Most anemia is related to the digestive system by dietary deficiency, malabsorption, or chronic bleeding. We review the World Health Organization definition of anemia, its morphological classifi cation (microcytic, macrocytic and normocytic) and pathogenic classi-fication (regenerative and hypo regenerative), and integration of these classifications. Interpretation of laboratory tests is included, from the simplest (blood count, routine biochemistry) to the more specific (iron metabolism, vitamin B12, folic acid, reticulocytes, erythropoietin, bone marrow examination and Schilling test). In the text and various algorithms, we propose a hierarchical and logical way to reach a diagnosis as quickly as possible, by properly managing the medical interview, physical examination, appropriate laboratory tests, bone marrow examination, and other comple-mentary tests. The prevalence is emphasized in all sections so that the gastroenterologist can direct the diagnosis to the most common diseases, although the tables also include rare diseases. Digestive diseases potentially causing anemia have been studied in preference, but other causes of anemia have been included in the text and tables. Primitive hematological diseases that cause anemia are only listed, but are not discussed in depth. The last section is dedicated to simplifying all items discussed above, using practical rules to guide diagnosis and medical care with the greatest economy of resources and time. 展开更多

Objective To investigate the possible involvement of erythropoietin (EPO)/erythropoietin receptor (EPOR) system in neovascularization and vascular regeneration in diabetic retinopathy (DR). Methods EPOR positive circulating progenitor cells (CPCs: CD34^+) and endothelial progenitor cells (EPCs: CD34^+KDR^+) were assessed by flow cytometry in type 2 diabetic patients with different stages of DR. The cohort consisted of age- and sex-matched control patients without diabetes (n=7), non-prolif- erative DR (NPDR, n=7), proliferative DR (PDR, n=8), and PDR complicated with diabetic nephropathy (PDR-DN, n=7). Results The numbers of EPOR^+ CPCs and EPOR^+ EPCs were reduced remarkably in NPDR corn pared with the control group (both P(0.01), whereas rebounded in PDR and PDR-DN groups in varying degrees. Similar changes were observed in respect of the proportion of EPOR^+ CPCs in CPCs (NPDR vs. control, P(0.01) and that of EPOR^+ EPCs in EPCs (NPDR vs. control, P〈0.05). Conclusion Exogenous EPO, mediated via the EPO/EPOR system of EPCs, may alleviate the impaired vascular regeneration in NPDR, whereas it might aggravate retinal neovascularization in PDR due to a rebound of EPOR^+ EPCs associated with ischemia. 展开更多

AIM: To investigate the expression of Erythropoietin (Epo) and its receptor (EpoR) in gastric adenocarcinoma (GAC) and the correlation with angiogenesis and clinicopathological features. METHODS: The expressions of Epo, EpoR and vascular endothelial growth factor (VEGF), as well as mi-crovessel density were evaluated in 172 GAC biopsies by immunohistochemical staining. The correlations between these parameters and patient’s clinicopathological features were analyzed statistically. RESULTS: The proportion of Epo and EpoR alterations in GAC was higher than that in adjacent normal mucosa (P = 0.035 and 0.030). Epo high-expression was associ-ated with EpoR high-expression, Lauren type, extensivelymph node metastasis and advanced stage of GAC (P = 0.018, 0.018, 0.004 and 0), while EpoR expression was linked with older age, World Health Organization type, extensive lymph node metastasis and advanced stage (P = 0.001, 0.013, 0.008 and 0.001). VEGF high expression was significantly correlated with EpoR low-expression, Lauren type, extensive lymph node metastasis and advanced stage (P = 0.001, 0.001, 0.001 and 0.007). The expression of Epo or EpoR was associated with microvessel density (P = 0.004 and 0.046). On multivariate analysis, only lymph node metastasis, abnormal Epo expression and tumor nodes metastases stage were independently associated with survival. In addition, a strong association with the immunohistochemical expression of EpoR and the angiogenic protein, VEGF, was noted. CONCLUSION: Increased expression of Epo and EpoR may play a signif icant role in the carcinogenesis, angiogenesis and progression of GAC. Epo may be an inde-pendent prognostic factor. 展开更多

This 6-week study was conducted to evaluate the effects of seven different levels of dietary chromium (Cr) (0,75,150,300,450,600,and 1200 ppb Cr) in the form of Cr nanoparticle (CrNano) on growth,body composition,serum hormones and tissue Cr in Sprague-Dawley (SD) rats. Seventy male SD rats (average initial body weight of (83.2±4.4) g) were randomly assigned to seven dietary treatments (n=10). At the end of the trial,body composition was assessed via dual energy X-ray absorptiometry (DEXA). All rats were then sacrificed to collect samples of blood,organs and tissues for determination of serum hormones and tissue Cr contents. The results indicated that lean body mass was significantly increased (P<0.05) due to the addition of 300 and 450 ppb Cr from CrNano. Supplementation of 150,300,450,and 600 ppb Cr decreased (P<0.05) percent body fat significantly. Average daily gain was increased (P<0.05) by addition of 75,150,and 300 ppb Cr and feed efficiency was increased (P<0.05) by supplementation of 75,300,and 450 ppb Cr. Addition of 300 and 450 ppb Cr decreased (P<0.05) the insulin level in serum greatly. Cr contents in liver and kidney were greatly increased (P<0.05) by the addition of Cr as CrNano in the dosage of from 150 ppb to 1 200 ppb. In addition,Supplementation of 300,450,and 600 ppb Cr significantly increased (P<0.05) Cr content in the hind leg muscle. These results suggest that supplemental CrNano has beneficial effects on growth performance and body composition,and increases tissue Cr concentration in selected muscles. 展开更多

Carotenoids, a class of natural pigments found in all photosynthetic organisms, are involved in a variety of physiological processes, including coloration, photoprotection, biosynthesis of abscisic acid (ABA) and chloroplast biogenesis. Although carotenoid biosynthesis has been well studied biochemically, the genetic basis of the pathway is not well understood. Here, we report the characterization of two allelic Arabidopsis mutants, spontaneous cell death1-1 (spcl-1) and spc1-2. The weak allele spc1-1 mutant showed characteristics of bleached leaves, accumulation of superoxide and mosaic cell death. The strong mutant allele spc1-2 caused a complete arrest of plant growth and development shortly after germination, leading to a seedling-lethal phenotype. Genetic and molecular analyses indicated that SPC1 encodes a putative ζ-carotene desaturase (ZDS) in the carotenoid biosynthesis pathway. Analysis of carotenoids revealed that several major carotenoid compounds downstream of SPC 1/ZDS were substantially reduced in spc1-1, suggesting that SPC 1 is a functional ZDS. Consistent with the downregulated expression of CAO and PORB, the chlorophyll content was decreased in spc1-1 plants. In addition, expression of Lhcb1. 1, Lhcbl. 4 and RbcS was absent in spc1-2, suggesting the possible involvement of carotenoids in the plastid-to-nucleus retrograde signaling. The spc1-1 mutant also displays an ABA-deficient phenotype that can be partially rescued by the externally supplied phytohormone. These results suggest that SPC1/ZDS is essential for biosynthesis of carotenoids and plays a crucial role in plant growth and development. 展开更多

Control and inadequate treatment leads to a worse quality of life and increased morbidity and hospitalization. Blood loss, and to a lesser extent, malabsorption of iron are the main causes of iron def iciency in IBD. There is also a variable component of anemia related to chronic inflammation. The anemia of chronic renal failure has been treated for many years with recombinant human erythropoietin (rHuEPO), which significantly improves quality of life and survival. Subsequently, rHuEPO has been used progressively in other conditions that occur with anemia of chronic processes such as cancer, rheumatoid arthritis or IBD, and anemia associated with the treatment of hepatitis C virus. Erythropoietic agents complete the range of available therapeutic options for treatment of anemia associated with IBD, which begins by treating the basis of the inflammatory disease, along with intravenous iron therapy as f irst choice. In cases of resistance to treatment with iron, combined therapy with erythropoietic agents aims to achieve near-normal levels of hemoglobin/hematocrit (11-12 g/dL). New formulations of intravenous iron (iron carboxymaltose) and the new generation of erythropoietic agents (darbepoetin and continuous erythropoietin receptor activator) will allow better dosing with the same eff icacy and safety. 展开更多

Objective To investigate the distribution of erythropoietin (EPO) and erythropoietin receptor ( EPOR ) expression in the postnatal rat retina development. Methods Forty-two male Sprague-Dawley rats were divided into 7 groups according to their various postnatal days: postnatal 1 d (D1 group), 3 d (D3 group), I week (W1 group), 2 weeks (W2 group), 3 weeks (W3 group), 4 weeks (W4 group) and8 weeks (W8 group) ( n = 6 ). Single eye was randomly chosen from each rat for the study. The retinal sections were stained with hematoxylin and eosin (HE) and used for the retina development observation. Immunohistochemical staining was used to localize EPO and EPOR expressions in retinas.of differentstages of development, and the expression intensities were determined by an image plus 4 program~~ Results The retinal inner nuclear layer (INL) and outer nuclear layer (ONL) were mixed together and had not yet fully differentiated in D1 and D3 groups. The INL and ONL formed their own independent regions and the outer plexiform layer (OPL) appeared between two layers in W1 group. With the postnatal retinal development, the inner plexiform layer ( IPL ) , rods and cones layer ( RCL ), and OPL were gradually widened and stabilized in W2 to W3 groups. EPO/EPOR expressions located prominently in the inner part of the postnatal rat developing retinas. The expression of EPO in GCL and INL gradually increased from DI to W4, then the expression decreased in W8. Expression of EPOR in GCL gradually increased from DI to WI , then decreased in W2 ; and it gradually increased again from W3 to W8. Expression of EPOR in INL gradually increased from D1 to W1, then decreased in W2 ; and it continued to decrease from W3 to W8. Expression of EPOR in the external segment of RCL gradually increased from D1 to W8. However, expression in the internal segment of RCL gradually decreased from D1 to W3 , then no obvious expression was seen in the internal segment of RCL in W4 and W8. Conclusion EPO/EPOR expressions locate prominently in the inner part of the postnatal rat developing retina. And EPO/EPOR expressions in the rat retinas exist the dynamic changes during the postnatal retina development period. 展开更多

The structure of 3 was characterized by IR, 1H NMR and MS, and the absolute configuration was further confirmed by X-ray crystal structure analysis. Compound 3 crystallizes in monoclinic, space group P21 with a = 11.030(2), b = 7.783(2), c = 12.797(2) , b = 93.44(1)o, Z = 2, Mr = 422.53 (C24H26N2O3S), V = 1096.3(6) 3, = 0.177 mm-1, Dc = 1.280 g/cm3, F(000) = 448 and T = 293(2) K. Objective: To observe the clinical effect of electroacupuncture in treating perimenopause syndrome (PMS) and explore its possible action mechanism. Methods: All of 110 cases with PMS were randomized into group A [Guanyuan (CV 4) and Sanyinjiao (SP 6)] and group B [Neiguan (PC 6) and Zusanli (ST 36)], 55 cases in each. After 4-week treatments, the Kupperman Menopausal Index (KMI) was adopted to evaluate the therapeutic effect. Estradiol (E2), follicle-stimulating hormone (FSH) and luteinizing hormone (LH) were assayed respectively before and after treatments. Results: The KMI decreased by 52.6% in group A, versus 47.4% in group B. The total effective rate was 93.6% in group A, versus 85.7% in group B, and the difference was not significant (P〈0.05). The content of E2 both increased in the two groups after 4-week treatments (P〈0.05); the contents of FSH and LH dropped but without statistical difference (P〈0.05). We investigated the androgen receptor(AR) gene mutation profiles of Chinese patients exhibiting severe androgen insensitivity syndrome(AIS) phenotypes. The present study enrolled 28 patients with genetically diagnosed AIS, who presented with severe phenotypes(Prader grade 0–3). Patients and some family members were screened via amplification and sequencing of their AR exons 1–8, including the corresponding intronic flanking regions. Luteinizing(LH), follicle-stimulating(FSH), and testosterone(T) hormone levels were found to be slightly, but not significantly, higher in patients with complete androgen insensitivity syndrome(CAIS) than in patients with partial androgen insensitivity syndrome(PAIS)(P>0.05). We identified 24 different AR mutations, including 12 that were novel. Ten patients(cases 2, 3, 10, 28, 11, 12, 19, 20, 24, and 25) were found to carry five recurrent mutations(p.Y572 S, p.P914 S, p.S176 R, p.Y782 N, and p.R841H); of these, p.Y572 S, p.S176 R, and p.Y782 N were novel. Among the mutations identified in patients with CAIS, six(66.7%) were characterized as single-nucleotide missense mutations, and six(66.7%) were found to be located in the AR ligand-binding domain(LBD). Among the mutations identified in patients with PAIS, 15(93.8%) were found to be missense, and 11(68.8%) were found to be located in the LBD. Patients 10 and 28 were determined to harbor the same missense mutation(p.P914S), but were diagnosed with CAIS and PAIS, respectively.Sex hormone levels were slightly, but not significantly, elevated in patients with CAIS compared to those with PAIS. Missense mutations spanning AR exons 1–8 were the predominant form of identified mutations, and these were mostly located in the AR LBD. Approximately 50% of the identified mutations were novel, and have enriched the AR gene-mutation database. Patients harboring identical mutations were in some instances found to exhibit divergent phenotypes. 展开更多

Objective: To observe the impact of electroacupuncture on the reproductive function of hydrocortisone acetate modeling rats. Methods: Thirty-six male SD rats were randomly allocated into three groups. The rats in the control group (n=6) underwent no treatment. The rats in the model group (n=15) were induced by hydrocortisone and not treated. The rats in the electroacupuncture group (n=15) were induced by hydrocortisone and treated with electroacupuncture. The luteinizing hormone (LH) and the testosterone (T) were tested with radioimmunoassay, while the hematoxylin-eosin (HE) staining was used for histological observation. Results: Compared with the pre-modeling, the serum T decreased significantly after the modeling (P0.05), and the LH increased without significant difference. The delamination of spermatids reduced dramatically. After the treatments, the serum LH of the rats in the electroacupuncture group were significantly higher than the rats in the model group (P0.05) but the serum T increased without significant difference. The delamination of spermatids increased notablely (P0.05). Conclusion: Electroacupuncture may regulate the reproductive function of the rats by improving the level of LH and T concentration. 展开更多

To promote spinosad biosynthesis by improving the limited oxygen supply during high-density fermentation of Saccharopolyspora spinosa, the open reading frame of the Vitreoscilla hemoglobin gene was placed under the control of the promoter for the erythromycin resistance gene by splicing using overlapping extension PCR. This was cloned into the integrating vector pSET152, yielding the Vitreoscilla hemoglobin gene expression plasmid pSET152EVHB. This was then introduced into S. spinosa SP06081 by conjugal transfer, and integrated into the chromosome by site-specific recombination at the integration site ФC31 on pSET152EVHB. The resultant conjugant, S. spinosa S078-1101, was genetically stable. The integration was further confirmed by PCR and Southern blotting analysis. A carbon monoxide differential spectrum assay showed that active Vitreoscilla hemoglobin was successfully expressed in S. spinosa S078-1101. Fermentation results revealed that expression of the Vitreoscilla hemoglobin gene significantly promoted spinosad biosynthesis under normal oxygen and moderately oxygen-limiting conditions (P〈0.01). These findings demonstrate that integrating expression of the Vitreoscilla hemoglobin gene improves oxygen uptake and is an effective means for the genetic improvement of S. spinosa fermentation. Neogene eolian successions are one of the most important terrestrial palaeoenvironmental archives in East Asia.However, they have received far less attention than Quaternary loess deposits, especially in the case of lipid biomarker analysis.In order to obtain a better insight into the early-middle Miocene palaeoenvironment, we conducted a study of n-alkane biomarkers from sediments of the QA-I section(Qinan) in the western Chinese Loess Plateau, and compared the results with those of previous n-alkane analyses of eolian and aquatic sediments of varying age. Our principal results are as follows:(1) All QA-I samples contain n-alkanes ranging from C_(14) to C_(35), among which the relative content of short-chain n-alkanes(C_(14)–C_(20)) from microorganisms is significantly greater than that of long-chain n-alkanes(C_(26)–C_(35)) from the waxes of terrestrial higher plants;the main peak is at C_(16)–C_(18). All samples have a relatively lower abundance of medium-chain n-alkanes(C_(21)–C_(25)) than that of long-and short-chain n-alkanes, similar to strongly weathered palaeosols in Quaternary loess and Late Miocene-Pliocene Hipparion Red-Earth; however, this distribution is significantly different from that in weakly-weathered loess of Quaternary loess and Late Miocene-Pliocene Hipparion Red-Earth, as well as from aquatic sediments.(2) Despite some odd-over-even carbon predominance of long-chain n-alkanes in the QA-I samples, the carbon preference index(CPI) values are significantly lower than those of most of the weakly-weathered sediments. Our results show that strong weathering and microbial processes have significantly altered the n-alkanes in the Miocene eolian deposits in Qinan, and led to a significant oxidation and degradation of long-chain n-alkanes and the predominance of short-chain n-alkanes from bacteria. Therefore, the contribution of microorganism to total organic carbon(TOC) and its resulting in carbon isotopic composition should be carefully assessed in future studies. 展开更多
