目的分析暂时性内支架治疗食管良性狭窄疗效,为食管良性狭窄提供有效的介入治疗方法。对象与方法12例食管良性狭窄患者食管最窄处直径1mm~5mm,吞咽困难评分为3~4级。在X线下置入可扩张金属带膜内支架,术后3~7天由胃... 目的分析暂时性内支架治疗食管良性狭窄疗效,为食管良性狭窄提供有效的介入治疗方法。对象与方法12例食管良性狭窄患者食管最窄处直径1mm~5mm,吞咽困难评分为3~4级。在X线下置入可扩张金属带膜内支架,术后3~7天由胃镜取出内支架。结果12例患者支架置入和支架取出成功率均为100%。操作死亡率为0%。支架置入后吞咽困难明显好转,吞咽困难评分0级-1级,平均0.6±0.4级;支架取出后吞咽困难评分0级~2级,平均0.8±0.5级。支架置入后管腔直径16~22mm,平均18.4±2.9mm。支架取出后一周内管腔直径10mm~18mm,平均14.5±2.7mm。患者术后随访3~50个月。随访期内管腔直径未见明显改变,吞咽能力在随访3个月内逐渐好转,3个月后处于维持状态。 展开更多

In recent decades, the outcomes of coronary heart disease (CHD) have markedly improved, which can be partly attributed to the use of novel drugs (especially statins and antiplatelet drugs) and partly to the evolution ofpercutaneous coronary intervention (PCI). From percutaneous transluminal coronary angioplasty to bare-metal stent and then to drug-eluting stent, every step of PCI is attractive to interventional cardiologist, great progress has been made for patients with CHD. In the past few years, some successor devices for treating CHD have cmerged. Undoubtedly, drug-coated balloon (DCB), which was recommended by 2014 ESC Guidelines on myocardial revascularization, is a "shining star" among them. DCB involves a semi-compliant angioplasty balloon coated with an anti-proliferative agent that can exert antirestenotic efficacy by permeating into the vessel wall during balloon contact. This review discusses the conception and merits, preclinical data, emerging clinical indications, and results from clinical trials of this novel interventional technology. Although DCB has shown authentic efficacy in the treatment ofin-stent restenosis, its use in de novo coronary lesions is still in dispute. Objectives To evaluate the very long-term safety and effectiveness of drug-eluting stents (DES) compared to bare-metal stents (BMS) for patients with large coronary vessels. Methods From April 2004 to October 2006... Objectives To evaluate the very long-term safety and effectiveness of drug-eluting stents (DES) compared to bare-metal stents (BMS) for patients with large coronary vessels. Methods From April 2004 to October 2006, 2407 consecutive patients undergoing de novo lesion percutaneous coronary intervention with reference vessel diameter greater than or equal to 3.5 mm at Fu Wai Hospital in Beijing, China, were prospectively enrolled into this study. We obtained 9-year clinical outcomes including death, myocardial infarction (MI), thrombosis, target lesion revascularization (TLR), target vessel revascularization (TVR), and major adverse cardiac events (MACE, the composite of death, MI, and TVR). We performed Cox's proportional-hazards models to assess relative risks of all the outcome measures after propensity match. Results After propensity scoring, 514 DES-treated patients were matched to 514 BMS-treated patients. The patients treated with BMS were associated with higher risk ofTLR (HR: 2.55, 95%CI: 1.520-4.277, P = 0.0004) and TVR (HR: 1.889, 95%CI: 1.185-3.011, P = 0.0075), but the rates of death/MI and MACE were not statistically different. All Academic Research Consortium definition stent thrombosis at 9-year were comparable in the two groups. Conclusions During long-term follow-up through nine years, use of DES in patients with large coronary arteries was still associated with significant reductions in the risks of TLR and TVR. 展开更多

OBJECTIVE: To assess the immediate- and long-term outcomes of stent supported coronary angioplasty in patients with severe left ventricular dysfunction. METHODS: Seventy-four consecutive patients with angiographic left ventricular ejection fractions or = 1 was seen in 29 (73%) of 40 successfully treated congestive heart failure patients at 6 months after the procedure. During long-term follow-up, 58 (87.9%) of 66 patients with clinical success were alive, including 44 (68.6%) free from cardiac events. CONCLUSIONS: Patients with severe left ventricular dysfunction treated with stent supported PTCA experience a high rate of success, low procedure related mortality and satisfactory long-term survival. 展开更多
