Infection with C. difficile (CDI) is characterized by diarrhea in clinical syndromes that vary from selflimited to mild or severe. Since its initial recognition as the causative agent of pseudomembranous colitis, C. difficile has spread around the world. CDI is one of the most common healthcare-associated infections and a significant cause of morbidity and mortality among older adult hospitalized patients. Due to extensive antibiotic usage, the number of CDIs has increased. Diagnosis of CDI is often difficult and has a substantial impact on the management of patients with the disease, mainly with regards to antibiotic management. The diagnosis of CDI is primarily based on the clinical signs and symptoms and is only confirmed by laboratory testing. Despite the high burden of CDI and the increasing interest in the disease, episodes of CDI are often misdiagnosed. The reasons for misdiagnosis are the lack of clinical suspicion or the use of inappropriate tests. The proper diagnosis of CDI reduces transmission, prevents inadequate or unnecessary treatments, and assures best antibiotic treatment. We review the options for the laboratory diagnosis of CDI within the settings of the most accepted guidelines for CDI diagnosis, treatment, and prevention of CDI.展开更多
Objective: To investigate the significance of extra-nodal spread in special histological sub-types of breast cancer and the relationship of such spread with prognostic parameters.Methods: A total of 303 breast cancer ...Objective: To investigate the significance of extra-nodal spread in special histological sub-types of breast cancer and the relationship of such spread with prognostic parameters.Methods: A total of 303 breast cancer cases were classified according to tumor type, and each tumor group was subdivided according to age, tumor diameter, lymph node metastasis, extra-nodal spread, vein invasion in the adjacent soft tissue, distant metastasis, and immunohistochemical characteristics [estrogen receptor(ER), progesterone receptor(PR) existence, p53, c-erbB-2, and proliferative rate(Ki-67)]. The 122 cases with extra-nodal spread were clinically followed up.Results: An extra-nodal spread was observed in 40%(122 cases) of the 303 breast cancer cases. The spread most frequently presented in micro papillary carcinoma histological sub-type(40 cases, 75%), but least frequently presents in mucinous carcinoma(2 cases, 8%). Patients with extra-nodal spread had a high average number of metastatic lymph nodes(8.3) and a high distant metastasis rate(38 cases, 31%) compared with patients without extra-nodal spread.Conclusion: The existence of extra-nodal spread in the examined breast cancer sub-types has predictive value in forecasting the number of metastatic lymph nodes and the disease prognosis.展开更多

METHODS:CD133 protein expression was evaluated by immunohistochemistry in 34 cholangiocarcinoma specimens.In addition,proliferation,chemoresistance and invasive properties of CD133-enriched(CD133 + ) and CD133-depleted(CD133 )RMCCA1 cholangiocarcinoma cells were studied and compared. RESULTS:Strong CD133 expression was observed in 67.6%(23/34)of the cholangiocarcinoma specimens. Strong expression of CD133 was significantly associated with nodal metastasis(P=0.009)and positive surgical margin status(P=0.011).In the in vitro study, both the CD133 + and CD133 cells had similar proliferation abilities and resistance to chemotherapeutic drugs.However,the CD133 + cells had a higher invasive ability compared with CD133 cells. CONCLUSION:CD133+cells play an important role in the invasiveness of cholangiocarcinoma.Targeting of the CD133+cells may be a useful approach to improve treatment against cholangiocarcinoma.展开更多
When using global positioning system/BeiDou navigation satellite(GPS/BDS)dual-mode navigation system to locate a train,Kalman filter that is used to calculate train position has to be adjusted according to the feature...When using global positioning system/BeiDou navigation satellite(GPS/BDS)dual-mode navigation system to locate a train,Kalman filter that is used to calculate train position has to be adjusted according to the features of the dual-mode observation.Due to multipath effect,positioning accuracy of present Kalman filter algorithm is really low.To solve this problem,a chaotic immune-vaccine particle swarm optimization_extended Kalman filter(CIPSO_EKF)algorithm is proposed to improve the output accuracy of the Kalman filter.By chaotic mapping and immunization,the particle swarm algorithm is first optimized,and then the optimized particle swarm algorithm is used to optimize the observation error covariance matrix.The optimal parameters are provided to the EKF,which can effectively reduce the impact of the observation value oscillation caused by multipath effect on positioning accuracy.At the same time,the train positioning results of EKF and CIPSO_EKF algorithms are compared.The eastward position errors and velocity errors show that CIPSO_EKF algorithm has faster convergence speed and higher real-time performance,which can effectively suppress interference and improve positioning accuracy.展开更多
