Total RNA was extracted from the...A full-length cDNA library from the testis of dark-spotted frogs ( Rana nigromaculata ) was constructed with the SMART (switching mechanism at 5' end of RNA transcript) technique. Total RNA was extracted from the testis and reverse transcripted into full-length cDNA using PowerScript reverse transcriptase. The first-strand cDNA was amplified using long-distance PCR (LD-PCR). After Sfi Ⅰ digestion and fractionation, cDNA ( 〉 500 bp) was ligated to λ TriplEx2 vector and packaged with GigapackⅢ Gold Packaging Extract. The titers of optimal primary libraries were 2.0×10^6 pfu/mL and 2.4 × 10^6 pfu/mL and the tlters of the amplified libraries were 0.48 × 10^9 pfu/mL and 3.0 × 10^9 pfu/ mL, respectively. The percentages of recombinant clones of primary libraries and amplified libraries were all over 90%. The libraries were converted into pTriplEx2 plasmids in E. coli BM 25.8 strain. The insert sizes were measured by PCR which showed most fragments were over 500 bp and the average length was 1.0 kb approximately. A positive clone of 1 171 bp was sequenced and named RnUb based on sequence similarity with the known ubiquitin genes in Gen- Bank. This sequence was a full-length cDNA with complete coding sequences, which indicated that the library built a base for screening the full-length cDNA. These data showed that this library attained to the requirements of a standard cDNA library. This library provided a useful resource for the functional genomic research of Rana nigromaculata.展开更多

METHODS: From January 2001 to October 2003, 134 patients with choledocholithiasis were assigned to either endocut mode group or conventional blended cut mode group at the time of sphincterotomy. The two groups were retrospectively compared for the complications after EST and serum amylase level before and 24 h after the procedure. RESULTS: Of the 134 patients treated, 79 were assigned to conventional blended cut mode group and 55 to endocut mode group. There was no significant difference in age, sex, and serum amylase level before EST between the two groups. Complications were found in 5 patients of the endocut mode group (9%): hyperamylasemia (5 times higher than normal) in 4 and moderate pancreatitis in 1. Complications were found in 13 patients of the conventional blended cut mode group (16%): hyperamylasemia in 12 and moderate pancreatitis in 1. Serum amylase levels were elevated in both groups 24 h after EST (P〈0.02). The average serum amylase level 24 h after EST in the conventional blended cut mode group was significantly higher than that in the endocut mode group (P〈 0.05). CONCLUSION: Endocut mode offers a safety advantage over conventional blended cut mode for pancreatitis after EST by reducing hyperamylasemia.展开更多

Choledocholithiasis develops in about 10%-20% of patients with gallbladder stones and the literature suggests that at least 3%-10% of patients undergoing cholecystectomy will have common bile duct (CBD) stones. CBD stones may be discovered preoperatively, intraoperatively or postoperatively Multiple modalities are available for assessing patients for choledocholithiasis including laboratory tests, ultrasound, computed tomography scans (CT), and magnetic resonance cholangiopancreatography (MRCP). Intraoperative cholangiography during cholecystectomy can be used routinely or selectively to diagnose CBD stones. The most common intervention for CBD stones is ERCP. Other commonly used interventions include intraoperative bile duct exploration, either laparoscopic or open. Percutaneous, transhepatic stone removal other novel techniques of biliary clearance have been devised. The availability, of equipment and skilled practitioners who are facile with these techniques varies among institutions. The aim of this study was to analyse its age-related prognostic value. Methods SYNTAX-score was calculated in 1331 all-comer patients undergoing percutaneous coronary intervention (PCI): 463 patients ≥ 75 years and 868 patients 〈 75 years. Outcomes of interest were all-cause mortality at one and two years. Results A significant interaction of age and SYNTAX-score for mortality was observed at two-year (Pinteraction= 0.019) but not at one-year follow-up (Pinteraction= 0.594). In multivariable analysis, SYNTAX-score independently predicted 1-year mortality in both age groups (〈 75 years, hazard ratio (HR): 1.43, 95% confidence intervals (CI): 1.03-2.00, P = 0.034; and 〉 75 years, HR: 1.37, 95% CI: 1.01-1.85, P = 0.042), but only two-year mortality among younger patients (〈 75 years, HR: 1.33, 95% CI: 1.01-1.76, P = 0.041; and ≥ 75 years, HR: 1.11, 95% CI: 0.87-1.41, P = 0.394). SYNTAX-score tertiles were useful to stratify 1-year mortality in both, patients 〈 75 years (SYNTAX-score 〈 9, 3.8%; 9-20, 5.3%; 〉 20, 10.3%; P = 0.004) and 〉 75 years (SYNTAX-score 〈 11, 5.7%; 11-22.5, 16.1%; 〉 22.5, 18.7%; P = 0.003), but two-year mortality only among patients 〈 75 years (SYNTAX-score 〈 9, 6.5%; 9-20, 7.6%; ≥ 20, 15%; P 〈 0.001) and not among ≥ 75 years old patients (SYNTAX-score 〈 11, 19.4%; 11-22.5, 26.3%; _〉 22.5, 27.9%; P = 0.138). Conclusions Age modi- fies the impact of the SYNTAX-score on longer-term mortality after PCI. Among patients 〈 75 years, the SYNTAX-score independently predicts the risk of death at one and two years after PCI, while among patients 〉 75 years its predictive role is limited to the first year after PCI. However, it is impossible to retrieve oversized stones because the conventional lithotripsy basket may not be able to grasp the stone. However, there is no es- tablished endoscopic extraction method for such gi- ant stone removal. We describe a case of successful extraction of a 4-cm large stone using a gastric bezoar basket. A 78-year-old woman had suffered from upper abdominal pain for 20 d. Contrast-enhanced computed tomogram revealed a 4-cm single stone in the distal common bile duct (CBD). Endoscopic stone retraction was decided upon and endoscopic papillary balloon dilation was performed using a large balloon. An at- tempt to capture the stone using a standard lithotripsy basket failed due to the large stone size. Subsequently, we used a gastric bezoar basket to successfully capturethe stone. The stone was fragmented into small pieces and extracted. The stone was completely removed after two sessions of endoscopic retrograde cholangio- pancreatography; each of which took 30 rain. No com- plications occurred during or after the procedure. The patient was fully recovered and discharged on day 11 of hospitalization. ML using a gastric bezoar basket is a safe and effective retrieval method in select cases, and is considered as an alternative nonoperative option for the management of difficult CBD stones.展开更多

METHODS:Two hundred ...AIM:To evaluate the role of endoscopic retrograde cholangiopancreatography (ERCP) for choledocholithiasis in patients with sickle cell anemia (SCA) in the era of laparoscopic cholecystectomy (LC). METHODS:Two hundred and twenty four patients (144 male,80 female; mean age,22.4 years; range,5-70 years) with SCA underwent ERCP as part of their evaluation for cholestatic jaundice (CJ). The indications for ERCP were:CJ only in 97,CJ and dilated bile ducts on ultrasound in 103,and CJ and common bile duct (CBD) stones on ultrasound in 42. RESULTS:In total,CBD stones were found in 88 (39.3%) patients and there was evidence of recent stone passage in 16. Fifteen were post-LC patients. These had endoscopic sphincterotomy and stone extraction. The remaining 73 had endoscopic sphincterotomy and stone extraction followed by LC without an intraoperative cholangiogram.CONCLUSION:In patients with SCA and cholelithiasis,ERCP is valuable whether preoperative or postoperative,and in none was there a need to perform intraoperative cholangiography. Sequential endoscopic sphincterotomy and stone extraction followed by LC is beneficial in these patients. Endoscopic sphincterotomy may also prove to be useful in these patients as it may prevent the future development of biliary sludge and bile duct stones.展开更多

The procedure...AIM: To analyze the indications, efficacy and safety of sphincteroplasty in our centre.METHODS: A retrospective study of sphincteroplasty in 53 cases of papilla at high risk was performed in 2004-2006. The procedure consisted of duodenoscopy with Olympus TJF 145 Videoduodenoscope, approach to the biliary tract using a catheter with a guidewire, and dilatation of the papilla with a dilatation balloon catheter using a syringe with a manometer for control of the filling pressure.RESULTS: The indications included intradiverticular papilla in 26 patients (49%), stenosis of a previous sphincterotomy in 19 patients (35.8%), small size of the papilla in 4 patients (7.5%), Billroth R gastrectomy in 3 patients (5.6%), and coagulopathy in one patient (1.9%). The efficacy was 97.8%, with all the calculi extracted from the common bile duct in 84.4% of the patients, even though 21 of the patients (39.6%) had calculi with a diameter equal to or greater than 10 ram. Seven patients (13.2%) presented complications: haemorrhage in 1 patient (1.9%) and mild pancreatitis in 6 patients (11.3%). The mean hospital stay in case of complications was of 3 ± 0.63 d.CONCLUSION: Sphincteroplasty is highly effective, with a compllcation rate similar to that of sphincterotomy, furthermore, the complications are of low clinical importance. The use of the 10 mm balloon makes it possible to extract calculi with a diameter of over 15 mm and to extract more than 3 calculi without increasing the rate of complications and reduces the need to resort to lithotripsy or rescue sphincterotomy.展开更多

In this paper, the process of IT project decision and implementation is fully analyzed, the real options that may be embedded in an IT project are identified, and a real option analysis (ROA) method is proposed for evaluation of an IT project under uncertain business environment. ROA employs Black-Scholes expansion model and cancels the assumption that the cost of project is certain. The numerical example manifests that the ROA can better evaluate IT project and select the IT investment alternative. Objective:To assess the diagnostic accuracy of whole-body MRI using diffusion-weighted sequence(WB-DWI)to determine the peritoneal cancer index(PCI)in correlation with surgical and histopathological findings.Meth...Objective:To assess the diagnostic accuracy of whole-body MRI using diffusion-weighted sequence(WB-DWI)to determine the peritoneal cancer index(PCI)in correlation with surgical and histopathological findings.Methods:Twenty-seven patients underwent preoperative WB-MRI,followed by cytoreductive surgery for primary tumors of the appendix(n=15),colorectum(n=12),and associated peritoneal disease.A total of 351 regions were retrospectively reviewed.The sensitivity,specificity,and accuracy were calculated at 13 anatomical sites.The WB-DWI PCI and PCI type were compared with surgical and histopathological findings.Results:No statistical difference was found between the WB-DWI PCI and surgical PCI(P=0.574).WB-DWI correctly predicted the PCI type in 24 of 27 patients with high accuracy(88.9%),including 10 of 10 patients with small-volume tumor,12 of 14 with moderate-volume tumor,and 2 of 3 with large-volume tumor.WB-DWI correctly depicted tumors in 163 of 203 regions,with 40false-negative and 23 false-positive regions.The overall sensitivity,specificity,and accuracy of WB-DWI for the detection of peritoneal tumors were 80.3%,84.5%,and 82.1%,respectively.For lesions<0.5 cm in diameter,WB-DWI demonstrated good sensitivity(69.4%).Conclusions:WB-DWI accurately predicted PCI before surgery in patients undergoing evaluation for cytoreductive surgery.展开更多

Using the approach, the designers may find the minimum of some user-defined objective functions under constrains. An example of the approach application for a surface combatant hull optimization is demonstrated. In the procedure, the Particle Swarm Optimization (PSO) algorithm is adopted for exploring the design space, and the Bezier patch method is chosen to automatically modify the geometry of bulb. The total resistance is assessed by RANS solvers. It's shown that the total resistance coefficient of the optimized design is reduced by about 6.6% comparing with the original design. The given combatant design optimization example demonstrates the practicability and superiority of the proposed approach for low carbon shipping.展开更多

Developed from China's nuclear R&D system and manufacturing capability, as well as APIO00 technology introduction and assimilation, CAP1400 is an advanced large passive nuclear power plant with independent intellectual property rights. By discussing the top design principle, main performance objectives, general parameters, safety design, and important improvements in safety, economy, and oth- er advanced features, this paper reveals the technology innovation and competitiveness of CAP1400 as an internationally promising Gen-IIl PWR model. Moreover, the R&D of CAP1400 has greatly promoted China's domestic nuclear power industry from the Gen-II to the Gen-III level.
文摘This paper presents a design scheme of intelligent transportation system based on the Internet of things. First, the paper elaborated the related technical and functional demand of intelligent traffic system, designed the gateway level model and the overall project. Then, we design gateway hardware circuit according to the overall plan, and design the gateway application software according to the functional requirements. Through the experiment and simulation results show that, the intelligent transportation system gateway based on Internet of things is ability to create ZigBee network through the way of wireless access, GPRS network, Ethernet access based on the wired way, to realizes multimode access, multi-protocol conversion gateway, ad hoc network functions.
文摘AIM:To evaluate the role of endoscopic retrograde cholangiopancreatography (ERCP) for choledocholithiasis in patients with sickle cell anemia (SCA) in the era of laparoscopic cholecystectomy (LC). METHODS:Two hundred and twenty four patients (144 male,80 female; mean age,22.4 years; range,5-70 years) with SCA underwent ERCP as part of their evaluation for cholestatic jaundice (CJ). The indications for ERCP were:CJ only in 97,CJ and dilated bile ducts on ultrasound in 103,and CJ and common bile duct (CBD) stones on ultrasound in 42. RESULTS:In total,CBD stones were found in 88 (39.3%) patients and there was evidence of recent stone passage in 16. Fifteen were post-LC patients. These had endoscopic sphincterotomy and stone extraction. The remaining 73 had endoscopic sphincterotomy and stone extraction followed by LC without an intraoperative cholangiogram.CONCLUSION:In patients with SCA and cholelithiasis,ERCP is valuable whether preoperative or postoperative,and in none was there a need to perform intraoperative cholangiography. Sequential endoscopic sphincterotomy and stone extraction followed by LC is beneficial in these patients. Endoscopic sphincterotomy may also prove to be useful in these patients as it may prevent the future development of biliary sludge and bile duct stones.
文摘AIM: To analyze the indications, efficacy and safety of sphincteroplasty in our centre.METHODS: A retrospective study of sphincteroplasty in 53 cases of papilla at high risk was performed in 2004-2006. The procedure consisted of duodenoscopy with Olympus TJF 145 Videoduodenoscope, approach to the biliary tract using a catheter with a guidewire, and dilatation of the papilla with a dilatation balloon catheter using a syringe with a manometer for control of the filling pressure.RESULTS: The indications included intradiverticular papilla in 26 patients (49%), stenosis of a previous sphincterotomy in 19 patients (35.8%), small size of the papilla in 4 patients (7.5%), Billroth R gastrectomy in 3 patients (5.6%), and coagulopathy in one patient (1.9%). The efficacy was 97.8%, with all the calculi extracted from the common bile duct in 84.4% of the patients, even though 21 of the patients (39.6%) had calculi with a diameter equal to or greater than 10 ram. Seven patients (13.2%) presented complications: haemorrhage in 1 patient (1.9%) and mild pancreatitis in 6 patients (11.3%). The mean hospital stay in case of complications was of 3 ± 0.63 d.CONCLUSION: Sphincteroplasty is highly effective, with a compllcation rate similar to that of sphincterotomy, furthermore, the complications are of low clinical importance. The use of the 10 mm balloon makes it possible to extract calculi with a diameter of over 15 mm and to extract more than 3 calculi without increasing the rate of complications and reduces the need to resort to lithotripsy or rescue sphincterotomy.
文摘There are many kinds of real options,which are valuable,in each phase of the lifetime of an information technology(IT)project.However,in the current IT investment decision theory,real options that embedded in IT projects are not considered. In this paper, the process of IT project decision and implementation is fully analyzed, the real options that may be embedded in an IT project are identified, and a real option analysis (ROA) method is proposed for evaluation of an IT project under uncertain business environment. ROA employs Black-Scholes expansion model and cancels the assumption that the cost of project is certain. The numerical example manifests that the ROA can better evaluate IT project and select the IT investment alternative. Finally, a road map is provided to help selecting the suitable evaluation method to make IT investment decision.

文摘Objective:To assess the diagnostic accuracy of whole-body MRI using diffusion-weighted sequence(WB-DWI)to determine the peritoneal cancer index(PCI)in correlation with surgical and histopathological findings.Methods:Twenty-seven patients underwent preoperative WB-MRI,followed by cytoreductive surgery for primary tumors of the appendix(n=15),colorectum(n=12),and associated peritoneal disease.A total of 351 regions were retrospectively reviewed.The sensitivity,specificity,and accuracy were calculated at 13 anatomical sites.The WB-DWI PCI and PCI type were compared with surgical and histopathological findings.Results:No statistical difference was found between the WB-DWI PCI and surgical PCI(P=0.574).WB-DWI correctly predicted the PCI type in 24 of 27 patients with high accuracy(88.9%),including 10 of 10 patients with small-volume tumor,12 of 14 with moderate-volume tumor,and 2 of 3 with large-volume tumor.WB-DWI correctly depicted tumors in 163 of 203 regions,with 40false-negative and 23 false-positive regions.The overall sensitivity,specificity,and accuracy of WB-DWI for the detection of peritoneal tumors were 80.3%,84.5%,and 82.1%,respectively.For lesions<0.5 cm in diameter,WB-DWI demonstrated good sensitivity(69.4%).Conclusions:WB-DWI accurately predicted PCI before surgery in patients undergoing evaluation for cytoreductive surgery.
文摘Combining modem Computational Fluid Dynamics (CFD) evaluator with optimization method, a new approach of hullform design for low carbon shipping is presented. Using the approach, the designers may find the minimum of some user-defined objective functions under constrains. An example of the approach application for a surface combatant hull optimization is demonstrated. In the procedure, the Particle Swarm Optimization (PSO) algorithm is adopted for exploring the design space, and the Bezier patch method is chosen to automatically modify the geometry of bulb. The total resistance is assessed by RANS solvers. It's shown that the total resistance coefficient of the optimized design is reduced by about 6.6% comparing with the original design. The given combatant design optimization example demonstrates the practicability and superiority of the proposed approach for low carbon shipping.
文摘The economy of the equestrian sector encompasses all the activities related to the equine world. All activities revolving around the use of a horse as entertainment, sport or business play a role in the "horse industry". These activities take place due to the existence of people demanding horses and a variety of goods and services associated with them Previous studies reveal that as income per capita becomes larger, the equestrian demand increases and, consequently, the global expenditure incurred by those individuals interested in horses. More than 500,000 horse, 9,000 breeders and 50,000 equestrian businesses, show the economic importance of the equestrian sector in Spain. That part of global expenditure that adds to the GDP of a country is what is generally defined as economic impact of the sector. Its relation will depend mostly on: the percentage of the expenditure that adds to national production and the participation in the country's global expenditure of those domestic goods and services associated with the equestrian demand. The activities related to sports are those with most economic impact, about 8,000 ∈ per horse, measured as total expenditure or with respect to the employment that they generate in the economy.
