BACKGROUND While Crohn’s disease has been studied extensively in high-income countries,its epidemiology and care in low and lower-middle income countries(LLMICs)is not well established due to a lack of disease regist...BACKGROUND While Crohn’s disease has been studied extensively in high-income countries,its epidemiology and care in low and lower-middle income countries(LLMICs)is not well established due to a lack of disease registries and diagnostic capacity.AIM To describe the published burden,diagnostic/treatment capacity,service utilization,challenges/barriers to individuals with Crohn’s in LLMICs and their providers.METHODS We conducted a scoping review utilizing a full search strategy was developed and conducted in PubMed,Embase and World Health Organization Global Index Medicus.Two independent reviewers screened the titles and abstracts of all of the publications found in this search,reviewed selected publications,and extracted relevant data,which underwent descriptive review and was analyzed in Excel.RESULTS The database search yielded 4486 publications,216 of which were determined to be relevant to the research questions.Of all 79 LLMICs,only 21(26.6%)have publications describing individuals with Crohn’s.Overall,the highest number of studies came from India,followed by Tunisia,and Egypt.The mean number of Crohn’s patients reported per study is 57.84 and the median is 22,with a wide range from one to 980.CONCLUSION This scoping review has shown that,although there is a severe lack of populationbased data about Crohn’s in LLMICs,there is a signal of Crohn’s in these settings around the world.展开更多
Data announced by the Chinese Ministry of Land and Resources in 2015 suggest that China has discovered more than 480 manganese ore-producing areas, with manganese ore resource reserves up to 1.22 billion tons, includi...Data announced by the Chinese Ministry of Land and Resources in 2015 suggest that China has discovered more than 480 manganese ore-producing areas, with manganese ore resource reserves up to 1.22 billion tons, including 0.303 billion tons of basic reserves and 0.915 billion tons amount of resource. Among these reserves, manganese carbonate ores account for 56%, manganese oxide ores (including spongy manganese ores) accounts for 25%, and other types of manganese ore occupy 19%. Rich ores with 〉30% manganese content only account for 5% of the total resource amount, and the remaining 95% are poor manganese ores. Since 1983 when China first began to import, the imported manganese ores have increased annually by 25%. In 2001, the annual imported manganese ores reached 1.71 million tons. From 2010, the annual imported manganese ores increased by greater than 45%, reaching up to 12 million tons in 2015. The goal of asthma management is to control symptoms, reduce the need for short acting beta agonist, and maintain optimal pulmonary function, and normal physical activities. Uncontrolled asthma can lead to obesity, su...The goal of asthma management is to control symptoms, reduce the need for short acting beta agonist, and maintain optimal pulmonary function, and normal physical activities. Uncontrolled asthma can lead to obesity, suboptimal pulmonary function, poor quality of life and mortality. Children with bronchial asthma in River State University Teaching Hospital were just receiving acute care in the emergency room with no concrete plan for a follow up care for their chronic asthma;as a result most of the patients had uncontrolled asthma. In the hospital, the situation led to increased emergency hospitalization, resulting in decreased bed availability, increased burden on manpower and health resource utilization thereby putting more pressure on the limited health resources. Using the Kotter’s model for change management, transformational and situational leadership style, the change in the desired quality and scope of health service rendered to asthmatic patients was successfully implemented;as a result there was a decrease in emergency room visit for acute asthma by 57.5% and an increase in the uptake of scheduled clinic visits for asthma control services. The effect of these changes was an improvement in the control of asthma and quality of life of our patient cohort. Implementation of change in health service delivery is a delicate process that needs a stepwise approach in order to successfully implement and sustain the desired change.展开更多

Africa can be"left behind"after other advanced continents recover from the coronavirus disease 2019(COVID-19)pandemic as reflected by the global pandemic of HIV/MDS.In this paper,we summarize potentially ada...Africa can be"left behind"after other advanced continents recover from the coronavirus disease 2019(COVID-19)pandemic as reflected by the global pandemic of HIV/MDS.In this paper,we summarize potentially adaptable,effective and innovative strategies from China,Italy,and the U.S.The purpose is to help African countries with weaker healthcare systems better respond to the COVID-19 pandemic.China,being the first to report COVID-19 infection swiftly swung into anti-epidemic actions by the use of innovative risk communication and epidemic containment strategies.Italy and U.S.,the next rapidly hit countries after China,however,experienced sustained infections and deaths due to delayed and ineffective response.Many African countries responded poorly to the COVID-19 pandemic as evidenced by the limited capacity for public health surveillance,poor leadership,low education and socioeconomic status,among others.Experience from China,Italy and U.S.suggests that a better response to the COVID-19 pandemic in Mrica needs a strong public health leadership,proactive strategies,innovative risk communication about the pandemic,massive tests and isolation,and scaling-up community engagement.Lastly,African countries must collaborate with other countries to facilitate real-time information and experience exchange with other countries to avoid being left behind.展开更多

Skopos theory has provoked a lot of research interest among scholars since it was introduced in the 1970s, Skopos theory has formed a new perspective of translation practice and research by breaking the traditional id...Skopos theory has provoked a lot of research interest among scholars since it was introduced in the 1970s, Skopos theory has formed a new perspective of translation practice and research by breaking the traditional idea of translation equivalence so to some extent it is an important supplement to translation theories. In Skopos theory, translation is considered to be a purposeful communicative action, and the specific translation purpose determines the whole translational action. Functionally adequate means that the translation should be adequate to the requirements of the translation and it works as the criterion of translation. Skopos theory enables translators to select appropriate translation strategies according to their own purposes, so as to effectively solve the dilemma in the translation process whether faithfully display the form of source text or flexibly retain the sense of the source text. The Great Gatsby is the masterpiece of the American writer Fitzgerald, F. S.. He not only depicted the intoxicating atmosphere of the unprecedented prosperity in American history after the First World War with vivid images and cheerful rhythms but also described the sorrows about the disillusionment of the "American Dream" after the war. This paper takes the Skopos of German functionalism as a point to analyze The Great Gatsby translated by Wu Ningkun from the perspective of skopos theory. Through the analysis of the specific translation of the selected paragraphs, I conclude Wu's translation strategies from lexical aspect, syntactic aspect and discourse aspect. In Wu's version he kept the styles, rhythms, and American thoughts of the source text by foreignization. His skopos is to offer an opportunity to broaden the horizon of readers who are hunger for new ideas and knowledge at that time therefor it is rather necessary to absorb the "exotic elements" in the his translations.展开更多
