Breast cancer is one of the most commonly diagnosed malignancies and the leading cause of cancer death of women over the world. A large number of females with breast cancer in Vietnam and other Southeast Asian(SEA) co...Breast cancer is one of the most commonly diagnosed malignancies and the leading cause of cancer death of women over the world. A large number of females with breast cancer in Vietnam and other Southeast Asian(SEA) countries present at an early age with more aggressive tumors compared with women in Australia. Despite experiencing a low incidence rate, the increasing incidence rate among SEA countries exceeds that of the Westernized world. Changes in reproductive factors, environmental exposures, and lifestyle are the possible causes of this trend. However, limited evidence shows that these factors are associated with breast cancer in the Vietnamese population. Breast cancer incidence rates within Vietnam are not uniform and appear to be dependent on geographic location. Findings from this review have important implications for breast cancer control and treatment in Vietnam. A good understanding of the morphology of the breast and the type and nature of breast cancers presenting in Vietnam is required to facilitate the introduction of an effective national breast screening program.展开更多
AIM: To evaluate the clinical and biochemical profile of patients with non alcoholic fatty liver disease (NAFLD) and to assess their histological severity at presentation.METHODS: Consecutive patients presenting t...AIM: To evaluate the clinical and biochemical profile of patients with non alcoholic fatty liver disease (NAFLD) and to assess their histological severity at presentation.METHODS: Consecutive patients presenting to the liver clinic of All India Institute of Medical Sciences (AIIMS) with raised transaminases to at least 1.5 times upper limit of normal, and histologically confirmed non-alcoholic fatty liver disease were included. Patients who had significant alcohol intake or positive markers of other liver diseases or who were taking drugs known to produce fatty liver were excluded. The clinical, biochemical and histological profile of this group was studied. RESULTS: Fifty-one patients with NAFLD formed the study population. Their median age and BMI were 34(17-58) years and 26.7(21.3-32.5) kg/m^2 respectively and 46 (90.1%) were males. The majority of the patients had mild inflammation, either grade 1 [32 (63%)] or grade 2 [16 (31%)] and only 3 (6%) patients had severe (grade 3) inflammation. Twenty-three (45%), 19 (37%), 8(16%) and 1(2%) patient had stage 0, 1, 2 and 3 fibrosis respectively on index biopsy and none had cirrhosis. On univariate analysis, triglyceride levels more than 150 mg % (OR = 7.1; 95% CI: 1.6-31.5, P = 0.002) and AST ALT ratio 〉 1 (OR = 14.3; 95% CI: 1.4-678.5, P = 0.008) were associated with high grades of inflammation and none was associated with advanced fibrosis. On multivariate logistic regression analysis, hypertriglyceridemia 〉150 mg% was the only factor independently associated with presence of high grade of inflammation (OR = 1.6; 95% CI: 1.3-22.7, P =0.02), while none was associated with advanced fibrosis. Triglyceride levels correlated positively with inflammatory grade (r = 0.412; P = 0.003). CONCLUSION: NAFLD in North Indian patients is a disease of young over-weight males, most of whom are insulin resistant and they tend to have a mild histological disease at presentation.展开更多

METHODS: Patients with reflux symptoms were enrolled and stratified to NERD and ERD after endoscopy (LA classification). Patients with ERD presenting with dyspepsia were included. Patients on proton pump inhibitors (PPI) or H2 receptor antagonists before endoscopy were excluded. Demographic data, Helicobacter pylori(H pylori) status and presence of minor psychiatric morbidity (based on General Health Questionnaire-28) were analyzed. RESULTS: Among 690 patients screened, 533 were eligible for analysis (male to female ratio: 3:2; Chinese: 75.4%; Malay: 9.8%; Indian: 14.8%). Clinical spectrum of GERD: N: 40.5%; A: 46%; B: 9.2%; C: 2.1%; D: 0.6%; Barrett's esophagus: 1.7%. Compared to patients with NERD, patients with ERD were significantly older (45 vs 39.4 years), more likely to be male (64.4% vs 53.7%), tended to smoke (19.6% vs 9.7%), less likely to have minor psychiatric morbidity (26.4 vs 46.7%) and were more likely to respond to PPI (79.7 vs 66.8%). There was also a trend towards a higher BMI (24.5 vs 23.5). Race, alcohol consumption and H pylori status were not significant. On multivariate analysis, age and presence of minor psychiatric morbidity remained with significant differences. CONCLUSION: The majority of patients who have typical symptoms of GERD have NERD or mild erosive reflux disease. Compared to patients with erosive reflux disease, patients with NERD were younger and had a higher prevalence of minor psychiatric morbidity.展开更多
The results inferred from experiments with analogue models carried out previously have shown that two types of plastic-flow waves, “fast-waves" and “slow-waves", are induced in the lower lithosphere (inclu...The results inferred from experiments with analogue models carried out previously have shown that two types of plastic-flow waves, “fast-waves" and “slow-waves", are induced in the lower lithosphere (including the lower crust and lithospheric mantle) under driving at plate boundaries and both of them are viscous gravity waves formed by the superposition of major and subsidiary waves. The major waves are similar to solitary waves and the subsidiary waves are traveling waves. The plastic-flow waves in the lower lithosphere control seismic activities in the overlying seismogenic layer and result in the distribution of earthquakes along the wave-crest belts. “Fast-waves" propagated with velocities of orders of magnitude of 100~102km/a have been verified by wave-controlled earthquake migration, showing the “decade waves" and “century waves" with the average periods of 10.8 and 93.4 a, respectively, which originate from the Himalayan driving boundary. According to the recognition of the patterns of the belt-like distribution of strong earthquakes with M S≥7.0, it is indicated further in this paper that the “slow-waves" with velocities of orders of magnitude of 100~101 m/a also originated under compression from the Himalayan driving boundary. Strong earthquakes with M S≥7.0 are controlled mainly by subsidiary waves, because the major waves with a duration of up to 106 a for each disturbance cannot result in the accumulation of enough energy for strong earthquakes due to the relaxation of the upper crust. The subsidiary waves propagate with an average wave length of 445 km, velocities of 0.81~2.80 m/a and periods of 0.16~0.55 Ma. The wave-generating time at the Himalayan driving boundary is about 1.34~4.59 Ma before present for the “slow-waves", corresponding to the stage from the Mid Pliocene to the Mid Early-Pleistocene and being identical with one of the major tectonic episodes of the Himalayan tectonic movement. It is shown from the recognition of the wave-controlled belts of strong earthquakes that two optimal patterns of wave-crest belts originated simultaneously from the eastern and western segments of the Himalayan arc, respectively. The overlap of wave-crest belts of these two systems is responsible for the relative concentration of energy and forms the seismic-energy-background zones for strong earthquakes with M S≥7.0.展开更多

The S wave velocity structure of the earth below Eastern Southeast Asia has been investigated by analyzing the seismogram from surface wave to multiple depth waves in the time domain and three Cartesian components sim...The S wave velocity structure of the earth below Eastern Southeast Asia has been investigated by analyzing the seismogram from surface wave to multiple depth waves in the time domain and three Cartesian components simultaneously.The wave passes across the front area of subduction zone between the Philippine plate and the Asian plate.The main data are waveform comparisons,instead of the arrival times.The synthetic seismogram is calculated using the GEMINI method.The synthetic seismogram constructed by PREMAN global earth model deviates greatly from the measured one.To solve this problem,corrections are needed for the β speed structure.Corrections cover the gradient change of β_h,which turns from negative to positive in upper mantle layers as in the PREMAN,change of earth crust depth and change of zero order coefficients of β velocity function in all earth mantle layers.So,the fitting is obtained,as well as the arrival time or the waveform of Love and Rayleigh surface waves,the S wave and the repetitive depth waves ScS_2 and ScS_3.This result reveals that the Southeast Asia,being stretched due to tectonic release,has a mantle in some parts with negative anomaly of S wave velocity and vertical anisotropy in all earth mantle layers.展开更多
Objective: To provide an overview of the incidence and mortality of female breast cancer for countries in the Asia-Pacific region.Methods: Statistical information about breast cancer was obtained from publicly availab...Objective: To provide an overview of the incidence and mortality of female breast cancer for countries in the Asia-Pacific region.Methods: Statistical information about breast cancer was obtained from publicly available cancer registry and mortality databases(such as GLOBOCAN), and supplemented with data requested from individual cancer registries. Rates were directly age-standardised to the Segi World Standard population and trends were analysed using joinpoint models.Results: Breast cancer was the most common type of cancer among females in the region, accounting for 18% of all cases in 2012, and was the fourth most common cause of cancer-related deaths(9%). Although incidence rates remain much higher in New Zealand and Australia, rapid rises in recent years were observed in several Asian countries. Large increases in breast cancer mortality rates also occurred in many areas, particularly Malaysia and Thailand, in contrast to stabilising trends in Hong Kong and Singapore, while decreases have been recorded in Australia and New Zealand. Mortality trends tended to be more favourable for women aged under 50 compared to those who were 50 years or older. Conclusion: It is anticipated that incidence rates of breast cancer in developing countries throughout the Asia-Pacific region will continue to increase. Early detection and access to optimal treatment are the keys to reducing breast cancerrelated mortality, but cultural and economic obstacles persist. Consequently, the challenge is to customise breast cancer control initiatives to the particular needs of each country to ensure the best possible outcomes.展开更多

Induce of phase II detoxification enzymes was also determined for each respective vegetable extract. Protection against authentic ONOO- and HOCl mediated cytotoxicity in human colon HCT116 cells was determined using the MTT 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyl tetrasodium bromide) viability assay.RESULTS: All of the extracts derived from green leaf vegetables exhibited antioxidant properties, while also having cytoprotective effects against ONOO- and HOCI mediated cytotoxicity. In addition, evaluation of the phase II enzyme inducing ability of each extract, as assessed by quinone reductase and glutathione- S-transferase activities, showed significant variation between the vegetables analyzed.CONCLUSION: Green leaf vegetables are potential sources of antioxidants and phase II detoxification enzyme inducers in the Asian diet. It is likely that consumption of such vegetables is a major source of beneficial phytochemical constituents that may protect against colonic damage.展开更多

文摘Coronary artery spasm (CAS) is found not only in Asian but also in white patients. CAS can lead to angina pecto- ris, myocardial infarction, ventricular arrhythmias and sud- den cardiac death. But CAS diagnosis is not necessarily easy, and it also remains highly challenging to therapy and prevention. We present a spontaneous extensive-CAS case with typical clinical features of acute myocardial infarction, despite the regular vasodilator treatment.

文摘The S wave velocity structure of the earth below Eastern Southeast Asia has been investigated by analyzing the seismogram from surface wave to multiple depth waves in the time domain and three Cartesian components simultaneously.The wave passes across the front area of subduction zone between the Philippine plate and the Asian plate.The main data are waveform comparisons,instead of the arrival times.The synthetic seismogram is calculated using the GEMINI method.The synthetic seismogram constructed by PREMAN global earth model deviates greatly from the measured one.To solve this problem,corrections are needed for the β speed structure.Corrections cover the gradient change of β_h,which turns from negative to positive in upper mantle layers as in the PREMAN,change of earth crust depth and change of zero order coefficients of β velocity function in all earth mantle layers.So,the fitting is obtained,as well as the arrival time or the waveform of Love and Rayleigh surface waves,the S wave and the repetitive depth waves ScS_2 and ScS_3.This result reveals that the Southeast Asia,being stretched due to tectonic release,has a mantle in some parts with negative anomaly of S wave velocity and vertical anisotropy in all earth mantle layers.

文摘Objective: To provide an overview of the incidence and mortality of female breast cancer for countries in the Asia-Pacific region.Methods: Statistical information about breast cancer was obtained from publicly available cancer registry and mortality databases(such as GLOBOCAN), and supplemented with data requested from individual cancer registries. Rates were directly age-standardised to the Segi World Standard population and trends were analysed using joinpoint models.Results: Breast cancer was the most common type of cancer among females in the region, accounting for 18% of all cases in 2012, and was the fourth most common cause of cancer-related deaths(9%). Although incidence rates remain much higher in New Zealand and Australia, rapid rises in recent years were observed in several Asian countries. Large increases in breast cancer mortality rates also occurred in many areas, particularly Malaysia and Thailand, in contrast to stabilising trends in Hong Kong and Singapore, while decreases have been recorded in Australia and New Zealand. Mortality trends tended to be more favourable for women aged under 50 compared to those who were 50 years or older. Conclusion: It is anticipated that incidence rates of breast cancer in developing countries throughout the Asia-Pacific region will continue to increase. Early detection and access to optimal treatment are the keys to reducing breast cancerrelated mortality, but cultural and economic obstacles persist. Consequently, the challenge is to customise breast cancer control initiatives to the particular needs of each country to ensure the best possible outcomes.
文摘AIM: To evaluate the antioxidant and phase II detoxification enzyme inducing ability of green leaf vegetables consumed in Asia.METHODS: The antioxidant properties of six commonly consumed Asian vegetables were determined using the ABTS, DPPH, deoxyribose, PR bleaching and ironascorbate induced lipid peroxidation assay. Induce of phase II detoxification enzymes was also determined for each respective vegetable extract. Protection against authentic ONOO- and HOCl mediated cytotoxicity in human colon HCT116 cells was determined using the MTT 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyl tetrasodium bromide) viability assay.RESULTS: All of the extracts derived from green leaf vegetables exhibited antioxidant properties, while also having cytoprotective effects against ONOO- and HOCI mediated cytotoxicity. In addition, evaluation of the phase II enzyme inducing ability of each extract, as assessed by quinone reductase and glutathione- S-transferase activities, showed significant variation between the vegetables analyzed.CONCLUSION: Green leaf vegetables are potential sources of antioxidants and phase II detoxification enzyme inducers in the Asian diet. It is likely that consumption of such vegetables is a major source of beneficial phytochemical constituents that may protect against colonic damage.
