Effects of health concept model-based detailed behavioral care on mood and quality of life in elderly patients with chronic heart failure

BACKGROUND With the intensification of social aging,the susceptibility of the elderly population to diseases has attracted increasing attention,especially chronic heart failure(CHF)that accounts for a large proportion of the elderly.AIM To evaluate the application value of health concept model-based detailed behavioral care in elderly patients with CHF.METHODS This study recruited 116 elderly CHF patients admitted from October 2018 to October 2020 and grouped them according to the nursing care that they received.The elderly patients who underwent health concept model-based detailed behavioral care were included in a study group(SG;n=62),and those who underwent routine detailed behavioral nursing intervention were included as a control group(CG;n=54).Patients’negative emotions(NEs),quality of life(QoL),and nutritional status were assessed using the self-rating anxiety/depression scale(SAS/SDS),the Minnesota Living with Heart Failure Questionnaire(MLHFQ),and the Modified Quantitative Subjective Global Assessment(MQSGA)of nutrition,respectively.Differences in rehabilitation efficiency,NEs,cardiac function(CF)indexes,nutritional status,QoL,and nursing satisfaction were comparatively analyzed.RESULTS A higher response rate was recorded in the SG vs the CG after intervention(P<0.05).After care,the left ventricular ejection fraction was higher while the left ventricular end-diastolic dimension and left ventricular end systolic diameter were lower in the SG compared with the CG(P<0.05).The post-intervention SAS and SDS scores,as well as MQSGA and MLHFQ scores,were also lower in the SG(P<0.05).The SG was also superior to the CG in the overall nursing satisfaction rate(P<0.05).CONCLUSION Health concept model-based detailed behavioral care has high application value in the nursing care of elderly CHF patients,and it can not only effectively enhance rehabilitation efficiency,but also mitigate patients’NEs and improve their CF and QoL. 展开更多

Information was a frequently used concept in many fields of investigation. However, this concept is still not really understood, when it is referred for instance to consciousness and its informational structure. In this paper it is followed the concept of information from philosophical to physics perspective, showing especially how this concept could be extended to matter in general and to the living in particular, as a result of the intimate interaction between matter and information, the human body appearing as a bipolar informed-matter structure. It is detailed on this way how this concept could be referred to consciousness, and an informational modeling of consciousness as an informational system of the human body is presented. Based on the anatomic architecture of the organism and on the inference of the specific information concepts, it is shown that the informational system of the human body could be described by seven informational subsystems, which are reflected in consciousness as corresponding cognitive centers. These results are able to explain the main properties of consciousness, both the cognitive and extra-cognitive properties of the mind, like that observed during the near-death experiences and other similar phenomena. Moreover, the results of such a modeling are compared with the existing empirical concepts and models on the energetic architecture of the organism, showing their relevance for the understanding of consciousness. 展开更多

On the long and well-worn road of many, but justifiable attempts of human to discover his origin, his trajectory as a species, and a suitable understanding consciousness, his system allowing the connection to the environment and to his own organism, the concepts and models of philosophy enunciated or experienced by millennia, meet today with modern science concepts of physics and of science of information. Based on recent discoveries of quantum physics and astrophysics, revealing a new understanding of our environment and starting from some philosophical concepts on information of matter and of living structures, this work discusses the dynamics of information within the frame of the Informational Model of Consciousness as an informational system of the human body, connected both to the environment and to the body itself, to control the adaptation for survival. It is shown that consciousness is actually an informational projection in the mind of seven informational subsystems, three of which forming the operative system of consciousness for the short-term adaptation, and other three forming the programmed operating system, dedicated to the maintenance of body and to the long-term survival of species, showing various inputs and outputs of information. The seventh subsystem is the information pole, connecting the organism with the external information, especially related to the extra-sensorial properties of the mind, the human body appearing as a bipolar info-matter structure, managed by the brain. The received information is progressively integrated into the informational system of the organism, which absorbs and emanates information as a reactive system for adaptation, able to operate both with matter-related (codified) and non-matter related (virtual) information. As both connections with external and internal environment (body itself) can be described in terms of information, this model opens the gate to investigate consciousness by means of the tools of the information science, offering also answers to the philosophic “mind-body” problem and to the “hard” problem and showing correspondences with some ancient philosophies. 展开更多

Mesoscopic characteristics of a clayey soil specimen subjected to macroscopic loading are examined using a medi- cal-use computerized tomography (CT) instrument. Disturbed state concept (DSC) theory is based on the utilization of the hard- ening model. DSC indirectly describes material behavior by claiming that the actual response of the material is expressed in terms of the relative intact (RI) response and the fully adjusted (FA) response. The occurrence of mesoscopic structural changes of material has similarities with the occurrence of a macroscopic response of the material under loadings. In general, the relative changing value of a softening material is three to five times more than that of a hardening material. Whether special zones exist or not in a specimen cross section does not affect the following conclusion: hardening material and softening material show me- chanical differences with CT statistical indices values prominently changing, and the change is related to the superposing of a disturbance factor. A new disturbance factor evolution function is proposed. Thus, mesoscopic statistical indices are introduced to describe macroscopic behavior through the new evolution function. An application of the new evolution function proves the effectiveness of the amalgamation of a macroscopic and a mesoscopic experimental phenomenon measurement methods. 展开更多

This paper presents a description of the DSC, andstatements for determination of parameters based on triaxial, multiaxial and interface tests. The headland-bay beach is one of the most common coastal types in the world.Its morphology reflects the changes that occurred during long-term evolution of the sandy coast.Several headland-bay beach models have been proposed to simulate the coastline’s configuration in equilibrium.In this paper,a new elliptical model is proposed,described,and applied.On the east coast of Laizhou Bay in Shandong Province from Longkou Port to Diaolongzui,four typical headland-bay beaches have developed,and four headland-bay beach models are used in this paper to simulate the morphology of these beaches to assess the applicability of each model.The simulation results of the elliptical model verify that it is applicable to the study area.In addition,the elliptical model is easy to use.Through simulation and field investigations,we concluded that most of the coastal segments in this area will remain in an erosion state,and the human activity has a significant impact on the shoreline’s evolution. 展开更多

To better evaluate the three-dimensional bay health and predict the dynamic bay health conditions, a concept of numerical bay health was introduced and a method of numerical bay health evaluation(NBHE) was developed.To support the NBHE method, a numerical bay health index(NBHI) system was constructed, which assess the natural and socio-economic effects on the entire bay. Five index groups are combined to formulate the NBHI,including geometry, hydrodynamics and sediment dynamics, bio-ecology, water quality and socio-economy.Each group has different number of indices selected and weighted using AHP method according to their importance. Data were mainly synthesized from a variety of numerical models together with monitoring programs, which provide superior to other approaches in discriminating data integrity and predicting data in future. The NBHE method using NBHI system was applied in the Yueqing Bay during spring tide in April 2007.According to the NBHE results, Sta. A, at the surface level of the estuarine mouth, has a healthy geometry condition, sub-healthy hydrodynamic and sediment dynamic condition, and unhealthy water quality and bioecology conditions. The integrated healthy score at Sta. A indicates its sub-healthy condition. 展开更多

目的探讨基于OBE理念的Mini-CEX评估模式在神经内科临床实习教学中的应用实效。方法选择2022年5月1日至2023年4月30日在神经内科实习的100名学生作为研究对象,将其分成实验组(n=50)和对照组(n=50)。实验组在OBE理念指导下,以学习成果为... 目的探讨基于OBE理念的Mini-CEX评估模式在神经内科临床实习教学中的应用实效。方法选择2022年5月1日至2023年4月30日在神经内科实习的100名学生作为研究对象,将其分成实验组(n=50)和对照组(n=50)。实验组在OBE理念指导下,以学习成果为导向,细化教学目标,并应用Mini-CEX评估模式进行测评和反馈,对照组采用与实验组相同的评估方式,并按照传统教学流程进行。结合观察数据,分析对比2组学生实习开始时和实习结束时的各项指标数据。结果实习结束时,实验组临床接诊、体格检查、人文医学、临床诊断、健康咨询、组织效果、整体评价等评分得到明显提升,且高于对照组P值;实习结束后,技能考试成绩方面,实验组得分高于对照组,差异有统计学意义(P<0.05),而且实验组满意度评价得分也比对照组更高。结论基于OBE理念的Mini-CEX评估教学模式应用于神经内科临床实习教学,可以增强学生临床实践技能培育效果。 展开更多

当前,为了释放公共数据价值、培育数据要素市场,公共数据授权运营成为重要的破局之策。然而,公共数据授权运营立法远落后于实践需求,是否立法以及如何立法仍成为悬而未决的难题。在立法条件方面,理论依据的充分、经济条件的具备为授权... 当前,为了释放公共数据价值、培育数据要素市场,公共数据授权运营成为重要的破局之策。然而,公共数据授权运营立法远落后于实践需求,是否立法以及如何立法仍成为悬而未决的难题。在立法条件方面,理论依据的充分、经济条件的具备为授权运营立法提供了可行性,而数据增值、企业利用、政府治理、国家愿景等需求,均证实了立法的必要性。在立法模式方面,权衡中央立法模式和地方立法模式的利弊,应推行“地方先行先试”的立法模式。在具体制度实现方面,地方立法应秉持效率与安全并重的立法理念,采用实用性立法结构,授权主体内容应设计为统一授权模式,运营主体内容应将基本业务能力和安全保障能力设置为准入条件,以法定情形和约定情形设置退出条件,授权运营的公共数据内容应遵循以混合分类法分类、以敏感程度分级的标准,安全监督与管理内容应以安全和效益为监督考核标准。 展开更多
