Objectives:In the sphere of occupational intensity, nursing stands recognized for its inherently demanding nature, marked by a rapid succession of tasks. Our primary aim was to assess the level of job intensity within nursing by analyzing the frequency and duration of activities performed by nurses, instances of task overlap, and the distribution of break times within a primary healthcare center. Methods:This study was conducted using a descriptive working method.Nursing activities were recorded with the observation of six nurses, each monitored for 37.5 h.We used the Maribor System for measuring Quantity in Nursing Care in Primary Health Care Settings to measure the nursing care activities about direct patient care, indirect patient care, and other unproductive aspects through direct non-participant observations. Results:About 41.5%(n=1,640)of all nursing activities involving direct contact with patients, about 38.2%(n=1,508)was indirect patient care, where a large amount worked with paper files(n=666,16.9%).In addition, about 15.0%were other nursing activities(mostly computer work),and just 5.3%(n=210)of tasks were unproductive. The observational findings indicate a pronounced level of work intensity experienced by nurses in primary health clinics.Significantly, nurses were predominantly engaged in direct patient care tasks, often managing multiple activities simultaneously.Their transitions between tasks occurred approximately every 3 min, frequently without adequate intervals for breaks. Conclusions:This study highlights the need to shift nurses’focus from a task-centric approach to one centered on patient care. The prevailing emphasis on tasks may contribute to enduring fatigue and professional dissatisfaction.Consequently, there is an urgent need to redefine the scope of a nurse’s role and implement a comprehensive computer information system as an integral part of this redefined approach. 展开更多

Objective:The entire mental health hazard among the nurses dealing with COVID-19 is just the tip of the iceberg. The goal of this study was to find acceptable and adaptive coping techniques for nurses in order to keep the healthcare system resilient during and after the COVID-19 pandemic.Method:The scoping review search took place from January 2020 to May 2021 published articles, and the results were saved in Endnote software. For data synthesis and review, the Joanna Briggs Institute manual and PRISMA-ScR principles were followed.Following backward and forward screening to exclude redundant, irrelevant, and ineligible studies,13 studies(3,613 nurses)were finally selected for this review. Results:The majorities of participants in these studies were female and had children, and worked in departments of critical care, emergency, and fever clinics.Safety concerns for family or colleagues,as well as a lack of safe working environment, were the primary causes of the mental illness.Professional re-sponsibility attitude,COVID-19 training, workplace safety, team spirit, and a few relaxation activities such as deep breathing, reading books, watching movies, and praying,on the other hand, were found to be beneficial for stress coping among nurses. Conclusion:Mentally fit nurses are more capable and flexible in the face of any life-threatening outbreak, even if it is very contagious.Several coping strategies were found to be effective for nurses during the COVID-19 pandemic, whereas problem-specific intervention trials might yield even better results. We should not delay specific mental health interventions to keep them viable and more efficient during a pandemic battlefield, whereas Post-Traumatic Stress Disorder and suicide rates can destroy the entire health care system. 展开更多

Objective:This study aimed to identify the challenges of community health nurses(CHNs)in delivering effective community health care to achieve universal health coverage(UHC)in Myanmar. Methods:A total of 30 CHNs from township health centers in the northeastern, southern, and western parts of Myanmar were purposefully recruited for quantitative and qualitative interviews.Quantitative data were processed using Microsoft Excel software, and qualitative data were analyzed using thematic analysis. This study is registered with researchregistry6201.Results:Around the country,30 CHNs uncovered their hardships in implementing primary health care to achieve UHC.Over 90%of the participants agreed to the problem of inadequate health infrastructure, while half of them felt unmotivated when they encountered role conflicts among various cadres of healthcare providers and poor opportunities for career promotion.Major problems arose from the lack of standard professional education at the entry point to community settings because most CHNs did not achieve specialized training in providing public health services.Complications are incapable of evaluating health services for policy-making and the inability to conduct health research to develop evidencebased practices.Insecure work and living conditions, unsupportive community relationships, and undereducation in professional practices were supportive major themes explored by CHNs to achieve a deeper understanding of the barriers to UHC.Not only the health system itself but also the population and other geographical factors have contributed to many challenges to CHNs.Conclusion:Myanmar’s CHNs face many challenges in achieving UHC.These challenges are not confined to the health sector. Some situations, such as geographical barriers and transportation problems, remain persistent challenges for healthcare providers. This study highlights the fact that current health systems should be strengthened by qualified healthcare providers and sufficient infrastructure.Meanwhile, public empowerment plays a critical role in promoting health development. 展开更多

Objective: To explore the fundamental theory, methods and data of the quality of life (QOL) of soldiers and its influence factors in the first response army. Methods: Totally 215 soldiers in the first response army of a military area in China were inquired by using self-regulating questionnaire and WHOQOL-BREF (Chinese Edition). Resalts: WHOQOL-BREF had good acceptability, reliability and validity in the first response army. QOL of soldiers in the first response army was middling. The influencing factors of QOL of the soldiers in first army includes self-report health, different areas where the army men come from, different arms of services, whether only child in family. And the difference was statistically significant. Conclusion: WHOQOL-BREF is fit for evaluating the quality of life of soldiers in the first response army. There are differences of the quality of life among the soldiers in the first response army. So we should take appropriate measures to improve the soldiers' quality of life. 展开更多

As more nurses embrace precision science, there is a tendency to utilize theoretical frameworks from other disciplines thus, placing nursing at risk of losing its autonomy and independence. The discipline has fallen prey to internal binary opposition, eliminating opportunities to engage in civil discourse. To explore how the roles nurses select might fit together in a theoretical framework and help nurses understand how the roles they choose to support their identity as nurses, this paper introduced a model of nursing that includes the bench scientists, the policy activists, and bedside nurses, using the Neuman Systems Model(NSM).The Nurse Role Integration Model(NRIM)espouses the basic tenets of NSM:prevention counteracts stressors from penetrating the client's lines of defense thus, reducing stress response.Primary prevention reflects the work of the nurse bench scientists, investigating the underlying mechanisms behind pathophysiology;secondary prevention is applied nurse scientists who build upon nurse researchers'work, identifying and testing potential interventions;tertiary prevention is nurse policy activists, the fulcrum, who leverage primary and secondary findings to argue policy change at all levels.Once policy change is adopted, bedside nurses are educated and implement the change. This lens provides an opportunity to create greater solidarity, strengthening the unity and autonomy of the discipline. 展开更多

Objectives:Intensive health services'utilization is common in older individuals affected by chronic diseases. This study assessed whether a structured family nurse-led educational intervention would be effective in reducing health services'use(readmissions and/or emergency service access)among older people affected by chronic conditions. Methods:This is a non-randomized before-after pilot study.A sample of 78 patients was recruited from two general practices in Italy and 70 among them were followed for 8 months.Standard home care was provided during the first four months'period(months 1-4),followed by the educational intervention until the end of the study(months 5-8).The intervention, based on the teach-back method, consisted of by-weekly 60-min home sessions targeting aspects of the disease and its treatment, potential complications, medication adherence, and health behaviours.Rates of health services'use were collected immediately before(T0),and after the interventions(T1).Differences in utilization rates were examined by the McNemar's test.Potential factors associated with the risk of health services'use were explored with a Cox proportional hazard regression model. Results:The sample(n=78)was predominantly female(n=50,64.1%),and had a mean age of 76.2(SD=4.8)years.Diabetes mellitus was the most frequent disease(n=27,34.6%).McNemar's test indicated a significant reduction in health services'use at T1(McNemar χ^(2)==28.03,P<0.001).Cox regressions indicated that time and patient education,as well as their interaction, were the only variables positively associated with the probability of health services'use. Conclusion:A teach-back intervention led by a family nurse practitioner has the potential to reduce health services'use in older patients with chronic diseases. 展开更多
