METHODS: Using a prospective database, the patients undergoing fully robotic surgery for biliary malignan- cies between January 2009 and January 2011 were in- cluded. Records of patients with confirmed malignancy were reviewed for clinicopathological data and informa- tion about PSM. RESULTS: Sixty-four patients with biliary tract cancers underwent robotic surgery, and sixty patients met the inclusion criteria. The median age was 67 year (range: 40-85 year). During a median 15-mo follow-up period, two female patients were detected solitary PSM after robotic surgery. The incidence of PSM was 3.3%. Pa- tient 1 underwent robotic anatomatic left hemihepa- tectomy and extraction of biliary tumor thrombi for an Klatskin tumor. She had a subcutaneous mass located at the right lateral abdominal wall near a trocar scar. Patient 2 underwent robotic pancreaticoduodenectomy for distal biliary cancer. She had two metachronous subcutaneous mass situated at the right lateral abdomi- nal wall under a same trocar scar at 7 and 26 mo. The pathology of the excised PSM masses confirmed meta- static biliary adenocarcinoma. CONCLUSION: The incidence of PSIVls after robotic surgery for biliary malignancies is relatively low, and biliary cancer can be an indication of robotic surgery.展开更多

An intuitive picture or mental image of reality from sense experience is apparently structured from orthogonal and independent sense inputs in association with past memories. Such images are not necessarily visual but may have such qualities. Play of mind with information in mental images processed by molecular actors is guided with rules of reality to assure non-contradictory outcome of actions for successful behaviors (Rogers and Jain 1993). Such functions have evolved to find food before becoming food, and fight or flight when in doubt. Shadows of ignorance that obscure mental images and associated states also have epistemic utility. Their success depends on learning from trials and errors to compensate for incomplete information and uncertainties with consideration of what could go wrong. Outcomes are also influenced by games played on mind by illusions, mistakes, surprises, ignorance. Loss of time, energy, and opportunity associated with ignored or misinterpreted information threaten survival. Equivocation, cynicism, wild goose chase, and vicious circle of sterile ideas encourage contradictory or inconsistent interpretations that compromise outcomes. Throughout the human history, ignorance of horsemen of apocalypse has unleashed havocs, perpetuated wars, epidemics, wrongful medical treatments, and economic disasters. What used to be crisis of ignorance has now become crisis of unintended consequences of inventions and other forms of knowledge. Ignorance of experts and head of states as horsemen of darkness brings misery to countless innocents. In Nay formalism, knowledge (gyan) is what knows with certainty (either as true or false). Identifying uncertainties introduces doubt (syad) in what one knows. For finding ones way around lack of relevant evidence (agyan), wisdom lies in recognition of ignorance and identifies problem to seek solution. Note that these states for propositions are not necessarily related by binary negations.展开更多

Eliot's personal attachment to religious knowledge and his reflections of religious studies inevitably manifest themselves in his literary works; be it his dramas or poems, mostly in the forms of philosophical and psychoanalytical analysis of his characters and the detected problems observed in their relationships. The Cocktail Party, a play starting off as a drawing-room comedy soon converts to a serious analysis of human psychology and the nature of human interactions. While engaging with these particular points, the play offers possibilities to discuss several religious allusions, though dealt with subtly. Integrated with religion, characters' attitudes towards a given situation and their final decision position them in their quest either in "hypothetical" enlightenment or in a "supposed" repetition of a vicious circle. The enforcing power behind the play is interestingly a psychiatrist, who adds dynamism and mystery to the plot and has a special task in making the play a drama of conversion. The inner conflicts of the characters which ultimately lead to a series of problems in their relationships are the main concern which causes the characters search for different solutions. Accordingly, they make their own choices to cease their inner struggles. This paper aims to explore the concept of conversion in Eliot's The Cocktail Party with references to the playwright's religious journey in his literary career.展开更多

The effect of Mad protein to the malignant phenotype of human hepatoma BEL7404 cell line was investigated experimentally. An eukarryotic vector pCDNA Ⅲ containing full ORF fragmentof mad cDNA was transfected into targeted cells. Under G418 selection, stable Mad-overexpressed cells were cloned.Studies on the effect of Mad over-expression in cell proliferation and cell cycle revealed that cell morphology of the Mad-overexpressed BEL-7404-M1 cells was significantly different from the parent and control vector transfected cells. DNA synthesis, cell proliferation and anchorage-independent growth in soft-agar of the madtransfected cel1s were partially inhibited in comparison to control cells.Flow Cytometry analysis indicated that mad over-expression might block more transfectant cells at G0/G1 phase, resulting in the retardation of cell proliferation. RT-PCR detected a marked inhibition of the expression of cdc25A, an important regulator gene of G0/G1to S phase in cell cycle. It was also found that Mad protein overexpression could greatly suppress p53-mediated apoptosis in BEL-7404-M1 cells in the absence of serume. Thus, Mad proteins may function as a negative regulator antagonizing c-Myc activity in the control of cell growth and apoptosis in human hepatocellular carcinoma BEL7404 cells.madoverexpression and regulation of cell growth and apoptosis展开更多

Historically, the cost of collecting structural data has been very high. Data was initially collected using Benkelman Beams and now by FWD (falling weight deflectometer). The structural strength of pavements weakens over time due to environmental and traffic loading factors but due to a lack of data, no structural deterioration curve for pavements has been implemented in a PMS (pavement management system). IRI (international roughness index) is a measure of the road longitudinal profile and has been used as a proxy for a pavement’s structural integrity. This paper offers two conceptual methods to develop PSDC (pavement structural deterioration curves). Firstly, structural data are grouped in sets by design ESA (equivalent standard axles). An ISN (“initial” SN), SNI (intermediate SN) and a TSN (terminal SN), are used to develop the curves. Using FWD data, the ISN is the SN after the pavement is rehabilitated (Financial Accounting “Modern Equivalent”). Intermediate SNIs, are SNs other than the ISN and TSN. The TSN was defined as the SN of the pavement when it was approved for pavement rehabilitation. The second method is to use TSD (traffic speed deflectometer) data. The road network already divided into road blocks, is grouped by traffic loading. For each traffic loading group, road blocks that have had a recent pavement rehabilitation, are used to calculate the ISN and those planned for pavement rehabilitation to calculate the TSN. The remaining SNs are used to complete the age-based or if available, historical traffic loading-based SNIs.展开更多

The patient reported was followed up with several hospital staff when she was diagnosed with gastric cancer, but unfortunately, she had already peritoneal implants in moment of the surgery approach. It has been reported in the literature that patients with this syndrome present greater toxicity to the chemotherapeutic treatment, however due to the rarity of the disease, it is not known exactly how the adjustment of these drugs should be performed and which is the better protocol to use. Palliative chemotherapy was proposed, but after receiving one dose of the initially treatment, she developed severe and limiting toxicity.展开更多

Based on negative selection algorithm, the mature detectors are generated. By introducing clonal selection algorithm, the detectors with higher affinity are selected to undergo a proliferation and somatic hyper-mutation process, so that more excellent detector offspring can be generated. Experimental result shows that the detection model has a higher detection rate for unknown smartphone malware, and better detection performance can be achieved by increasing the clone generation.展开更多

These slums represent a fertile environment for diseases caused by pollution in all its forms and are subject to crimes, rape and theft. There is a huge difference in the state of conditions when comparing the formally planned areas to the informally planned ones. People living in the formally planned settlements, like Nasr City, and in informal ones, like Ezbet El-Haggana, are similar to the kings and queens and slaves of legends. This paper tries to find solutions to the problem of slums and random areas.展开更多

Simultaneously, cyber criminals are executing malicious actions, such as tracking user activities, stealing personal data, and committing bank fraud. These criminals gain numerous benefits as too many people use Android for their daily routines, including important communications. With this in mind, security practitioners have conducted static and dynamic analyses to identify malware. This study used static analysis because of its overall code coverage, low resource consumption, and rapid processing. However, static analysis requires a minimum number of features to efficiently classify malware. Therefore, we used genetic search(GS), which is a search based on a genetic algorithm(GA), to select the features among 106 strings. To evaluate the best features determined by GS, we used five machine learning classifiers, namely, Na?ve Bayes(NB), functional trees(FT), J48, random forest(RF), and multilayer perceptron(MLP). Among these classifiers, FT gave the highest accuracy(95%) and true positive rate(TPR)(96.7%) with the use of only six features.展开更多

His workup included a barium esophagogram that revealed a fistula between the esophagus and a right lower lobe bronchus. The diagnosis should be considered in certain individuals with suggestive symptomatology and unexplained respiratory pathology. The fistula was divided and resected, The patient had an uneventful recovery.

文摘In order to solve the problem that me traditional signature-based detection technology cannot effectively detect unknown malware, we propose in this study a smartphone malware detection model (SP-MDM) based on artificial immune system, in which static malware analysis and dynamic malware analysis techniques are combined, and antigens are generated by encoding the characteristics extracted from the malware. Based on negative selection algorithm, the mature detectors are generated. By introducing clonal selection algorithm, the detectors with higher affinity are selected to undergo a proliferation and somatic hyper-mutation process, so that more excellent detector offspring can be generated. Experimental result shows that the detection model has a higher detection rate for unknown smartphone malware, and better detection performance can be achieved by increasing the clone generation.
文摘AIM:To determine which features of history and demographics predict a diagnosis of malignancy or peptic stricture in patients presenting with dysphagia.METHODS:A prospective case-control study of 2000 consecutive referrals(1031 female, age range:17-103 years) to a rapid access service for dysphagia, based in a teaching hospital within the United Kingdom, over 7 years. The service consists of a nurse-led telephone triage followed by investigation(barium swallow or gastroscopy),if appropriate, within 2 wk.Logistic regression analysis of demographic and clinical variables was performed. This includes age, sex, duration of dysphagia, whether to liquids or solids, and whether there are associated features(reflux, odynophagia, weight loss, regurgitation). We determined odds ratio(OR) for these variables for the diagnoses of malignancy and peptic stricture. We determined the value of the Edinburgh Dysphagia Score(EDS) in predicting cancer in our cohort.Multivariate logistic regression was performed and P < 0.05 considered significant. The local ethics committee confirmed ethics approval was not required(audit).RESULTS:The commonest diagnosis is gastro-esophageal reflux disease(41.3%).Malignancy(11.0%) and peptic stricture(10.0%) were also relatively common.Malignancies were diagnosed by histology(97%) or on radiological criteria, either sequential barium swallows showing progression of disease or unequivocal evidence of malignancy on computed tomography. The majority of malignancies were esophago-gastric in origin but ear, nose and throat tumors, pancreatic cancer and extrinsic compression from lung or mediastinal metastatic cancer were also found.Malignancy was statistically more frequent in older patients(aged >73 years,OR 1.1-3.3,age < 60 years 6.5%,60-73 years 11.2%,> 73 years 11.8%,P < 0.05),males(OR 2.2-4.8,males 14.5%,females 5.6%,P < 0.0005),short duration of dysphagia(≤ 8 wk,OR 4.5-20.7,16.6%,8-26 wk 14.5%,> 26 wk 2.5%,P < 0.0005),progressive symptoms(OR 1.3-2.6:progressive 14.8%,intermittent 9.3%,P < 0.001),with weight loss of ≥ 2 kg(OR 2.5-5.1,weight loss 22.1%,without weight loss 6.4%,P < 0.0005) and without reflux(OR 1.2-2.5,reflux 7.2%,no reflux 15.5%,P < 0.0005).The likelihood of malignancy was greater in those who described true dysphagia(food or drink sticking within 5 s of swallowing than those who did not(15.1%vs 5.2% respectively,P < 0.001).The sensitivity, specificity, positive predictive value and negative predictive value of the EDS were 98.4%,9.3%,11.8% and 98.0% respectively.Three patients with an EDS of 3(high risk EDS ≥ 3.5) had malignancy.Unlike the original validation cohort, there was no difference in likelihood of malignancy based on level of dysphagia(pharyngeal level dysphagia 11.9% vs mid sternal or lower sternal dysphagia 12.4%).Peptic stricture was statistically more frequent in those with longer duration of symptoms(> 6 mo,OR 1.2-2.9,≤ 8 wk 9.8%,8-26 wk 10.6%,> 26 wk 15.7%,P < 0.05) and over 60 s(OR 1.2-3.0,age < 60 years 6.2%,60-73 years 10.2%,> 73 years 10.6%,P < 0.05).CONCLUSION:Malignancy and peptic stricture are frequent findings in those referred with dysphagia. The predictive value for associated features could help determine need for fast track investigation whilst reducing service pressures.
文摘Despite being one of the symbols of civilization throughout human history, Greater Cairo has many slum areas, hosting a population of three million people living in deteriorating conditions and lacking the basic human necessities. This paper tries to find solutions to the problem of slums and random areas.

文摘Mobile device manufacturers are rapidly producing miscellaneous Android versions worldwide. Among these classifiers, FT gave the highest accuracy(95%) and true positive rate(TPR)(96.7%) with the use of only six features.
