解剖型髓内钉和股骨近端防旋髓内钉治疗老年股骨转子间骨折的临床对照研究 被引量:5

目的比较解剖型髓内钉(ZNN)和股骨近端防旋髓内钉(PFNA)治疗老年股骨转子间骨折的临床疗效。方法回顾性分析上海市第十人民医院2015年6月至2018年6月手术治疗的208例大于60岁的老年股骨转子间骨折患者的临床资料,所有患者均获得1年以上... 目的比较解剖型髓内钉(ZNN)和股骨近端防旋髓内钉(PFNA)治疗老年股骨转子间骨折的临床疗效。方法回顾性分析上海市第十人民医院2015年6月至2018年6月手术治疗的208例大于60岁的老年股骨转子间骨折患者的临床资料,所有患者均获得1年以上随访。ZNN治疗96例,PFNA治疗112例。比较两组患者的手术时间、术中失血量、术后尖顶距、骨折愈合时间、术后并发症发生率、髋部外侧疼痛情况和术后1年随访时髋关节Harris评分。结果两组患者平均随访(18.7±5.6)个月。手术时间ZNN组(71.3±15.1)min,PFNA组(67.3±7.2)min,两组间存在差异(P=0.013)。术中失血量ZNN组为(60.1±3.8)mL,PFNA组为(59.1±5.8)mL,差异无统计学意义(P>0.05)。术后尖顶距ZNN组(17.3±3.2)mm,PFNA组(16.8±4.6)mm,差异无统计学意义(P>0.05)。骨折愈合时间ZNN组(15.5±2.5)周,PFNA组(15.1±1.7)周,差异无统计学意义(P>0.05)。术后并发症ZNN组4例(4.2%),其中1例股骨头坏死,3例髋螺钉切割,PFNA组6例(5.4%),其中2例股骨头坏死,4例髋螺钉切割,并发症发生率两组无统计学差异(P>0.05)。术后1年随访时髋关节Harris评分ZNN组为(91.2±4.5)分,PFNA组为(92.5±6.7)分,无统计学差异(P>0.05)。术后1年随访时髋部外侧疼痛发生率ZNN组为3.1%(3例),PFNA组为11.6%(13例),ZNN组明显低于PFNA组,有统计学差异(P=0.022)。结论ZNN和PENA均是治疗老年股骨转子间骨折的有效手段,ZNN组手术时间略长于PFNA组,对于不能耐受较长时间手术的老年患者推荐使用PFNA。ZNN结合了人体股骨前弓设计,对于股骨前弓较大的患者,推荐使用ZNN治疗。 展开更多

利用HTRI软件对某进料蒸汽加热器进行了设计优化。采用单变量分析的方法,从工艺和结构两个方面,考察了折流板间距和切割率对加热器的影响,以及饱和蒸汽和过热蒸汽在热阻分布和传热系数等方面的差异,最终发现切割率为40%、折流板间距350... 利用HTRI软件对某进料蒸汽加热器进行了设计优化。采用单变量分析的方法,从工艺和结构两个方面,考察了折流板间距和切割率对加热器的影响,以及饱和蒸汽和过热蒸汽在热阻分布和传热系数等方面的差异,最终发现切割率为40%、折流板间距350 mm时进料蒸汽加热器性能最优。计算结果表明:随着折流板间距的提高,壳侧传热系数和总传热系数逐渐减小;随着折流板切割率的增大,壳侧传热系数及总传热系数均先增大后减小;与过热蒸汽相比,饱和蒸汽可以增大总传热系数,提高加热器设计裕量,并使生产操作更易于控制。 展开更多

The wire electrical discharge machining(EDM) of 6061 aluminium alloy in terms of material removal rate,kerf/slit width,surface finish and wear of electrode wire for different pulse on time and wire tension was studied.Eight experiments were carried out in a wire EDM machine by varying pulse on time and wire tension.It is found that the material removal rate increases with the increase of pulse on time though the wire tension does not affect the material removal rate.It seems that the higher wire tension facilitates steady machining process,which generates low wear in wire electrode and better surface finish.The surface roughness does not change notably with the variation of pulse on time.The appearance of the machined surfaces is very similar under all the machining conditions.The machined surface contains solidified molten material,splash of materials and blisters.The increase of the pulse on time increases the wear of wire electrode due to the increase of heat input.The wear of wire electrode generates tapered slot which has higher kerf width at top side than that at bottom side.The higher electrode wear introduces higher taper. 展开更多

An evaluation was performed on the influence of mowing height (2.5,5.0 or 7.5 cm) and mowing frequency (weekly or fortnightly) on levels of ergot alkaloids (ergine and ergonovine) formed in drunken horse grass,Achnatherum inebrians,grown under greenhouse conditions.Samples were taken monthly and alkaloids were extracted and analyzed by reverse-phase HPLC.Alkaloid levels increased more or less linearly with plant age over the first four months following establishment.Levels were higher in samples cut fortnightly compared to those cut weekly,and were higher when plants were cut at a mowing height of 7.5 cm vs.2.5 cm.In most cases,the highest alkaloid levels observed were almost three times those of the lowest.If plant protection applications are developed for the endophytic fungus,Neotyphodium gansuense,it will be necessary to be aware of the potential role of plant husbandry practices (e.g.,defoliation frequency and intensity) for reducing or enhancing levels of plant alkaloids.Similar husbandry factors may affect alkaloid levels in other Neotyphodium-grass associations,which would be interesting for further study. 展开更多
