In 2010, a panel of Chinese pathologists reported the first expert consensus for the pathological diagnosis of primary liver cancers to address the many contradictions and inconsistencies in the pathological characteristics and diagnostic criteria for PLC. Since then considerable clinicopathological studies have been conducted globally, prompting us to update the practice guidelines for the pathological diagnosis of PLC. In April 18, 2014, a Guideline Committee consisting of 40 specialists from seven Chinese Societies(including Chinese Society of Liver Cancer, Chinese Anti-Cancer Association; Liver Cancer Study Group, Chinese Society of Hepatology, Chinese Medical Association; Chinese Society of Pathology, Chinese Anti-Cancer Association; Digestive Disease Group, Chinese Society of Pathology, Chinese Medical Association; Chinese Society of Surgery, Chinese Medical Association; Chinese Society of Clinical Oncology, Chinese Anti-Cancer Association; Pathological Group of Hepatobiliary Tumor and Liver Transplantation, Chinese Society of Pathology, Chinese Medical Association) was created for the formulation of the first guidelines for the standardization of the pathological diagnosis of PLC, mainly focusing on the following topics: gross specimen sampling, concepts and diagnostic criteria of small hepatocellular carcinoma(SHCC), microvascular invasion(MVI), satellite nodules,and immunohistochemical and molecular diagnosis. The present updated guidelines are reflective of current clinicopathological studies, and include a novel 7-point baseline sampling protocol, which stipulate that at least four tissue specimens should be sampled at the junction of the tumor and adjacent liver tissues in a 1:1 ratio at the 12, 3, 6 and 9 o'clock reference positions. For the purposes of molecular pathological examination, at least one specimen should be sampled at the intratumoral zone, but more specimens should be sampled for tumors harboring different textures or colors. Specimens should be sampled at both adjacent and distant peritumoral liver tissues or the tumor margin in order to observe MVI, satellite nodules and dysplastic foci/nodules distributed throughout the background liver tissues. Complete sampling of whole SHCC ≤ 3 cm should be performed to assess its biological behavior, and in clinical practice, therapeutic borders should be also preserved, even in SHCC. The diagnostic criteria of MVI and satellite nodules, immunohistochemical panels, as well as molecular diagnostic principles, such as clonal typing, for recurrent HCC and multinodule HCC were also proposed and recommended. The standardized process of pathological examination is aimed at ensuring the accuracy of pathological PLC diagnoses as well as providing a valuable frame of reference for the clinical assessment of tumor invasive potential, the risk of postoperative recurrence, long-term survival, and the development of individualized treatment regimens. The updated guidelines could ensure the accuracy of pathological diagnoses of PLC, and provide a valuable frame of reference for its clinical assessment. 展开更多

Purpose: The purpose of the study was to examine the psychometric properties of the Chinese-translated Behavioral Regulation in Exercise Questionnaire-2 (C-BREQ-2) among Chinese university students from the Mainland and Hong Kong of China. Methods: University students from the Mainland (n = 191) and Hong Kong (n=194) of China participated in this study. Factorial validity, discriminant validity, nomological validity, internal reliability, and measurement invariance across sample of the C-BREQ-2 were examined. Results: Confirmatory factor analysis provided support for the factorial validity of the 18-item, 5-factor structure C-BREQ-2. Examination of the 95% confidence interval of the inter-factor correlations suggested that the C-BREQ-2 assesses related but distinct constructs, which provided support for its discriminant validity. The internal consistency reliability of the C-BREQ-2 was found acceptable. Examination of the pattern of inter-factor correlations between different regulations suggested that a simplex-like pattern was displayed, which provided evidence for the nomological validity of C-BREQ-2. The results from multi-group confirmatory factor analysis suggested that the factor loadings and variances/ covariances of the C-BREQ-2 measurement model were invariant across the Chinese university students in the Mainland and Hong Kong of China. Conclusion: The current study provided further psychometric evidence for the C-BREQ-2, which makes the further application and research of self-determination theory (SDT) based motivation in relation to exercise and physical activity in the Mainland of China context possible. 展开更多

Objectives:This study aimed to assess the effects of modified pulmonary rehabilitation(PR)on patients with moderate to severe chronic obstructive pulmonary disease(COPD).Methods:A total of 125 patients(63 in the PR group and 62 in the control group)were recruited in this study.The patients in the PR group received 12 weeks of conventional treatment,nursing,and modified pulmonary rehabilitation,while the patients in the control group underwent 12 weeks of conventional treatment,nursing,pursed-lip breathing training,and abdominal breathing training.Baseline characteristics,St.George's Respiratory Questionnaire(SGRQ),the six-minute walk test(6MWT),modified medical research council(MMRC)dyspnea scale,and lung function were compared between the two groups.Results:A total of 112 patients(58 patients in the PR group and 54 patients in the control group)completed the 12-week monitoring and follow-up.The SGRQ scores,symptoms(54.933±11.900),activity(52.644±14.334),impact(55.400±9.905),and total score(54.655±10.681)of the PR group did not significantly differ in pre-and post-treatments(P<0.05).No significant change was also observed in the control group(P>0.05).6MWT[(372.089±67.149)m]was significantly improved in the PR group(P<0.05)but was not significantly different in the control group(P>0.05).MMRC(actual rank sum 1719,rank sum 2047.5)was significantly reduced in the PR group(P<0.05)but not in the control group(P>0.05).The lung function(FVC,FEV1,FEV1/FVC,FEV1%and PEF)of the patients in both groups did not significantly change(P>0.05).Conclusion:Modified PR reduces the symptoms of dyspnea,increases exercise capacity,and improves the quality of life of patients with moderate to severe COPD. 展开更多

In speech recognition, acoustic modeling always requires tremendous transcribed samples, and the transcription becomes intensively time-consuming and costly. In order to aid this labor-intensive process, Active Learning (AL) is adopted for speech recognition, where only the most informative training samples are selected for manual annotation. In this paper, we propose a novel active learning method for Chinese acoustic modeling, the methods for initial training set selection based on Kullback-Leibler Divergence (KLD) and sample evaluation based on multi-level confusion networks are proposed and adopted in our active learning system, respectively. Our experiments show that our proposed method can achieve satisfying performances. 展开更多

Use of the stepwise progression return-to-play protocol following concussion among practicing athletic trainers

Purpose: The purpose of this study was to determine whether practicing athletic trainers(ATs) were using the stepwise progression to make return-to-play(RTP) decisions after concussion and to determine what factors influenced their decision to use the stepwise progression.Methods: A total of 166 ATs(response rate = 16.6%) completed a 21-item questionnaire that evaluated participant demographics, methods of concussion management, and RTP decision-making using the stepwise progression. Descriptive statistics and a logistic regression were completed to analyze data.Results: Factors such as education level(p = 0.05) and number of concussions treated(p = 0.05) predicted use of the stepwise progression,whereas sex(p = 0.17), employment setting(p = 0.17), state law(p = 0.86), and years practicing(p = 0.17) did not predict whether ATs were following the stepwise progression.Conclusion: The majority of the ATs from this study are employing the stepwise progression to safely return athletes to play after sustaining a concussion. This demonstrates that ATs are providing a standard of care for concussed athletes across various athletic training settings; however,having a graduate degree and treating more concussions per year are predictors of whether an AT follows all steps of the stepwise progression. 展开更多

Methods A questionnaire survey was conducted amongst Chinese orthopedic surgeons. A total of 293 surgeons were surveyed concerning five key aspects of thromboembolic prophylaxis after major orthopedic surgery at the proseminar of Chinese guidelines for prevention of venous thromboembolism (VTE) after major orthopedic surgery in January of 2009. Results Totally, 208 surgeons (71.0%) responded, successfully completing the questionnaire. Of them, 57.6% respondents selected combined basic, mechanical, and pharmacologic methods for thromboprophylaxis; 51.0% respondents prefer starting prophylaxis 12-24 hours after surgery; 60.3% surgeons would use chemoprophylaxis for 7-10 days; 47.6% respondents prefer VTE prevention based on patients' special conditions and needs upon discharge. "Safety" was the most repeated and emphasized factor during VTE prophylaxis. Conclusions Multimodal thromboprophylaxis is frequently used after major orthopedic surgery. Half surgeons prefer to start chemoprophylaxis 12-24 hours after surgery. Thromboprophylaxis regimen varies for discharged patients. 展开更多

The gap between the theories of second language acquisition and their practice in materials development has been a controversial issue in the field of applied linguistics. Recently, the findings of corpus linguistics have narrowed the gap. However, their practice, especially in the realm of vocabulary acquisition, has encountered pedagogic problems in the classroom because the authentic nature of the words in corpus-based examples was beyond the level of learners' comprehension which paves the way for the practice of pedagogic corpus linguistics. Therefore, the purpose of this study was to practice pedagogic corpus linguistics, and mainly data-driven learning, with the first year secondary school English language learners in Iran in terms of their vocabulary acquisition. To do so, 28 students participated in the study. Examples from the British National Corpus for the main words of each unit of their course book, Prospect 1, were presented to them in a course of 16 sessions. Those which were beyond their understanding were eliminated and those comprehensible for them, the pedagogic ones, were selected based on their reports regarding the comprehensibility of each corpus-based example. 展开更多

The identity of paramedic professions is bound to develop in order to adapt to the changes of the world of health. Careproviders' identity is based on the interpersonal dimension, yet French massage-physiotherapy bases this interpersonal competence on touch. This is why many practitioners try to develop this aspect of their practice through continuing professional education. Danis Bois Method (MOthode Danis Bois: MDB) of fasciatherapy, characterized by its relational and sensitive approach to care and touch, stands in this field. This paper will present the impacts of this manual therapy on the professional identity of 446 masseurs-physiotherapists who took part in a self-assessment survey. The results show that, in spite of the few difficulties they meet within their duties, practicing fasciatherapy integrates perfectly well into the framework of massage-physiotherapy. It largely reinforces the feeling of therapeutical efficacy and the capacity to meet the evolution in health needs (chronic diseases, mental suffering, and patients' quality of life). It also restores meaning and value to the profession and boosts motivation as well as it fosters the congruence of both considered as an added value to masso-physiotherapy, a means to accom professional identity pets plish onal and professional identities. Machine Learning(ML) techniques have been widely applied in recent traffic classification.However, the problems of both discriminator bias and class imbalance decrease the accuracies of ML based traffic classifier. In this paper, we propose an accurate and extensible traffic classifier. Specifically, to address the discriminator bias issue, our classifier is built by making an optimal cascade of binary sub-classifiers, where each binary sub-classifier is trained independently with the discriminators used for identifying application specific traffic. Moreover, to balance a training dataset,we apply SMOTE algorithm in generating artificial training samples for minority classes.We evaluate our classifier on two datasets collected from different network border routers.Compared with the previous multi-class traffic classifiers built in one-time training process,our classifier achieves much higher F-Measure and AUC for each application. 展开更多
