We aimed to investigate the risk factors and the results of CABG along with the long term survival in patients at an age of 80 and older. Methods Between Januaa-y 2002 and December 201 I, a total of i01 consecutive patients at an age of S0 and older who underwent CABG in our hospital were included in the study. The patients were followed and the long-term survival was estimated. Results The mean age of the patients was 82.98 ~ 2.27 years. Sixty-four (63.4%) were males and 37 (36.6%) were females. Emergency surgery, duration of cardiopulmonary bypass, the intensive care unit (ICU) stay, inotropic support, intra aortic balloon pulsation application, amount oferythrocyte transfusion and flesh frozen plasma transfusion and ventilation period were significantly higher in the patients who died in the hospital. The duration of cardiopulmonary bypass (CPB) was found to be an inde- pendent predictor of mortality (OR: 1.18, 95% CI 1.01-1.38, P = 0.034). The in-hospital mortality was 16.8%. Kaplan-Meier analyses revealed a survival ratio of 91.3% at one year, 82.9% at three years and 69.0% at five years. Conclusions Patients at the age of 80 and older can be candidates for the CABG procedure bearing in mind that they may have a longer ventilation period and intensive care unit stay. The morbidity and mortality of this age group is considered within an acceptable range. Approaches to minimize CPB, or the choice of off-pump surgery, may be a preventive method to lower the incidence of mortality. Hence, CABG may be performed in this age group with a satisfactory survival ratio.展开更多

One of the four main sub-groups, the p38 group of MAP kinases, serve as a nexus for signal transduction and play a vital role in numerous biological processes. In this review, we highlight the known characteristics and components of the p38 pathway along with the mechanism and consequences of p38 activation. We focus on the role of p38 as a signal transduction mediator and examine the evidence linking p38 to inflammation, cell cycle, cell death, development, cell differentiation, senescence and tumorigenesis in specific cell types. Upstream and downstream components of p38 are described and questions remaining to be answered are posed. Finally, we propose several directions for future research on p38.展开更多

The constitution of population dramatically turns into an elder and low-birthrate society. Therefore, the ratio of elders living along is increasing yearly. When the elders need to deal the illness and go to hospital along, decays come from degeneration might have affect on their activity in accessible environment in hospital. Signs is a vital tool for delivering and communicating information in the road sign system. Whether the elders can identify the signs in hospital clearly becomes an important issue in the current research. To understand the affect of signs design in hospital to elders, the research reviewed and gained insight of degeneration in both physical and psychological of elders and the proper principal of signs design for elders. The research surveys in Taipei City, which has densest medical center, and mainly focus on National Taiwan University College of Medicine, which has hundred years of history (founded in 1895). The research conducts field survey in both old and new buildings of National Taiwan University College of Medicine in August, 2015. The survey includes five types of signs design: Public Facilities, Prohibition, Safety, Warning, and Commercial Facilities. The result from article review indicates the most salient psychological degenerations includes the change in cognitive function, decay of memory, and deceleration of information processing; physical degenerations includes the decay of vision. As a result, the following properties should be taken into consideration when designing sings for elders: (1) The proper ratio of graphic and text. (2) Avoid using colors or matches difficult to identify by elders, like purple, blue, and green. (3) Take the visibility, legibility, identity, attention, and memory into consideration, there should be 70% differentiation in contrast between text of sign and background. (4) Avoid using multi-meaning graphic, keep the design simple. The result from the survey of National Yaiwan University College of Medicine indicates: (1) Using mainly on graphic and text as subsidiary, part of the sign design using both Chinese and English, and make sure the appropriateness of ratio between graphic and text. Use more of sans-serif font, which has better legibility. (2) Match the color mainly as yellow background with black graphic, blue background with white graphic, white background with black graphic, white background with blue graphic, a strong contrast between background and graphic on brightness and chroma matches the principle of visibility, legibility, and identity. (3) Simplify the signs design may safeguard its visibility. When elder need to handle illness comes from degeneration alone and go to large and complex hospitals frequently, the decay of their mental and physical might have a huge negative affect. Methods From December 2005 to December 201 l, 84 graft lesions were treated percumneously. Seventeen were located at proximal anastomosis, 48 were located at SVG body, 19 were located at distal anastomosis. Pri mary endpoint was defined as major adverse cardiovascular events (MACE, composite of cardiac death, target vessel revascularization, acute myocardial infarction). Results The graft age was 6.7 i 4.0 years. Most anastomosis lesions (80.0%) presented within one year post coro-nary artery bypass grafting (CABG). Proximal anastomosis lesion had the lowest successful rate for PCI compared with graft body and distal anastomosis lesions (70.6% vs. 91.7%, 79.0%, P 〈 0.05). The distal embolic protection device was used in 19.1% of patients, most frequently used in body graft PCI (29.2%, P 〈 0.01). The diameter of the stent was smallest in distal anastomosis group (2.9 ±0.4 mm, P 〈 0.05). The highest post dilatation pressure was required in the proximal anastomosis (17.8 ± 2.7 atm, P 〈 0.05). The patients were followed up for 24.3 ±16.9 months. MACE occurred in 18.57% of patients. Incidence of MACE was highest among proximal anastomosis PCI (47.1% vs. body graft PCI 16.7%, distal anastomosis PCI 21.1%; P 〈 0.05). Old myocardial infarction was the predictive factor for the poor clinical outcomes (P〈 0.04). Conclusions PCI of SVG lesions is feasible with lower success rate. PCI of ostial graft anastomosis lesions had the lowest procedure success rate and highest MACE rate compared with graft body and distal anastomosis lesions. Aging sensor is a good online way to detect the circuit aging, which performs during the operating time with no influence of the normal operation of circuits. In this paper, a Dou- ble-edge-triggered Detection Sensor for circuit Aging (DSDA) is proposed, which employs data signal of logic circuits as its clock to control the sampling process. The simulation is done by Hspice using 45 nm technology. The results show that this technique is not case of the detection precision is more than 80% under aging fault effectively with the 8% power cost and 30% sensitive to the process variations. This paper describes advantages and concerns related to the usage of, and the design with, polymeric drainboards in tunnel construction. Common degradation mechanisms associated with HDPE (high density polyethylene) sheets are described. The stringent requirements for the Gotthard Alpine Railway Tunnel through the Swiss Alps, e.g., high ambient temperatures of up to 45 ℃ and an expected service life of up to 100 years require outstanding aging resistance of polymeric drainage materials. The paper describes the methods deployed to investigate the long-term performance of HDPE drainboards, focusing on aging mechanisms. Details associated with the test procedures developed to reflect the specific properties of drainboards, as well as the results obtained, are presented. A summary table shows recommended product specifications needed to confine the aging properties of drainboards and to design a system performing adequately during the entire lifetime of the structure.展开更多
When a liquid is supercooled,its structural equilibration timeτeqincreases sharply approaching the glass transition temperature Tg,below which it is kinetically arrested in the out of equilibrium glassy state.Upon an...When a liquid is supercooled,its structural equilibration timeτeqincreases sharply approaching the glass transition temperature Tg,below which it is kinetically arrested in the out of equilibrium glassy state.Upon annealing below Tg,such glassy state relaxes toward the equilibrium state.There is growing evidence that such relaxation process is quite complex,influenced by the complexity of the potential energy landscape.Here we report the observation of a hierarchical aging pathway in the process of the glass-to-supercooled liquid transition in various molecular glass forming systems.Differential scanning calorimetry reveals that the glassy state upon annealing below Tgfirstly enters a transient metastable supercooled liquid state with slightly higher enthalpies than that of the equilibrium supercooled liquid state when extrapolated to below Tg.This observation is also confirmed by structural measurements via Raman scattering.The dynamics of the transient metastable-to-stable supercooled liquid transition exhibits characteristics of thermodynamic transition at spinodal temperature Tspslightly above Tg,leading to anomalous behaviors such as the appearance of a transition-like behavior in the plot of fragility measurements.These observations imply that the free energy landscape of the supercooled liquid develops complexity with qualitative changes approaching Tgand could have strong influence on the process of the glass transition.展开更多
:Objective:To study the epidemiologic characteristics of traffic injuries among people over 60 years old in the Nan' an district(urban)and Jiangjin district(rural)of Chongqing,and to discuss the corresponding s...:Objective:To study the epidemiologic characteristics of traffic injuries among people over 60 years old in the Nan' an district(urban)and Jiangjin district(rural)of Chongqing,and to discuss the corresponding strategies for its prevention and cure.Methods:Records of traffic injuries in people over 60 years old registered by the traffic police between 2000 and 2006 in Nan'an district and Jiangjin district were collected in the Database of Road Traffic Accidents and Traffic Injuries.Epidemiologic characteristics of traffic injuries among the aged people were analyzed and compared.Results:Between the year 2000 and 2006,the average annual incidence of traffic injuries and mortality rate in the aged people in Nan' an district were 124.62/100 000 and 13.85/100 000 respectively,higher than that in Jiangjin district(27.49/100 000,7.13/100 000,P〈0.01).However,the mortality rate for the aged people who were involved in traffic injuries in Jiangjin district was 20.60%,higher than that in Nan'an district(10.00%,P〈0.01).Head injury was the primary cause of death.Totally 76.58% of casualties were pedestrians.Over 90% of the traffic accidents occurred in the areas with no traffic signal or traffic control system.Conclusions:The traffic environment is unfavorable to the aged people.It is important to enhance traffic safety consciousness of drivers and the elderly and to strengthen traffic safety system and traffic law,so as to provide a safe road traffic environment for the aged people.展开更多
