In order to develop the high photocatalytic activity of TiO2 under visible light as that under ultraviolet light and make it easy to be separated from treated liquor, a visible light response and spherical activated c...In order to develop the high photocatalytic activity of TiO2 under visible light as that under ultraviolet light and make it easy to be separated from treated liquor, a visible light response and spherical activated carbon (SAC) supported photocatalyst doped with upconversion luminescence agent Er3+:YAlO3 was prepared by immobilizing Er3+:YAlO3/TiO2, which was obtained by combination of Er3+:YAlO3 and TiO2 using sol-gel method, on the surface of SAC. The crystal phase composition, surface structure and element distribution, and light absorption of the new photocatalysts were examined by X-ray diffraction (XRD), energy dispersive X-ray spectra (EDS) analysis, scanning electron microscopy (SEM) and fluorescence spectra analysis (FSA). The photocatalytic oxidation activity of the photocatalysts was also evaluated by the photodegradation of methyl orange (MO) in aqueous solution under visible light irradiation from a LED lamp (λ400 nm). The results showed that Er3+:YAlO3 could perform as the upconversion luminescence agent which converts the visible light up to ultraviolet light. The Er3+:YAlO3/TiO2 calcinated at 700 °C revealed the highest photocatalytic activity. The apparent reaction rate constant could reach 0.0197 min-1 under visible light irradiation.展开更多
Cholangiocarcinomas arise from the epithelial cells of the bile ducts and are associated with poor prognosis. C...Cholangiocarcinomas arise from the epithelial cells of the bile ducts and are associated with poor prognosis. Despite new diagnostic approaches, the definite diagnosis of this malignancy continues to be challenging. Cholangiocarcinomas often grow longitudinally along the bile duct rather than in a radial direction. Thus, large tumor masses are frequently absent and imaging techniques, including ultrasound, CT, and MRI have only limited sensitivity. Tissue collection during endoscopic (ERCP) and/or percutaneous transhepatic (PTC) procedures are usually used to confirm a definitive diagnosis of cholangiocarcinoma. However, forceps biopsy and brush cytology provide positive results for malignancy in about only 50% of patients. Percutaneous and peroral cholangioscopy using fiber-optic techniques were therefore developed for direct visualization of the biliary tree, yielding additional information about endoscopic appearance and tumor extension, as well as a guided biopsy acquistion. Finally, endoscopic ultrasonography (EUS) complements endoscopic and percutaneous approaches and may provide a tissue diagnosis of tumors in the biliary region through fine-needle aspiration. In the future, new techniques allowing for early detection, including molecular markers, should be developed to improve the diagnostic sensitivity in this increasing tumor entity.展开更多

In order to explore the physiological and biochemical characteristics of fresh upper leaves and roots of different qualities of tobacco, the malonaldehyde (MDA), superoxide dismutase (SOD) activity, chlorophyllase...In order to explore the physiological and biochemical characteristics of fresh upper leaves and roots of different qualities of tobacco, the malonaldehyde (MDA), superoxide dismutase (SOD) activity, chlorophyllase activity, amylase activity, medium trace element content, chemical component content, root fresh weight, dry weight and activity in the normal tobacco leaves, tobacco leaves rich in nutrients and premature grey tobacco leaves were studied. The results showed that compared with the normal tobacco leaves, the root fresh weight, dry weight and activity in the premature grey tobacco leaves decreased by 41.22%, 14.44% and 60.29% respectively, while malonaldehyde content increased by 34.82%, and chlorophyllase, α-amylase, β-amylase and SOD activity declined by 89.55%, 65.71%, 71.26% and 37.25% respectively. Chlorophyllase, co-amylase and 13-amylase activity in the tobacco leaves rich in nutrients decreased by 58.76%, 36.19% and 41.93% respectively. The content of medium trace elements in the tobacco leaves rich in nutrients and premature grey tobacco leaves was lower than that of the normal tobacco leaves. Starch, total sugar, reducing sugar and potassium content in the normal tobacco leaves were higher than that of the tobacco leaves rich in nutrients and premature grey tobacco leaves.展开更多

METHODS: Previously diagnosed GIM patients underwent targeted biopsy fro...AIM: To evaluate the efficacy of non-sequential narrow band imaging (NBI) for a better recognition of gastric intestinal metaplasia (GIM). METHODS: Previously diagnosed GIM patients underwent targeted biopsy from areas with and without GIM, as indicated by NBI, twice at an interval of 1 year. The authors compared the endoscopic criteria such as light blue crest (LBC), villous pattern (VP), and large long crest (LLC) with standard histology. The results from two surveillance endoscopies were compared with histology results for sensitivity, specificity, positive predic-tive value (PPV), negative predictive value (NPV), and likelihood ratio of positive test (LR+). The number of early gastric cancer cases detected was also reported. RESULTS: NBI targeted biopsy was performed in 38 and 26 patients during the first and second surveillance endoscopies, respectively. There were 2 early gastric cancers detected in the first endoscopy. No cancer was detected from the second study. Surgical and endoscopic resections were successfully performed in each patient. Sensitivity, specificity, PPV, NPV, and LR+ of all 3 endoscopic criteria during the first/second surveillances were 78.8%/91.3%, 82.5%/89.1%, 72.8%/77.8%, 86.8%/96.1, and 4.51/8.4, respectively. LBC provided the highest LR+ over VP and LLC. CONCLUSION: Nonequential NBI is useful for GIM targeted biopsy. LBC provides the most sensitive reading. However, the optimal duration between two surveillances requires further study.展开更多

[Method] The amylase specific activities and starch degradation of the leaves were determined by using spectrophotometry. [Result] The 8 U/g exogenous α-amy-lase could improve the specific activity of the leaf α-amylase at yel owing and color-fixing stages, but could not at stem-drying stage, and similarly, the 80 U/g exoge-nous β-amylase could improved the specific activity of the leaf β-amylase at the yel owing stage and the early period of color-fixing stage. The leaf starch could be enhanced to degrade by the exogenous α- or β-amylases and the enhancing effect of the former was stronger than that of the later. 1.50 mg/ml Ca2+ improved the specific activity of the leaf (α+β)-amylase mainly due to its enhancing effect on the leaf α-amylase, and increased the starch degradation. 4 mmol/L Mn2+ inhibited the leaf α-amylase from yel owing to the early period of color-fixing and the β- and (α+β)-amylases from the yel owing to the later period of color-fixing, but enhanced the leafα-amylase from the later period of color-fixing to the later period of stem-drying and the β- and (α+β)-amylases at the later period of stem-drying. Meanwhile, Mn2+ ham-pered the starch degradation during yel owing, but promoted it from the early period of color-fixing to stem-drying. 1 mg/ml K+ enhanced the leaf α-, β- and (α+β)-amy-lases during the yel owing stage, but lowered them from the early period of color-fix-ing to the later period of stem-drying, and always inhibited the leaf starch degrada-tion. [Conclusion] The exogenous α-, β- amylases and Ca2+ of suitable concentra-tions could be used to treat the tobacco leaves before flue-curing to improve the leaf starch degradation during the curing.展开更多

AIM: To prospectively investigate the efficacy of the revised Vienna Classification for diagnosing colorectal epithelial neoplastic lesions in cold biopsy specimens.METHODS: Patients were selected for inclusion if t...AIM: To prospectively investigate the efficacy of the revised Vienna Classification for diagnosing colorectal epithelial neoplastic lesions in cold biopsy specimens.METHODS: Patients were selected for inclusion if they had colorectal epithelial lesions that were not considered suitable for direct endoscopic resection,These included colorectal polyps ≥ 10 mm and lesions suspected of being carcinomas capable of invading the colorectal submucosa or beyond, including strictures, based on the cold biopsies obtained from each lesion prior to resection. We investigated the relationship between diagnoses based on cold biopsy samples using the revised Vienna Classification and resected specimens of the same lesions, and the therapeutic implications of diagnoses made using the revised Vienna Classification. The same cold biopsy specimens were also examined using the Japanese Group Classification guidelines, and compared with the resected specimens of the same lesions for reference.RESULTS: A total of 179 lesions were identified. The sensitivity, specificity, positive and negativepredictive values of the revised Vienna Classification for distinguishing between intramucosal lesions and submucosal invasive carcinomas in cold biopsy specimens was 22.2%, 100%, 100%, and 71.4%,respectively, and for distinguishing between intramucosal lesions and those invading the submucosa or beyond was 59.7%, 100%, 100%, and 37.6%,respectively. The sensitivity, specificity, positive and negative predictive values of the Japanese Group Classification for distinguishing between intramucosal lesions and submucosal invasive carcinomas in cold biopsy specimens was 83.3%, 91.4%, 83.3%. and 91.4%, respectively, and for distinguishing between intramucosal lesions and those invading the submucosa or beyond was 95.1%, 91.4%, 97.9%, and 82.1%, respectively. A total of 137 of 144 carcinomas that had invaded the submucosa or beyond and three high-grade intraepithelial neoplasias were diagnosed as "carcinoma" using the Japanese Group Classification system.CONCLUSION: The revised Vienna Classification for cold biopsy specimens has high positive predictive value in the diagnosis of colorectal carcinoma invasive to the subrnucosa or beyond.展开更多

Chitin is the structural component of fungi, crustaceans, insects and parasitic nematodes, but is completely absent in mammals. Exposure to antigens containing chitin- or chitin-like structures sometimes induces strong T helper type-I responses in mammals, which may be associated with the induction of mammalian chitinases. Chitinase 3-like-1 (CHI3L1), a member of the mammalian chitinase family, is induced specifically during the course of inflammation in such disorders as inflammatory bowel disease, hepatitis and asthma. In addition, CHI3L1 is expressed and secreted by several types of solid tumors including glioblastoma, colon cancer, breast cancer and malignant melanoma. Although the exact function of CHI3L1 in inflammation and cancer is still largely unknown, CHI3L1 plays a pivotal role in exacerbating the inflammatory processes and in promoting angiogenesis and remodeling of the extracellular matrix. CHI3L1 may be highly involved in the chronic engagement of inflammation which potentiates development of epithelial tumorigenesis presumably by activating the mitogen-activated protein kinase and the protein kinase B signaling pathways. Anti-CHI3L1 antibodies or pan-chitinase inhibitors may have the potential to suppress CHI3Ll-mediated chronic inflammation and the subsequent carcinogenic change in epithelial cells.展开更多
The purpose of this study was to examine the sedimentary facies characteristics of lower Cambrian Niutitang Formation(∈1n)in South China,to reveal the mechanism of organic matter enrichment,and to guide exploration o...The purpose of this study was to examine the sedimentary facies characteristics of lower Cambrian Niutitang Formation(∈1n)in South China,to reveal the mechanism of organic matter enrichment,and to guide exploration of shale gas.Macro investigation and experimental analyses were used to assess the lithology in detail,total organic matter mass fraction w(TOC),mineral composition,and trace element characteristics of∈1n.The influencing factors of organic matter enrichment were discussed extensively,and a sedimentary facies mode was suggested.In the early stage of∈1n,the locations of Well E’yangye 1,Well Ciye 1,Well Changye 1,and Well Anye 1 respectively develop,platform inner sag,outer shelf,Jiangnan slope belt,and South China detention basin.In the late stage of∈1n,the sedimentary facies evolve with decreasing sea level.The study area presents a complete three-step basin in the Early Cambrian.In the early stage of∈1n,the first step is the Yangtze carbonate platform,the second step is the outer shelf and slope,and the third step is the deep-water basin.From the Yangtze carbonate platform to the deep-water basin,w(TOC)and the mass fraction of quartz gradually increase,the mass fraction of carbonate mineral decreases,and the mass fraction of clay mineral is higher in the second step.The sea level fluctuation results in a higher w(TOC)vertically in the lower∈1n shale,and the paleogeographic(provenance)conditions lead to better horizontal development of organic matter in the outer shelf,slope and detention basin.Trace elements are abundant in the lower∈1n,and w(TOC)is correlated positively with many trace elements.In the outer shelf,slope,and adjacent areas,hydrothermal activity and upwelling current bring nutrient-rich material and promote organic matter enrichment under a strong reducing condition.Deep-shelf,slope and deep-water basin are the best facies for the formation and preservation of organic matter,especially deep-water basin facies.It remains necessary to strengthen the exploration of shale gas in the deep-water basin of∈1n in central Hunan,China.展开更多
Epithelial-to-mesenchymal and mesenchymal-to-epi- thelial transitions are well established biological events which have an important role in not just normal tissue and organ development, but in the pathogenesis of dis...Epithelial-to-mesenchymal and mesenchymal-to-epi- thelial transitions are well established biological events which have an important role in not just normal tissue and organ development, but in the pathogenesis of diseases. Increasing evidence has established their presence in the human colon during colorectal carcinogenesis and cancer invasion, chronic inflammation-related fibrosis and in the course of mucosal healing. A large body of evidence supports the role for transforming growth factor-13 and its downstream Smad signaling, the phosphatidylinositol 3'-kinase/Akt/mTOR axis, the Ras-mitogen-activated protein kinase/Snail/Slug and FOXC2 pathway, and Hedgehog signaling and microR- NAs in the development of colorectal cancers via epi- thelial-to-mesenchymal transition. C-met and Frizzled-7, among others, seem to be the principle effectors of mesenchymal-to-epithelial transition, hence have a role not just in mucosal regeneration but in the progression of colonic wall fibrosis. Here we discuss a role for these pathways in the initiation and development of the transition events. A better understanding of their induction and regulation may lead to the identification of pathways and factors that could be potent therapeu- tic targets. The inhibition of epithelial-to-mesenchymal transition using mTOR kinase inhibitors targeting theATP binding pocket and which inhibit both mTORC1 and mTORC2, RNA aptamers or peptide mimetics, such as a Wnt5A-mimetic, may all be useful in both cancer treatment and delaying fibrosis, while the induction of mesenchymal-to-epithelial transition in induced pluripotent stem cells may enhance epithelial healing in the case of severe mucosal damage. The preliminary results of the current studies are promising, but more clinical investigations are needed to develop new and safe therapeutic strategies for diseases of the colon.展开更多
In addition to squamous cell carcinoma,the incidence of Barrett's esophagus with high-grade dysplasia and esophageal adenocarcinoma is rapidly increasing worldwide.Unfortunately,the current standard of care for es...In addition to squamous cell carcinoma,the incidence of Barrett's esophagus with high-grade dysplasia and esophageal adenocarcinoma is rapidly increasing worldwide.Unfortunately,the current standard of care for esophageal pathology involves resection of the affected tissue,sometimes involving radical esophagectomy.Without exception,these procedures are associated with a high morbidity,compromised quality of life,and unacceptable mortality rates.Regenerative medicine approaches to functional tissue replacement include the use of biological and synthetic scaffolds to promote tissue remodeling and growth.In the case of esophageal repair,extracellular matrix(ECM) scaffolds have proven to be effective for the reconstruction of small patch defects,anastomosis reinforcement,and the prevention of stricture formation after endomucosal resection(EMR).More so,esophageal cancer patients treated with ECM scaffolds have shown complete restoration of a normal,functional,and disease-free epithelium after EMR.These studies provide evidence that a regenerative medicine approach may enable aggressive resection of neoplastic tissue without the need for radical esophagectomy and its associated complications.展开更多

As our intellectual perception of the world is becoming more universal and all-inclusive, so sh...In this paper, I first create new frameworks for a reflection on “human” and “humanism” in the time of globalization. As our intellectual perception of the world is becoming more universal and all-inclusive, so should also our hearts. The heart is the inner most core of being human, to become able to expand in unprecedented dimensions of human existence. In this context, I propose forgiveness as the guiding principle of expansion and inclusion, and consequently of becoming more human. If memory helps us to survive, forgetting allows us to go on living. The practice of forgetting, and consequently of reconciliation, is the distinctive and complex moment of human existence, when the human agent makes him/herself open and available to the present moment, and lives fully in the present moment. Once doing so, the present moment becomes the space in which s/he can reconcile with his/her past and shape his/her future. In the second part, I check possible impediments on our way to expansion and inclusion, as well as in our process of forgiveness and reconciliation. Pata?jali, in his instructions to yogis talks about kleshas, i.e. impediments or afflictions, which are weakening our way of thinking and acting. These kleshas include ignorance, ego, attachment, aversion, and the clinging to life. I take these kleshas also as afflictions on our way to becoming more human, or as impediments in our process of forgiveness and reconciliation. Finally, I reflect on the human urge to become more human. In our time of globalization, we are challenged to look at this urge from a universal perspective. This urge, bound with an inevitable process of forgiveness and reconciliation, challenges each religion and its dogmatic teaching, and represents the evaluative criterion. The spirituality, or religion, that which promulgates and lives forgiveness and reconciliation will automatically excel because it will succeed, without losing its own identity, to integrate and include otherness of the other.展开更多

LBC provided the highest LR+ over VP and LLC. CONCLUSION: Nonequential NBI is useful for GIM targeted biopsy. LBC provides the most sensitive reading. However, the optimal duration between two surveillances requires further study.

文摘Objective] The aim of this study was to investigate the effects of exoge-nous amylases and Ca2+, Mn2+ and K+ on the amylase specific activities and starch degradation of the upper leaves of 'KRK26' planted in Yunnan Province during flue-curing. [Method] The amylase specific activities and starch degradation of the leaves were determined by using spectrophotometry. [Result] The 8 U/g exogenous α-amy-lase could improve the specific activity of the leaf α-amylase at yel owing and color-fixing stages, but could not at stem-drying stage, and similarly, the 80 U/g exoge-nous β-amylase could improved the specific activity of the leaf β-amylase at the yel owing stage and the early period of color-fixing stage. The leaf starch could be enhanced to degrade by the exogenous α- or β-amylases and the enhancing effect of the former was stronger than that of the later. 1.50 mg/ml Ca2+ improved the specific activity of the leaf (α+β)-amylase mainly due to its enhancing effect on the leaf α-amylase, and increased the starch degradation. 4 mmol/L Mn2+ inhibited the leaf α-amylase from yel owing to the early period of color-fixing and the β- and (α+β)-amylases from the yel owing to the later period of color-fixing, but enhanced the leafα-amylase from the later period of color-fixing to the later period of stem-drying and the β- and (α+β)-amylases at the later period of stem-drying. Meanwhile, Mn2+ ham-pered the starch degradation during yel owing, but promoted it from the early period of color-fixing to stem-drying. 1 mg/ml K+ enhanced the leaf α-, β- and (α+β)-amy-lases during the yel owing stage, but lowered them from the early period of color-fix-ing to the later period of stem-drying, and always inhibited the leaf starch degrada-tion. [Conclusion] The exogenous α-, β- amylases and Ca2+ of suitable concentra-tions could be used to treat the tobacco leaves before flue-curing to improve the leaf starch degradation during the curing.

文摘AIM: To prospectively investigate the efficacy of the revised Vienna Classification for diagnosing colorectal epithelial neoplastic lesions in cold biopsy specimens.METHODS: Patients were selected for inclusion if they had colorectal epithelial lesions that were not considered suitable for direct endoscopic resection,These included colorectal polyps ≥ 10 mm and lesions suspected of being carcinomas capable of invading the colorectal submucosa or beyond, including strictures, based on the cold biopsies obtained from each lesion prior to resection. We investigated the relationship between diagnoses based on cold biopsy samples using the revised Vienna Classification and resected specimens of the same lesions, and the therapeutic implications of diagnoses made using the revised Vienna Classification. The same cold biopsy specimens were also examined using the Japanese Group Classification guidelines, and compared with the resected specimens of the same lesions for reference.RESULTS: A total of 179 lesions were identified. The sensitivity, specificity, positive and negativepredictive values of the revised Vienna Classification for distinguishing between intramucosal lesions and submucosal invasive carcinomas in cold biopsy specimens was 22.2%, 100%, 100%, and 71.4%,respectively, and for distinguishing between intramucosal lesions and those invading the submucosa or beyond was 59.7%, 100%, 100%, and 37.6%,respectively. The sensitivity, specificity, positive and negative predictive values of the Japanese Group Classification for distinguishing between intramucosal lesions and submucosal invasive carcinomas in cold biopsy specimens was 83.3%, 91.4%, 83.3%. and 91.4%, respectively, and for distinguishing between intramucosal lesions and those invading the submucosa or beyond was 95.1%, 91.4%, 97.9%, and 82.1%, respectively. A total of 137 of 144 carcinomas that had invaded the submucosa or beyond and three high-grade intraepithelial neoplasias were diagnosed as "carcinoma" using the Japanese Group Classification system.CONCLUSION: The revised Vienna Classification for cold biopsy specimens has high positive predictive value in the diagnosis of colorectal carcinoma invasive to the subrnucosa or beyond.

文摘The family of mammalian chitinases includes members both with and without glycohydrolase enzymatic activity against chitin, a polymer of N-acetylglucosamine. Chitin is the structural component of fungi, crustaceans, insects and parasitic nematodes, but is completely absent in mammals. Exposure to antigens containing chitin- or chitin-like structures sometimes induces strong T helper type-I responses in mammals, which may be associated with the induction of mammalian chitinases. Chitinase 3-like-1 (CHI3L1), a member of the mammalian chitinase family, is induced specifically during the course of inflammation in such disorders as inflammatory bowel disease, hepatitis and asthma. In addition, CHI3L1 is expressed and secreted by several types of solid tumors including glioblastoma, colon cancer, breast cancer and malignant melanoma. Although the exact function of CHI3L1 in inflammation and cancer is still largely unknown, CHI3L1 plays a pivotal role in exacerbating the inflammatory processes and in promoting angiogenesis and remodeling of the extracellular matrix. CHI3L1 may be highly involved in the chronic engagement of inflammation which potentiates development of epithelial tumorigenesis presumably by activating the mitogen-activated protein kinase and the protein kinase B signaling pathways. Anti-CHI3L1 antibodies or pan-chitinase inhibitors may have the potential to suppress CHI3Ll-mediated chronic inflammation and the subsequent carcinogenic change in epithelial cells.

文摘The purpose of this study was to examine the sedimentary facies characteristics of lower Cambrian Niutitang Formation(∈1n)in South China,to reveal the mechanism of organic matter enrichment,and to guide exploration of shale gas.Macro investigation and experimental analyses were used to assess the lithology in detail,total organic matter mass fraction w(TOC),mineral composition,and trace element characteristics of∈1n.The influencing factors of organic matter enrichment were discussed extensively,and a sedimentary facies mode was suggested.In the early stage of∈1n,the locations of Well E’yangye 1,Well Ciye 1,Well Changye 1,and Well Anye 1 respectively develop,platform inner sag,outer shelf,Jiangnan slope belt,and South China detention basin.In the late stage of∈1n,the sedimentary facies evolve with decreasing sea level.The study area presents a complete three-step basin in the Early Cambrian.In the early stage of∈1n,the first step is the Yangtze carbonate platform,the second step is the outer shelf and slope,and the third step is the deep-water basin.From the Yangtze carbonate platform to the deep-water basin,w(TOC)and the mass fraction of quartz gradually increase,the mass fraction of carbonate mineral decreases,and the mass fraction of clay mineral is higher in the second step.The sea level fluctuation results in a higher w(TOC)vertically in the lower∈1n shale,and the paleogeographic(provenance)conditions lead to better horizontal development of organic matter in the outer shelf,slope and detention basin.Trace elements are abundant in the lower∈1n,and w(TOC)is correlated positively with many trace elements.In the outer shelf,slope,and adjacent areas,hydrothermal activity and upwelling current bring nutrient-rich material and promote organic matter enrichment under a strong reducing condition.Deep-shelf,slope and deep-water basin are the best facies for the formation and preservation of organic matter,especially deep-water basin facies.It remains necessary to strengthen the exploration of shale gas in the deep-water basin of∈1n in central Hunan,China.
文摘Epithelial-to-mesenchymal and mesenchymal-to-epi- thelial transitions are well established biological events which have an important role in not just normal tissue and organ development, but in the pathogenesis of diseases. Increasing evidence has established their presence in the human colon during colorectal carcinogenesis and cancer invasion, chronic inflammation-related fibrosis and in the course of mucosal healing. A large body of evidence supports the role for transforming growth factor-13 and its downstream Smad signaling, the phosphatidylinositol 3'-kinase/Akt/mTOR axis, the Ras-mitogen-activated protein kinase/Snail/Slug and FOXC2 pathway, and Hedgehog signaling and microR- NAs in the development of colorectal cancers via epi- thelial-to-mesenchymal transition. C-met and Frizzled-7, among others, seem to be the principle effectors of mesenchymal-to-epithelial transition, hence have a role not just in mucosal regeneration but in the progression of colonic wall fibrosis. Here we discuss a role for these pathways in the initiation and development of the transition events. A better understanding of their induction and regulation may lead to the identification of pathways and factors that could be potent therapeu- tic targets. The inhibition of epithelial-to-mesenchymal transition using mTOR kinase inhibitors targeting theATP binding pocket and which inhibit both mTORC1 and mTORC2, RNA aptamers or peptide mimetics, such as a Wnt5A-mimetic, may all be useful in both cancer treatment and delaying fibrosis, while the induction of mesenchymal-to-epithelial transition in induced pluripotent stem cells may enhance epithelial healing in the case of severe mucosal damage. The preliminary results of the current studies are promising, but more clinical investigations are needed to develop new and safe therapeutic strategies for diseases of the colon.

文摘In addition to squamous cell carcinoma,the incidence of Barrett's esophagus with high-grade dysplasia and esophageal adenocarcinoma is rapidly increasing worldwide.Unfortunately,the current standard of care for esophageal pathology involves resection of the affected tissue,sometimes involving radical esophagectomy.Without exception,these procedures are associated with a high morbidity,compromised quality of life,and unacceptable mortality rates.Regenerative medicine approaches to functional tissue replacement include the use of biological and synthetic scaffolds to promote tissue remodeling and growth.In the case of esophageal repair,extracellular matrix(ECM) scaffolds have proven to be effective for the reconstruction of small patch defects,anastomosis reinforcement,and the prevention of stricture formation after endomucosal resection(EMR).More so,esophageal cancer patients treated with ECM scaffolds have shown complete restoration of a normal,functional,and disease-free epithelium after EMR.These studies provide evidence that a regenerative medicine approach may enable aggressive resection of neoplastic tissue without the need for radical esophagectomy and its associated complications.
文摘In this paper, I first create new frameworks for a reflection on “human” and “humanism” in the time of globalization. As our intellectual perception of the world is becoming more universal and all-inclusive, so should also our hearts. The heart is the inner most core of being human, to become able to expand in unprecedented dimensions of human existence. In this context, I propose forgiveness as the guiding principle of expansion and inclusion, and consequently of becoming more human. If memory helps us to survive, forgetting allows us to go on living. The practice of forgetting, and consequently of reconciliation, is the distinctive and complex moment of human existence, when the human agent makes him/herself open and available to the present moment, and lives fully in the present moment. Once doing so, the present moment becomes the space in which s/he can reconcile with his/her past and shape his/her future. In the second part, I check possible impediments on our way to expansion and inclusion, as well as in our process of forgiveness and reconciliation. Pata?jali, in his instructions to yogis talks about kleshas, i.e. impediments or afflictions, which are weakening our way of thinking and acting. These kleshas include ignorance, ego, attachment, aversion, and the clinging to life. I take these kleshas also as afflictions on our way to becoming more human, or as impediments in our process of forgiveness and reconciliation. Finally, I reflect on the human urge to become more human. In our time of globalization, we are challenged to look at this urge from a universal perspective. This urge, bound with an inevitable process of forgiveness and reconciliation, challenges each religion and its dogmatic teaching, and represents the evaluative criterion. The spirituality, or religion, that which promulgates and lives forgiveness and reconciliation will automatically excel because it will succeed, without losing its own identity, to integrate and include otherness of the other.
