METHODS: Eighty patients undergoing diagnostic esophagogastroduodenoscopy in a tertiary care medical center were randomized to either lidocaine swab or spray. Intravenous meperidine and midazolam were given as needed during the procedure. RESULTS: Patients in the lidocaine swab group (SWG) tolerated the procedure better than those in the spray group (SPG) with a median tolerability score of 2 (1, 4) compared to 4 (2, 5) (P < 0.01). The endoscopists encountered less diffi culty performing the proceduresin the SWG with lower median difficulty scores of 1 (1, 5) compared to 4 (1, 5) in the SPG (P < 0.01). In addition, the need for intravenous sedation was also lower in the SWG compared to the SPG with fewer patients requiring intravenous sedation (13/40 patients vs 38/40 patients, respectively, P < 0.01). The patients in the SWG were more satisfi ed with the mode of local anesthesia they received as compared to the SPG. In addition, the endoscopists were happier with the use of lidocaine swab. CONCLUSION: The use of a posterior lingual lidocaine swab in esophagogastroduodenoscopy improves patient comfort and tolerance and endoscopist satisfaction and decreases the need for intravenous sedation.展开更多

The Altay granulite (AG) is an amphibole plagioclase two-pyroxene granulite and is mainly composed of two pyroxenes, plagioclase, amphibole and biotite. Its melano-minerals are rich in Mg/(Mg+Fe2+), and its amphibole and biotite are rich in TiO2. The AG is rich in Mg/(Mg+Fe2+), Al2O3 and depletion of U, Th and Rb contents. The AG has moderate ∑REE and LREE-enriched with weak positive Eu anomaly. The AG shows island-arc pattern with negative Nb, P and Ti anomalies, reflecting that formation of the AG may be associated with subduction. Geochemical and mineral composition data reflect that the protolith of the AG is calc-alkaline basalt and formed by granulite facies metamorphism having peak P-T conditions of 750 °C?780 °C and 0.6?0.7 Gpa. The AG formation underwent two stages was suggested. In the early stage of oceanic crustal subduction, calc-alkaline basalt with island-arc environment underwent granulite facies metamorphism to form the AG in deep crust, and in the late stage, the AG was thrust into the upper crust.展开更多

The part of the residents with initially antibody positive as well as the part of residents with initially antibody negative have been followed up for 3 consecutive years by serology and clinical examination for studying kinetic changes of antibody to M.leprae and its relation with clinical disease. The results showed that the rates of subclinical infection of leprosy in a high endemic area are different from village to village, and the risk of developing clinical disease does not associate with subclinical infection rate. It correlates with the number of cured accumulative leprosy cases and active cases within the village. The authors consider that in leprosy high endemic villages, especially those cropped up new multi bacillary leprosy cases frequently in recent years, it may be helpful to use serology to detect early leprosy cases.展开更多

Three study areas are selected to consider different rocks, including gneiss in Jangheung, granite in Sangju and sedimentary rocks in Pohang, South Korea. Many landslides have occurred in these three areas during the rainy season.Precipitation records indicate that landslides occurring in the gneiss area of Jangheung and granite area of Sangju may be influenced by the hourly rainfall intensity rather than cumulative rainfall. However, landslides occurring in the sedimentary rock area of Pohang may be influenced by hourly rainfall intensity and cumulative rainfall. To investigate the factors that influence these types of landslides, a detailed landslide survey was performed and a series of laboratory soil tests were conducted.According to the detailed field survey, most landslides occurred on the flanks of mountain slopes, and the slope inclination where they occurred mostly ranged from 26 to 30 degrees, regardless of the geological conditions. The landslide in the gneiss area of Jangheung is larger than the landslides in the granite area of Sangju and sedimentary rock area of Pohang.Particularly, the landslide in the sedimentary rock area is shorter and shallower than the landslides in the gneiss and granite areas. Thus, the shape and size of the landslide are clearly related to the geological conditions. According to the integrated soil property and landslide occurrence analyses results, the average dry unit weight of the soils from the landslide sites is smaller than that of the soils obtained from the nonlandslide site. The average coefficient of permeability of soils obtained from the landslide sites is greater than that of soils obtained from the non-landslide sites with the same geology. These results indicate that the soils from the landslide sites are more poorly graded or looser than the soils from the non-landslide sites.展开更多

Sesame, cv. Kenana-2, was grown in 6-m wide alleys formed by 3 year old Acacia ampliceps and Acacia stenophylla. Air temperature, relative humidity, wind speed, solar energy and shade behaviour were measured throughout the growing season. Irrigation was measured by water meter and soil moisture by gravimetric sampling at depths of 15, 30, 45 and 60 cm, in alley cropping and control plots. Sesame was evaluated for growth and yield performance in southern, central and northern parts of each alley. The results showed substantial reduction in wind runs, solar radiation and air temperature, while relative humidity was increased under alley conditions. Soil moisture content, especially in the upper 045 cm soil depth, was much higher in alley cropped plots than in the control plots. A. stenophylla resulted in the highest saving of irrigation water and considerably increased (40%) sesame seed yield. In contrast, sesame seed yield was reduced by 46% under A. ampliceps-alley cropping. Competition for light and water were highly responsible for variation in sesame seed yield in the different zones of the alley. A. stenophylla tree is, therefore, recommended in an alley cropping system in the semi-desert region of northern Sudan for its superiority in optimizing radiation and saving of irrigation water.展开更多

It occurs when the strong band of the tissue supporting the arch of foot becomes irritated and inflamed. The majority of patients can be treated conservatively but some resistant cases need surgery eventually. This study aims to evaluate the outcome results of percutaneous planter fascia release under local anesthesia for chronic planter fasciitis. Methods: This prospective study was conducted in the Department of Orthopaedic Surgery in the School of Medical Science and Research, Sharda University, India from December 2010 to December 2013. Totally 78 patients with planter fasciitis for more than 6-12 months were recruited from the outpatient department. All patients were operated on under local anesthesia and followed up for a year. Results: The clinical results were evaluated in terms of pain, activity level and patient satisfaction. Pain relief was achieved averagely at eight weeks after surgery. The results were excellent in 88.46% (69/78) patients and good in 6.41% (9/78) patients. Neither complications of lateral column instability, sinus tarsitis and metatarsalgia nor wound-related complications were encountered. On subjective evalua- tions, 88.46% (69J78) patients reported full satisfaction and 6.41% (9/78) reported partial satisfaction after treatment. Conclusion: Percutaneous planter fasciitis release under local anesthesia is a minimally invasive pro- cedure that can be performed in the outpatient setting. It is easy, quick, effective and moreover with few coml^lications.展开更多

In the outcrops, rocks are gradually changed from wall rock (slate or schist) to pluton (gneissic granite); some residual blocks of sandy rock occur in the margin of pluton, and the foliations of residual blocks are parallel to those of both wail rock and gneissic granite. The thin-section observations show that the four plutons contain peraluminous minerals such as muscovite and sillimanite. The flattened and elongated feldspar and quartz grains are often visible in the gneissic granite, parallel to direction of lineation, suggesting that the granitic rock were subjected to a strong ductile sheafing. Geochemically, the A/CNK values from 13 granitic samples are between 1.03 and 1.37 with an average of 1.16, indicating that the granites are of strongly peraluminous plutons. The REE compositions of the 13 samples are similar, showing higher EREE contents, with enrichment in LREEs, depletion in Eu and REE patterns with relative LREE-enrichment and negligible Eu anomalies. They show enrichment in Rb, Th, U and depletion in Ba, Sr, Nb, Ti, belonging to a low Ba-Sr type of granite. Thus, the four bodies should be derived from the same magmatic source. Zircons used as U-Pb dating mostly exhibit euhedral shape and high Th/U values from 0.52 to 1.54 with an average of 1.08, suggesting that most zircons are of magmatic genesis. The zircons from four plutons yielded rather similar 206pb/238U vs. 207Tpb/235U concordia ages: 436.1±5.7 Ma for the Tangwan granite, 440.6±4 Ma for the Jiekou gneissic granite, 435.9±6.2 Ma for the Dongbao gneissic granite, and 441.9±3.1 Ma for the Jinxi K-granite, respectively, corresponding to Silurian Llandovery. Several xenocrysts yielded U-Pb ages around 700 Ma, implying that a breakup event took place during Neoproterozoic in the South China Block. In situ Lu-Hf isotopic analysis shows that all εHf(t) values of zircons are negative and have two-stage Hf model ages (TDM2) from 1.4 to 3.6 Ga, indicating that the Silurian granitic magma came from the re- cycle of Meso-Paleoproterozoic basement and even partly Archean rocks, and had not been effected by mantle magma. Re- searches on regional geology suggest that an intracontinental tectono-magmatic event took place during the early Paleozoic in the study areas, which is characterized by folding and thrusting, leading to crustal shortening and thickening, up to 20 km thickness. The high geothermal temperature from thickening crust and accumulation of producing high-heat radioactive elements gradually softened crustal rocks and caused a partial melting, forming peraluminous granitic magma. Under the post-orogenic extensional and de-pressure condition, these granitic magma rose and was emplaced in the upper crust, leading to development of S-type plutons展开更多

In this study, we report the Archean(2802 ± 13 Ma) granodioritic gneisses in the Taihua Complex from the Xiaoqinling area, located in the southern margin of the NCC. The zircon Lu–Hf isotope analysis of the rocks showed ^(176)Hf/^(177)Hf ratios of0.280977–0.281228, corresponding to ε_(Hf)(t) values ranging from-3.5 to +6.6, and two–stage Hf model ages varying from 2836 to 3409 Ma. It was confirmed that late Mesoarchean(2.9–2.8 Ga) juvenile crust made contribution to the source material of these ca. 2.8 Ga granodioritic gneisses in the Xiaoqinling area. The whole rock geochemical data indicate that the granodioritic gneisses are high-K calc-alkaline series, probably generated at relatively high pressure and temperature, and formed under the continental arc setting. Statistically, we conclude that the magmatic activities during 2.9–2.7 Ga may represent the most intense crustal growth events in the NCC and these Archean rocks at different locations in southern NCC underwent a similar crustal evolution history.展开更多

However, many plant species have adapted to the harsh sand environment along the southeastern coast of China. Studying the microbial biomass in such an ecosystem can improve our understanding of the roles that microbes play in soil fertility and nutrient cycling. We investigated the differences in soil microbial biomass carbon(MBC) and nitrogen(MBN) contents and their seasonal dynamics in five forest types(a secondary forest and plantations of Casuarinas, Pine, Acacia, and Eucalyptus). The results indicated that the seasonal variations of soil MBC and MBN contents in all five forest stands were higher in spring and winter, but lower in summer and autumn. The MBC content was lower in the Casuarinas plantation than in the other plantations in the same soil layer. However, no significant differences were observed in MBN contents among the different forest types. The MBC and MBN concentrations were positively correlated with soil moisture, but negatively correlated with soil temperature. The MBC and MBN contents also decreased with increasing soil depth. Across all soil layers, secondary forest had the highest MBC and MBN concentrations. Our study also showed that the MBC and MBN contents were positively affected by total soil carbon(TC), pH, and litter N content, but were negatively impacted by soil bulk density and litter C content. Moreover, the MBN content was positively correlated with root N content. In summary, environmental factors and the differences in litter and fine roots, soil nutrient contents, as well as the soil physical and chemical properties caused by different tree species collectively affected the concentrations of the soil MBC and MBN.展开更多

Many long-term studies of birds have reported a climatic influence on breeding performance. We analysed long-term changes in first laying date and clutch size for first clutches of sedentary, hole-nesting and socially monogamous tree sparrows Passer montanus during 1980-2009 in northwestern Croatia. Laying date advanced and spring temperature increased significantly during the study. Date of laying was significantly correlated with mean air spring temperature and advanced by 8.6 days during the 30 years period. Laying date was delayed in extremely cold, but not in extremely warm springs compared to years with normal temperatures. These results suggest that current climate change including extreme change does not have negative effects on timing of laying or clutch size. In contrast to laying date, clutch size did not vary systematically during the study period or with mean spring air temperature changes [Current Zoology 57 (3): 414-418, 2011].展开更多

A. stenophylla tree is, therefore, recommended in an alley cropping system in the semi-desert region of northern Sudan for its superiority in optimizing radiation and saving of irrigation water.
文摘Purpose: Plantar fasciitis is the most common cause of pain on the bottom of the heel. It is easy, quick, effective and moreover with few coml^lications.

文摘This paper reports the systematic study on petrology, geochemistry, LA ICPMS zircons U-Pb dating, and in situ Hf isotope geology of the four plutons in the central-southern Jiangxi Province, an important part of the South China Block. Under the post-orogenic extensional and de-pressure condition, these granitic magma rose and was emplaced in the upper crust, leading to development of S-type plutons

文摘Recently, more attention has been paid to Precambrian magma events and crustal growth of the North China Craton(NCC), accompanying with controversy in activity stages and dynamic mechanism. Statistically, we conclude that the magmatic activities during 2.9–2.7 Ga may represent the most intense crustal growth events in the NCC and these Archean rocks at different locations in southern NCC underwent a similar crustal evolution history.

文摘Coastal sand dune ecosystems generally have infertile soil with low water-holding capacity and high salinity. In summary, environmental factors and the differences in litter and fine roots, soil nutrient contents, as well as the soil physical and chemical properties caused by different tree species collectively affected the concentrations of the soil MBC and MBN.
文摘Global surface air temperatures increased during the past 100 years. In contrast to laying date, clutch size did not vary systematically during the study period or with mean spring air temperature changes [Current Zoology 57 (3): 414-418, 2011].
