Application of multidisciplinary team-based integrated traditional Chinese medicine and Western medicine in rotator cuff injury patients undergoing arthroscopic surgery

BACKGROUND Arthroscopic rotator cuff repair is a common surgical treatment for rotator cuff injuries(RCIs).Although this procedure has certain clinical advantages,it requires rehabilitation management interventions to ensure therapeutic efficacy.AIM To investigate the effect of integrated traditional Chinese medicine and Western medicine(TCM-WM)under the multidisciplinary team(MDT)model on the postoperative recovery of patients undergoing arthroscopic surgery for RCIs.METHODS This study enrolled 100 patients who underwent arthroscopic rotator cuff repair for RCIs at the Seventh People’s Hospital of Shanghai University of Traditional Chinese Medicine between June 2021 and May 2024.They were divided into a control group(n=48)that received routine rehabilitation treatment and an experimental group(n=52)that received TCM-WM under the MDT model(e.g.,acupuncture,TCM traumatology and orthopedics,and rehabilitation).The results of the Constant–Murley Shoulder Score(CMS),Visual Analogue Scale(VAS),Shoulder Pain and Disability Index(SPADI),muscular strength evaluation,and shoulder range of motion(ROM)assessments were analyzed.RESULTS After treatment,the experimental group showed significantly higher CMS scores in terms of pain,functional activity,shoulder joint mobility,and muscular strength than the baseline and those of the control group.The experimental group also exhibited significantly lower VAS and SPADI scores than the baseline and those of the control group.In addition,the experimental group showed significantly enhanced muscular strength(forward flexor and external and internal rotator muscles)and shoulder ROM(forward flexion,abduction,and lateral abduction)after treatment compared with the control group.CONCLUSION TCM-WM under the MDT model improved shoulder joint function,relieved postoperative pain,promoted postoperative functional recovery,and facilitated the recovery of muscular strength and shoulder ROM in patients with RCIs who underwent arthroscopic rotator cuff repair.

Molecular mechanism and research progress of traditional Chinese medicine in the treatment of prostate cancer

Prostate cancer(PCa)is one of the most common malignant tumors in the male genitourinary system,ranking second in incidence worldwide.Traditional Chinese medicine(TCM),as an important component of complementary and alternative medicine,shows unique advantages in cancer treatment.Chinese herbal medicine is usually composed of multiple ingredients and involves multiple signaling pathways,which showed function of inducing apoptosis of cancer cells,arresting the cell cycle,inhibiting invasion and metastasis,reducing drug resistance,and regulating immune function.Physical therapy is also an important treatment of TCM.Currently,Physical therapy such as acupuncture or Tai Chi and Qigong are gaining increased recognition in the management of PCa,particularly in addressing issues like urinary incontinence and bone metastasis-related pain.This article reviews the TCM treatment and therapy of PCa,in order to provide new research avenues and treatment options for the treatment of PCa with TCM and improve the quality of life of patients.

探讨Chinese way术式在关节镜下治疗巨大肩袖损伤的应用价值

目的探讨Chinese way术式在关节镜下治疗巨大肩袖损伤的应用价值,为临床治疗提供参考。方法选取2022年3月至2024年3月于扬州市江都中医院接受治疗的70例巨大肩袖损伤患者的临床资料,进行回顾性分析。根据治疗方式不同分为双排锚钉缝线桥组和Chinese way组,每组35例。双排锚钉缝线桥组患者采用关节镜下双排锚钉缝线桥固定术治疗,Chinese way组患者采用关节镜下Chinese way术式治疗。比较两组患者肩关节活动度、并发症发生情况、Fugl-Meyer评定量表(FMA-UE)评分。结果术后3个月,两组患者前屈、后伸、内旋、外旋的肩关节活动度均增加,且Chinese way组均大于双排锚钉缝线桥组(均P<0.05)。Chinese way组患者并发症总发生率低于双排锚钉缝线桥组(均P<0.05)。术后3个月,两组患者运动功能、感觉功能、平衡功能、关节活动度、疼痛的各项FMA-UE评分均升高,且Chinese way组均高于双排锚钉缝线桥组(均P<0.05)。结论Chinese way术式在关节镜下治疗巨大肩袖损伤的效果较好,可有效提升患者肩关节活动度和上肢运动功能,且安全性较高,值得临床应用。

Traditional Chinese medicine in the treatment of Helicobacter pylorirelated gastritis:The mechanisms of signalling pathway regulations

Helicobacter pylori-associated gastritis(HPAG)is a common condition of the gastrointestinal tract.However,extensive and long-term antibiotic use has resulted in numerous adverse effects,including increased resistance,gastrointestinal dysfunction,and increased recurrence rates.When these concerns develop,traditional Chinese medicine(TCM)may have advantages.TCM is based on the concept of completeness and aims to eliminate pathogens and strengthen the body.It has the potential to prevent this condition while also boosting the rate of Helicobacter pylori eradication.This review elaborates on the mechanism of TCM treatment for HPAG based on cellular signalling pathways,which reflects the flexibility of TCM in treating diseases and the advantages of multi-level,multipathway,and multi-target treatments for HPAG.

Literature study on traditional Chinese medicine syndromes after renal transplantation

BACKGROUND Kidney transplantation is the most effective means to treat patients with renal failure,but its postoperative problems such as rejection reactions,immunosuppressant poisoning,chronic transplant kidney nephropathy,etc.still have not been effectively solved.This study searched for literature on traditional Chinese medicine(TCM)syndromes after kidney transplantation in China,conducted statistical analysis of the results,and sought to identify the underlying patterns.AIM To understand the TCM syndromes after renal transplantation and associated rules and provide a theoretical basis for further clinical research.METHODS The literature pertaining to TCM syndromes in renal transplantation,published in the China National Knowledge Infrastructure,Wanfang database,and WIP database from 1970 to 2021,was meticulously searched and comprehensively and statistically analyzed.RESULTS Following the established inclusion and exclusion criteria,13 studies were selected for analysis.Post-renal transplantation,no significant discrepancy was noted among the groups based on the location of TCM viscera.However,when categorized according to TCM pathogenic factors,the groups with spleen and kidney yang deficiency,as well as liver and kidney yin deficiency,exhibited a statistically significant difference in the frequency.CONCLUSION Currently,the research on TCM syndromes pertaining to renal transplantation is in its nascent phase.It is imperative to conduct a multicentric,large-scale survey of TCM syndromes subsequent to renal transplantation in the ensuing years.

Cohort study on the treatment of BRAF V600E mutant metastatic colorectal cancer with integrated Chinese and western medicine

BACKGROUND Patients with BRAF V600E mutant metastatic colorectal cancer(mCRC)have a low incidence rate,poor biological activity,suboptimal response to conventional treatments,and a poor prognosis.In the previous cohort study on mCRC conducted by our team,it was observed that integrated Chinese and Western medicine treatment could significantly prolong the overall survival(OS)of patients with colorectal cancer.Therefore,we further explored the survival benefits in the population with BRAF V600E mutant mCRC.AIM To evaluate the efficacy of integrated Chinese and Western medicine in the treatment of BRAF V600E mutant metastatic colorectal cancer.METHODS A cohort study was conducted on patients with BRAF V600E mutant metastatic colorectal cancer admitted to Xiyuan Hospital of China Academy of Chinese Medical Sciences and Traditional Chinese Medicine Hospital of Xinjiang Uygur Autonomous Region from January 2016 to December 2022.The patients were divided into two cohorts.RESULTS A total of 34 cases were included,with 23 in Chinese-Western medicine cohort(cohort A)and 11 in Western medicine cohort(cohort B).The median overall survival was 19.9 months in cohort A and 14.2 months in cohort B,with a statistically significant difference(P=0.038,hazard ratio=0.46).The 1-3-year survival rates were 95.65%(22/23),39.13%(9/23),and 26.09%(6/23)in cohort A,and 63.64%(7/11),18.18%(2/11),and 9.09%(1/11)in cohort B,respectively.Subgroup analysis showed statistically significant differences in median OS between the two cohorts in the right colon,liver metastasis,chemotherapy,and first-line treatment subgroups(P<0.05).CONCLUSION Integrated Chinese and Western medicine can prolong the survival and reduce the risk of death in patients with BRAF V600E mutant metastatic colorectal cancer,with more pronounced benefits observed in patients with right colon involvement,liver metastasis,combined chemotherapy,and first-line treatment.

Roles of traditional Chinese medicine extracts in hyperuricemia and gout treatment:Mechanisms and clinical applications

In this manuscript,we comment on the article by Liu et al published in the recent issue of the journal.Hyperuricemia(HUA)has become the second most common metabolic disease after type 2 diabetes mellitus and is the most important risk factor for gout.This discussion focuses on the targets and clinical application value of traditional Chinese medicine(TCM)extracts in the treatment of HUA and gout,emphasizing the role of gut microbiota.Liu et al’s study demonstrated that Poecilobdella manillensis protein extract alleviated HUA through multiple mechanisms,including inhibition of uric acid(UA)reabsorption,promotion of UA excretion,repair of intestinal barrier function,and regulation of gut microbiota and metabolome.Unlike the commonly used urate-lowering drugs such as allopurinol and febuxostat,which have clear and single targets,many TCMs have multi-target effects.However,the active components and mechanisms of TCMs are not fully understood,limiting their clinical application in the treatment of HUA and gout.Additionally,the role of gut microbiota in UA metabolic homeostasis needs to be further explored.

Research Progress on Modern Biological Mechanism of Chinese Medicine in Prevention and Treatment of Gouty Nephropathy

Chinese medicine prevention and treatment of gouty nephropathy often uses single drugs and compound formulae to regulate uric acid,which has the effect of lowering uric acid,protecting the kidneys and improving clinical symptoms.In conclusion,the mechanism of action of Chinese medicine against gouty nephropathy mainly involves oxidative stress,inflammatory response,etc.,with a view to providing new ideas and methods for the prevention and treatment of gouty nephropathy by Chinese medicine.

Lightweight and polarized self-attention mechanism for abnormal morphology classification algorithm during traditional Chinese medicine inspection

Objective To propose a Light-Atten-Pose-based algorithm for classifying abnormal morphol-ogy in traditional Chinese medicine(TCM)inspection to solve the problem of relying on manual labor or expensive equipment with personal subjectivity or high cost.Methods First,this paper establishes a dataset of abnormal morphology for Chinese medi-cine diagnosis,with images from public resources and labeled with category labels by several Chinese medicine experts,including three categories:normal,shoulder abnormality,and leg abnormality.Second,the key points of human body are extracted by Light-Atten-Pose algo-rithm.Light-Atten-Pose algorithm uses lightweight EfficientNet network and polarized self-attention(PSA)mechanism on the basis of AlphaPose,which reduces the computation amount by using EfficientNet network,and the data is finely processed by using PSA mecha-nism in spatial and channel dimensions.Finally,according to the theory of TCM inspection,the abnormal morphology standard based on the joint angle difference is defined,and the classification of abnormal morphology of Chinese medical diagnosis is realized by calculat-ing the angle between key points.Accuracy,frames per second(FPS),model size,parameter set(Params),and giga floating-point operations per second(GFLOPs)are chosen as the eval-uation indexes for lightweighting.Results Validation of the Light-Atten-Pose algorithm on the dataset showed a classification accuracy of 96.23%,which is close to the original AlphaPose model.However,the FPS of the improved model reaches 41.6 fps from 16.5 fps,the model size is reduced from 155.11 MB to 33.67 MB,the Params decreases from 40.5 M to 8.6 M,and the GFLOPs reduces from 11.93 to 2.10.Conclusion The Light-Atten-Pose algorithm achieves lightweight while maintaining high ro-bustness,resulting in lower complexity and resource consumption and higher classification accuracy,and the experiments prove that the Light-Atten-Pose algorithm has a better overall performance and has practical application in the pose estimation task.

Culture teaching is an inseparable part of International Chinese Language Education,and it has great significance in international Chinese language teaching.Students can enhance their interest in Chinese language learning and improve their learning effect through understanding the excellent traditional Chinese culture,at the same time,culture teaching can help to disseminate the Chinese culture and promote the development of a multicultural world.Taking the teaching of Chinese New Year culture as an example,we analyze the current teaching materials involving Chinese New Year culture with high popularity and acceptance,put forward effective teaching methods matching the feasibility of teaching Chinese New Year culture,and then present the specific teaching design of Chinese New Year culture,in order to provide references for the teaching of culture in International Chinese Language Education.

A Study on the Correlation Between LIWC Word Categories and Chinese Composition Writing Performance of Fourth,Fifth,and Sixth Grade Students

This study focuses on the analysis of the Chinese composition writing performance of fourth,fifth,and sixth grade students in 16 selected schools in Longhua District,Shenzhen during the spring semester of 2023.Using LIWC(Linguistic Inquiry and Word Count)as a text analysis tool,the study explores the impact of LIWC categories on writing performance which is scaled by score.The results show that the simple LIWC word categories have a significant positive influence on the composition scores of lower-grade students;while complex LIWC word categories have a significant negative influence on the composition scores of lower-grade students but a significant positive influence on the composition scores of higher-grade students.Process word categories have a positive influence on the composition scores of all three grades,but the impact of complex process word categories increases as the grade level rises.

Study on the Changes of Medical Income Structure in Governmentrun Hospitals of Traditional Chinese Medicine from 2012 to 2021

Objective To study the changing characteristics and trend of medical income structure in the government-run hospitals of traditional Chinese medicine(TCM),evaluate the effects of relevant reform measures,and to put forward corresponding suggestions for further optimizing their income structure.Methods The data related to the average medical income of government-run hospitals of TCM from 2012 to 2021 were sorted out.Then,descriptive analysis method was used to analyze the changes of related indicators.Besides,structural change method was applied to investigate the changes of outpatient income and inpatient income.Results and Conclusion From 2012 to 2021,the growth of medical income in government-run hospitals of TCM tended to be stable,and the proportion of medical service income increased from 22.62%(2012)to 29.38%(2021),but the average annual growth rate was only 0.68%.The main items that caused the change of outpatient income structure were medicine revenue,laboratory tests,diagnosis and treatment,and the cumulative contribution rate was 89.15%.The main items that caused the change of inpatient income structure were medicine revenue,sanitary materials,and auxiliary examinations income,with a cumulative contribution rate of 80.04%.However,the contribution rate of registration,diagnosis,treatment,surgery and nursing income reflecting the value of medical personnel’s technical labor was relatively small.The medical income structure of government-run hospitals of TCM underwent great changes and gradually became reasonable,but the medical service income increased slowly,and not all indicators achieved the expectations.To promote the sustainable development of public hospitals of TCM and enable them to provide high-quality and efficient TCM medical and health services,it is necessary to further improve the relevant policy mechanism.

Lipoprotein(a) as a Marker for Predicting the Presence and Severity of Coronary Artery Disease in Untreated Chinese Patients Undergoing Coronary Angiography

Objective Low-density lipoprotein cholesterol（LDL-C） has been well known as the risk factor of coronary artery disease（CAD）. However, the role of lipoprotein（a） [Lp（a）] in the development of CAD is of great interest but still controversial. Thus, we aim to explore the effect of Lp（a） on predicting the presence and severity of CAD in Chinese untreated patients, especially in combination with LDL-C. Methods We consecutively recruited 1,980 non-treated patients undergoing coronary angiography, among which 1,162 patients were diagnosed with CAD. Gensini score（GS） was used to assess the severity of CAD. Lp（a） was measured by immunoturbidimetric method. Results Patients with CAD had higher level of LDL-C and Lp（a） compared with non-CAD（P 〈 0.05）. Multivariable logistic regression revealed that Lp（a） 〉 205 mg/L（highest tertile） predicted 1.437-fold risk for CAD（95% CI： 1.108-1.865, P = 0.006） and 1.480-fold risk for high GS（95% CI： 1.090-2.009, P = 0.012） respectively. Interestingly, concomitant elevated level of Lp（a） and LDL-C conferred the highest risk for both presence [OR = 1.845, 95% CI： 1.339-2.541, P 〈 0.001] and severity [OR = 1.736, 95% CI： 1.188-2.538, P = 0.004] of CAD. Conclusion Lipoprotein（a） is a useful marker for predicting the presence and severity of CAD, especially combined with LDL-C.

In the 21st century,Chinese online fantasy fiction is gaining popularity overseas.Lai Jingping,a Chinese-American widely known online as RWX,inspired by the popularity of his translation of a Chinese fantasy fiction,Coiling Dragon,established a website called Wuxiaworld.com to disseminate Chinese fantasy culture.Now it has become the largest website in the world to translate these kinds of works.This paper,in order to explore the advantages of Wuxiaworld.com,examines the translation landscape and its trends as presented on this website,and further analyzes the translation of fiction and interactive platforms using I Shall Seal the Heavens,one of the stories on the website,as an example.Our study,through text analysis and data collection,found that Wuxiaworld.com is good at hyperlink-aided translations,and has translation awareness based on the analysis of covers,synopses and glossaries;foreignization is the main translation strategy when it comes to the fantasy terms in I Shall Seal the Heavens,while annotation in text as well as hyperlinks in the forms of text,pictures and music are adopted for cultural compensation;interactive platforms,such as comments,forums and announcements,are conducive to improving translation quality and stimulating readers’enthusiasm for these translations and Chinese culture.

The Comparison of Chinese and Western Cultures from the Perspective of Chinese Culture “Going Global”: With Confucius and Socrates’ Thoughts as Examples

Chinese culture “going global” is an inevitable requirement for China’s economic and social development.As an important part of Chinese traditional culture,the dissemination of Confucian thought is just the spread of Chinese culture.Therefore,the spread of Chinese Confucian thought abroad is also the key to Chinese culture “going global”.However,there are some cultural hinders between China and the West due to the differences in cultures.On the basis of summarizing Confucius and Socrates’ thoughts,this paper analyzes their essence of thoughts by contrast,which represent the eastern and Western cultures respectively.From the perspective of cross-cultural communication,the paper adopts the methods of multiculturalism,criticism and down-to-earth attitudes to deal with Chinese and Western cultures in order to make Chinese go global and share China’s wisdom with the world.

On the Strategic Paths of the Going Global Strategy of Chinese Literature

The going global strategy of Chinese literature is a vital part of the going global strategy of Chinese culture. In recent years,the Chinese government has launched and strongly supported a series of activities and projects to promote this strategy,but little effect has been produced. Starting from analyzing the predicament and reasons of the current strategy,the essay suggests four strategic paths: enhancing the overall strength of the country to promote the international influence of Chinese culture,strengthening cultural awareness and cultural introspection,integrating the nationality and cosmopolitan of literature and establishing a market-oriented literary translation mechanism.

Chinese herbal medicine for incomplete immune reconstruction in patients with AIDS undergoing antiretroviral treatment: A systematic review of randomized trials

Objective:To evaluate the effectiveness and safety of Chinese herbal medicines(CHMs)for incomplete immune reconstruction in patients with HIV/AIDS.Methods:Eight electronic databases were searched for randomized controlled trials(RCTs)on the use of CHM for patients with HIV/AIDS with incomplete immune reconstruction.Outcomes included CD4þcell count,quality of life,and adverse events/effects.The Cochrane Risk of Bias was employed to evaluate the methodological quality of the included RCTs.Results:We identified 13 eligible RCTs,with an overall high risk of bias,on 10 different CHMs.There was a significant increase in CD4þcell count after the use of Jianpi Yiqi medicinal paste for 3 months;tripterygium glycosides tablets(TGTs)for 3 months(mean difference[MD]52.63 cells/mL,95%confidence interval[CI,46.98,58.28]),6,9,and 12 months;Wenshen Jianpi granules for 6 months;Shenling Fuzheng capsules for 6 months(MD 49.53 cells/mL,95%CI[8.45,90.61])and 12 months;Aikeqing granules for 9 months(MD 61.51 cells/mL,95%CI[16.25,106.77])and 12 months;Guipi decoction for 12 months;Mianyi No.2 granules(JT)for 12 and 18 months;and Chinese medicine granules for 18 months.The increase in the mean difference of CD4þcell count from 6 to 18 months was larger in Chinese medicine granules and Mianyi No.2 granules(JT).Guipi decoction and Jianpi Qushi decoction improved the Karnofsky score.Four RCTs reported the outcome of adverse events/effects,while four cases of minor adverse effects were reported in the TGTs group.Conclusion:Jianpi Yiqi medicinal paste,Wenshen Jianpi granules,Shenling Fuzheng capsules,Aikeqing granules,Guipi decoction,and TGTs may be effective in increasing CD4þwithin 12 months,and Mianyi No.2 granules(JT)and Chinese medicine granules may show long-term effects.High-quality large RCTs on the effectiveness and safety of CHMs are still warranted.

Empathy and Cognitive Schema:The Ways of Chinese Culture Going Global Reconsidered

In spite of its acknowledged achievements and contributions in recent years,it is still necessary to review the diplomacy of Chinese culture going global.Approached from the perspective of cognition and affection,the main reason leading to the low efficiency of Chinese official cross-cultural communication is that the collectivism implied in the governmental institutes is in conflict with the individualism schema in Western mainstream discourse,making it difficult to invoke empathy from Western audience.By contrast,some popular culture spread through the Internet is empathy-provoking only because it contains two schemata of nostalgia and individualism that have been embedded in many Western netizens’cognition.In other words,popular culture has little communicating power to transmit the essence of Chinese culture.Given the limitations of these two ways,China should at least make adjustments on four aspects to improve the effect of its culture going out:(1)integrate more mutually shared schemata into the selected Chinese culture;(2)modernize the cultural contents and transmission methods;(3)reduce the role of the government and insist on diversifying and individualizing communication subjects;(4)encourage the creation of fine culture.
