Sepsis one-hour bundle management combined with psychological intervention on negative emotion and sleep quality in patients with sepsis

BACKGROUND Sepsis is a serious infectious disease caused by various systemic inflammatory responses and is ultimately life-threatening.Patients usually experience depression and anxiety,which affect their sleep quality and post-traumatic growth levels.AIM To investigate the effects of sepsis,a one-hour bundle(H1B)management was combined with psychological intervention in patients with sepsis.METHODS This retrospective analysis included 300 patients with sepsis who were admitted to Henan Provincial People’s Hospital between June 2022 and June 2023.According to different intervention methods,the participants were divided into a simple group(SG,n=150)and combined group(CG,n=150).H1B management was used in the SG and H1B management combined with psychological intervention was used in the CG.The changes of negative emotion,sleep quality and post-traumatic growth and prognosis were compared between the two groups before(T0)and after(T1)intervention.RESULTS After intervention(T1),the scores of the Hamilton Anxiety scale and Hamilton Depression scale in the CG were significantly lower than those in the SG(P<0.001).Sleep time,sleep quality,sleep efficiency,daytime dysfunction,sleep disturbance dimension score,and the total score in the CG were significantly lower than those in the SG(P<0.001).The appreciation of life,mental changes,relationship with others,personal strength dimension score,and total score of the CG were significantly higher than those of the SG(P<0.001).The scores for mental health,general health status,physiological function,emotional function,physical pain,social function,energy,and physiological function in the CG were significantly higher than those in the SG(P<0.001).The mechanical ventilation time,intensive care unit stay time,and 28-d mortality of the CG were significantly lower than those of the SG(P<0.05).CONCLUSION H1B management combined with psychological intervention can effectively alleviate the negative emotions of patients with sepsis and increase their quality of sleep and life.

Negative effects of artiffcial nest boxes on birds:A review

Artificial nest boxes are placed to attract birds to nest and breed in a specific location,and they are widely used in avian ecology research and in the attraction of insectivorous birds.There is evidence that artificial nest boxes can adversely affect breeding fitness but no great focus has been placed on this issue by researchers.Therefore,we retrieved 321 research papers regarding artificial nest boxes published from 2003 to 2022 and used the'Biblioshiny'program to extract and integrate keywords;we then summarized the adverse effects of artificial nest boxes on avian breeding success.The studies highlighted many drawbacks and misuses in the designing and placement of nest boxes;furthermore,bird attraction was decreased by their inappropriate selection,thus reducing breeding success.Regarding nest box production,there were shortcomings in the construction material,color,smell,and structural design of the boxes used.Nest boxes were also placed at inappropriate densities,locations,orientations,heights,and managed incorrectly.Finally,we propose suggestions for more efficient and safer artificial nest boxes for future use in avian ecology research and bird conservation.

Evaluation of urban underground space resources using a negative list method: Taking Xi'an City as an example in China

Utilization of urban underground space has become a vital approach to alleviate the strain on urban land resources,and to optimize the structure and pattem of the city.It is also very important to improve the city environment,build livable city and increase the capacity of the city.Based on the analysis of existing evaluation methods and their problems,a method for evaluating underground space resources based on a negative list of adverse factors affecting underground space development is proposed,to be primarily used in urban planning stages.A list of the adverse factors is established,including limiting factors,constraining factors and influencing factors.Taking Xi'an as an example,using a geographical information system platform,a negative list of adverse factors for the underground space resources in Xi'an City are evaluated,and preventive measures are proposed.Natural resources,exploitable resources,and the potential growth of exploitable underground space resources are evaluated.Underground space assessment in the different development stages of the city,collaborative utilization and safety evaluation for multiple subsurface resources,environmental impact and assessment,as well as evaluation methods based on big data and intelligent optimization algorithms are all discussed with the aim of serving city planning and construction.

Effect Study of Management Buyouts of Chinese Listed Companies ——On the Basis of the Company Financial Indexes

This paper selects 11 Chinese listed companies as the sample and compares the effect of premanagement buyouts and post-management buyouts （hereafter MBO）. The authors evaluate the effects of MBO according to the change of earnings capacity, management ability, debt paying ability, development ability and capital structure. The result shows that the obvious change does not take place in earnings capacity of the company and management ability, but the development ability of the company improves notably, and most of companies have taken advantage of financial leverage withir two years of post-MBO.

Rural Carbon Emissions Based on Community Management

Rural community is an effective way to simultaneously achieve manage- ment of ecological resources and protection of natural resources, depending on its management mode based on cooperation, motivation, local experiences and extensi- ble boundary. In the research, carbon emission in community is a kind of ecological resources and negative externality is the cause of increase of rural carbon emission. Compared with governmental ruling and marketing approach, community manage- ment mode proves more effective to solve the problem of negative externality of carbon emission in community. Furthermore, rural carbon, emission was analyzed in detail on basis of community management mode and extending mode of carbon emis- sion in rural areas. In addition, some policies and suggestions were proposed to im- prove community management of carbon emission in rural areas, providing an ef- fective way for low-carbon economy in rural areas.

Can the improvement of competitive adequacy and fairness reduce discriminatory M&A behavior?Evidence from the market access negative list pilot in China

Corporate mergers and acquisitions(M&As)are subject to skewed logic due to excessive government regulation.China is progressively adopting the Market Access Negative List(MANL)pilot to transfer the power of resource allocation from the government to the market.Using the DID method,we examine the impact of relaxing market access regulation on firms’M&A behavior against China’s institutional background and the M&A events of listed companies from 2012 to 2019.The MANL significantly increases firms’M&A tendency and amount and strengthens the competitive adequacy and fairness of market-oriented M&A decisions.Post-M&A financial performance does not increase,but human capital productivity,innovation effectiveness and total factor productivity do,demonstrating the dynamic balance of profit and efficiency in M&As.

Management protocol for Fournier’s gangrene in sanitary regime caused by SARS-CoV-2 pandemic:A case report

BACKGROUND Fournier's gangrene(FG)is a serious,aggressive and often deadly polymicrobial infection of the soft tissues of the perineum,the rectum and the external genital organs.It is an anatomical subcategory of necrotizing fasciitis,which has a similar etiology and treatment strategy.CASE SUMMARY A 60-year-old man was admitted to the hospital during severe acute respiratory syndrome coronavirus 2 pandemic with complaints of fever up to 38.9°C,abdominal pain,and edema of the scrotum,the penis,the perineum,and the right gluteal region for 2 d.Computed tomography of the abdomen and the pelvis revealed extensive inflammatory infiltrations of the subcutaneous tissue of the hypogastrium,and the penis;along with liquefaction and presence of gas in the subcutaneous tissues of the scrotum,the perineum,and the right gluteal region.The patient was diagnosed with FG,and was urgently qualified to undergo surgery in the Department of Urology.After performing the necessary examinations,a resection of the necrotic tissues with bilateral orchiectomy and excision of the penile and scrotal skin was performed.After surgery,he was transferred to the intensive care unit for further management.CONCLUSION Early management prevents the resection of the other organs by inhibiting the contiguous spread of infection.

This review begins with an introduction of groundwater resources in China and their distribution characteristic,followed by an elaboration of the exploitation and utilization of groundwater and the negative environmental effects from groundwater overexploitation, and a description of the existing groundwater protection and management measures. At last, the existing problems in groundwater protection and management, with some suggestions, are presented.

Research on Higher Education Collaborative Management Platform Based on Relationship Graph

According to the current problems of higher education management informatization,this paper puts forward a development scheme of collaborative platform on education management.The main technology includes three parts.First,integrate the distributed database and use two-tier linked list to realize dynamic data access.Second,the relation graph is used to display the data of each student,so as to realize the visual sharing of data.Third,realize the collaborative information security mechanism from three aspects to ensure the legal sharing of data.Finally,the platform development is completed with Java.It can help to improve the effectiveness of educating students.

Prognostic Significance of Vascular Endothelial Growth Factor-A (VEGF-A) and Ki-67 Expression in Head and Neck Cancer Patient with Negative Neck

Background: Management of N0 neck in patients with head and neck squamous cell carcinoma (HNSCC) remains a subject of continued debate. Prognostic biomarkers might provide useful information for treatment selection and adjustment. Objective: To evaluate the prognostic relevance of VEGF-A and Ki-67 expression to types of neck management. Methods: This prospective study included 140 patients with HNSCC. Tumor expression of VEGF-A and Ki-67 was measured by immunohistochemistry. Based on tumor size and site criteria, 88 patients with N0 neck were categorized as high, intermediate and low risk of subclinical neck diseases and accordingly treated by elective neck dissection (END), irradiation (ENI) and observation. Adjuvant treatment was given to tumor with close or positive margins. A multivariate Cox regression model was used to identify prognostic factors. Impact of biomarker expression, treatment type and risk category on disease-specific survival (DSS) in the setting of N0 neck were evaluated by Kaplan-Meier survival and adjusted hazard ratio (HR). Results: Coexpression of VEGF-A and Ki-67 (HR = 2.351, p = 0.021) and positive node (HR = 2.301, p = 0.009) were independent prognostic factors for HNSCC. In the setting of N0 neck, marker coexpression has an HR of 4.97 (p = 0.004) independent of treatment modalities (p = 0.069) and risk categories (p = 0.971). Alternatively, neither marker expression was predictive of a better treatment outcome for END compared to ENI, as suggested by the odds of patients being survived 15.4 times greater (p = 0.01) and the 5-year DSS rates of 85.1% versus 44.7% (p = 0.008). Conclusion: Coexpression of VEGF-A and Ki-67 is a suggestion of tumor microinvasiveness in addition to risk of lymph node metastasis and may indicate the need of adjuvant treatment despite negative tumor margins. Neither marker expression serves an indicator for the selection of END over ENI in neck management.

大数据驱动的税收征管与非上市公司高质量发展——基于创新行为的研究视角

作为我国经济发展的重要力量,非上市公司创新行为是社会技术进步不可或缺的推动力。利用独特的全国税收调查数据,以金税三期工程在我国各省市的逐步试点作为政策冲击,探讨大数据驱动的税收征管是否以及如何影响非上市公司的创新行为。实证研究发现,在金税三期工程试点实施后,非上市公司加大了创新投入。机制检验发现,这种影响主要是通过大数据税收征管的公司治理效应和信息质量效应获得实现。异质性检验发现,大数据税收征管对非上市公司创新投入的影响与金税三期工程实施的客观条件和主观能动性,以及企业融资约束程度密切相关。进一步分析发现,在金税三期工程试点实施后,非上市公司的创新产出有了明显提升。

长江流域重要饮用水水源地保护长效机制

指导重要饮用水水源地保护是《中华人民共和国水法》《中华人民共和国长江保护法》和“三定”规定赋予水行政主管部门的法定职责,也是维系流域供水安全保障的底线。通过对长江流域重要饮用水水源地保护管理现状的回顾分析,针对保护管理中存在的水源地名录准入退出机制不健全、安全评估制度不完善、资金投入保障机制不足等问题,提出了完善水源地安全保障规划体系、规范水源地名录管理机制、强化水源地安全评估和保护、严格水源地保护考核等长效机制构建思路和后续工作建议。研究成果对于落实《中华人民共和国水法》《中华人民共和国长江保护法》饮用水水源地保护要求、贯彻长江经济带绿色发展和长江大保护建设的要求、推动长江流域重要饮用水水源地保护长效机制建设具有重要意义。

口腔修复门诊实施院内感染清单式管理的效果分析

目的分析口腔修复门诊实施院内感染清单式管理的效果。方法针对院内感染监测发现的问题,于2021年8月—2022年12月在医院口腔修复门诊实施包括诊室空气质量、医疗器械、口腔印模、医护人员手卫生的清单式管理。经过3次诊室空气培养,器械表面、印模表面及医护人员手部采样和细菌学检测,详细记录监测结果,对比实施前后口腔修复门诊院内感染控制质量监测数据的差异,分析实施效果。结果清单式管理实施后,口腔科门诊空气菌落计数较清单式管理实施前下降(P<0.01);医护人员手部、口腔印模表面、医疗器械表面菌落计数较清单式管理实施前下降(P<0.01);口腔科门诊空气、医护人员手部、托盘消毒储存监测合格率较清单式管理实施前提升(P<0.01);患者院内感染发生率较实施前下降(P<0.01)。结论口腔修复门诊实施院内感染清单式管理可有效减少诊室空气粉尘、器械表面、印模表面、医护人员手部的细菌学检出率,降低院内感染风险。
