目的回顾性分析106例老年晚期胃癌病例,评价内镜下缩瘤术对老年晚期胃癌预后的影响.方法将收集的老年晚期胃癌病例分为采用内镜下缩瘤术治疗的治疗组和仅接受最佳支持治疗的同期对照组,主要观察指标为有效率和生存期.完成4疗程以上缩瘤...目的回顾性分析106例老年晚期胃癌病例,评价内镜下缩瘤术对老年晚期胃癌预后的影响.方法将收集的老年晚期胃癌病例分为采用内镜下缩瘤术治疗的治疗组和仅接受最佳支持治疗的同期对照组,主要观察指标为有效率和生存期.完成4疗程以上缩瘤术治疗后评估短期疗效及不良反应.采用Cox多因素分析方法 ,在基线均衡的条件下比较两组胃癌病例的生存期,Kaplan-Meier法估算中位生存时间.结果收集中国人民解放军第455医院消化肿瘤科2010-01-01/2015-06-30诊治的老年晚期胃癌患106例,治疗组为51例,对照组为55例,治疗组和对照组的近期有效率分别为76.5%和27.3%(P<0.05).C o x多因素回归分析示:内镜下缩瘤治疗(P=0.000)、肿瘤大小(P=0.000)是影响老年晚期胃癌病例生存期的独立的预后因素,在组间基线均衡的条件下,治疗组(51例)的中位生存期较对照组(55例)延长(12.496 mo vs 8.384 mo,P=0.000),采用内镜下缩瘤治疗的相对危险度:0.430(95%CI:0.274-0.675).治疗组1、2年生存率较对照组明显延长(52.8%vs 29.1%、22.9%vs 2.8%).结论内镜下缩瘤术对晚期胃癌具有较好临床疗效,可延长老年晚期胃癌患者的生存期,使其生存获益.展开更多

By us...To deal with the effect of compressible fluids on the supercavitating flow over the subsonic disk cavitator of a projectile, a finite volume method is formulated based on the ideal compressible potential theory. By using the continuity equation and Tait state equation as well as Riabouchinsky closure model, an“inverse problem”solution is presented for the supercavitating flow. According to the impenetrable condition on the surface of supercavity, a new iterative method for the supercavity shape is designed to deal with the effect of compressibility on the supercavity shape, pressure drag coefficient and density field. By this method, the very low cavitation number can be computed. The calculated results agree well with the experimental data and empirical formula. At the subsonic condition, the fluid compressibility will make supercavity length and radius increase. The supercavity expands, but remains spheroid. The effect on the first 1/3 part of supercavity is not obvious. The drag coefficient of projectile increases as the cavitation number or Mach number increases. With Mach number increasing, the compressibility is more and more significant. The compressibility must be considered as far as the accurate calculation of supercavitating flow is concerned.展开更多

By means of this invariant and the Lewis-Riesenfeld quantum invariant theory, we obtain closed formulae of the quantum state and the evolution operator of the system. We show that the time evolution of the quantum system directly leads to production of various generalized one- and two-mode combination squeezed states, and the squeezed effect is independent of the driving term of the Hamiltonian. In some special cases, the current solution can reduce to the results of the previous works.展开更多

This study presents an underwater acoustic data compression method that is based on compressed sensing. Underwater acoustic signals are transformed into the sparse domain for data storage at a receiving terminal, and the improved orthogonal matching pursuit(IOMP) algorithm is used to reconstruct the original underwater acoustic signals at a data processing terminal. When an increase in sidelobe level occasionally causes a direction of arrival estimation error, the proposed compression method can achieve a 10 times stronger compression for narrowband signals and a 5 times stronger compression for wideband signals than the orthogonal matching pursuit(OMP) algorithm. The IOMP algorithm also reduces the computing time by about 20% more than the original OMP algorithm. The simulation and experimental results are discussed.展开更多
Objective: To discuss the method of functional reconstruction of ischemic contracture in the lower limb and propose a classification protocol for ischemic contracture in the lower limb based on its severity and progn...Objective: To discuss the method of functional reconstruction of ischemic contracture in the lower limb and propose a classification protocol for ischemic contracture in the lower limb based on its severity and prognosis. Methods: Atotal of 42 patients with ischemic contracture in the lower limb were included in this study. According to different types of disturbance and degrees of severity, surgical reconstructions consisting of nerve decompression, tendon lengthening or transfer, intrinsic foot muscle release and sural-tibial nerve anastomosis were performed in every patient. Results: Postoperatively, all patients were able to walk on flat ground. Drop foot was corrected in 10 patients, and 5 patients still felt some difficulty during stair activity. Split Achilles tendon transfer to flexor hallucis longus tendon was performed in 12 patients, and their walking stability was improved. Seven patients accepted ipsilateral suraltibial nerve anastomosis, and sensitivity recovery reached to S2 in 2 patients and S3 in 5 patients. Conclusions: Iscbemic contracture in the lower limb is a devastating complication after lower limb trauma. The prevention of contracture is much more important than the treatment of an established contracture. Split Achilles tendon transfer to flexor hallucis longus tendon and sural-tibial nerve anastomosis, which was initially implemented by us, could improve the functional recovery of ischemic contracture in lower limbs, and thus provides a new alternative for functional reconstruction of ischemic contracture in the lower limb.展开更多

However, based on compressed sensing...Due to the low sound propagation speed, the tradeoff between high azimuth resolution and wide imaging swath has severely limited the application of sonar underwater target imaging. However, based on compressed sensing(CS) technique, it is feasible to image targets with merely one pulse and thus avoid the above tradeoff. To investigate the possible waveforms for CS-based underwater imaging, the deterministic M sequences widely used in sonar applications are introduced in this paper. By analyzing the compressive matrix constructed from M sequences, the coherence parameter and the restricted isometry property(RIP) of the matrix are derived. Also, the feasibility and advances of M sequence are demonstrated by being compared with the existing Alltop sequence in underwater CS imaging framework. Finally, the results of numerical simulations and a real experiment are provided to reveal the effectiveness of the proposed signal.展开更多
