The catalytic activities of the catalysts as well as influencing factors on catalytic activity have been investigated for the simultaneous removal of NOx and diesel soot particulate. An increase in catalytic activity for the selective reduction of NOx was observed with Ag addition in these perovskite oxides, especially with 5% Ag loading. Advanced glycation end products(AGEs)are a complex and heterogencous group of compounds that have been implicated in diabetes related complfcations.Sk in autofluorescence was recently introduced as an altemative tool ...Advanced glycation end products(AGEs)are a complex and heterogencous group of compounds that have been implicated in diabetes related complfcations.Sk in autofluorescence was recently introduced as an altemative tool for skin AGEs accumulation assessment in diabetes.Sucossful optical diagnosis of diabetes requires a rapid and accurate classification algorithm.In order to improve the performance of noninvasive and optical diagnosis of type 2 diabetes,support vector machines(SVM)algorithm was implemented for the clasification of skin autofluorescence from diabetics and control subjects.Cross-validation and grid optimization methods were employed to calculate the optimal parameters that ma ximize classification accuracy.Classification model was set up according to the training set and then veri fied by the testing set.The results show that radical basis fiunction is the best choice in the four common kernels in SVM.Moreover,a diagnostic accuracy of 82.61%,a sensitivity of 69.57%,and a specificity of 95.65%for discriminating diabetics from control subjects were achieved using a mixed kemel function,which is based on liner kernel function and radical basis function.In comparison with fasting plasma glucose and HbAue test,the clasifcation method of skin autofuorescence spectrum based on SVM shows great potential in screening of diabetes.展开更多

K-type and inverted-K-type jacket support structures have superior potential due to their fewer joints and lower cost of manufacture and installation. A numerical study was presented on the dynamic responses of K-type and inverted-K-type jacket support structures subjected to different kinds of dynamic load. The results show that the inverted-K-type jacket structure has higher natural frequencies than the K-type. The wave force spectrum response shows that the maximum displacement of the K-type jacket structure is larger than that of the inverted-K-type. The time-history responses under wind and wave-current load indicate that the inverted-K-type jacket structure shows smaller displacement and stress compared with the K-type, and presents different stress concentration phenomena. The dynamic responses reveal that the inverted-K-type of jacket support structure has greater stiffness and superior mechanical properties, and thus is more applicable in the offshore area with relatively deep water.展开更多

Methods: Patients with ...Objective: This study aims to explore the effect of peer support in improving self-efficacy of rural patients with type 2 diabetes and the application of drugs to reduce the pain from diabetes. Methods: Patients with type 2 diabetes in the rural area were selected and they were divided into the peer group and control group with voluntary. The patients in peer support group applied six kinds of activity patterns, while the control group was carried out with routine activities. Results: Comparison indexes for the two groups are mastering degree of related knowledge to diabetes, control compliance rate of indicators related to diabetes, and management of diabetes self-efficacy. Comparing with the control group, indexes in peer support group were significantly increased (P < 0.01). Meanwhile, in the table of diabetes distress, 17 items have been improved and the occurrence rate of painful diabetic decreased significantly (P < 0.01). Conclusion: Peer support can improve the rural patients’ self-efficacy of type 2 diabetes and decrease patients’ pain from type 2 diabetes mellitus.展开更多

Social support can buffer effects of stress. No previous study has been found comparing perceptions of foreign-born persons with DM, concerning the content of and need for social support with a population of origin. The aim was to compare the meaning of support and its impact on the life situation in Swedish- and foreign-born persons diagnosed with type 2 DM living in Sweden. A purposive sample of 74 persons;34 foreign- and 40 Swedish-born, aged 32-80 years, diagnosed with type 2 DM, was included. The foreign-born persons were mostly Middle Eastern and Ex-Yugoslavian refugees. Mixed methods were used and data were collected by qualitative interviews and quantitative data collected by the Norbeck Social Support Questionnaire (NSSQ). Studying the figures measured by NSSQ, no significant differences were found according to gender but foreign-born men scored lower than Swedish, especially in emotional support and aid. The main findings showed that respondents, irrespective of origin, described the meaning of the concept of support mainly as information support in learning to manage DM. However, foreign-born persons also focused on medical support with regular follow-ups while Swedes emphasized the need for emotional support. Foreign- and Swedish-born persons are in general more similar than dissimilar in describing the meaning, need and impact of support. Irrespective of origin they want to learn to manage their disease and thus desire and need regular follow-up and information immediately from being diagnosed. Further studies are needed as the study is based on a limited sample.展开更多

Some working points for achieving sufficient accuracy in free surface grinding are discussed. GSX-F is constructed using the patch division method and is used to test grinding. The life-altering change was determined to be a diagnosis of type 2 diabetes,the journey is the ups and downs of coping with the diagnosis as people work toward acceptance of type 2 diabetes.The review includes a synthesis of findings from 22 quantitative studies of psychosocial factors and coping in adults with type 2 diabetes.Anxiety,depression,stress,and diabetes distress were identified as key influential psychosocial factors.Increased social support was inversely related to emotional distress and coping styles were related to social well-being,psychological health,and physical health outcomes.The positive coping style of problem-focused coping was linked to improved psychological and physical health.Emotional responses to diagnosis were related to depression and anxiety.Negative coping styles of resignation,protest,or isolation were higher in women and linked to poorer quality of life,while avoidance was linked to increased diabetes-related distress and depressive symptoms.展开更多
BACKGROUND Large-scale functional connectivity(LSFC)patterns in the brain have unique intrinsic characteristics.Abnormal LSFC patterns have been found in patients with dementia,as well as in those with mild cognitive ...BACKGROUND Large-scale functional connectivity(LSFC)patterns in the brain have unique intrinsic characteristics.Abnormal LSFC patterns have been found in patients with dementia,as well as in those with mild cognitive impairment(MCI),and these patterns predicted their cognitive performance.It has been reported that patients with type 2 diabetes mellitus(T2DM)may develop MCI that could progress to dementia.We investigated whether we could adopt LSFC patterns as discriminative features to predict the cognitive function of patients with T2DM,using connectome-based predictive modeling(CPM)and a support vector machine.AIM To investigate the utility of LSFC for predicting cognitive impairment related to T2DM more accurately and reliably.METHODS Resting-state functional magnetic resonance images were derived from 42 patients with T2DM and 24 healthy controls.Cognitive function was assessed using the Montreal Cognitive Assessment(MoCA).Patients with T2DM were divided into two groups,according to the presence(T2DM-C;n=16)or absence(T2DM-NC;n=26)of MCI.Brain regions were marked using Harvard Oxford(HOA-112),automated anatomical labeling(AAL-116),and 264-region functional(Power-264)atlases.LSFC biomarkers for predicting MoCA scores were identified using a new CPM technique.Subsequently,we used a support vector machine based on LSFC patterns for among-group differentiation.The area under the receiver operating characteristic curve determined the appearance of the classification.RESULTS CPM could predict the MoCA scores in patients with T2DM(Pearson’s correlation coefficient between predicted and actual MoCA scores,r=0.32,P=0.0066[HOA-112 atlas];r=0.32,P=0.0078[AAL-116 atlas];r=0.42,P=0.0038[Power-264 atlas]),indicating that LSFC patterns represent cognition-level measures in these patients.Positive(anti-correlated)LSFC networks based on the Power-264 atlas showed the best predictive performance;moreover,we observed new brain regions of interest associated with T2DM-related cognition.The area under the receiver operating characteristic curve values(T2DM-NC group vs.T2DM-C group)were 0.65-0.70,with LSFC matrices based on HOA-112 and Power-264 atlases having the highest value(0.70).Most discriminative and attractive LSFCs were related to the default mode network,limbic system,and basal ganglia.CONCLUSION LSFC provides neuroimaging-based information that may be useful in detecting MCI early and accurately in patients with T2DM.展开更多

METHODS: Adolescents with type 2 diab...AIM: To investigate whether assigning young, healthy and motivated lay volunteer partners("buddies") to adolescents with type 2 diabetes improves hemoglobin A1c(HbA 1c). METHODS: Adolescents with type 2 diabetes were randomized to partnering with a "buddy" or to conventional treatment. During the initial screening visit, which coincided with a routine outpatient diabetes clinic visit, patients with type 2 diabetes underwent a physical examination, detailed medical history, laboratory measurement of HbA 1c, and completed two questionnaires(Pediatric Quality of Life Inventory and Children's Depression Inventory) to assess their overall quality of life and the presence of depressive symptoms. Patients were then randomized to the intervention(the buddy system) or conventional treatment(standard care). All patients were scheduled to return for followup at 3- and 6-mo after their initial visit. Hb A1 c was determined at all visits(i.e., at screening and at the 3- and 6-mo follow-up visits) and quality of life and depressive symptoms were evaluated at the screening visit and were reassessed at the 6-mo visit. RESULTS: Ten adolescents, recruited from a pool of approximately 200 adolescents, enrolled over a twoyear time period, leading to premature termination of the study. In contrast, we easily recruited motivated lay volunteers. We found no change in HbA 1c from the initial to the 6-mo visit in either group, yet our small sample size limited systematic assessment of this outcome. Participants repeatedly missed clinic appointments, failed to conduct self-glucose-monitoring and rarely brought their glucometers to clinic visits. Total quality of life scores(72.6 ± 6.06) at screening were similar to previously reported scores in adolescents with type 2 diabetes(75.7 ± 15.0) and lower than scores reported in normal-weight(81.2 ± 0.9), overweight(83.5 ± 1.8), and obese youths without diabetes(78.5 ± 1.8) or in adolescents with type 1 diabetes(80.5 ± 13.1). Among adolescents who returned for their 6-mo visit, there were no differences in total quality of life scores(70.2 ± 9.18) between screening and follow-up.CONCLUSION: Our approach, effective in adults with type 2 diabetes, was unsuccessful among adolescents and emphasizes the need for innovative strategies for diabetes treatment in adolescent patients.展开更多

In particular, patients without gastric stenosis who can consume a meal usually experience better quality of l...Patients with type 4 gastric cancer and peritoneal metastasis respond better to chemotherapy than surgery. In particular, patients without gastric stenosis who can consume a meal usually experience better quality of life (QOL). However, some patients with unsuccessful chemotherapy are unable to consume a meal because of gastric stenosis and obstruction. These patients ultimately require salvage surgery to enable them to consume food normally. We evaluated the outcomes of salvage total gastrectomy after chemotherapy in four patients with gastric stenosis. We determined clinical outcomes of four patients who underwent total gastrectomy as salvage surgery. Outcomes were time from chemotherapy to death and QOL, which was assessed using the Support Team Assessment Schedule-Japanese version (STAS-J). Three of the patients received combination chemotherapy [tegafur, gimestat and otastat potassium (TS-1); cisplatin]. Two of these patients underwent salvage chemotherapy after 12 and 4 mo of chemotherapy. Following surgery, they could consume food adequately and their STAS-J scores improved, so their treatments were continued. The third patient underwent salvage surgery after 7 mo of chemotherapy. This patient was unable to consume food adequately after surgery and developed surgical complications. His clinical outcomes at 3 mo were very poor. The fourth patient received combination chemotherapy (TS-1 and irinotecan hydrochloride) for 6 mo and then underwent received salvage surgery. After surgery, he could consume food adequately and his STAS-J score improved, so his treatment was continued. After the surgery, he enjoyed his life for 16 mo. Of four patients who received salvage total gastrectomy after unsuccessful chemotherapy, the QOL improved in three patients, but not in the other patient. Salvage surgery improves QOL in most patients, but some patients develop surgical complications that prevent improvements in QOL. If salvage surgery is indicated, the surgeon and/or oncologist must provide the patient with a clear explanation of the purpose of surgery, as well as the possible risks and benefits to allow the patient to reach an informed decision on whether to consent to the procedure.展开更多

The study assessed adherence to treatment by type 2 DM patients aged 20 years and above at Monze Mission Hospital in Monze district, Zambia. Research questions: 1) What is the level of adherence to treatment by type 2 DM patients receiving care from Monze Mission Hospital? 2) What factors influence adherence to treatment by type 2 DM patients? <strong>Methods:</strong> The study was conducted in 2017. It was a cross-sectional design. Simple random sampling method was used to select respondents and data was collected using a structured interview schedule. Data was entered and analyzed using the modified self-reported Morisky Medication Adherence Scale with six scores, modified self-care management questionnaire and IBM<span style="white-space:nowrap;"><sup><span style="white-space:nowrap;"><span style="white-space:nowrap;">©</span></span></sup> </span>Statistical Package for Social Sciences. Chi-Square was used to test associations between variables and binary logistic regression was used for multivariate analysis. The Morisky Medication Adherence scale was interpreted as follows: patients who scored between 4 - 6 points on knowledge had high knowledge while those who scored between 1 - 3 points had low knowledge about the disease. <strong>Results: </strong>The sample size of the study was 138 respondents. The study showed, only 44.2% of respondents had good adherence to treatment as they scored above 4 using the Morisky Medication Adherence Scale. Majority of the patients (55.6%) had knowledge about treatment despite 65.2% of respondents reported distance and financial challenges as hindrances to adherence. The study revealed a statistical association between adherence and knowledge of type 2 DM treatment. <strong>Conclusion:</strong> The study showed that poor adherence to treatment reduced as knowledge about DM increased. There is need to consider educational programs to strengthen adherence to dietary advice, regular exercise and follow up, to achieve normal glycemic levels.展开更多

BACKGROUND In transradial intervention procedures,poor back-up support and noncoaxial alignment of the guide catheter(GC)may result in failure of the balloon or stent to reach the targeted lesion.Methods to provide ex...BACKGROUND In transradial intervention procedures,poor back-up support and noncoaxial alignment of the guide catheter(GC)may result in failure of the balloon or stent to reach the targeted lesion.Methods to provide extra back-up support using the original GC and wire can improve procedural success with reduced complications.A rapid exchange guide extension catheter provides convenient and efficient back-up support while preserving the initial GC and inserted wire.AIM To evaluate the efficacy and safety of rapid exchange extension catheter in the treatment of type B2/C nonocclusive coronary lesions via the radial access.METHODS A total of 135 patients with type B2/C nonocclusive lesions who were treated via the transradial approach were enrolled in the study.The clinical characteristics,indications for use of the rapid exchange extension catheter,and procedural details and results were reviewed and analyzed.All procedure-related complications and major adverse cardiovascular events were recorded during the in-hospital stay and follow-up period.RESULTS The most common indication for the use of a rapid exchange extension catheter was vascular tortuosity(37.8%),followed by heavy calcification(28.9%),long lesions(20.0%),proximal stent(6.7%),in-stent restenosis(5.2%),and coronary origin anomalies(1.5%).The following technologies failed in passing targeted lesions before delivering the rapid exchange catheter:Multiple predilatation technique(57%),buddy wire technique(33.4%),balloon anchoring technique (5.9%), and cutting balloon modification (3.7%). The mean depth of the extensioncatheter intubation was 20.56 ± 13.05 mm, and the mean rapid exchange catheterservice time was 18.9 ± 9.7 min. The mean length and diameter of stents were 33.5± 14.4 mm and 2.75 ± 0.45 mm, respectively. The total rate of technique success(balloon or stent successful crossing of the target lesion with this technique) was94.8%.CONCLUSIONThe rapid exchange extension catheter technique showed acceptable safety andefficacy in the transradial coronary interventions of type B2/C nonocclusivecoronary lesions. We recommend this technique to assist in complex lesionintervention via radial access.展开更多

The crystal structure of scheelite-type compounds having M^1M^′Ⅲ (XO4)2(X = Mo, W) as common formula and the formability of the continuous solid solution between these compounds and rare earth molybdates or tungstates were also investigated. Besides, the cell constants of these compounds can be calculated by some semi-empirical equations. Based on the obtained relationships, the results of computerized prediction of the solid solubility of T1Pr (MoO4)2-Pr2 (MoO4)3 system have good agreement with experimental results.展开更多

The dynamic responses reveal that the inverted-K-type of jacket support structure has greater stiffness and superior mechanical properties, and thus is more applicable in the offshore area with relatively deep water.
文摘Objective: This study aims to explore the effect of peer support in improving self-efficacy of rural patients with type 2 diabetes and the application of drugs to reduce the pain from diabetes. Methods: Patients with type 2 diabetes in the rural area were selected and they were divided into the peer group and control group with voluntary. The patients in peer support group applied six kinds of activity patterns, while the control group was carried out with routine activities. Results: Comparison indexes for the two groups are mastering degree of related knowledge to diabetes, control compliance rate of indicators related to diabetes, and management of diabetes self-efficacy. Comparing with the control group, indexes in peer support group were significantly increased (P < 0.01). Meanwhile, in the table of diabetes distress, 17 items have been improved and the occurrence rate of painful diabetic decreased significantly (P < 0.01). Conclusion: Peer support can improve the rural patients’ self-efficacy of type 2 diabetes and decrease patients’ pain from type 2 diabetes mellitus.

文摘Diabetes (DM) requires self-care that is demanding and may cause stress. Social support can buffer effects of stress. No previous study has been found comparing perceptions of foreign-born persons with DM, concerning the content of and need for social support with a population of origin. The aim was to compare the meaning of support and its impact on the life situation in Swedish- and foreign-born persons diagnosed with type 2 DM living in Sweden. A purposive sample of 74 persons;34 foreign- and 40 Swedish-born, aged 32-80 years, diagnosed with type 2 DM, was included. The foreign-born persons were mostly Middle Eastern and Ex-Yugoslavian refugees. Mixed methods were used and data were collected by qualitative interviews and quantitative data collected by the Norbeck Social Support Questionnaire (NSSQ). Studying the figures measured by NSSQ, no significant differences were found according to gender but foreign-born men scored lower than Swedish, especially in emotional support and aid. The main findings showed that respondents, irrespective of origin, described the meaning of the concept of support mainly as information support in learning to manage DM. However, foreign-born persons also focused on medical support with regular follow-ups while Swedes emphasized the need for emotional support. Foreign- and Swedish-born persons are in general more similar than dissimilar in describing the meaning, need and impact of support. Irrespective of origin they want to learn to manage their disease and thus desire and need regular follow-up and information immediately from being diagnosed. Further studies are needed as the study is based on a limited sample.

文摘Methyl-(γ-chloropropyl)dichlorosilane was synthesized under the catalysis of a silicasupported Karstedt-type catalyst. By orthogonal experimental design method, the optimum reaction parameters such as reactants ratio, reaction temperature and time, and the dosage of catalyst, were determined. At the optimum reaction condition the product yield reached 78.42%, which is higher than that reported in the literatures.
文摘To obtain accurate forms and surfaces in free surface grinding, it is important to provide grinding conditions suitable for a curved surface. A grinding support system for the free surface (GSX-F) is proposed to help the operator grind a free surface with the high accuracy and the high productivity. To succeed in free surface grinding, the property of a ball type wheel must be known. Therefore, a basic study of free surface grinding with a ball type wheel is carried out based on the grinding center (GC). Some working points for achieving sufficient accuracy in free surface grinding are discussed. GSX-F is constructed using the patch division method and is used to test grinding. Reasonable results are obtained.

文摘Type 2 diabetes mellitus contributes to poor health outcomes including mortality,yet there is a gap in the literature when seeking to understand the influence of psychosocial factors on coping in this population.The paper presents a systematic review of quantitative studies that examined relationships among psychosocial determinants and coping in adults with type 2 diabetes.This review is the second layer of knowledge discovery for the concept,"Taking on a life-altering change is a rhythmical journey of experiencing ups and downs on the way to acceptance." The life-altering change was determined to be a diagnosis of type 2 diabetes,the journey is the ups and downs of coping with the diagnosis as people work toward acceptance of type 2 diabetes.The review includes a synthesis of findings from 22 quantitative studies of psychosocial factors and coping in adults with type 2 diabetes.Anxiety,depression,stress,and diabetes distress were identified as key influential psychosocial factors.Increased social support was inversely related to emotional distress and coping styles were related to social well-being,psychological health,and physical health outcomes.The positive coping style of problem-focused coping was linked to improved psychological and physical health.Emotional responses to diagnosis were related to depression and anxiety.Negative coping styles of resignation,protest,or isolation were higher in women and linked to poorer quality of life,while avoidance was linked to increased diabetes-related distress and depressive symptoms.

文摘BACKGROUND Large-scale functional connectivity(LSFC)patterns in the brain have unique intrinsic characteristics.Abnormal LSFC patterns have been found in patients with dementia,as well as in those with mild cognitive impairment(MCI),and these patterns predicted their cognitive performance.It has been reported that patients with type 2 diabetes mellitus(T2DM)may develop MCI that could progress to dementia.We investigated whether we could adopt LSFC patterns as discriminative features to predict the cognitive function of patients with T2DM,using connectome-based predictive modeling(CPM)and a support vector machine.AIM To investigate the utility of LSFC for predicting cognitive impairment related to T2DM more accurately and reliably.METHODS Resting-state functional magnetic resonance images were derived from 42 patients with T2DM and 24 healthy controls.Cognitive function was assessed using the Montreal Cognitive Assessment(MoCA).Patients with T2DM were divided into two groups,according to the presence(T2DM-C;n=16)or absence(T2DM-NC;n=26)of MCI.Brain regions were marked using Harvard Oxford(HOA-112),automated anatomical labeling(AAL-116),and 264-region functional(Power-264)atlases.LSFC biomarkers for predicting MoCA scores were identified using a new CPM technique.Subsequently,we used a support vector machine based on LSFC patterns for among-group differentiation.The area under the receiver operating characteristic curve determined the appearance of the classification.RESULTS CPM could predict the MoCA scores in patients with T2DM(Pearson’s correlation coefficient between predicted and actual MoCA scores,r=0.32,P=0.0066[HOA-112 atlas];r=0.32,P=0.0078[AAL-116 atlas];r=0.42,P=0.0038[Power-264 atlas]),indicating that LSFC patterns represent cognition-level measures in these patients.Positive(anti-correlated)LSFC networks based on the Power-264 atlas showed the best predictive performance;moreover,we observed new brain regions of interest associated with T2DM-related cognition.The area under the receiver operating characteristic curve values(T2DM-NC group vs.T2DM-C group)were 0.65-0.70,with LSFC matrices based on HOA-112 and Power-264 atlases having the highest value(0.70).Most discriminative and attractive LSFCs were related to the default mode network,limbic system,and basal ganglia.CONCLUSION LSFC provides neuroimaging-based information that may be useful in detecting MCI early and accurately in patients with T2DM.

文摘To address the problem of the weak anti-noise and macro-trend extraction abilities of the current methods for identifying radar antenna scan type,a recognition method for radar antenna scan types based on limited penetrable visibility graph(LPVG)is proposed.Firstly,seven types of radar antenna scans are analyzed,which include the circular scan,sector scan,helical scan,raster scan,conical scan,electromechanical hybrid scan and two-dimensional electronic scan.Then,the time series of the pulse amplitude in the radar reconnaissance receiver is converted into an LPVG network,and the feature parameters are extracted.Finally,the recognition result is obtained by using a support vector machine(SVM)classifier.The experimental results show that the recognition accuracy and noise resistance of this new method are improved,where the average recognition accuracy for radar antenna type is at least 90%when the signalto-noise ratio(SNR)is 5 dB and above.

文摘AIM: To investigate whether assigning young, healthy and motivated lay volunteer partners("buddies") to adolescents with type 2 diabetes improves hemoglobin A1c(HbA 1c). METHODS: Adolescents with type 2 diabetes were randomized to partnering with a "buddy" or to conventional treatment. During the initial screening visit, which coincided with a routine outpatient diabetes clinic visit, patients with type 2 diabetes underwent a physical examination, detailed medical history, laboratory measurement of HbA 1c, and completed two questionnaires(Pediatric Quality of Life Inventory and Children's Depression Inventory) to assess their overall quality of life and the presence of depressive symptoms. Patients were then randomized to the intervention(the buddy system) or conventional treatment(standard care). All patients were scheduled to return for followup at 3- and 6-mo after their initial visit. Hb A1 c was determined at all visits(i.e., at screening and at the 3- and 6-mo follow-up visits) and quality of life and depressive symptoms were evaluated at the screening visit and were reassessed at the 6-mo visit. RESULTS: Ten adolescents, recruited from a pool of approximately 200 adolescents, enrolled over a twoyear time period, leading to premature termination of the study. In contrast, we easily recruited motivated lay volunteers. We found no change in HbA 1c from the initial to the 6-mo visit in either group, yet our small sample size limited systematic assessment of this outcome. Participants repeatedly missed clinic appointments, failed to conduct self-glucose-monitoring and rarely brought their glucometers to clinic visits. Total quality of life scores(72.6 ± 6.06) at screening were similar to previously reported scores in adolescents with type 2 diabetes(75.7 ± 15.0) and lower than scores reported in normal-weight(81.2 ± 0.9), overweight(83.5 ± 1.8), and obese youths without diabetes(78.5 ± 1.8) or in adolescents with type 1 diabetes(80.5 ± 13.1). Among adolescents who returned for their 6-mo visit, there were no differences in total quality of life scores(70.2 ± 9.18) between screening and follow-up.CONCLUSION: Our approach, effective in adults with type 2 diabetes, was unsuccessful among adolescents and emphasizes the need for innovative strategies for diabetes treatment in adolescent patients.
文摘Patients with type 4 gastric cancer and peritoneal metastasis respond better to chemotherapy than surgery. In particular, patients without gastric stenosis who can consume a meal usually experience better quality of life (QOL). However, some patients with unsuccessful chemotherapy are unable to consume a meal because of gastric stenosis and obstruction. These patients ultimately require salvage surgery to enable them to consume food normally. We evaluated the outcomes of salvage total gastrectomy after chemotherapy in four patients with gastric stenosis. We determined clinical outcomes of four patients who underwent total gastrectomy as salvage surgery. Outcomes were time from chemotherapy to death and QOL, which was assessed using the Support Team Assessment Schedule-Japanese version (STAS-J). Three of the patients received combination chemotherapy [tegafur, gimestat and otastat potassium (TS-1); cisplatin]. Two of these patients underwent salvage chemotherapy after 12 and 4 mo of chemotherapy. Following surgery, they could consume food adequately and their STAS-J scores improved, so their treatments were continued. The third patient underwent salvage surgery after 7 mo of chemotherapy. This patient was unable to consume food adequately after surgery and developed surgical complications. His clinical outcomes at 3 mo were very poor. The fourth patient received combination chemotherapy (TS-1 and irinotecan hydrochloride) for 6 mo and then underwent received salvage surgery. After surgery, he could consume food adequately and his STAS-J score improved, so his treatment was continued. After the surgery, he enjoyed his life for 16 mo. Of four patients who received salvage total gastrectomy after unsuccessful chemotherapy, the QOL improved in three patients, but not in the other patient. Salvage surgery improves QOL in most patients, but some patients develop surgical complications that prevent improvements in QOL. If salvage surgery is indicated, the surgeon and/or oncologist must provide the patient with a clear explanation of the purpose of surgery, as well as the possible risks and benefits to allow the patient to reach an informed decision on whether to consent to the procedure.
文摘<strong>Background:</strong> Diabetes mellitus (DM) is one of the most common non-communicable diseases and among the leading causes of disability, morbidity and mortality globally. The study assessed adherence to treatment by type 2 DM patients aged 20 years and above at Monze Mission Hospital in Monze district, Zambia. Research questions: 1) What is the level of adherence to treatment by type 2 DM patients receiving care from Monze Mission Hospital? 2) What factors influence adherence to treatment by type 2 DM patients? <strong>Methods:</strong> The study was conducted in 2017. It was a cross-sectional design. Simple random sampling method was used to select respondents and data was collected using a structured interview schedule. Data was entered and analyzed using the modified self-reported Morisky Medication Adherence Scale with six scores, modified self-care management questionnaire and IBM<span style="white-space:nowrap;"><sup><span style="white-space:nowrap;"><span style="white-space:nowrap;">©</span></span></sup> </span>Statistical Package for Social Sciences. Chi-Square was used to test associations between variables and binary logistic regression was used for multivariate analysis. The Morisky Medication Adherence scale was interpreted as follows: patients who scored between 4 - 6 points on knowledge had high knowledge while those who scored between 1 - 3 points had low knowledge about the disease. <strong>Results: </strong>The sample size of the study was 138 respondents. The study showed, only 44.2% of respondents had good adherence to treatment as they scored above 4 using the Morisky Medication Adherence Scale. Majority of the patients (55.6%) had knowledge about treatment despite 65.2% of respondents reported distance and financial challenges as hindrances to adherence. The study revealed a statistical association between adherence and knowledge of type 2 DM treatment. <strong>Conclusion:</strong> The study showed that poor adherence to treatment reduced as knowledge about DM increased. There is need to consider educational programs to strengthen adherence to dietary advice, regular exercise and follow up, to achieve normal glycemic levels.
文摘A control problem containing support functions in the integrand of the objective of the functional as well as in the inequality constraint function is considered. For this problem, Fritz John and Karush-Kuhn-Tucker type necessary optimality conditions are derived. Using Karush-Kuhn-Tucker type optimality conditions, Wolfe type dual is formulated and usual duality theorems are established under generalized convexity conditions. Special cases are generated. It is also shown that our duality results have linkage with those of nonlinear programming problems involving support functions.
文摘BACKGROUND In transradial intervention procedures,poor back-up support and noncoaxial alignment of the guide catheter(GC)may result in failure of the balloon or stent to reach the targeted lesion.Methods to provide extra back-up support using the original GC and wire can improve procedural success with reduced complications.A rapid exchange guide extension catheter provides convenient and efficient back-up support while preserving the initial GC and inserted wire.AIM To evaluate the efficacy and safety of rapid exchange extension catheter in the treatment of type B2/C nonocclusive coronary lesions via the radial access.METHODS A total of 135 patients with type B2/C nonocclusive lesions who were treated via the transradial approach were enrolled in the study.The clinical characteristics,indications for use of the rapid exchange extension catheter,and procedural details and results were reviewed and analyzed.All procedure-related complications and major adverse cardiovascular events were recorded during the in-hospital stay and follow-up period.RESULTS The most common indication for the use of a rapid exchange extension catheter was vascular tortuosity(37.8%),followed by heavy calcification(28.9%),long lesions(20.0%),proximal stent(6.7%),in-stent restenosis(5.2%),and coronary origin anomalies(1.5%).The following technologies failed in passing targeted lesions before delivering the rapid exchange catheter:Multiple predilatation technique(57%),buddy wire technique(33.4%),balloon anchoring technique (5.9%), and cutting balloon modification (3.7%). The mean depth of the extensioncatheter intubation was 20.56 ± 13.05 mm, and the mean rapid exchange catheterservice time was 18.9 ± 9.7 min. The mean length and diameter of stents were 33.5± 14.4 mm and 2.75 ± 0.45 mm, respectively. The total rate of technique success(balloon or stent successful crossing of the target lesion with this technique) was94.8%.CONCLUSIONThe rapid exchange extension catheter technique showed acceptable safety andefficacy in the transradial coronary interventions of type B2/C nonocclusivecoronary lesions. We recommend this technique to assist in complex lesionintervention via radial access.
文摘The regularities of the solid solutions between the scheelite-type compounds and rare earth molybdates or tungstates were investigated by the atomic parameter-support vector machine method and the intelligent database of phase diagrams of molten salt systems. The crystal structure of scheelite-type compounds having M^1M^′Ⅲ (XO4)2(X = Mo, W) as common formula and the formability of the continuous solid solution between these compounds and rare earth molybdates or tungstates were also investigated. Besides, the cell constants of these compounds can be calculated by some semi-empirical equations. Based on the obtained relationships, the results of computerized prediction of the solid solubility of T1Pr (MoO4)2-Pr2 (MoO4)3 system have good agreement with experimental results.
