BACKGROUND Polyneuropathy,organomegaly,endocrinopathy,M-protein,skin changes(POEMS)syndrome is a rare paraneoplastic syndrome that encompass multiple systems.The most common clinical symptoms of POEMS syndrome are pro-gressive sensorimotor polyneuropathy,organ enlargement,endocrine disorders,darkening skin,a monoclonal plasma cell proliferative disorder,and lymph node hyperplasia.The organomegaly consists of hepatosplenomegaly and/or lym-phadenopathy;cases of cardiomyopathy are rare.Diagnoses are often delayed because of the atypical nature of the syndrome,exposing patients to possibly severe disability.Therefore,identifying atypical symptoms can improve the prognosis and quality of life among POEMS syndrome patients.lenalidomide and dexamethasone.CONCLUSION When patients with cardiomyopathy have systemic manifestations such as numb limbs and darkening skin,the POEMS syndrome is the most possible diagnosis. 展开更多

Mandatory lane change(MLC)is likely to cause traffic oscillations,which have a negative impact on traffic efficiency and safety.There is a rapid increase in research on mandatory lane change decision(MLCD)prediction,which can be categorized into physics-based models and machine-learning models.Both types of models have their advantages and disadvantages.To obtain a more advanced MLCD prediction method,this study proposes a hybrid architecture,which combines the Evolutionary Game Theory(EGT)based model(considering data efficient and interpretable)and the Machine Learning(ML)based model(considering high prediction accuracy)to model the mandatory lane change decision of multi-style drivers(i.e.EGTML framework).Therefore,EGT is utilized to introduce physical information,which can describe the progressive cooperative interactions between drivers and predict the decision-making of multi-style drivers.The generalization of the EGTML method is further validated using four machine learning models:ANN,RF,LightGBM,and XGBoost.The superiority of EGTML is demonstrated using real-world data(i.e.,Next Generation SIMulation,NGSIM).The results of sensitivity analysis show that the EGTML model outperforms the general ML model,especially when the data is sparse. 展开更多

BACKGROUND Polyneuropathy,organomegaly,endocrinopathy,M-protein,and skin changes(POEMS)syndrome is a rare paraneoplastic syndrome caused by a plasma cell proliferative disorder.The syndrome is characterized by elevated plasma cells,platelets,and vascular endothelial growth factor levels.Although heart disease rarely occurs in POEMS syndrome,the death rate increases sharply after heart failure.We report a patient who initially presented with an endocrine disease and developed congestive heart failure related to POEMS syndrome 9 years later.CASE SUMMARY A 23-year-old woman with no history of menstruation and a 9-year history of type I diabetes reported feeling breathless after activities.She could not lie down and rest at night.Three months prior,she experienced pain and increased tension in her left thigh accompanied by tenderness and edema in both lower extremities.The chief complaint upon hospital admission was that blood sugar has increased for more than 9 years,pain in the left thigh,and edema in both legs for more than 2 mo.After a multisystem evaluation,she was diagnosed with POEMS syndrome.Her echocardiogram showed left ventricular dilation with systolic dysfunction,and the left ventricular ejection fraction was only 38%with severely elevated brain natriuretic peptide.She received a combination of dexamethasone and thalidomide for 1 mo,but her symptoms did not improve.Therefore,we added a two-per-week bortezomib injection.After 2 wk,the patient’s heart function had improved significantly.CONCLUSION This case provides information about the treatment of POEMS syndrome with complications and highlights the challenges of developing a standardized treatment. 展开更多

BACKGROUND Polyneuropathy organomegaly endocrinopathy M-protein and skin changes(POEMS)syndrome is a rare paraneoplastic syndrome caused by a potential plasma cell tumor.The clinical manifestations of POEMS syndrome are diverse.Due to the insidious onset and lack of specific early-stage manifestations,POEMS syndrome is easily misdiagnosed or never diagnosed,leading to delayed treatment.Neurological symptoms are usually the first clinical manifestation,while ascites is a rare symptom in patients with POEMS syndrome.CASE SUMMARY A female patient presented with unexplained ascites as an initial symptom,which is a rare early-stage manifestation of the condition.After 1 year,the patient gradually developed progressive renal impairment,anemia,polyserosal effusion,edema,swollen lymph nodes on the neck,armpits,and groin,and decreased muscle strength of the lower extremities.The patient was eventually diagnosed with POEMS syndrome after multidisciplinary team discussion.Treatment comprised bortezomib+dexamethasone,continuous renal replacement therapy,chest and abdominal closed drainage,transfusions of erythrocytes and platelets,and other symptomatic and supportive treatments.The patient’s condition initially improved after treatment.However,then her symptoms worsened,and she succumbed to the illness and died.CONCLUSION Ascites is a potential early manifestation of POEMS syndrome,and this diagnosis should be considered for patients with unexplained ascites.Furthermore,multidisciplinary team discussion is helpful in diagnosing POEMS syndrome. 展开更多

The aim of this paper is to reveal the mechanism of compromise and change in coordination where players agree in general but disagree on coordination methods. When players agree on the need to collaborate but are in conflict regarding the specific method, one player must always compromise. This situation is known as the Battle of the Sexes in game theory. It has ever been believed that if an agreement is reached under such circumstances, the players do not have the incentive to withdraw from the agreement. However, this study shows that this belief is not always true if the players were able to revise the outcome of their negotiations later. The wide-ranging fields use game theories for their analysis frameworks to analyze the success or failure of coordination. However, comparing with the possibility of betrayal illustrated as a well-known Prisoner Dilemma, it has been rare to discuss conflict regarding the specific method of coordination, although such situations are often observed in today's interdependent real world. The repeated Battle of the Sexes games presented in this study would be a useful framework to analyze conflict regarding the specific method of coordination. 展开更多

All countries of the world should take action to deal with climate change. However, such an action is actually a game process where each country chooses to cooperate or defect under constrained condition. Here we study the group behavior of countries in dealing with climate change. In our study, the countries of the world are considered as players who are distributed on an assumed spatial network. A new evolution rule is proposed to model the game process among the players. Our extensive experiments demonstrate that under constrained condition, the local psychology of players possibly results in the formation of local group. Some local and global oscillations can be found where part or all of the players change their choices periodically. 展开更多

Large yellow croaker(Larimichthys crocea), one of the most important marine fish species in China, can change its abdominal skin color when it is shifted from light to dark or from dark to light, providing us an opportunity of investigating the molecular responding mechanism of teleost in light stress. The gene expression profile of fish under light stress is rarely documented. In this research, the transcriptome profiles of the abdominal skin of L. crocea exposed to light or dark for 0 h, 0.5 h and 2 h were produced by next-generation sequencing(NGS). The cluster results demonstrated that stress period, rather than light intensity(e.g., light or dark), is the major influencing factor. Differently expressed genes(DEGs) were identified between 0 h and 0.5 h groups, between 0 h and 2 h groups, between 0.5 h light and 0.5 h dark, and between 2 h light and 2 h dark, respectively. The gene ontology(GO) and Kyoto Encyclopedia of Genes and Genomes(KEGG) annotation revealed that the genes relating to immunity, energy metabolism, and cytoskeletal protein binding were significantly enriched. The detailed analysis of transcriptome profiles also revealed regular gene expression trends, indicating that the elaborate gene regulation networks underlined the molecular responses of the fish to light stress. This transcriptome analysis suggested that systematic and complicated regulatory cascades were functionally activated in response to external stress, and coloration change caused by light stress was mainly attributed to the change in the density of chromatophores for L. crocea. This study also provided valuable information for skin coloration or light stress research on other marine fish species. 展开更多

The incidence of nonmelanoma skin cancer is rising worldwide.The major cardinogenic factor for most skin cancers is solar ultraviolet light,in particular,the chronic exposure to UVB(280-320nm).In this study,the optical characteristics of skin canceration process induced by UVB were analyzed.Optical coherence tomography(OCT)was used for monitoring the morphologic changes and compared with histological analysis.Meanwhile,the optical property,such as the attenuation cofficient(A4)was systematically extracted and analyzed.In addition,characteristic text ures,including energy,entropy and correlation were revealed from OCT images.Results suggest that OCT is a useful tool for monitoring the process of UVB-induced skin cancer and changes of optical property during this process. 展开更多

车辆换道时可能存在与其他车辆发生行驶目的冲突问题,传统的方法没有考虑到车辆之间的动态博弈。为此,根据城市交通的特点以及车辆换道要考虑的各种因素,采用演化博弈的方法进行车辆换道情况分析,为车辆的行驶决策提供方案。首先利用车... 车辆换道时可能存在与其他车辆发生行驶目的冲突问题,传统的方法没有考虑到车辆之间的动态博弈。为此,根据城市交通的特点以及车辆换道要考虑的各种因素,采用演化博弈的方法进行车辆换道情况分析,为车辆的行驶决策提供方案。首先利用车辆在换道时要考虑的时间损失、冲突损失、互让损失、通过收益和换道成本等因素,来构建换道车辆与所要博弈车辆的博弈矩阵,利用博弈矩阵建立车辆冲突的动力学模型,在换道车辆和所要博弈车辆都基于自身利益最大化条件下对存在的各种情况的均衡点的存在和稳定性进行分析,并对演化情况的实际状况进行说明。然后用MATLAB进行仿真验证。在不同情况下,影响车辆进行占道的主导因素也不尽相同,对影响车辆行驶结果的时间损失、冲突损失、互让损失之间的关系进行灵敏度分析。结果表明:车辆在经过博弈后演化的结果可能为采取换道冲突、采取换道不冲突、不采取换道冲突、不采取换道不冲突;发现车辆的互让损失、时间损失会促使车辆进行占道,换道成本和冲突损失会抑制车辆的占道行为。该结果解释了城市交通中存在交通冲突的原因,为车辆驾驶员行驶决策和缓解交通拥堵提供了参考。 展开更多

利用人工气候室模拟了不同季节对应的9种温度突变工况,对24名受试者开展了中性→热(冷)→中性往返式温度突变对人体热反应影响的实验研究,得到了环境温度发生突变时人体生理反应、主观反应及全身热负荷的变化规律。研究发现:在各突变工... 利用人工气候室模拟了不同季节对应的9种温度突变工况,对24名受试者开展了中性→热(冷)→中性往返式温度突变对人体热反应影响的实验研究,得到了环境温度发生突变时人体生理反应、主观反应及全身热负荷的变化规律。研究发现:在各突变工况下,主观反应变化超前于生理反应变化;突变前后人体生理和心理反应出现不对称现象;在热→中性突变工况下人体出现“冷感超越”,通过分析实验过程中人体各项散热量的变化规律,发现“冷感超越”的原因是皮肤蒸发散热量显著增大进而导致人体总散热量远大于产热量。温度突变动态环境中的热感觉与平均皮肤温度及其变化率均呈线性关系,得到了基于皮肤温度的热感觉预测模型。将人体热负荷及其变化量引入热感觉预测模型中,得到了基于热负荷的热感觉预测模型。PMV模型、基于皮肤温度的热感觉预测模型、基于热负荷的热感觉预测模型的对比显示,基于热负荷的预测模型能够更好地预测温度突变环境下的人体热感觉。 展开更多
