Objective:Compared with long-term renal replacement therapy,kidney transplantation is the ideal treatment for end-stage renal disease(ESRD),significantly extending patient life and improving quality of life.Kidney transplant patients need to adhere to lifelong immunosuppressive medication regimens,but their medication adherence is generally poor compared with other organ transplant recipients.Medication adherence is closely related to medication literacy and psychological status,yet related studies are limited.This study aims to investigate the current status of medication adherence,inner strength,and medication literacy in kidney transplant patients,analyze the relationships among these 3 factors,and explore the mediating role of inner strength in the relationship between medication literacy and medication adherence.Methods:A cross-sectional survey was conducted from March to October 2023 involving 421 patients aged≥18 years who visited kidney transplantation outpatient clinics at 4 tertiary hospitals in Hunan Province.The inner strength,medication literacy,and medication adherence of kidney transplant patients were investigated using the Inner Strength Scale(ISS),the Chinese version of the Medication Literacy Assessment in Spanish and English(MedLitRxSE),and the Chinese version of the Morisky Medication Adherence Scale-8(C-MMAS-8),respectively.Univariate analysis was performed to examine the effects of demographic and clinical data on medication adherence.Correlation analysis was conducted to explore the relationships among medication literacy,medication adherence,and inner strength.Significant variables from univariate and correlation analyses were further analyzed using multiple linear regression,and the mediating effect of inner strength was explored.Results:Among the 421 questionnaires collected,408 were valid,with an effective rate of 96.91%.The scores of C-MMAS-8,MedLitRxSE,and ISS were 6.64±1.16,100.63±14.67,and 8.47±4.03,respectively.Among the 408 patients,only 86(21.08%)patients had a high level of medication adherence,whereas 230(56.37%)patients had a medium level of medication adherence,and 92(22.55%)patients had poor medication adherence.Univariate analysis indicated that the kidney transplant patients’age,marital status,education levels,years since their kidney transplant operation,number of hospitalizations after the kidney transplant,and adverse drug reactions showed significant differences in medication adherence(all P<0.05).Correlation analysis showed that inner strength positively correlated with both medication literacy(r=0.183,P<0.001)and medication adherence(r=0.201,P<0.001).Additionally,there was a positive correlation between medication adherence and medication literacy(r=0.236,P<0.001).Inner strength accounted for 13.22%of the total effect in the mediating role between medication literacy and medication adherence.Conclusion:The level of medication adherence among kidney transplant patients needs improvement,and targeted intervention measures are essential.Inner strength mediates the relationship between medication literacy and medication adherence in these patients.Healthcare professionals should focus on enhancing medication literacy and supporting patients’inner strength to improve medication adherence. 展开更多

Oxygen-enriched top-blown smelting is a promising technology for processing waste printed circuit boards(WPCBs).The distribution behavior of valuable elements in WPCBs during smelting was investigated by varying the oxygen-enriched concentration,oxygen volume,CaO/SiO_(2)(mass ratio),and Fe/SiO_(2)(mass ratio).The optimal operating conditions were obtained by implementing a one-factor-at-a-time method.X-ray diffractometer,scanning electron microscopy−energy dispersive spectrometer,and inductive coupled plasma-atomic emission spectroscopy methods were utilized to detect the chemical composition,occurrence state as well as elemental contents of alloy and slag.It is found that the elements of Cu,Sn and Ni are mainly accumulated in the alloy while Fe is mainly oxidized into the slag.The direct yields of Cu,Sn and Ni are 90.18%,85.32%and 81.10%under the optimal conditions of temperature 1250℃,oxygen-enriched concentration 30%,oxygen volume 24 L,CaO/SiO_(2) mass ratio 0.55,and Fe/SiO_(2) mass ratio 1.05.The results show that the valuable metals are mainly lost in the slag through mechanical entrainment. 展开更多

Then, this paper estimates China's future human capital demand and shortage based on economic and demographic forecast. [Objective] The aim of this paper is to investigate the influences of exogenous 5-aminolevulinic acid (5-aLA) application on the growth and mechanism of winter oilseed rape (Brassica napus L.) before winter. [Method] For this purpose a split-plot experimental design with four treatments of seed soaking with 0,25,50 and 75 mg/L ALA solution (CK,A1,A2 and A3) was carried out from October 2009 to January 2010 in order to observe the influences upon the growth and relevant physiological indexes of Zhongshuang 9. [Result] Seed soaking with ALA solution significantly promotes the growth of double-9 before winter,which is probably due to the fact that ALA application enhances its photosynthesis,absorption of nutrients,and its anti-oxidation abilities. The research also finds that ALA treatment notably increases its photosynthetic pigment contents,root activities,leaf nitrate reductase activities,soluble sugar contents,antioxidase activities,and significantly decreases malondialdehyde(MDA) content. [Conclusion] These results indicate that seed soaking with 50 mg/L ALA solution might have a potential application in increasing the productivity of winter oilseed rape in agricultural management. 展开更多

[Method] Alliinase activity was measured to analysis the influence of temperature, pH, substrate concentration and metal iron. [Result] Alliinase was an enzyme with thermal instability. Its optimum reaction temperature was 29℃ and pH value was 6.1. The Vmax was 0. 439 IU/mg and Km was 0.483 mmol/L by using natural extract as substrate. Alliinase activity was activated when the K^+ , Mg^2+ , Na^+ and Cd^2+ existed and alliinase activity was inhibited when Cu^2+ existed. [Condusion] Results showed that the kinetic characteristics of alliinase supply the academic foundation for development and application of garlic medical products. 展开更多

By creating a competitiveness framework and a system of heterogeneous indicators, this paper investigates the competitiveness of China and the US in terms of current status, historic change and global environment. Our research led to the following findings: core factors determine the level of competitiveness for China and the US; the national competitiveness of both countries is evolving towards structural homogeneity; and China and the US lead most countries in many common areas. China has the potential to overtake the US in competitiveness in the future. We suggest that China increase its competitiveness by promoting its advantages, addressing its weaknesses and focusing on core areas. 展开更多

In this study,circular dichroism(CD)and molecular dynamics(MD)simulation were used to investigate the thermal unfolding pathway of staphylococcal enterotoxin B(SEB)at temperatures of 298–371 and 298–500 K,and the relationship between the experimental and simulation results were explored.Our computational findings on the secondary structure of SEB showed that at room temperature,the CD spectroscopic results were highly consistent with the MD results.Moreover,under heating conditions,the changing trends of helix,sheet and random coil obtained by CD spectral fitting were highly consistent with those obtained by MD.In order to gain a deeper understanding of the thermal stability mechanism of SEB,the MD trajectories were analyzed in terms of root mean square deviation(RMSD),secondary structure assignment(SSA),radius of gyration(R_(g)),free energy surfaces(FES),solvent-accessible surface area(SASA),hydrogen bonds and salt bridges.The results showed that at low heating temperature,domain Ⅰ without loops(omitting the mobile loop region)mainly relied on hydrophobic interaction to maintain its thermal stability,whereas the thermal stability of domain Ⅱ was mainly controlled by salt bridges and hydrogen bonds.Under high heating temperature conditions,the hydrophobic interactions in domain Ⅰ without loops were destroyed and the secondary structure was almost completely lost,while domain Ⅱ could still rely on salt bridges as molecular staples to barely maintain the stability of the secondary structure.These results help us to understand the thermodynamic and kinetic mechanisms that maintain the thermal stability of SEB at the molecular level,and provide a direction for establishing safer and more effective food sterilization processes. 展开更多

Methods HPMC was used as matrix to formulate the sustained release tablet. The influences of the size of melatonin, type and amount of HPMC, drug loading, type and amount of additives, and compressing pressure were investigated. Plasma concentration of melatonin in dogs after intravenous injection of two doses and oral administration of sustained release tablets and unmodified release capsules was detected by HPLC using fluorescence detector. Results The drug release from sustained release tablets was influenced by the size of melatonin, type and amount of HPMC, drug loading, and type and amount of additives. Melatonin was found to fit two compartment model after intravenous injection, AUC was proportional to doses, and t(1/2β) of two doses has no significant difference. Relative bioavailability of melatonin sustained release tablet to normal capsule was 83.8%, and absolute bioavailability was 3.75% for sustained release tablet and 4.49% for capsule. Conclusion The melatonin sustained release tablet was well formulated. The absolute bioavilability for oral administration of either sustained release tablet or unmodified release capsule of melatonin was less than 5%. The bioavailability of melatonin sustained release tablet was lower than that of unmodified release capsule, but MRT of sustained release tablet was significantly longer than that of capsule. 展开更多

Effect of Milrinone Induced Insulin Resistance on Glucose and Lipid Metabolism in Rats

Aim To investigate the effects of milrinone (a selective phosphodiesteraseIII inhibitor PDE_3 ) on insulin secretion, blood glucose, plasma free fatty acids (FFA) anddose-response relationship, and assess possible effects of milrinone on glucose metabolism andinsulin sensitivity in conscious rats. Methods The catheterized nonstressed rats were administeredvarious doses of milrinone (1, 5, 25μmoL·kg^(-1)) and were compared with controls. Ahyperinsulinaemic-eugly-caemic clamp was established in counscious rats, andmilrinone(25μmoL·kg^(-1)) and 25% dimethyl sulfoxide (DMSO, as a control) were given at 120 minduring hyperinsulinaemic-euglycaemic clamping. Glucose turnover was determind with by gaschromatograph mass spectrometer (GC-MS). Results After dosing, plasma FFA levels in 3 milrinonegroups significantly increased, compared with the controls and before dosing. The percentages ofelevation of FFA by the different milrinone doses were very similar, 50%, and 52% , 55% for 1, 5,and 25 μmoL·kg^(-1), repectively, at 2 min after dosing. Plasma insulin levels were significantlyelevated in the 5 and 25 μmoL·kg^(-1) groups, and the effect of milrione on glucose concentrationwas detectable only in 25μmoL·kg^(-1) group. During hyperinsulinaemic clamping, there weresignificant increase, in plasma FFA (from 173 +- 15 to 634 +- 87μmoL·kg^(-1)) and hepatic glucoseproduction (HGP), and a significant decrease in glucose infusion rates (GIR) to about 21% and aslight increase in plasma insulin after milrinone treatment. Conclusion Milrinone impaires theability of insulin to suppress lipolysis and HGP, and insulin-mediated glucose utilization inperipheral tissue. Therefore, milrinone administration may induce an acute insulin resistance invivo. 展开更多

Four winter wheat (Triticum aestivum L.) varieties ('JD 8', 'Jing 411','Centurk' and 'Tam 202') were used to study the effect of heat stress on photosynthetic characteristics of flag leaf blade, nag leaf sheath, peduncle, glume, lemma and awn during grain-filling stage. The results showed that heat acclimation during grain-filling stage increased thermotolerance of wheat with significant differences among different green organs. During heat stress, the decreases of the efficiency of primary light energy conversion (F-v/F-m) of PS II and pigment (chlorophyll and carotenoid) content were much slower in peduncle, flag leaf sheath and glume than in nag leaf blade, lemma and ann; and the percentage of decrease in net photosynthetic rate (P-n) of ear was lower than that of the nag leaf blade. The measured photosynthetic parameters (F-v/F-m, P-n and pigment content) of 'JD 8', a relatively heat tolerant variety, declined more slowly than those of the other three varieties during the whole heat stress period. 展开更多
