Results from RT-PCR showed that expression of LBPmRNA in the liver, lungs and kidneys was increased after shock and resuscitation. It is suggested thathemorrhagic shock could significantly enhance endotoxin-induced TNFa production, which might be due toup-regulation of LBP expression in tissues after shock, and tissue macrophages might be the main source ofcytokine production.展开更多
Objective:To observe the clinical efficacy of row needling at the Governor Vessel on the head for poststroke insomnia,analyze the changes of sleep structure,serum amino acid neurotransmitters,vascular endothelial cell...Objective:To observe the clinical efficacy of row needling at the Governor Vessel on the head for poststroke insomnia,analyze the changes of sleep structure,serum amino acid neurotransmitters,vascular endothelial cell function and cognitive function,in order to explore the mechanism of this therapy.Methods:A total of 41 patients with poststroke insomnia were included in an observation group and treated with row needing at the Governor Vessel on the head;33 patients with poststroke insomnia were included in a control group and given routine acupuncture treatment.The patients in both groups were treated consecutively for one month.The efficacy was observed and the changes of related indicators were compared.Results:After treatment,the efficacy of the observation group was better than that of the control group(P<0.05).The sleep latency in the observation group was shorter than that in the control group(P<0.05);the total sleep duration,deep sleep duration,light sleep duration and rapid eye movement sleep duration in the observation group were significantly longer than those in the control group(P<0.05);the total waking duration and the waking times over 5 min were significantly less than those in the control group(P<0.05).The serum glutamic acid,γ-aminobutyric acid,calcitonin gene-related peptide,brain-derived neurotrophic factor,nerve growth factor and nitric oxide levels were significantly higher than those in the control group(P<0.01).The tumor necrosis factor-α,interleukin-6,bradykinin,and endothelin-1 levels were lower than those in the control group(P<0.01).The Pittsburgh sleep quality index score in the observation group was lower than that in the control group(P<0.01),and the mini-mental state examination score was higher than that in the control group(P<0.05).Conclusion:Row needling at the Governor Vessel on the head can effectively improve the sleep structure,cognitive function,and sleep quality of patients with poststroke insomnia.The possible mechanisms are regulating serum amino acid neurotransmitters and improving the function of vascular endothelial cells.展开更多
Successful embryo implantation depends on a well-functioning endometrium and high-grade quality embryos.An increasing body of evidence suggests that sildenafil citrate plays an important role in endometrial receptivit...Successful embryo implantation depends on a well-functioning endometrium and high-grade quality embryos.An increasing body of evidence suggests that sildenafil citrate plays an important role in endometrial receptivity and implantation.However,few studies have investigated the effects of oral sildenafil citrate on the clinical outcomes of patients with recurrent implantation failure(RIF).In the present study,we aimed to determine the impact of sildenafil citrate on improving the endometrial blood flow and pregnancy rate in RIF patients.A total of 346 frozen-thawed transfer cycles from 305 RIF patients undergoing IVF at the Center for Reproductive Medicine of Peking University Third Hospital from June 2013 to December 2017 were enrolled.Oral sildenafil citrate was given to the patients,and the related effects on endometrial blood flow,endometrial thickness,and clinical outcomes were investigated.In the 346 transferred cases,the proportion of three groups(A:undetectable blood flow;B:sub-endometrial blood flow;C:endometrial and sub-endometrial blood flow)before and after oral administration of sildenafil citrate was as follows:group A:36.7%(n=127)vs.7.5%(n=26);group B:42.8%(n=148)vs.24.0%(n=83);group C:20.5%(n=71)vs.68.5%(n=237).Moreover,the endometrial blood flow in 137 FET cycles was improved(61.8%).Besides,the implantation rate was 19.47%(132/678),and the clinical pregnancy rate reached 32.66%(113/346),while the endometrial blood flow of the pregnancy group was better compared with the non-pregnancy group.This study explored the improving effects of sildenafil citrate on the endometrial receptivity and clinical outcomes of RIF patients,confirming the crucial role of good endometrial blood flow in a successful pregnancy.Meanwhile,the low incidence of adverse events also supported the security and feasibility of sildenafil citrate in the clinical therapy of RIF patients.展开更多
