使用降钙素原轨迹识别脓毒症亚表型及风险分层研究

背景脓毒症是一种异质性疾病,识别脓毒症亚表型有助于优化脓毒症管理。目的利用降钙素原(PCT)轨迹识别脓毒症亚表型并进行风险分层。方法回顾性分析2021-01-01至2023-08-01宁夏医科大学总医院收住的800例及甘肃省人民医院收住的202例成... 背景脓毒症是一种异质性疾病,识别脓毒症亚表型有助于优化脓毒症管理。目的利用降钙素原(PCT)轨迹识别脓毒症亚表型并进行风险分层。方法回顾性分析2021-01-01至2023-08-01宁夏医科大学总医院收住的800例及甘肃省人民医院收住的202例成年脓毒症患者(年龄>18岁),随机将宁夏医科大学总医院其中597例患者纳入开发队列(60%),另外203例及甘肃省人民医院202例共405例患者纳入验证队列(40%)。首先根据患者28 d生存情况,将开发队列分为存活组与死亡组,分析不同时间PCT测量值(PCT第1天、第3天、第5天、第7天分别标记为PCT d1、PCT d3、PCT d5、PCT d7)对脓毒症的预后价值,并绘制受试者工作特征(ROC)曲线评估预测效能;然后,基于PCT重复测量进行组基轨迹建模以识别脓毒症亚表型,根据PCT变化趋势及临床特征对亚表型进行特征分析,并进行生存分析与风险分层;最后,对预测模型进行验证。结果开发队列中512例存活、85例死亡,总体28 d死亡率为14.2%;验证队列中341例存活、64例死亡,总体28 d死亡率为16.3%。开发队列中死亡组PCT d3、PCT d5、PCT d7高于存活组(P<0.01);ROC曲线结果显示,PCT d7预测效能较高,ROC曲线下面积为0.833。开发队列组基轨迹建模确定了4种脓毒症亚表型:“中起点快速上升型”的特征是呼吸功能障碍;“低起点缓慢下降型”的特征是并发症及危重症评分均较低,视为基线组;“高起点快速下降型”的特征是合并症及危重症评分均较高;“高起点缓慢下降型”的特征是多器官功能障碍,危重症评分较高,视为入院时最严重组。通过对4种亚表型进行生存分析,结果显示“中起点快速上升型”死亡率最高,定义为高危组;其次为“高起点缓慢下降型”,定义为中危组;“低起点缓慢下降型”和“高起点快速下降型”死亡率较低,定义为低危组。验证队列与开发队列PCT轨迹和合并症的相对分布基本一致。结论利用PCT轨迹可以识别脓毒症亚表型,结合PCT数值与变化轨迹可以实现对脓毒症的风险分层,为临床医师利用PCT变化轨迹评估患者预后提供理论依据。 展开更多

人工智能辅助多参数流式细胞术检测淋巴细胞亚群免疫表型的应用分析

探讨淋巴细胞亚群(LS)免疫表型应用人工智能辅助多参数流式细胞术(FCM)检测的价值。方法 选择医院检验科2023年1月~2024年1月采集的外周血样本研究,样本量为1000例,均接受多参数FCM检测LS免疫表型,包括T淋巴细胞、B淋巴细胞、NK细胞等,... 探讨淋巴细胞亚群(LS)免疫表型应用人工智能辅助多参数流式细胞术(FCM)检测的价值。方法 选择医院检验科2023年1月~2024年1月采集的外周血样本研究,样本量为1000例,均接受多参数FCM检测LS免疫表型,包括T淋巴细胞、B淋巴细胞、NK细胞等,分别予以传统人工分析、人工智能辅助分析(基于高斯混合模型的多维数据聚类算法),对两种分析方法的一致性进行评价。结果 人工智能辅助分析结果和传统人工分析结果通过一致性检测有974例、未通过一致性检测有26例,一致性检测通过比例为97.40%;人工智能辅助分析单个样本时间均值为1.39±0.21s,传统人工分析单个样本时间均值为55.63±6.94s,差异显著(P<0.05);人工智能辅助分析与传统人工分析结果在不同LS免疫表型差值上对比,CD3+、CD3+CD8+、CD3+CD4+/CD3+CD8+比值、CD3-CD19+上有明显差异(P<0.05),但其余项目上对比无差异(P>0.05);人工智能辅助分析质控重复系数、95%CI分别为2.8411%、2.7255%~2.9541%,均在±5%可接受临界值范围,提示人工智能辅助分析测定LS免疫表型百分比可快速检出,且具备良好的重复性。结论 LS免疫表型检测中应用人工智能辅助多参数FCM检测,经人工智能辅助分析不仅可快速分析结果,而且分析结果准确度高,有良好的重复性,与传统人工分析结果一致性高,但可明显缩短分析时间,能满足临床应用基本需求,有应用价值。 展开更多

目的观察抑制KLF8基因对肝癌干细胞CD133^+表型细胞亚群比例、裸鼠体内成瘤和肺转移能力的影响。方法收集肝癌患者的肺转移灶,分离培养原代肝癌细胞,无血清悬浮培养形成肝癌细胞球,经流式分选筛选出CD133^+表型的肝癌干细胞细胞亚群(LCS... 目的观察抑制KLF8基因对肝癌干细胞CD133^+表型细胞亚群比例、裸鼠体内成瘤和肺转移能力的影响。方法收集肝癌患者的肺转移灶,分离培养原代肝癌细胞,无血清悬浮培养形成肝癌细胞球,经流式分选筛选出CD133^+表型的肝癌干细胞细胞亚群(LCSCs)。将KLF8基因的RNA干扰重组慢病毒质粒(sh-KLF8)及其阴性对照空载体质粒(sh-control)分别感染LCSCs,经2. 0μg/m L嘌呤霉素筛选,成功构建稳定敲除KLF8基因的CD133^+表型肝癌干细胞株LCSCs_(sh-KLF8)及对照细胞株LCSCs_(sh-control),qRT-PCR法检测KLF8 mRNA,流式细胞术检测CD133^+表型细胞亚群比例。分别注射LCSCs_(sh-KLF8)和LCSCs_(sh-control)构建肝癌干细胞裸鼠移植瘤模型,观察敲除KLF8基因对肝癌干细胞裸鼠体内成瘤能力的影响(成瘤重量)。另将LCSCs_(sh-KLF8)和LCSCs_(sh-control)分别经尾静脉注射到裸鼠体内,构建肝癌干细胞的裸鼠肺转移模型,观察敲除KLF8基因对肝癌干细胞裸鼠体内肺转移能力的影响(肺转移灶个数、KLF8蛋白)。结果与LCSCs_(sh-control)比较,LCSCs_(sh-KLF8)中KLF8 mRNA相对表达水平降低,分选时CD133^+表型干细胞亚群的比例显著降低,裸鼠体内的成瘤重量和肺转移的数量降低,KLF8蛋白阳性着色较少(P均<0. 05)。结论抑制KLF8基因可降低肝癌干细胞CD133^+表型细胞亚群比例及体内成瘤、肺转移能力,KLF8基因可能是抑制肝癌发生和进展的关键分子。 展开更多

目的分析肺癌间质表型亚系A3细胞中上皮间质转化(EMT)相关的miR-194-5p鉴定、分析及其靶基因功能富集的生物学过程和信号通路。方法使用微流控芯片筛选母系肺癌A549细胞获得间质表型亚系A3细胞,利用miRCURYTM芯片筛选差异性表达的miRNA... 目的分析肺癌间质表型亚系A3细胞中上皮间质转化(EMT)相关的miR-194-5p鉴定、分析及其靶基因功能富集的生物学过程和信号通路。方法使用微流控芯片筛选母系肺癌A549细胞获得间质表型亚系A3细胞,利用miRCURYTM芯片筛选差异性表达的miRNA。利用MTT、Transwell实验检测miR-194-5p对亚系细胞增殖、迁移、侵袭能力的影响。应用Miranda、MiRBase、TargetScan数据库和DAVID在线软件对miR-194-5p进行靶基因预测及相关信号通路、生物学功能富集分析。结果成功筛选获得具有间质表型亚系A3细胞,与母系A549细胞相比,在A3亚系中miR-194-5p的表达水平显著下调,转染miR-194-5pmimics后细胞迁移和侵袭能力发生显著降低但增殖未受影响;转染miR-194-5pinhibitor后细胞的迁移和侵袭能力增强。miR-194-5p的48个靶基因主要在蛋白质结合等生物学过程中起作用,信号通路显著富集于TGF-β等EMT相关信号转导通路。结论从微流控芯片获得的间质表型细胞亚系成功筛选EMT相关miRNA,其中miR-194-5p抑制肺癌EMT进程,是潜在的治疗靶点。 展开更多

目的:探讨T细胞表型及Th1/Th2功能亚群在早期稽留流产发病中的免疫机制。方法:选取14例早期稽留流产患者(妊娠8~10周),采用血液自动分析仪检测其外周血中淋巴细胞和粒细胞百分率,采用流式细胞仪检测其外周血中T淋巴细胞及其亚群的百分... 目的:探讨T细胞表型及Th1/Th2功能亚群在早期稽留流产发病中的免疫机制。方法:选取14例早期稽留流产患者(妊娠8~10周),采用血液自动分析仪检测其外周血中淋巴细胞和粒细胞百分率,采用流式细胞仪检测其外周血中T淋巴细胞及其亚群的百分率,用酶联免疫吸附法测定其血清中细胞因子干扰素-γ(Interferon-γ,IFN-γ)和白介素-4(Inter-leukin-4,IL-4)的浓度。同时选取18例正常妊娠早期妇女(妊娠8~10周)检测上述各项指标作为对照。结果:①早期稽留流产患者外周血中淋巴细胞比率29.77±4.37,明显高于对照组的23.4±5.96(P<0.05);粒细胞比率为60.09±4.58,明显低于对照组的67.76±7.31(P<0.05)。②早期稽留流产患者外周血中CD3+细胞和CD4+细胞的百分率以及CD4+/CD8+的比值分别为66.76±8.76、41.01±8.47、1.83±0.50,均显著高于正常妊娠早期妇女的56.72±10.86、31.22±7.70、1.20±0.42(P均<0.05);CD8+细胞的百分率为25.05±5.87,与对照组的27.35±6.28比较,无显著差异。③早期稽留流产患者血清中IFN-γ浓度为28.96±10.76ng/L,明显高于对照组18.11±7.08ng/L(P<0.01);IL-4为13.16±8.37ng/L,显著低于对照组的25.91±15.96ng/L(P<0.05)。结论:稽留流产的发生可能与CD4+/CD8+T细胞比例失调,以及Th1/Th2功能亚群向Th1偏移有关。 展开更多

AIM:To investigate M2 isoform of pyruvate kinase(PKM2) expression in gastric cancers and evaluate its potential as a prognostic biomarker and an anticancer target.METHODS:All tissue samples were derived from gastric cancer patients underwent curative gastrectomy as a primary treatment.Clinical and pathological information were obtained from the medical records.Gene expression microarray data from 60 cancer and 19 noncancer gastric tissues were analyzed to evaluate the expression level of PKM2 mRNA.Tissue microarrays were constructed from 368 gastric cancer patients.Immunohistochemistry was used to measure PKM2 expression and PKM2 positivity of cancer was determined by proportion of PKM2-positive tumor cells and staining intensity.Association between PKM2 expression and the clinicopathological factors was evaluated and the correlation between PKM2 and cancer prognosis was evaluated.RESULTS:PKM2 mRNA levels were increased more than 2-fold in primary gastric cancers compared to adjacent normal tissues from the same patients(log transformed expression level:7.6 ± 0.65 vs 6.3 ± 0.51,P < 0.001).Moreover,differentiated type cancers had significantly higher PKM2 mRNA compared to undifferentiated type cancers(log transformed expression level:7.8 ± 0.70 vs 6.7 ± 0.71,P < 0.001).PKM2 protein was mainly localized in the cytoplasm of primary cancer cells and detected in 144 of 368(39.1%) human gastric cancer cases.PKM2 expression was not related with stage(P = 0.811),but strongly correlated with gastric cancer differentiation(P < 0.001).Differentiated type cancers expressed more PKM2 protein than did the undifferentiated ones.Well differentiated adenocarcinoma showed 63.6% PKM2-positive cells;in contrast,signet-ring cell cancers showed only 17.7% PKM2-positive cells.Importantly,PKM2 expression was correlated with shorter overall survival(P < 0.05) independent of stage only in signet-ring cell cancers.CONCLUSION:PKM2 expression might be an adverse prognostic factor for signet-ring cell carcinomas.Its function and potential as a prognostic marker should be further verified in gastric cancer. 展开更多

A reliable chronology is essentially critical for correlating loess records with other paleoenvironmental time series, as well as for continuing improvements in the reconstruction of paleoenvironment and paleoclimate changes. It is exactly that the scarcity of chronologies across the Sea of Azov has limited the interpretation of climatic and environmental information in the East European Plain. In view of this, this paper conducted an exploratory study to investigate whether the optically stimulated luminescence(OSL) dating of medium-grained quartz could be used to obtain a set of chronologies and the age models could be used to establish an independent time scale since the Late Pleistocene for the Sea of Azov loess. The results showed that an internally consistent set of optical ages for the Azov loess deposited up to ~76 ka. In addition, the ages developed based on magnetic susceptibility and grain size ages models showed good comparability with independent OSL ages at an acceptable range, suggesting that it might be practicable to establish an independent time scale using age models at the Sea of Azov loess, at least for the uppermost part of the Chumbur-Kosa section. Comparison with the ages based on two age models,the grain size ages using fine-grain fractions may provide a more reliable chronological sequence at the Azov loess since the Late Pleistocene. With the help of absolute ages and climate proxies(magnetic susceptibility and grain size), paleoclimatic change in the Sea of Azov have been traced for the Late Pleistocene. 展开更多

Pseudomonas stutzeri caused an outbreak of freshwater fish in Luwuk Banggai (tilapia and catfish), Bali (tilapia), Jambi (tilapia and catfish) and Tanjung Pinang (catfish). The study was purposed to comprehensively identify special phenotypic and genotypic characteristics of P. stutzeri isolated from several areas in Indonesia, including its morphometric and biochemical characteristics and molecular variation. Bacteria were isolated from internal organs (kidney, ulcer and eye) of fish. They were then identified using morphology and biochemical test. DNA isolates were entirely extracted, amplified and reversed on 16S rRNA region, and further then were sequenced. Phylogenetic trees of bacteria were constructed using neighbor-joining and maximum-parsimony methods. The colony were similar, such as rod shape (Jambi, Tanjung Pinang, Bali), bacil shape (Luwuk Banggai), transparant in tryptic soy agar (TSA) (Luwuk Banggai), creamy beige in glutamate starch phenol red (GSP) (Bali), gram negative, motile, no reaction in the oxidative-fermentative test, positive result in catalase and oxidase test, negative in lysine decarboxylase and ornithine decarboxylase test and positive result in indole test; gelatin was degraded (only Bali), urea was not degraded, no color change in Methyl-red and Voges-proskaeur (MR-VP) test; acid not produce from glucose, inositol or sucrose. Citrate was utilized by some isolates: positive (Jambi, Tanjung Pinang) and negative (Bali, Luwuk Banggai). Results showed us that isolates of Jambi, Bali and Tanjung Pinang were monophyletic species with P. stutzeri $8 and ZH-1 comparing to gen bank. However, merely phenotypic analysis among Pseudomonas sp. was confused compared to each other. 展开更多

A comparative study on the phenotypic and genetic characteristics among Acidithiobacillus ferrooxidans (AF2), a typic strain ATCC23270 and a previously isolated strain AF3 was performed. AF2 can use ferrous ion (Fe^2+) or elemental sulfur (S^0) as sole energy source, but oxidizes So more effectively than Fe^2+, which is different from ATCC23270 and AF3. The G+C content of AF2 is 51.8% (molar fraction), however, ATCC23270 and AF3 strains have G+C content of 63.7% and 64.8% (molar fraction), respectively. The DNA-DNA hybridization results show that AF2 has 41.53% and 52.38% genome similarity to ATCC 23270 and AF3, respectively, but AF3 has a high genome similarity of 89.86% to ATCC 23270 strain. Rusticyanin (rus) and subunit III of aa3-type cytochrome oxidase (coxC) genes are not detected in AF2, but Fe^2+ oxidase (iro) gene can be detected. To understand the genomic organization of iro gene, a cosmid library of AF2 genome was constructed and iro gene-containing clone was screened. The sequencing result shows that although the nucleotide sequence of iro gene in AF2 is completely identical to that of ATCC 23270 strain, its genomic organization is different from that of ATCC 23270. In AF2, iro is located at downstream ofpurA gene, while it is located at downstream ofpetC-2 gene in ATCC 23270 strain. These results indicate that AF2 is a novel strain ofA. ferrooxidans, and that phenotypic differences among the strains ofA. ferrooxidans are closely correlated with their genetic polymorphisms. 展开更多

The considerable plasticity of its shoot and root structure helps it to capture light more effectively in winter and early spring, and to adapt to the soil moisture conditions in its narrow habitat. Another medicinal plant, Anthriscus sylvestris, is of similar economic importance but commonly distributed widely. In contrast to C. smyrnioides, it has low structural plasticity. Symbiotic and phenotypic characteristics of thirty rhiobial isolates obtained from root nodules of two cowpea (Vigna unguiculata L. Walp) cultivars that grown in different sites of Fezzan (Southern part of Libya) were studied. Cultural characteristics and cross-nodulation with Arachis hypogega and Faidherbia albida showed that they were slow-growing rhizobia. Each isolate was found to coexist with non-symbiotic bacteria similar in their cultural characteristics to fast-growing rhizobia. All isolates formed symbiosis with the test plants, but different in their nitrogen-fixation efficiency. Numerical analysis of phenotypic characteristics showed that at boundary level of 70% average similarity, the isolates formed four distinguished groups and two isolates remained separate. Most isolates exhibited wide tolerance to acidity, alkalinity and extreme temperatures. They also resistant to some heavy metals such as mercury, copper, zinc, lead, cadmium and aluminum at low concentrations and antibiotics like polymyxin, colistin, bacitracin and nalidixic acid. Isolates displayed different response to salinity ranging from sensitive, which unable to grow in 1% NaCI to resistant and grow at 2% NaCl or above. Urea was hydrolyzed by most of them and carbohydrates utilizations were different. Sucrose and maltose were metabolized by most of the test isolates, whereas, monosaccharide and sugar alcohols were poorly utilized. 展开更多

The transformer-2(tra-2) gene plays a key role in the regulatory hierarchy of sexual differentiation in somatic tissues and in the germline of Drosophila melanogaster.In this study,sequences and expression profiles of tra-2 in the Chinese mitten crab Eriocheir sinensis were characterized.Four tra-2 isoforms,designated as Estra-2a,Estra-2b,Estra-2c,and Estra-2d,were isolated.They all contained an RNA-recognition motif(RRM) and a linker region,which shared high similarity with other reported tra-2s.Sequence analysis revealed that Estra-2a,Estra-2b and Estra-2c are encoded by the same genomic locus and are generated by alternative splicing of the pre-mRNA.Compared with the other three isoforms,Estra-2d lacks the RS2 domain.Quantitative real-time PCR showed that all four isoforms were highly expressed in the fertilized egg,and in the 2-4 cell and blastula stages compared with larval stages(P<0.01),suggesting their maternal origin in early embryonic developmental stages.Notably,Estra-2a was highly expressed in male somatic tissues,while Estra-2c was significantly highly expressed in the ovary.These results suggest that Estra-2c is involved in sexual differentiation of the Chinese mitten crab.Our findings provide basic information for further functional studies of the tra-2 gene/protein in this species. 展开更多
