为了定量研究空管人误致因因素,分析各人误因素关联度和验证其可靠性,达到控制人误的最终目的而开展此研究。采用D.A.Wiegmann等人的"人误分析分类系统"对空管不安全事件进行分类,运用灰色关联理论进行关联度分析。影响人误... 为了定量研究空管人误致因因素,分析各人误因素关联度和验证其可靠性,达到控制人误的最终目的而开展此研究。采用D.A.Wiegmann等人的"人误分析分类系统"对空管不安全事件进行分类,运用灰色关联理论进行关联度分析。影响人误的主因素次序为组织影响、不安全行为的前提条件、不安全行为。采用灰色关联理论所得到的计算结果与Shappell等人采用专家函询方法对飞行机组所进行的研究结果基本一致;在不安全行为层面,导致不安全事件的人误因素顺序是技能差错、决策差错、违规;在团队层面,班组内部的沟通与协调不畅导致的不安全事件在各个飞行阶段都存在;在不安全行为的前提条件层面,环境因素导致的不安全事件所占比例最大。 展开更多

The combined moving mechanism is composed of sprocket wheel and drum-like small wheel installed on the chain. It can make the robot move independently in the direction parallel to the tubes and in the direction perpendicular to the tubes. This paper made a detailed discussion on the composition of the combined moving mechanism, the design method of the conjugate outline curve and the circular-arc outline curve of the drum-like small wheel that meshes with the tubes. Error analysis is directed to the propagations and accumula-tions of various errors and their effect on assembly success.Using the screw coordinates, assembly errors are represented as 'error twist', the extremely compact expression. According to the law of screw composition, relative position and orientation errors of mating parts are computed and the necessary condition of assembly success is concluded. A new simple method for measuring assembly errors is also proposed based on the transformation law of a screw.Because of the compact representation of error, the model presented for error analysis can be applied to various part- mating types and especially useful for error analysis of complexity assembly. 展开更多

Accident at Fukushima Dai-ichi nuclear power plant significantly affected the nuclear industry at time when everybody was expecting the so called nuclear renaissance. There is no question that the accident has at least slowed it down. Research on this accident is taking place all over the world, in this paper, we present the findings of research on Fukushima nuclear power plant accident in relation to the Czech Republic. The paper focuses on the analysis of human performance during the accident. Lessons learned from the accident and main human errors are presented. First, the brief factors affecting the human performance are discussed. They are followed by the short description of activities on units 1-3. The key human errors in the accident mitigation are then identified. On unit 1, the fuel damage was probably impossible to prevent, however, on units 2 and 3, it could be probably prevented. 展开更多

Human reliability analysis(HRA) is an expansion of man-machine engineering. It is also a new multidisciplinary based on behavioral science, cognitive science, information processing, system analysis and probability statistics in order to analyze, predict, reduce and prevent human errors. Firstly, the quantitative analysis model of HRA is proposed based on Markov process theory by using human error probability(HEP) and error correction cycle(ECC) as parameters. And human reliability evaluation criterion is built. Then, the HRA process considering error correction is proposed based on cognitive reliability and error analysis method(CREAM). Finally, according to the characteristics of armored vehicle system, common performance condition(CPC) in CREAM is improved.A reliability impact index is characterized by the overall contexts of tasks. Human reliability evaluation criterion of armored vehicle system is formulated. And the result of HRA is obtained based on the method presented in this paper. In addition, the relative weights are estimated by combining scale of 10/10—18/2 and analytical hierarchy process(AHP), and the triangular fuzzy number considering confidence factor and optimism index is adopted in order to reduce the subjectivity. The analysis results show that the method presented in this paper is reasonable and feasible. Meantime, the method can provide guidance for human reliability analysis of other weapon systems. 展开更多
