Cytomegalovirus(CMV)infection of the gastrointestinal (GI)tract has been reported in both immunocompetent and,more frequently,in immunocompromised patients.We describe a case of a 19-year-old male who developed CMV infection of the terminal ileum while receiving immunosuppression for lupus nephritis. This was a distinctly unusual site of infection which clinically mimicked Crohn's ileitis.We note that reports of terminal ileal CMV infection have been infrequent. Despite a complicated hospital course,ganciclovir therapy was effective in resolving his symptoms and normalizing his ileal mucosa.This report highlights the importance of accurate histological diagnosis and clinical follow-up of lupus patients with GI symptoms undergoing intense immunosuppression. 展开更多

The paper uses the idea of mechanism design and simulation to analyze and solve environmental pollution improvement. By establishing a simple environmental pollution improvement model, the paper transforms pollution control into binary decision established by a sewage treatment plant and different decisions by the fisherman on the establishment of sewage treatment plants. Extramedullary hematopoiesis consists in the appearance and proliferation of hematopoietic cells outside the bone marrow. In this article, the authors describe a case of hepatosplenic hematopoiesis in a 9-year-old, male Goeldi's monkey concurrent with a Calodium hepaticum infestation, belonging to the Lisbon's Zoo primate collection (Portugal). Lesions were identified upon necropsy after euthanasia due to the presence of an apparently non-excisable, metastatic aortic mass. Histopathological analysis of samples taken was carried out and immunohistochemical staining was used to characterize the cellular population involved, confirming the diagnosis of extramedullary hematopoiesis. To the best of the authors' knowledge, this is the first report of hepatosplenic extramedullary hematopoiesis in a Goeldi's monkey. 展开更多

Hepatic involvement in aggressive systemic mastocytosis(ASM) is relatively common, and the main clinical features of this disease include hepatomegaly, portal hypertension, ascites, and fibrosis. Cirrhosis is a rare ASM symptom. We report an ASM case that initially mimicked cirrhosis based on clinical and radiographic analyses. The portal tract was expanded by mononuclear inflammatory cells, and an increase in collagen amount was observed in routine histological sections of the biopsied liver. A diagnosis of systemic mastocytosis(SM) was made after ancillary tests for mast cells using bone marrow aspirates. Extensive involvement of the liver and gastrointestinal tract was observed. Clinicians and pathologists need to consider ASM as a diagnosis or differential diagnosis in a clinical case of cirrhosis with unknown etiology. The diagnosis can be confirmed or disregarded by immunohistochemical staining and molecular analysis. 展开更多

The solution of this eq... In this work the authors simulate a contaminant transport problem in three dimensions that takes place in the soil of waste disposals. Such problem is modeled by a diffusion-dominated equation. The solution of this equation is addressed by using mixed finite element method for the spatial discretization of the equation. The resulting linear algebraic system is handled by an iterative domain decomposition procedure. This procedure is naturally parallelizable, and permits to implement computational codes in distributed memory machines in order to save on CPU time. Numerical results of the serial and parallel codes were compared with experimental results, and their performance measures were evaluated. The results indicate that the parallelizable procedure is an efficient tool for performing simulations of the problem. 展开更多

Patch models involve explicit movements of people between distinct locations. The aim of the present work has been designed and explored a patch model with population mobility between different patches and between each patch and an external population. The authors considered a SIR (susceptible-infected-recovered) scheme. The model was explored by computer simulations. The results show how endemic levels are reached in all patches of the system. Furthermore, the performed explorations suggest that the people mobility between patches, the immigration from outside the system and the infection rate in each patch, are factors that may influence the dynamics of epidemics and should be considered in health policy planning. 展开更多

When the liquid propellant thruster works,its plume field would contain many propellant liquid droplets,especially at pulse state.Liquid droplets may move along with the gas flow and deposit on the components of spacecraft as contamination.The simulation of the plume field involving the gas molecules and liquid droplets is an important part in contamination studies of thruster plume.Based on the PWS software developed by Beihang University(BUAA),axial-symmetric two-phase direct simulation Monte Carlo(DSMC) method is used with the liquid droplet taken as a kind of solid particle.The computation of gas-to-particle effect and gas reflection on the particle surface are decoupled.The inter-particle collision is also considered.The gas parameters at nozzle exit of 120N engine after 20 ms pulse work are taken as the entrance condition of the numerical simulation.Four test cases are conducted for comparison of different collision modules.Simulation results show that the effects of liquid propellant droplets mainly concentrate near the axis line of engine.The particle-to-gas collision would cause evident differences in the gas field and subtle differences in the particle phase.The liquid droplets in the plume field are generally accelerated and convected by the gas molecules.The DSMC method is proved to be a feasible solver to numerically simulate the two-phase flow involving solid phase and rarefied gas flow. 展开更多

Monitoring air pollution in China from geostationary satellite: A synthetic study using simulated observations

We simulated geostationary satellite observations to assess the potential for high spatial-and temporal-resolution monitoring of air pollution in China with a focus on tropospheric ozone(O_3), nitrogen dioxide(NO_2), sulfur dioxide(SO_2), and formaldehyde(HCHO). Based on the capabilities and parameters of the payloads onboard sun-synchronous satellites, we simulated the observed spectrum based on a radiative transfer model using a geostationary satellite model. According to optimal estimation theory, we analyzed the sensitivities and retrieval uncertainties of the main parameters of the instrument for the target trace gases. Considering the retrieval error requirements of each trace gas, we determined the major instrument parameter values(e.g., observation channel, spectral resolution, and signal-to-noise ratio). To evaluate these values, retrieval simulation was performed based on the three-dimensional distribution of the atmospheric components over China using an atmospheric chemical transportation model. As many as 90% of the experiments met the retrieval requirements for all target gases. The retrieval precision of total-column and stratospheric O_3 was 2%. In addition, effective retrieval of all trace gases could be achieved at solar zenith angles larger than 70°. Therefore, the geostationary satellite observation and instrument parameters provided herein can be used in air pollution monitoring in China. This study offers a theoretical basis and simulation tool for improving the design of instruments onboard geostationary satellites. 展开更多
