Acupoint to acupoint penetrative needling method has a long history in traditional Chinese medicine. Chinese physicians in various dynasties hold that this type of needling method can strengthen the needling sensations and raise the therapeutic effect in clinical practice. This needling method can be divided into straight penetration, oblique penetration and transverse penetration. In clinical application, it can also be divided into external medial meridian penetration, a single meridian penetration, neighboring meridians penetration, and neighboring acupoints penetration, etc. in accordance with the involved acupoints and meridians. Clinical practice demonstrates that penetrative needling possesses advantages of stronger needling sensations, better therapeutic effect and fewer acupoints selection. For this reason, it is extensively applied in clinic. 展开更多

Objective: To observe the clinical effect of acupuncture on peripheral facial paralysis in acute period and changes in facial nerve F-wave.Methods: A total of 148 eligible cases were randomly allocated into an observation group(n=74) and a control group(n=74) by their visiting sequence. In addition to oral Prednisone and Aciclovir tablets, patients in the observation group received point-towards-point acupuncture therapy, whereas patients in the control group received routine acupuncture therapy. The treatment was done once a day and 6 times made up a course of treatment. The efficacy evaluation was made after 2 courses. The electromyography(EMG) F-wave was monitored before and after treatment. Results: After 2 courses of treatment, the recovery rate and total effective rate were 47.3% and 94.6% respectively in the observation group, versus 20.3% and 82.4% in the control group, showing statistical differences(P〈0.01). Before treatment, the facial nerve F-wave latency on the affected side was prolonged and its frequency of occurrence was decreased in both groups. After treatment, the F-wave latency and frequency of occurrence were significantly improved in both groups(P〈0.05) and there were significant between-group differences(P〈0.05). Conclusion: In addition to Western medication, point-towards-point acupuncture therapy can obtain more accurate and better effect than routine acupuncture therapy for acute peripheral facial paralysis. 展开更多

Objective: To observe the effect of point-toward-point needling at points on the low back regions on residual back pain after percutaneous kyphoplasty (PKP), thoracolumbar function and quality of life (QOL) in patients with osteoporotic vertebral compression fracture (OVCF). Methods: A total of 72 patie nts with OVCF and residual back pain after PKP were in eluded and allocated into a treatme nt group (n=36) and a control group (n=36) by the random number table. Cases in the control group received salmon calcitonin injection (miacalcic), for 1 mL each time, once a day, and oral intake of calcium carbonate D3 pill, 600 mg each time, once a day, whereas cases in the treatment group received point-toward-point needling at points on the low back regions and electroacupuneture (EA) for 30 min each time, once a day and 6 times a week on the basis of the treatment in the control group. Treatment in both groups lasted for 2 weeks. Therapeutic efficacy indicators including visual analog scale (VAS), Oswestry disability index (ODI) and Barthel index (Bl) were evaluated before and after treatment and at follow-up visit (1 month after treatment). Results: The total effective rate was 94.4% and 88.9% in the treatment group after treatment and at the follow-up visit, respectively, higher than 77.8% and 69.4% in the control group, and the between-group differences were statistically significant (both P<0.05). After treatment and at the follow-up visit, the VAS and ODI scores decreased, and Bl scores in creased in both groups, and the in tra-group differe nces were statistically sign ifica nt (all P<0.05). After treatme nt and at the follow-up visit, between-group differences of VAS, ODI and Bl scores were statistically significant (all P<0.05). Conclusion: On the basis of conventional medication treatment, point-toward-point needling at points on the low back regions has a good therapeutic effect in relieving residual back pain after PKR improving thoracolumbar function and QOL, better than conventional medication treatment alone. 展开更多

In his over seventy-year practice of Chinese medicine, Prof. HUANG Xian-ming inherited plentiful academic experience from his father, Mr. HUANG Hong-fang and has a thorough understanding of the needling techniques described in Nei Jing ( Inner Canon ) and has developed his own school in the clinical treatment. Prof. HUANG believes that the manipulating skills of the needling techniques are the key to the therapeutic effects, and the needling techniques include the tonifying, reducing, warming, clarifying, ascending, descending, diminishing and opening skills. Among them, the tonifying technique is able to support the anti-pathogenic ability and get rid of deficiency, often used for diseases and patterns induced by insufficiency of qi and blood and low immunity. The reducing technique is used to reduce excessive symptoms and dissipate the pathogens, often used for pathogenic patterns of hyperactive pathogenic heat, qi stagnation and blood stasis, and muscular spasm and swelling. 展开更多

Objective:To observe the effect of complex reinforcing [Shao Shan Huo (Mountain-burning Fire)] and reducing [Tou Tian Liang (Heaven-penetrating Cooling)] manipulations on motion and balance of the lower limbs in patients with subacute combined degeneration of the spinal cord (SCD). Methods:A total of 100 SCD cases were randomly allocated into an observation group and a control group by their visit sequence, 50 cases in each group. On the basis of Western medical treatment, cases in the observation group were also treated with acupuncture therapy plus complex reinforcing-reducing manipulation; whereas cases in the control group were only treated with the same Western medical treatment as those in the observation group. Before and 2 months after treatment, kinematic parameters including muscle force, muscle tone and range of motion of the hip and knee joints were measured. In addition, balancing parameters including the path length of center of pressure (COP), peripheral area, COP path length per unit area and rectangular area were also measured before and after treatment. Results:After treatment, there were statistical intra-group differences in COP path length, peripheral area, COP path length per unit area, left-right offset, rectangular area, coefficient of stability and coefficient of weight distribution (P〈0.01). There were statistical inter-group differences in muscle force, muscle tone and range of motion of hip and knee joints (P〈0.01). The total effective rate was 83.3% in the observation group, versus 60.0% in the control group, showing a statistical difference (P〈0.05). Conclusion:Complex reinforcing-reducing manipulations can improve the lower limb motion and balance in SCD patients. 展开更多
