METHODS:Studies using the EMR technique to resect large colorecta...AIM:To evaluate the proportion of successful complete cure en-bloc resections of large colorectal polyps achieved by endoscopic mucosal resection(EMR). METHODS:Studies using the EMR technique to resect large colorectal polyps were selected.Successful complete cure en-bloc resection was defined as one piece margin-free polyp resection.Articles were searched for in Medline,Pubmed,and the Cochrane Control Trial Registry,among other sources. RESULTS:An initial search identified 2620 reference articles,from which 429 relevant articles were selected and reviewed.Data was extracted from 25 studies(n =5221)which met the inclusion criteria.All the studies used snares to perform EMR.Pooled proportion of en-bloc resections using a random effect model was 62.85%(95%CI:51.50-73.52).The pooled proportion for complete cure en-bloc resections using a random effect model was 58.66%(95%CI:47.14-69.71).With higher patient load(>200 patients),this complete cure en-bloc resection rate improves from 44.19%(95%CI: 24.31-65.09)to 69.17%(95%CI:51.11-84.61). CONCLUSION:EMR is an effective technique for the resection of large colorectal polyps and offers an alternative to surgery.展开更多

Many applications are included such as the energy generation and purchasing. The aim of the present study is to find the most suitable models for the peak load of the Kingdom of Bahrain. Many mathematical methods have been developing for maximum load forecasting. In the present paper, the modeling of the maximum load, population and GDP (gross domestic product) versus years obtained. The curve fitting technique used to find that models, where Graph 4.4.2 as a tool used to find the models. As well, Neuro-Fuzzy used to find the three models. Therefore, three techniques are used. These three are exponential, linear modeling and Neuro-Fuzzy. It is found that, the Neuro-Fuzzy is the most suitable and realistic one. Then, the linear modeling is the next suitable one.展开更多

Methods: The clinical data of ...Objective: To evaluate the effect of the treatment modality guided by intraventricular intracranial pressure (ICP) monitoring on patients with severe traumatic brain injury (TBI). Methods: The clinical data of a group of 136 severely brain-injured patients admitted to Shanghai Neurosurgical Emergency Center from December 2004 to February 2006 were studied. Results: The intraventricular ICP monitor was placed in all the 136 patients via Kocher's pathway, Paine's pathway or intraoperative opened ventricle. In this series, the probe was placed during the procedure of craniotomy in 98 patients; for other 38 patients, the probe was placed initially to measure or to monitor ICE A stepwise protocol targeting at ICP control (420 mm Hg) and optimal cerebral perfusion pressure (CPP) maintenance (60-90 mm Hg) was deployed.Among them, 76 patients survived with good recovery, 14 with moderate disability, 24 with severe disability, 10 with vegetative state, and 12 died. Complications associated with intraventricular ICP monitoring included hemorrhage and infection. Hemorrhage occurred in 1 patient and infection in 5 patients. There were no unacceptable complications related to ICP monitoring. Conclusions: Ventricular access for 1CP monitoring can be safely and accurately achieved. ICP monitoring via ventriculostomy may facilitate an early and accurate intervention for severely brain-injured patients. The intraventricular ICP monitoring is a low-risk procedure and can yield great benefits for management of patients with severe TBI.展开更多
