AIM: To evaluate the role of N-myc downstream- regulated gene 1 (NDRG1) expression in prognosis and survival of colorectal cancer patients with different ethnic backgrounds. METHODS: Because NDRG1 is a downstream ...AIM: To evaluate the role of N-myc downstream- regulated gene 1 (NDRG1) expression in prognosis and survival of colorectal cancer patients with different ethnic backgrounds. METHODS: Because NDRG1 is a downstream target of p53 and hypoxia inducible factor-1α (HIF-1α), we examined NDRG1 expression together with p53 and HIF-1α by irnmunohistochernistry. A total of 157 colorectal cancer specimens including 80 from Japanese patients and 77 from US patients were examined. The correlation between protein expression with clinicopathological features and survival after surgery was analyzed. RESULTS: NDRG1 protein was significantly increased in colorectal tumor compared with normal epithelium in both Japanese and US patient groups. Expression of NDRG1 protein was significantly correlated with lymphatic invasion, venous invasion, depth of invasion, histopathological type, and Dukes' stage in Japanese colorectal cancer patients. NDRG1 expression was correlated to histopathological type, Dukes' stage and HIF-1α expression in US-Caucasian patients but not in US-African American patients. Interestingly, Kaplan-Meier survival analysis demonstrated that NDRG1 expression correlated significantly with poorer survival in US-African American patients but not in other patient groups. However, in p53-positive US cases, NDRG1 positivity correlated significantly with better survival. In addition, NDRG1 expression also correlated significantly with improved survival in US patients with stages Ⅲ and IV tumors without chemotherapy. In Japanese patients with stages Ⅱ and Ⅲ tumors, strong NDRG1 staining in p53- positive tumors correlated significantly with improved survival but negatively in patients without chemotherapy. CONCLUSION: NDRG1 expression was correlated with various clinicopathological features and clinical outcomes in colorectal cancer depending on the race/ethnicity of the patients. NDRG1 may serve as a biological basis for the disparity of clinical outcomes of colorectal cancer patients with different ethnic backgrounds.展开更多

METHODS: Patients with reflu...AIM:To examine the clinical spectrum of gastroesophageal reflux disease (GERD) and compare erosive (ERD) with non-erosive (NERD) in terms of clinical, demographic and psychiatric profiles. METHODS: Patients with reflux symptoms were enrolled and stratified to NERD and ERD after endoscopy (LA classification). Patients with ERD presenting with dyspepsia were included. Patients on proton pump inhibitors (PPI) or H2 receptor antagonists before endoscopy were excluded. Demographic data, Helicobacter pylori(H pylori) status and presence of minor psychiatric morbidity (based on General Health Questionnaire-28) were analyzed. RESULTS: Among 690 patients screened, 533 were eligible for analysis (male to female ratio: 3:2; Chinese: 75.4%; Malay: 9.8%; Indian: 14.8%). Clinical spectrum of GERD: N: 40.5%; A: 46%; B: 9.2%; C: 2.1%; D: 0.6%; Barrett's esophagus: 1.7%. Compared to patients with NERD, patients with ERD were significantly older (45 vs 39.4 years), more likely to be male (64.4% vs 53.7%), tended to smoke (19.6% vs 9.7%), less likely to have minor psychiatric morbidity (26.4 vs 46.7%) and were more likely to respond to PPI (79.7 vs 66.8%). There was also a trend towards a higher BMI (24.5 vs 23.5). Race, alcohol consumption and H pylori status were not significant. On multivariate analysis, age and presence of minor psychiatric morbidity remained with significant differences. CONCLUSION: The majority of patients who have typical symptoms of GERD have NERD or mild erosive reflux disease. Compared to patients with erosive reflux disease, patients with NERD were younger and had a higher prevalence of minor psychiatric morbidity.展开更多

In nonCaucasian populations Crohn's disease continues to increase in incidence but this increase appears not to be a consequence of variation in NOD2, further advancing the accumulating evidence for other susceptibility loci. Frequencies of the known alleles are compared across populations in health and disease and evidence for additional alleles in NOD2 is reviewed. Based on its position on chromosome 16 coincident with some other autoimmune disease susceptibility Iocalizations, research has targeted NOD2 variation as the potential cause of other autoimmune disorders. While these investigations have mostly returned negative findings, two diseases, Blau Syndrome and Graft versus Host Disease, have been shown to be caused by risk alleles in NOD2. As is frequent in complex disease investigations, some results await validation, but the identification of NOD2 and the differences within and across population raises intriguing questions about the population genetics of the variation at this locus.展开更多
A novel Sn-2.5Ag-2.0Ni alloy was used for soldering SiCp/Al composites substrate deposited with electroless Ni(5%P) (mass fraction)and Ni(10%P)(mass fraction)layers.It is observed that variation of P contents in the e...A novel Sn-2.5Ag-2.0Ni alloy was used for soldering SiCp/Al composites substrate deposited with electroless Ni(5%P) (mass fraction)and Ni(10%P)(mass fraction)layers.It is observed that variation of P contents in the electroless Ni(P)layer results in different types of microstructures of SnAgNi/Ni(P)solder joint.The morphology of Ni3Sn4 intermetallic compounds(IMCs)formed between the solder and Ni(10%P)layer is observed to be needle-like and this shape provides high speed diffusion channels for Ni to diffuse into solder that culminates in high growth rate of Ni3Sn4.The diffusion of Ni into solder furthermore results in the formation of Kirkendall voids at the interface of Ni(P)layer and SiCp/Al composites substrate.It is observed that solder reliability is degraded by the formation of Ni2SnP,P rich Ni layer and Kirkendall voids.The compact Ni3Sn4 IMC layer in Ni(5%P)solder joint prevents Ni element from diffusing into solder,resulting in a low growth rate of Ni3Sn4 layer.Meanwhile,the formation of Ni2SnP that significantly affects the reliability of solder joints is suppressed by the low P content Ni(5%P)layer.Thus,shear strength of Ni(5%P) solder joint is concluded to be higher than that of Ni(10%P)solder joint.Growth of Ni3Sn4 IMC layer and formation of crack are accounted to be the major sources of the failure of Ni(5%P)solder joint.展开更多

METHODS: Data were obtained retrospectively from a review ...AIM: To determine the prevalence of ulcerative colitis (UC) in Malaysian patients and to establish the spectrum of the disease seen in Malaysian patients. METHODS: Data were obtained retrospectively from a review of the medical records of in- and out-patients with a diagnosis of UC at the University Hospital, Kuala Lumpur between 1985 and 1998. RESULTS: There were 45 confirmed cases of UC, of which 3 were foreigners, who were excluded from analysis. Thirty new cases of UC were diagnosed during the study period. Their mean age at presentation was 33.0±10.0 years. The highest prevalence of UC was 17.9/100 000 hospital admissions in the Indians, followed by 11.2/100 000 hospital admissions in the Chinese. The lowest prevalence was 3.7/100 000 hospital admissions in the Malays. The prevalence of UC was significantly higher in the Indians and the Chinese when compared with the Malays with an OR of 4.89 (CI = 2.02-12.24; x^2 = 15.45, P〈0.001) and 3.06 (CI = 1.24-7.78; x^2= 6.30; P= 0.012) respectively. The extent of colonic disease Was similar in the Malay and Indian patients. In contrast, distal or left-sided colitis predominated in the Chinese with an OR of 8.17 (95%CI = 1.31-64.87; x^2 = 5.53, P = 0.02). Extraintestinal manifestations were uncommon (11.9%). CONCLUSION: UC is an uncommon disease in Malaysia, but racial differences exist. The Indians had the highest prevalence of UC with the Chinese demonstrating the least extensive disease.展开更多
Ethnicity is a carrier of language and culture.Spatial distribution of ethnic diversity is fundamental for identifying and reconstructing the migration patterns and evolution histories of cultures and languages.Utiliz...Ethnicity is a carrier of language and culture.Spatial distribution of ethnic diversity is fundamental for identifying and reconstructing the migration patterns and evolution histories of cultures and languages.Utilizing the Chinese 4th National Census (1990) data,we investigated the specific time geographical patterns of population and diversity of Chinese ethnicminorities.As anticipated,results show that Chineseminorities are chiefly concentrated in dis-tant plateaus and mountains in the southwest,northwest and northeast of China.Further,population density centers of the 10 majorminorities are rather scattered,alternatively dominating at different parts of the country.This study pro-vides a first comprehensive quantitative test on a prevailing notion of 'six plates and three corridors' on the empirical clustering patterns of Chinese ethnicminorities.There are more consistent evidences supporting this notion in the north of China,with the central and southern regions showing more complex patterns,potentially transformed by processes such as migration,fragmentation,and percolation.The results of this study suggest that a geographical ap-proach can provide heuristic and complementary information for better understanding of historical social processes.展开更多

In order to cond...This paper explores Du Boisian notion of double consciousness and the notion of black nihilism suggested by Cornel West in Ralph Ellison's Invisible Man (1952) and August Wilson's Fences (1986). In order to conduct this study, the researcher adopts African American approach and shows that double consciousness and black nihilism are the immediate effects of African Americans' marginalisation. According to W. E. B. Du Bois, all African Americans develop the sense of double consciousness which means being both African and American simultaneously. On the other hand, Cornel West believes that black Americans suffer from nihilism which means loss of hope and absence of meaning. The paper is an attempt to compare the novel Invisible Man and the play Fences thematically based on these two theories. It then looks to find the considerable similarities in the conditions of the protagonists of these two literary works. Although both protagonists migrate to the North to escape Southern racism in the era prior to the Civil Rights Movement, they become totally disillusioned, for they are treated as the invisible marginal. Interestingly though, at the end of both stories, there remains a ray of hope for reactivation and the creation of democracy for black Americans.展开更多

Th...As the construction of identities is said to be within and not outside discourse, identities are thus produced in specific historical and institutional sites and within specific discursive formations and practices. These formations and practices, under which identities are produced, are explored within the Diaspora space of West Yorkshire occupied by Sudanese Muslim women. The paper interrogates Sudanese women's identities and questions how the experiences of exile and Diaspora transform women's identities and influence the gender roles to which they relate. This study is an attempt to add to migration, refugee, exile and "race" studies and the studies of Black and religious identity in the Western Diaspora space. The paper maintains that if all identities are thought to operate through exclusion, then identities are constructed through difference. Therefore, the identities of African Muslim women are constructed through the relation to the Other.展开更多
AIM: To determine rates of hepatitis C (HCV) risk factor ascertainment, testing, and referral in urban primary care practices, with particular attention to the effect of race and ethnicity. METHODS: Retrospective char...AIM: To determine rates of hepatitis C (HCV) risk factor ascertainment, testing, and referral in urban primary care practices, with particular attention to the effect of race and ethnicity. METHODS: Retrospective chart review from four primary care sites in Philadelphia; two academic primary care practices and two community clinics was performed. Demographics, HCV risk factors, and other risk exposure information were collected. RESULTS: Four thousand four hundred and seven charts were reviewed. Providers documented histories of injection drug use (IDU) and transfusion for less than 20% and 5% of patients, respectively. Only 55% of patients who admitted IDU were tested for HCV. Overall, minorities were more likely to have information regarding a risk factor documented than their white counterparts (79% vs 68%, P < 0.0001). Hispanics were less likely to have a risk factor history documented, compared to blacks and whites (P < 0.0001). Overall, minorities were less likely to be tested for HCV than whites in the presence of a known risk factor (23% vs 35%, P = 0.004). Among patients without documentation of risk factors, blacks and Hispanics were more likely to be tested than whites (20% and 24%, vs 13%, P < 0.005, respectively). CONCLUSION: (1) Documentation of an HCV risk factor history in urban primary care is uncommon, (2)Racial differences exist with respect to HCV risk factor ascertainment and testing, (3) Minority patients, positive for HCV, are less likely to be referred for subspecialty care and treatment. Overall, minorities are less likely to be tested for HCV than whites in the presence of a known risk factor.展开更多

The paper examines the development of "Negro poetry" from the Negritude movement through the analysis of Sartre as featured in Black Orpheus (1976). Following the analysis of Sartre, the author looks at the Black/...The paper examines the development of "Negro poetry" from the Negritude movement through the analysis of Sartre as featured in Black Orpheus (1976). Following the analysis of Sartre, the author looks at the Black/White binary and its cultural and political context, which is based on the dominance of Europeans over Blacks. Then the author examines how Blacks have attempted to subvert this binary through alternative political stands, such as the practice of anti-racism, and innovative cultural art and literary forms, such as the Negritude movement. Like Sartre, the author focuses on "Negro poetry" and examines its limits and the challenges it put forth against the White supremacist elements in White dominant culture. The author concludes that while "Negro poetry" reflects a step forward in the anti-racist development of Black political and cultural identities, it is a limiting method of approach that does not subvert enough White dominant culture. This position enables the author to argue that, for example, the essentialist, racist, and gender--and sex--biases within the representations and images of the "feminine" and Black women are not sufficiently critically re-represented in "Negro poetry" of the Negritude movement. The method of this approach is (1) to look at Sartre's writing in Black Orpheus, (2) to include the secondary literature of this text within my analysis, (3) to examine sections of "Negro poetry" as featured in Black Orpheus, and (4) to critically access the racial, sex, and gender dimensions of "Negro poetry" as they relate to the achievements of the men or artists of the Negritude movement.展开更多

文摘AIM:To investigate associations between ethnicity,age and sex and the risk,colon distribution and density scores of diverticular disease(DD).METHODS:Barium enemas were examined in 1000 patients:410 male,590 female;760 whites,62 Asians,44 black africans(BAs),and 134 other blacks(OBs).Risks and diverticula density of left-sided DD(LSDD) and rightsided-component DD(RSCDD = right-sided DD + right and left DD + Pan-DD) were compared using logistic regression.RESULTS:Four hundred and forty-seven patients had DD(322 LSDD and 125 RSCDD).Adjusted risks:(1) LSDD:each year increase in age increased the odds by 6%(95% CI:5-8,SE:0.8%,P < 0.001);Asians:odds ratio(OR):0.23(95% CI:0.10-0.53,SE:0.1,P ≤ 0.001) and OBs:OR:0.25(95% CI:0.14-0.43,SE:0.07,P ≤ 0.001) appeared protected vs Whites;(2) RSCDD:each year increase in age increased the odds by 4%(95% CI:2-6,SE:1%,P < 0.001);females were 0.60 times(95% CI:0.40-0.90,SE:0.12,P = 0.01) less likely than males to have RSCDD;BAs were 3.51 times(95% CI:1.70-7.24,SE:1.30,P < 0.001) more likely than Whites to have RSCDD;and(3) DD density scores:each year increase in age increased the odds of highdensity scores by 4%(95% CI:1-6,SE:1%,P < 0.001);RSCDD was 2.77 times(95% CI:1.39-3.32,SE:0.67,P < 0.001) more likely to be of high density than LSDD.No further signif icant differences were found in the adjusted models.CONCLUSION:Right colonic DD might be more common and has higher diverticula density in the west than previously reported.BAs appear predisposed to DD,whereas other ethnic differences appear conserved following migration.

文摘Investigations into the inheritance of the three risk alleles R702W, G908R and 1007 fsInsC in NOD2 associated with susceptibility to Crohn's disease have demonstrated a remarkable amount of heterogeneity across ethnicities and populations, with regional variation across Europe for example, suggesting local founder effects. In nonCaucasian populations Crohn's disease continues to increase in incidence but this increase appears not to be a consequence of variation in NOD2, further advancing the accumulating evidence for other susceptibility loci. Frequencies of the known alleles are compared across populations in health and disease and evidence for additional alleles in NOD2 is reviewed. Based on its position on chromosome 16 coincident with some other autoimmune disease susceptibility Iocalizations, research has targeted NOD2 variation as the potential cause of other autoimmune disorders. While these investigations have mostly returned negative findings, two diseases, Blau Syndrome and Graft versus Host Disease, have been shown to be caused by risk alleles in NOD2. As is frequent in complex disease investigations, some results await validation, but the identification of NOD2 and the differences within and across population raises intriguing questions about the population genetics of the variation at this locus.

文摘A novel Sn-2.5Ag-2.0Ni alloy was used for soldering SiCp/Al composites substrate deposited with electroless Ni(5%P) (mass fraction)and Ni(10%P)(mass fraction)layers.It is observed that variation of P contents in the electroless Ni(P)layer results in different types of microstructures of SnAgNi/Ni(P)solder joint.The morphology of Ni3Sn4 intermetallic compounds(IMCs)formed between the solder and Ni(10%P)layer is observed to be needle-like and this shape provides high speed diffusion channels for Ni to diffuse into solder that culminates in high growth rate of Ni3Sn4.The diffusion of Ni into solder furthermore results in the formation of Kirkendall voids at the interface of Ni(P)layer and SiCp/Al composites substrate.It is observed that solder reliability is degraded by the formation of Ni2SnP,P rich Ni layer and Kirkendall voids.The compact Ni3Sn4 IMC layer in Ni(5%P)solder joint prevents Ni element from diffusing into solder,resulting in a low growth rate of Ni3Sn4 layer.Meanwhile,the formation of Ni2SnP that significantly affects the reliability of solder joints is suppressed by the low P content Ni(5%P)layer.Thus,shear strength of Ni(5%P) solder joint is concluded to be higher than that of Ni(10%P)solder joint.Growth of Ni3Sn4 IMC layer and formation of crack are accounted to be the major sources of the failure of Ni(5%P)solder joint.
文摘AIM: To determine the prevalence of ulcerative colitis (UC) in Malaysian patients and to establish the spectrum of the disease seen in Malaysian patients. METHODS: Data were obtained retrospectively from a review of the medical records of in- and out-patients with a diagnosis of UC at the University Hospital, Kuala Lumpur between 1985 and 1998. RESULTS: There were 45 confirmed cases of UC, of which 3 were foreigners, who were excluded from analysis. Thirty new cases of UC were diagnosed during the study period. Their mean age at presentation was 33.0±10.0 years. The highest prevalence of UC was 17.9/100 000 hospital admissions in the Indians, followed by 11.2/100 000 hospital admissions in the Chinese. The lowest prevalence was 3.7/100 000 hospital admissions in the Malays. The prevalence of UC was significantly higher in the Indians and the Chinese when compared with the Malays with an OR of 4.89 (CI = 2.02-12.24; x^2 = 15.45, P〈0.001) and 3.06 (CI = 1.24-7.78; x^2= 6.30; P= 0.012) respectively. The extent of colonic disease Was similar in the Malay and Indian patients. In contrast, distal or left-sided colitis predominated in the Chinese with an OR of 8.17 (95%CI = 1.31-64.87; x^2 = 5.53, P = 0.02). Extraintestinal manifestations were uncommon (11.9%). CONCLUSION: UC is an uncommon disease in Malaysia, but racial differences exist. The Indians had the highest prevalence of UC with the Chinese demonstrating the least extensive disease.

文摘Ethnicity is a carrier of language and culture.Spatial distribution of ethnic diversity is fundamental for identifying and reconstructing the migration patterns and evolution histories of cultures and languages.Utilizing the Chinese 4th National Census (1990) data,we investigated the specific time geographical patterns of population and diversity of Chinese ethnicminorities.As anticipated,results show that Chineseminorities are chiefly concentrated in dis-tant plateaus and mountains in the southwest,northwest and northeast of China.Further,population density centers of the 10 majorminorities are rather scattered,alternatively dominating at different parts of the country.This study pro-vides a first comprehensive quantitative test on a prevailing notion of 'six plates and three corridors' on the empirical clustering patterns of Chinese ethnicminorities.There are more consistent evidences supporting this notion in the north of China,with the central and southern regions showing more complex patterns,potentially transformed by processes such as migration,fragmentation,and percolation.The results of this study suggest that a geographical ap-proach can provide heuristic and complementary information for better understanding of historical social processes.

文摘This paper explores Du Boisian notion of double consciousness and the notion of black nihilism suggested by Cornel West in Ralph Ellison's Invisible Man (1952) and August Wilson's Fences (1986). In order to conduct this study, the researcher adopts African American approach and shows that double consciousness and black nihilism are the immediate effects of African Americans' marginalisation. According to W. E. B. Du Bois, all African Americans develop the sense of double consciousness which means being both African and American simultaneously. On the other hand, Cornel West believes that black Americans suffer from nihilism which means loss of hope and absence of meaning. The paper is an attempt to compare the novel Invisible Man and the play Fences thematically based on these two theories. It then looks to find the considerable similarities in the conditions of the protagonists of these two literary works. Although both protagonists migrate to the North to escape Southern racism in the era prior to the Civil Rights Movement, they become totally disillusioned, for they are treated as the invisible marginal. Interestingly though, at the end of both stories, there remains a ray of hope for reactivation and the creation of democracy for black Americans.
文摘As the construction of identities is said to be within and not outside discourse, identities are thus produced in specific historical and institutional sites and within specific discursive formations and practices. These formations and practices, under which identities are produced, are explored within the Diaspora space of West Yorkshire occupied by Sudanese Muslim women. The paper interrogates Sudanese women's identities and questions how the experiences of exile and Diaspora transform women's identities and influence the gender roles to which they relate. This study is an attempt to add to migration, refugee, exile and "race" studies and the studies of Black and religious identity in the Western Diaspora space. The paper maintains that if all identities are thought to operate through exclusion, then identities are constructed through difference. Therefore, the identities of African Muslim women are constructed through the relation to the Other.
文摘AIM: To determine rates of hepatitis C (HCV) risk factor ascertainment, testing, and referral in urban primary care practices, with particular attention to the effect of race and ethnicity. METHODS: Retrospective chart review from four primary care sites in Philadelphia; two academic primary care practices and two community clinics was performed. Demographics, HCV risk factors, and other risk exposure information were collected. RESULTS: Four thousand four hundred and seven charts were reviewed. Providers documented histories of injection drug use (IDU) and transfusion for less than 20% and 5% of patients, respectively. Only 55% of patients who admitted IDU were tested for HCV. Overall, minorities were more likely to have information regarding a risk factor documented than their white counterparts (79% vs 68%, P < 0.0001). Hispanics were less likely to have a risk factor history documented, compared to blacks and whites (P < 0.0001). Overall, minorities were less likely to be tested for HCV than whites in the presence of a known risk factor (23% vs 35%, P = 0.004). Among patients without documentation of risk factors, blacks and Hispanics were more likely to be tested than whites (20% and 24%, vs 13%, P < 0.005, respectively). CONCLUSION: (1) Documentation of an HCV risk factor history in urban primary care is uncommon, (2)Racial differences exist with respect to HCV risk factor ascertainment and testing, (3) Minority patients, positive for HCV, are less likely to be referred for subspecialty care and treatment. Overall, minorities are less likely to be tested for HCV than whites in the presence of a known risk factor.
文摘After experiencing racism in Canada, where multiculturalism emphasized ethnic differences and fixed identities rather than allowing for cultural interaction and hybridity, the author, B. Mukherjee, moves to the USA whose biculturalism favors cultural interactions and fluid identities. Here she experiences the transformative powers of cultural interactions and frees herself and her work from the static power of cultural disjunction. Her personal experience highlights the need of immigrant characters to connect to the mainstream and not to be isolated from it The paper explores the problem of cultural adaptability and integration as experienced by Dimple, the main character in Mukherjee's novel Wife (1975). Based upon contemporary research on cultural and social identity formation, the paper analyses Dimple's inner struggle of identity in the context of her immigrant status, and it relates her ultimately tragic response to loneliness and alienation resulting in cultural disjunction, non-adaptability, and non-assimilation.
文摘The paper examines the development of "Negro poetry" from the Negritude movement through the analysis of Sartre as featured in Black Orpheus (1976). Following the analysis of Sartre, the author looks at the Black/White binary and its cultural and political context, which is based on the dominance of Europeans over Blacks. Then the author examines how Blacks have attempted to subvert this binary through alternative political stands, such as the practice of anti-racism, and innovative cultural art and literary forms, such as the Negritude movement. Like Sartre, the author focuses on "Negro poetry" and examines its limits and the challenges it put forth against the White supremacist elements in White dominant culture. The author concludes that while "Negro poetry" reflects a step forward in the anti-racist development of Black political and cultural identities, it is a limiting method of approach that does not subvert enough White dominant culture. This position enables the author to argue that, for example, the essentialist, racist, and gender--and sex--biases within the representations and images of the "feminine" and Black women are not sufficiently critically re-represented in "Negro poetry" of the Negritude movement. The method of this approach is (1) to look at Sartre's writing in Black Orpheus, (2) to include the secondary literature of this text within my analysis, (3) to examine sections of "Negro poetry" as featured in Black Orpheus, and (4) to critically access the racial, sex, and gender dimensions of "Negro poetry" as they relate to the achievements of the men or artists of the Negritude movement.
