We evaluated the survival benefit of resection in this group of patients. Methods: We retrospectively reviewed the hospital records of 288 patients with T4 gastric carcinoma to compare the clinicopathological results in patients with curative resection (n = 49) with patients with non-curative resection (n = 113) from 1980 to 2000. Results: Curative resection was performed in 49 (30.2%) patients, with the pancreas being the most frequently resected organ. The 5-year survival rate was 9.3% and the median survival time was 12 months for all 162 registered patients. The 5-year survival rates in patients after curative and non-curative resection were 14.3% vs 7.1% respectively. These values differed considerably between the two groups (P = 0.033). Operative type and curability were independent statistically significant prognostic parameters. Conclusion: Curability was an independent prognostic factor among all registered patients. Patients with T4 gastric carcinoma might be benefited from curative resection.展开更多

Methods: Thirty-three eligible patients with histologically proven advanced MPM with ECOG PS of 〈 2 and had adequate liver and kidney functions received gemcitabine (1000 mg/m2) on days 1 and 8 combined with cisplatin (80 mg/m^2) on day 1. Results: The majority of the study group were males and constituted 87.9% (n = 29), while females constituted 12.1% (n = 4). The median age was 52 years and ranged from 37 to 69 years. Regarding SWOG performance status, 5 patients were PS 0, 21 patients were PS 1 and 7 patients were PS 2. Regarding pathological subtypes, epithelial histology represented 84.8% of the study (n = 28) while sarcomatoid type represented 6% (n = 2) and biphasic type accounted for 9% (n = 3). Regarding staging according to IMIG staging system, 12.1% of patients were stage Ⅱ, 54.5% were stage Ⅲand 33.3% were stage Ⅳ. Only one patient (3%) had attained a complete response, 18 patients (54.5%) showed partial response, 12 patients (36.4%) showed stationary disease, while 2 patients (6.1%) showed progressive disease. The correlation between response rate obtained and PS was statistically significant (P = 0.029). After a median follow-up of 14.4 months, the median survival time was 20.3 months (95% Cl: 14.58-26.01 months) and the progression free survival time was 11.8 months (95% CI: 10.76-12.83 months) Conclusion: The combination of gemcitabine and cisplatin is an active and safe treatment in patients with MPM. Further larger comparative studies are warranted to document this finding.展开更多

METHODS Forty patients with advanced soft tissue sarcomas refractory to first-line chemotherapy treatment were enrolled. They received combination of gemcitabine at dose of 900 mg/m2 on days 1 and 8 and docetaxel at dose of 100 mg/m2 on day 8, and had this regimen repeated every 3 weeks. If the patients had received the pelvic irradiation in advance, gemcitabine dose was reduced to 675 mg/m2 on days 1 and 8 and docetaxel to 75 mg/m2 on day 8, and had it repeated every 3 weeks. RESULTS Gemcitabine/docetaxel combination was well tolerated by the patients with an overall response of 20%. After median follow-up of 15 months, a median overall survival time was 12 months (95% CI 7.042-16.958) and a median progression free survival time was 6 months (95% CI 5.445-6.545). The most common hematologic toxicity was neutropenia (47.5%) while mucositis was the most common nonhematologic toxicity (45%). The 1- and 2- year survival rates were 50% and 15%, respectively. CONCLUSION This regimen of gemcitabine/docetaxel combination as second-line treatment for the patients with advanced soft tissue sarcomas is effective with acceptable toxicities. These results should be evaluated in a large phase III trial.展开更多

However, the primary events of NH4^+ toxicity at the cellular level are still unclear. Here, we showed that NH4^+ toxicity can induce the root cell death in a temporal pattern which primarily occurs in the cells of root maturation and elongation zones, and then spreads to the ceils in the meristem and root cap. The results from the NH4^+-hypersensitive mutant hsn1 further confirmed our findings. Taken together,NH4^+ toxicity inhibits primary root growth by inhibiting cell elongation and division and inducing root cell death.展开更多

B cells play a central role in the immunopathogenesis via antibody-dependent and antibody-independent ways. Excessive autoantibodies production, hyperresponsiveness and prolonged survival of autoreactive B cells are characteristics of SLE. In this article, mechanisms of self-tolerance loss of B cells and promising B cell-targeting therapies in lupus are reviewed and discussed.展开更多

This paper studies the survivability assessment method, which has mainly two parts, namely susceptibility assessment and vulnerability assessment. Our emphasis focuses on how to assess the vulnerability of spacecraft to space debris. The basic vulnerability assessment process includes: (1) the analyses of the threat environment of space debris; (2) the process of identification of critical components of spacecraft, in which a DMECA (damage mode, effects, and criticality analysis) is introduced and an expert system is proposed to get the damage ratio of each component; (3) the spacecraft geometrical description method, in which " shot line scanning method" is described to calculate the projected/overlapping area of components and determine the masking/shielding relationship among components; (4) the calculation of spacecraft kill probability and vulnerable area. At last, the authors conclude that the proposed assessment method is systemic and feasible, and it can give a quantitative assessment result of survivability/vulnerability in the spacecraft conceptual design. The theory contained in this paper is useful to develop computerized tool for spacecraft survivability analysis.展开更多
