-olution computed tomography.This review focuses on the role of FDG-PET scanning in staging and response assessment in malignancies of the upper gastrointesti-nal tract,specif ically gastric,esophageal and pancreas carcinoma. 展开更多

Twenty three samples, ranging in age from 200 to 9000 a,including multiple styles and shapes and drawn from diverse and remote areas in China have been used to establish the typical annual dose as 5.5mGy/a and its associated deviation. Behind what is called "Needham's Grand Question" (why was China overshot by the West in science and technology?) lies a deeper question of how China came to lose the capacity of deeply reflective thought clearly present in the ancient Chinese philosophers. This is a loss felt by all Chinese as a psychological sense of hollowness, a loss of identity, made worse by the seeming inaccessibility of the ancient Chinese wisdom to the modern Chinese mind. It is clear that at some historical point China suffered an extreme psychological blow sufficient to traumatise it at the threshold of reflective thought, unable to look inwards any more. The paper identifies that point as the utter devastation wrought by Kublai Khan and the Mongols 750 years ago. What devastates reflective thought is wilfulness, the insistent focusing of all attention and energy on external, material things, and Kublai Khan was wilful in the extreme. What confirms this as the crippling point is that, in response to Kublai Khan's Mongol invasion the Chinese, over time, not only completely altered the geography of China itself, moving their capital to the North (Beijing), but have ever since fought to establish as "China" all the territory over which Kublai Khan ruled. China is clearly not free of Kublai Khan's shadow. But even more precisely, in the process of doing this--and showing their own wilfulness--in building the Forbidden City in Beijing they built it in the shape of the Chinese metaphysical model of the universe, the Chinese version of the Tree of Life metaphysical glyph But it has an error in it. And the error is precisely that in where it places things, it makes what would have been the attributes of reflective thought subservient to wilfulness. The model itself thus shows the hollowness of the Chinese mind from that moment on. The outer form--the "appearance"---of the ancient wisdom was still there. But the content--the "substance"--of it was not. And with no reflective thought, true creativity disappears. 展开更多

Geological Structure and Quaternary Activity of the Zhangjiakou Fault, Northern China

Based on detailed field investigations, this paper describes the geometrical characteristics and tectonic activities of the Zhangjiakou fault at the northwest of Beijing. This fault strikes mainly northwest to west, short parts of which strike near east to west, dipping north, and extends over a length of 70km. It is a major geological and geomorphologicai margin, controlling the neotectonic movement in this region. On the south side of the Zhangjiakou fault are the Late Quaternary unconsolidated deposits, forming basins; while on the other side are Mesozoic volcano debris and Pre-Mesozoic metamorphic rocks, forming lower mountains and hills. The Zhangjiakou fault consists mainly of high-angle inverse strike-slip fault and partially of normal strike-slip fault. Among these, the north-dipping NW-NWW-trending secondary faults, constituting the main fracture of the fault, have inverse characteristics; those near the EW-trending secondary faults are links of the former faults, with a smaller length and normal faulting characteristics. Thus, the Zhangjiakou fault is a north-dipping inverse and partially south-dipping normal strike-slip fault. The Zhangjiakou fault has been continuously active since the Quaternary. With the exception of the western end extension, which has been active since the late Pleistocene, the main part of the fault has been active since the Holocene. The central main segment of the Zhangjiakou fault is more active. Since the mid-late period of the late Pleistocene, the average vertical slip rate of a single fault has been over 0. 07mm/a - 0. 30mm/a. The Zhangjiakou fault has multi-slip surfaces, and the total vertical slip rate reaches 1.33mm/a, estimated from the Qingshuihe river terraces and the relevant drilling data. 展开更多

This paper reviews the history and progress of research on active tectonics in China and overseas.By giving a brief introduction on the history of active tectonic research in China and other countries,the paper sums up the process and development of quantitative investigation of active tectonics since the 1980s.The focus is on the main efforts and progress made in China on certain aspects of research,such as basic surveys and applied investigation of active tectonics,the study of theories related to regional active tectonics and their kinematics and geodynamics,surveys on coupling relations between deep and shallow structures,active fault surveys and prospecting and seismic hazard assessment in urban areas,as well as the efforts made using Quaternary geochronology.Furthermore,the paper looks back on Chinese quantitative investigation of active tectonics in China and sums up cognitions derived from studies on the determination of several basic and measurable parameters of active tectonics.These parameters include the length of fault and fault segmentation,coseismic slip and cumulative slip,fault slip rate,the sequence of paleoearthquake events and the time elapsed since the most recent event.At the same time,efforts and progress made in China on assessing the long-term seismic potential for active faults and evaluating the risk from potential active fault movement have been reviewed by summarizing research on developing theories,models,methods and the application of time-dependent seismic potential to probabilistic assessment,magnitude estimation for potential earthquakes on active faults,and the forecast of potential risk caused by active fault movement.Finally,in consideration of the realities and problems in the research of active tectonics in China,the authors put forward several suggestions for issues worthy of more attention for further investigation in the future. 展开更多

Elderly patients over 65, especially those over 75 years are more vulnerable to being prescribed inappropriate medications because of the high incidence of co-medication in this population resulting from the presence of multiple chronic diseases and also age-related changes in pharmacokinetics and pharmacodynamics. The aim at this study was to explore the habits of taking medications without recommendation fracture among older patients, to determine which medications are most often used without prescription, and if there is difference in this habits considering gender and age of respondents. Methods: A cross-sectional study was conducted by interviewing all patients 〉 65 years that have arrived from 01.10. to 15.12.2015 in one ambulance of the Health Center of Mostar using the adapted questionnaire made out for the research. 90 questionnaires were properly filled for analysis, 60.0% were women and 40.0% of them were men. There was 45.6% respondents between 65 and 75 years, and 54.4% 76 years and above. Resaults: 15.6% of respondents said that they don't take their therapy regularly. Women compared to men, significantly more often use non-prescription drugs (P = 0.007). Analgesics and sedatives are more often used as non-prescription drugs, compare to supplements (P = 0.002). Conclusion: This study, although conducted on a small sample should be an alarm for general practitioners. They should think more about adverse effects of drugs on the aged, and use Beers criteria in every day practice. In addition, pharmacists should respect the legislation and not sell drugs without doctor's prescription to which it applies. 展开更多

Sinian, Cambrian, Ordovician, Devonian, Carboniferous,Triassic, Jurassic and Cretaceous strata are involved in the thrust nappe system. The nappe structure is of the type of duplex structures formed as a result of the earlier stage migration from SE to NW and late stage migration from E to W of sedimentary cover or basement strata. Formation of the nappe structure in the studied area involves two main epochs: Early Yanshanian and Late Yanshanian to Early Himalayan. The mineral deposits and the buried coalfields in the area, especially the latter, are extensively controlled by the nappe structure. 展开更多

Structural characteristics of the Jurassic basins of Xining, Minhe, and Xiji in the east of middle Qilian were researched based on the data obtained by gravitational, magnetic, and seismic methods. The result shows that each of these three basins is an independent structural unit with a NW strike and being separated by upheavals. Two groups of faults with NW and NE directions are developed in the basin, which controls the formation and evolution of the (Jurassic basins). The NW faults are the main ones while the NE faults are the secondary for controlling the sedimentation. Of the three basins, the Minhe basin is the favorable prospecting area. 展开更多

The new oxovanadium (V) complex, [PyH][VO2(L)] 1 (salicyladehyde 5-bromo salicyloylhydrazone is abbreviated as H2L; Hpy is protonated pyridine) was obtained from a refluxed solution of VOSO4 and H2L in acetonitrile-methanol-pyridine. Similarly, another new complex, [VO(L)(OCH3)] 2 was synthesized by refluxing VOSO4 and H2L in methanol-pyridine. Crystal data for 1: C19H15N3O5BrV, Mr= 496.2, monoclinic, P21/n, a = 7.1885(3), b = 9.2718(3), c = 28.803(1) A, β = 96.185(1)°, Z = 4 and V = 1908.6(1) A3; for 2: C15H12N2O5BrV, Mr= 431.1, monoclinic, P2,/n, a = 12.202(2), b = 8.045(2), c = 16.604(3) A, β = 101.29(3)°, Z = 4 and V = 1598.4(2) A3. The structures of 1 and 2 have been determined by X-ray analyses and reveal that the coordination environments of V atoms in both complexes are of square-based pyramid. Three of the four based donor atoms are from the tridentate 'ONO' donor ligand while the fourth is one terminal oxygen atom with the V(1) - O(3) distance 1.646(4) A for 1 and one -OCH3 group with the V(1)-O(3) distance 1.753(3) A for 2. The V(1)-O(4) terminals occupy the axial sites in both cases. The complexes are also characterized by IR and 1H NMR spectroscopies. 展开更多
