METHODS: Patients with reflux symptoms were enrolled and stratified to NERD and ERD after endoscopy (LA classification). Patients with ERD presenting with dyspepsia were included. Patients on proton pump inhibitors (PPI) or H2 receptor antagonists before endoscopy were excluded. Demographic data, Helicobacter pylori(H pylori) status and presence of minor psychiatric morbidity (based on General Health Questionnaire-28) were analyzed. RESULTS: Among 690 patients screened, 533 were eligible for analysis (male to female ratio: 3:2; Chinese: 75.4%; Malay: 9.8%; Indian: 14.8%). Clinical spectrum of GERD: N: 40.5%; A: 46%; B: 9.2%; C: 2.1%; D: 0.6%; Barrett's esophagus: 1.7%. Compared to patients with NERD, patients with ERD were significantly older (45 vs 39.4 years), more likely to be male (64.4% vs 53.7%), tended to smoke (19.6% vs 9.7%), less likely to have minor psychiatric morbidity (26.4 vs 46.7%) and were more likely to respond to PPI (79.7 vs 66.8%). There was also a trend towards a higher BMI (24.5 vs 23.5). Race, alcohol consumption and H pylori status were not significant. On multivariate analysis, age and presence of minor psychiatric morbidity remained with significant differences. CONCLUSION: The majority of patients who have typical symptoms of GERD have NERD or mild erosive reflux disease. Compared to patients with erosive reflux disease, patients with NERD were younger and had a higher prevalence of minor psychiatric morbidity.展开更多

Methods: Male and female (n = 197) college basketball players completed the Psychological Performance Inventory-Alternative (PPI-A), a measure of characteristics and skills consistent with mental toughness, and the PERK an objective measure of basketball performance. Results: Findings suggest that basketball performance can be partially predicted by mental toughness and starting status. Males reported greater mental toughness than females. Starters and nonstarters did not differ in mental toughness. Moderated hierarchical regression analysis indicated that mental toughness was related to performance for male players as both a main effect and interaction with starter status. For female players, in contrast, starter status was the only significant predictor of performance. Practitioners are encouraged to foster the psychological skills associated with mental toughness in females and non-starters in basketball. Conclusion: Discussion of the PPI-A as a measure of mental toughness and suggestions for its improvement are explored. A need exists for additional research on mental toughness and objective performance, as performance enhancement is a major impetus for research on mental toughness. Patients were treated with conventional interferon alfa-2b (48/98patients; 5 MIU interferon alfa-2b thrice weekly) or peginterferon alfa-2b (50/98 patients; 80-150 μgpeginterferon alfa-2b) in combination with weight-adapted ribavirin (800-1 200 mg/d). Repeated psychometric testing was performed before, three times during and once after antiviral therapy: Depression was evaluated by the Hospital Anxiety and Depression Scale (HADS), anger/hostility by the Symptom Checklist-90 Items Revised (SCL-90-R).RESULTS: Therapy with pegylated interferon alfa-2bproduces comparable scores for depression (ANOVA:P = 0.875) as compared to conventional interferon.Maximums of depression scores were even higher and cases of clinically relevant depression were frequent during therapy with peginterferon. Many points of the twelve regular channels and extra-ordinary points are named after 'Shen' (mind) such as Shenmen (HT 7), Shenting (GV 24), Shentang (BL 44), Shendao (GV 11), Benshen (GB 13), and Sishencong (EX-HN1). As their names indicate, these points yield specific therapeutic effects for disorders of the mind. Based on the above knowledge, we have made clinical observations on the acupuncture treatment for mental disorders, and obtained quite satisfactory therapeutic effects. NF (non-specific factors) involved in treatment are all those effects that do not depend on the pharmacological properties of the drug. Materials and Methods: The job lasted a year. The sample consisted of patients with mental health disorders and was divided into two groups. Treatment compliance was measured with the Morisky-Green Test. Results were compared using the chi square test and relative risk. Results and Discussion: Group A had 23 patients ending 91.3% of them and group B of 22 patients, ending the 77.27%. At the beginning of the study, we found in the group A 0.86 NF/patients while in the group B 0.82 NF/patient. At the end of the study there was a 54.65% decrease in group A while in group B the proportion remained. At the beginning of the study both groups had approximately 40% of compliant patients. Data that remained in the control group rose to 80.95% in group A at the end of the study. Conclusions: The work demonstrates the negative influence of these factors on adherence to treatment.展开更多

The ways in which art is similar to religion are discussed. Pertinent concepts are explored, with awareness of shifting definitions and application of these concepts. This conversation seems to present a consensus that there are aspects of art that overlap with the spiritual or religious. There is not, however, consensus on what these aspects are or their significance to society, as perspectives and understanding continue to change with growing complexities and multiple viewpoints within this interdisciplinary field. The study is based on analysis of personal counseling experience, where literature was...Our voluntary project ran in different groups of adults (healthy, depressed, schizophrenic, and clients with anxiety disorders). The study is based on analysis of personal counseling experience, where literature was used as a tool to disclose client's personal meanings. During our sessions, clients were observed phenomenologically, and their speach was written down. Reflecting on the qualitative data of transformative learning in language arts, we developed techniques for facilitation and disclosure of personal meanings. Grounded theory was used for data generalization: Personal educational insights and its comparison with other researchers' theoretical insights were the basis to develop the methodical system for psychological guidance and counseling and to explain it. We revealed that focus on personally important meanings during discussion after literature reading has influence on the mental state of personality and deepens the interactions with the literature art, so we suggest a spiral model to explain the methodical system of our method. Transformative learning in language arts as a method of guidance and counseling can be understood as spiritual motion by spirale. It has three levels: (1) art level (interaction with the literature); (2) psychology level (counseling); and (3) art level (interaction with the same peace of literature as in the beginning of the session, poetic summary of the session). Levels (1) and (3) mean interaction with literature art, but at the third level, this interaction has new quality, because personality has better perception of Self and more ability to connect personal meanings and literature wisdom. At the first level, we have direction from literature to Personality. At the third level, we have direction from the Self (Speaking from Within) to literature. Disclosure of personally important meanings is a key to self-understanding and poetical thinking; our developed methodical system reduces emotional tension and strengthens interconnectedness between inner and outer world and improves poetical understanding. In this paper, the methodical system of transformative learning and guidance is discussed.展开更多

The aim of the research was to achieve optimal performance by the athletes during the kumite1 in karate. The research involved initial, control and final experiment, where the karate practitioners were focused on the training model given (group; n = 13) and working in a group with additional individual intensive sessions provided for each participant. All athletes were male contestants aged 26.4 - 6.8. The aim of the research was to explore how their performance can be influenced using psychological techniques during karate trainings, or how not to fall into one of the four undesirable states of mind called Shikai2. Luther elevated marriage and family life to a place of respect and honor in the church and society at a time when singleness and celibacy were celebrated as ways of reaching a higher state of spirituality. Martin Luther's teachings were so radically different from the traditional Catholic views of his days that the church became furious with him and tried almost everything in their power to silence him. Although Luther is known primarily for his views on Justification, but it is his teachings on marriage and family that has been more consequential. Many in the West who have never heard the name of Martin Luther, today enjoy a more biblical view on marriage and family because of his influence. He advocated divorce according to biblical principles, at a time when divorce was almost impossible; he encouraged priests to marry by showing that there was no conflict between their calling and marriage; he denounced celibacy, blaming it for encouraging lust rather than aiding chastity; he restored marriage and family life back to the arena of spirituality and respectability in society. Luther lived out his own advice by getting married and living an exemplary married life. What made Luther so effective was the passionate intensity with which he advocated these reforms. The statistical data showed that the total effective rate in the treatment group (80.6%) was significantly higher than that in the control group (25%), and the differences in the observed indexes before and after treatment were significant (P 0.05), indicating that the acupuncture treatment was superior in immediate therapeutic effect on senile vascular dementia to drug treatment.展开更多

Plato characterizes eros in the Symposium and the Phaedrus as an invincible psychic force, capable, taking into account the fallibility of man, of compelling him toward the acquisition of that which will entail his completion. Eros, considered thusly, contributes to human ontogeny. Plotinus, who experiences beauty as eros and terror accompanied by pleasure, to the same degree will characterize it as the outcome and consequence of an intellectually and morally superior human soul. By this definition, the greatness of art lies in the fact that it compels the souls of men to eros for its spiritual content, the true nature of a work of art being the idea. As such, as long as it is not easy to know if this beauty exists as an ontologically, gnoseologically, and aesthetically idealized entity, so much the more we consider that we cannot entirely prove Freud's erotic theory of primordial and desirable phantasms, which seems to differ only slightly from the Platonic theory, or from Jung's theory of archetypes, as it seems not to recognize to an appropriate degree individual human experience. Transformative learning in language arts as a method of guidance and counseling can be understood as spiritual motion by spirale. It has three levels: (1) art level (interaction with the literature); (2) psychology level (counseling); and (3) art level (interaction with the same peace of literature as in the beginning of the session, poetic summary of the session). Levels (1) and (3) mean interaction with literature art, but at the third level, this interaction has new quality, because personality has better perception of Self and more ability to connect personal meanings and literature wisdom. At the first level, we have direction from literature to Personality. At the third level, we have direction from the Self (Speaking from Within) to literature. Disclosure of personally important meanings is a key to self-understanding and poetical thinking; our developed methodical system reduces emotional tension and strengthens interconnectedness between inner and outer world and improves poetical understanding. In this paper, the methodical system of transformative learning and guidance is discussed.

文摘This text represents a research that by individual treatment explores the influence and effect of the so-called advanced karate training (combined training program for development of physical and mental skills) in strengthening the person's tolerance to difficult and stressful situations. The aim of the research was to achieve optimal performance by the athletes during the kumite1 in karate. The research involved initial, control and final experiment, where the karate practitioners were focused on the training model given (group; n = 13) and working in a group with additional individual intensive sessions provided for each participant. All athletes were male contestants aged 26.4 - 6.8. The aim of the research was to explore how their performance can be influenced using psychological techniques during karate trainings, or how not to fall into one of the four undesirable states of mind called Shikai2. Results confirmed that the model of combined physical and mental training for athletes improves their physical skills and optimizes performance during competitions.
文摘This paper describes and analyzes Martin Luther's views on marriage and family primarily from his own writings. Luther elevated marriage and family life to a place of respect and honor in the church and society at a time when singleness and celibacy were celebrated as ways of reaching a higher state of spirituality. Martin Luther's teachings were so radically different from the traditional Catholic views of his days that the church became furious with him and tried almost everything in their power to silence him. Although Luther is known primarily for his views on Justification, but it is his teachings on marriage and family that has been more consequential. Many in the West who have never heard the name of Martin Luther, today enjoy a more biblical view on marriage and family because of his influence. He advocated divorce according to biblical principles, at a time when divorce was almost impossible; he encouraged priests to marry by showing that there was no conflict between their calling and marriage; he denounced celibacy, blaming it for encouraging lust rather than aiding chastity; he restored marriage and family life back to the arena of spirituality and respectability in society. Luther lived out his own advice by getting married and living an exemplary married life. What made Luther so effective was the passionate intensity with which he advocated these reforms. He wrote and spoke with such power and backed up his words with such a bold and courageous life, although living in the shadow of constant threats, that centuries after his death, the power and conviction of his ideas still resonate.
文摘63 patients with senile vascular dementia were randomly divided into the treatment group (treated by acupuncture) and the control group (treated with piracetam). The authors observed the changes in the score of Hasegawa's dementia scale (HDS), p300, rheoencephalogram, topographic EEG, superoxide dismutase (SOD) activity in erythrocytes, and lipid peroxide (LPO) level in plasma before and after treatment. The statistical data showed that the total effective rate in the treatment group (80.6%) was significantly higher than that in the control group (25%), and the differences in the observed indexes before and after treatment were significant (P 0.05), indicating that the acupuncture treatment was superior in immediate therapeutic effect on senile vascular dementia to drug treatment.
文摘Art composed of categories of the scale of beauty transmutes the aesthetic into eroticism, such that as an element of life it may be displayed as a love relationship between beauty and eros, the knowledge of which is referenced as early as Plato's philosophical contemplations. Plato characterizes eros in the Symposium and the Phaedrus as an invincible psychic force, capable, taking into account the fallibility of man, of compelling him toward the acquisition of that which will entail his completion. Eros, considered thusly, contributes to human ontogeny. Plotinus, who experiences beauty as eros and terror accompanied by pleasure, to the same degree will characterize it as the outcome and consequence of an intellectually and morally superior human soul. By this definition, the greatness of art lies in the fact that it compels the souls of men to eros for its spiritual content, the true nature of a work of art being the idea. As such, as long as it is not easy to know if this beauty exists as an ontologically, gnoseologically, and aesthetically idealized entity, so much the more we consider that we cannot entirely prove Freud's erotic theory of primordial and desirable phantasms, which seems to differ only slightly from the Platonic theory, or from Jung's theory of archetypes, as it seems not to recognize to an appropriate degree individual human experience. On the contrary and in agreement with the Lacanian theory on eros, in the environment of which eros is directed towards the other, so like it, it emerges in the life of the other, rupturing and reformulating it, we consider that each of us, in reality, in the erotic phenomenon, meets the other, and, behind him, our own self, which, enchanted by all that we sense that the other represents, rates all his characteristics as charmingly beautiful, because they have the privilege of coinciding, or at least of converging towards the aesthetic, entirely unique foundation of our self.
